Fountain View Recreation Center Fountain View Recreation Center is the perfect venue for birthdays, receptions, showers, graduation/sweet sixteen parties or any special occasion. With scenic lake views, access to the outdoor patio and state-of-the-art AV equipment, this is the perfect place for your party or meeting. The banquet room can accommodate 160 people and can be split into two or three individual rooms, each with AV capability and a variety of room set up options. At Your House Party Wagon
Wow the crowd at your next party or picnic with our Party Wagon! The fun-making trailer is stocked with 10 tables, 100 chairs, a 10’ by 10’ pop up canopy, a supersized charcoal grill, ladder toss, bocce set, volleyball net, jump ropes and bean bag game. We deliver right to homes within the Carol Stream Park District. The Party Wagon is available April - October. Cost is $125 per rental.

How to Register
To register for these programs, complete the ActivKids registration forms and submit with payment. Forms are available online, at your school’s ActivKids site, or at the Fountain View or Simkus Recreation Centers. Registration must be at least 24 business hours prior to your child’s start date. Dance Class Information Instructional dance classes are offered on a seasonal basis. Participants do not perform in annual spring dance recital. Parent Observation will be held during the last class of the session. Only the registered child will be allowed to attend and participate in class. All classes are held at Simkus Recreation Center. There are no classes on July 4. Girls may wear leotard, tights, pants or capris depending on the class. Boys may wear athletic pants or shorts, t-shirt and socks. WAIVER & RELEASE The Carol Stream Park District is committed to conducting its recreation programs and activities in a safe manner and holds the safety of participants in high regard. The Carol Stream Park District continually strives to reduce such risks and insists that all participants follow safety rules and instructions that are designed to protect the participants’ safety. However, participants registering for Forever Young trips, outings and activities must recognize that there is an inherent risk of injury arising out of this activity. You are solely responsible for determining if you are physically fit and/or adequately skilled for the activities contemplated by this agreement. It is always advisable, especially if the participant is disabled in any way or recently suffered an illness, injury or impairment, to consult a physician before undertaking any physical activity.
WARNING OF RISK Despite careful and proper preparation, instruction, medical advice, conditioning and equipment, there is still a risk of serious injury when participating in any recreational activity/program. Understandably, not all hazards and dangers can be foreseen. Participants must understand that depending upon the particular activity, certain risks, dangers and injuries due to acts of God, inclement weather, slips and falls, inadequate or defective equipment, inadequate supervision or instruction, premises defects, carelessness, horseplay, vehicle accidents and all other circumstances inherent to recreational activities/programs exist. In this regard, it must be recognized that it is impossible for the Carol Stream Park District to guarantee absolute safety.
WAIVER AND RELEASE OF ALL CLAIMS AND ASSUMPTION OF RISK Please read this form carefully and be aware that in signing up and participating in this trip/outing/ activity, you will be expressly assuming the risk and legal liability and waiving and releasing all claims for injuries, damages or loss which you might sustain as a result of participating in any and all activities connected with and associated with trip/outing/ activity (including transportation services and vehicle operations), when provided.
I recognize and acknowledge that there are certain risks of physical injury to participants in trip/outing/ activity, and I voluntarily agree to assume the full risk of any and all injuries, damages or loss, regardless of severity, that I may sustain as a result of said participation. Say cheese! Next time you are visiting Simkus or Fountain View, please allow time for your photo to be taken at the service desk for safety purposes. If you register online or by mail, just stop by the service desk when you arrive for your next trip or event. Those who have a fitness or pool membership will not need to have their account updated with a new photo.
Departure Times
Departure times listed for each trip reflect the time when the bus departs. Please arrive ten minutes early in order to check in and board the bus. Seating is first come, first served. Specific individual seats will not be reserved for participants.
Accessibility
The park district strives to provide trips and activities for everyone. Most Forever Young programs are accessible; however, participants using walkers or wheelchairs should confirm the trip location’s accessibility in advance. Use the Activity Key as a guide when determining if a program suits your capabilities. Indicate any accommodation requests on your registration form. Front bus seating may be reserved in advance for those with applicable medical special needs. Participants requesting front of bus seating should arrive fifteen minutes prior to departure time. Pick-up Locations
The park district bus picks up at two Carol Stream locations. Remember to write your pick-up location on your registration form. If you wish to switch locations after you have registered, let us know at least one day in advance.

Trip Cancellations
We understand that trip cancellations sometimes cannot be avoided. Withdrawals are only processed after the payment deadline if you find a replacement or a participant on the waitlist takes your spot. If a friend is taking your place on a trip, they must be paid with a registration form and waiver completed in advance. Passengers who are not on our roster are not permitted to attend.

The Carol Stream Travel Softball and Baseball Association (CSTSBA) is for those boys and girls who are ready to play at the next level and are getting ready for High School sports. The Cougars Baseball teams (9U-15U) play in the WSBL (West Suburban Baseball League) and play additional tournaments all over Illinois. Parent Code of Conduct
Register at csparks.org or pick up a registration packet at the Simkus Recreation Center. By registering your child you agree to comply with the Parent Code of Conduct which can be accessed on the home web page of each youth sports league and is discussed by the coach at the preseason parent meeting.
What is the meet and greet?
The meet and greet is a set time where players, parents, and coaches meet each other and hand out any necessary information, and in some cases will receive their uniforms.
What is the player evaluation?
Players get rated based on each skill set for that particular sport. All players are welcome but new players and players who are playing up a level are required to attend.
How do I register for soccer and tee ball leagues?
Registration is by grade level. Players are to be registered for the grade the player will be in for 2017/2018 school year. Preschoolers must be 4 years old by program start date. Participants can participate in Instructional Tee Ball and Instructional Soccer without conflict.
How do I register for youth baseball?
Registration is by age. Players are to be registered based on their age as of April 30, 2018.
MCCASLIN FAMILY CHALLENGE Sunday, Aug 6 10am-3pm Grades 3-8
Come and enjoy a day out at McCaslin Park. Father and Son, Father and Daughter, Mother and Son, or Mother and Daughter teams compete at McCaslin throughout the day at everything that the park has to offer. Competitions will include Hot Spot Basketball, Bankshot Basketball, Volleyball, Bags, Mini Golf, and Wiffleball. Points are awarded throughout the day and awards are given out to the winners. How do I register for youth softball?
Registration is by age. Players are to be registered based on their age as of December 31, 2017.
What happens if I register after the deadline?
Players placed on a wait list are not guaranteed placement on a team. If you register before team formation the player will be moved off the wait list if there are enough to form an additional team. However, if you register after team formation, or if another team cannot be formed, placement will only be done when a team or teams are below the established maximum number of team participants. Players will not be added to any team one week prior to the first scheduled game.
Can my child play up?
Playing up is not allowed for instructional soccer or tee ball. For all other leagues playing up is allowed by one age or grade level but the following process must be adhered to: • The player must register at his or her age or grade. • A request to play up must be made to the program supervisor. • The player must attend the evaluation day for the sport and must attend at the time designated for the requested league/ level (i.e. if a player should be playing in the Mustang league but wants to play in the Bronco league, player must attend the Bronco player evaluation).
Can I request for my child to be placed on a specific team? No, due to the nature of the program, we cannot accept any other coach, player, or carpool requests. However, one mutual friendship request will be honored for players in an instructional league, Pinto (8U) baseball, or grade 1 and 2 soccer. Forms are available at csparks.org/youthsportsfriendshiprequest/ Request deadlines are the same as registration deadlines (not early bird).
Can my business sponsor my son or daughter's sports team?
Yes, if your business would like to sponsor a youth sports team, contact Pam at 630-784-6123, pamf@csparks.org. Specific sponsorship requests based on availability.

Gary Ave., Carol Stream, in the 25-yard indoor pool and therapy pool. • New swimmers — select a swim class level based on your child’s age and previous lesson history or attend a skill test day. Returning swimmers — select a swim class level based on your child’s previous swim lesson report card. • Class levels may be combined if deemed appropriate. Classes fill quickly; transferring from one level to another is not always possible. If you have questions, please call our Coordinator at 630-784-6187. • First day arrive 15 minutes early for check-in. • Stay for the entire swimmer’s class, in case of emergency where consent is required. • Swimmers will receive a report card at the end of the session indicating which level to register for in the next session. • Please bring — towel, swimsuit already on child; non-potty trained participants must wear a swim diaper with tightly fitting plastic pants under swimsuit; report card from previous session. • Do not bring — swimming aids; lifejackets, water wings, flippers, or snorkel masks that cover the nose. Snacks and treats are recommended to be eaten after lessons. • To receive a full refund withdrawals and transfers must be completed 24 business hours prior to the first day of lessons. Refunds and make-ups will not be given for classes missed. Please refer to Refund and Withdrawal Policy for further details.

Want a private swim lesson but none of our scheduled times work for you? Contact our Swim Lesson Coordinators to schedule a custom private lesson. Participants receive a total of three hours of instruction. Lessons cannot be scheduled during swim team practices, group lessons or regularly scheduled private lessons. Family members living in the same house may be added for an additional fee. Maximum of 3 students per class. Swim Team Dolphins Swim Team
A volunteer-based program for youth who want to join the world of competitive swimming. Dolphins compete in the DuPage Swim & Dive Conference along with 12 other park district teams. Competition is from mid-May through July. Swim meets are on Wednesday nights and Saturday mornings. Swimmers will not be allowed to compete until they can swim 25 yards without stopping. Each family is assessed a $50 volunteer deposit upon registration. Volunteer deposits are refunded on a full or partial basis at the end of the season based on fulfillment of volunteer requirements. Junior Golf Lessons
These golf lessons are designed to teach junior golfers the fundamentals of the golf swing, chipping and putting skills, basic rules and etiquette of golf while offering on course playing time (last class). These lessons will be taught at both Glendale Lakes Golf Club and Green Valley Driving Range. Classes will be held weather permitting. Make-ups will be arranged for classes canceled due to inclement weather only. Golf Clubs will be provided to students if needed, please indicate when registering. Adult Group Golf Lessons
These golf lessons are designed to teach all beginning golfers basic fundamentals of the game. Fundamentals covered include the golf swing, chipping and putting as well as basic rules and etiquette. What We Do
We build and strengthen the financial resources of the District by receiving gifts, grants, bequests and endowments on the District’s behalf. Contributions from individuals and businesses are used to improve or add to existing park projects or programs, or to help fund future projects.
Make a Donation
Your donation will help us fund our Scholarship Program, Parks & Facility Improvements, Program Enhancements and Green Initiatives. As a 501(c)3 corporation, all contributions to the organization are tax deductible. Identified donors will receive a written receipt.
Leave a Legacy
Including a gift of any size in your will is a way you can leave a legacy for your parks.
Help us raise the funds to raise the Flag! Contribute to this Community Fundraising Effort to renovate Memorial Park through the Carol Stream Parks Foundation. Pick up an envelope to make a donation to the project. Make your check payable c/o Carol Stream Parks Foundation or drop it off at the Fountain View Recreation Center (910 N Gary Ave) or the Simkus Recreation Center (849 W Lies Rd).
Raise the Flag for Memorial Park
Payment Full payment for most programs is due at the time of registration. Payment may be made by cash, check or credit card (Visa/MC/AMEX/DISC). Make checks payable to the Carol Stream Park District. There is a $20 fee for returned checks. Activities with recurring or monthly payments must have an EZ Pay Agreement on file. Your payments are charged to your credit/ debit card. For more information, please visit our Service Team at Simkus or Fountain View Recreation Centers to complete an EZ Pay authorization form.
Photography Policy
Carol Stream Park occasionally takes photographs and videos of participants at our parks or facilities, in park district programs, classes or events for marketing purposes.
Oops!
The Carol Stream Park District staff has made every effort to prepare our marketing material as accurately as possible, however, errors can occur. Circumstances may require that adjustments be made to programs, fees, schedules, etc. The Park District reserves the right to make any such adjustments. The Carol Stream Park District apologizes for any inconvenience these errors may cause.
Lost & Found
Contact us if you lost something at one of our parks (630-784-6107) or facilities (630-784-6100).
Avoid Cancelation
Most classes require a set minimum number of participants. Register at least a week prior to the start date to avoid cancellation.
Program Capacity
All programs have minimum and maximum enrollments. If the minimum enrollment is not met one week prior to the program date, the Park District may have to cancel, postpone or consolidate the program with another program. If the program is cancelled, full credits will be issued.
Waiting Lists
Once the maximum enrollment is met for a class, a waiting list will be formed. If space becomes available or additional classes are created, you will be contacted. Payment is due when being waitlisted. Credits will be issued after the first class for those who are not able to be registered.
Inclusion Services
Inclusion Services are designed to provide the least restrictive environment while providing the maximum opportunity to participate in regular park district programs. There is no charge for WDSRA’s support. To register for services, simply indicate that special accommodations are needed on the park district registration form. Two-week advance notice is requested for inclusion support.

Insurance
The Park District does not carry medical or accident insurance for program participants or park patrons. Review your personal health insurance plan to be certain that you and your family have proper coverage.

Dedication trees
Choose from a variety of wonderful trees. Each tree is $350 and includes: Nursery quality tree. Dedication plaque and a dedication leaf placed on the Living Memories display located in the Simkus Recreation Center. Visit csparks.org or contact maryr@csparks.org.
Financial Assistance
We offer financial assistance/scholarships for qualified Carol Stream Park District residents. Confidential applications available at Fountain View Fitness Center and Simkus Recreation Center.
Refund and Withdrawal Policy
It is the District’s intent to provide quality recreation programs and services. The District recognizes however, that there may be occasions when the event/service doesn’t match a customer’s needs, or when the customer is unable to participate in an activity. For those occasions, the following withdrawal policy will apply. All requests for refunds will be assessed a ‘Service Fee’ of 10% of the original, total registration fee - $5 minimum / $50 maximum. No refunds under $5; credit will be placed on account. For monthly/quarterly payments, or memberships, there is no refund of the prior or current period; a cancellation of the future months’ charges will be allowed. No refunds after 2nd meeting of class/program. Forever Young Programs – Service Fee will apply after the payment deadline, and refunds may be issued prior to the event as long as Supervisor approval is given. No refunds after event. Exceptions in which a Withdrawal Service Fee will not be assessed: Medical note exception is valid any time prior to the start of the last class and will not be subject to a ‘Service Fee’; a prorated refund or credit will be issued. Transferring to another program in lieu of receiving a refund. The credit available to transfer towards another program will be calculated on the prorated balance from the original registration. Leaving on account in lieu of receiving a refund. If funds are not used by the end of any calendar year, the credits will be returned less the applicable ‘Service Fee’. Refunds are not offered: if participant did not attend all or any portion of the class/program (forgot about class, moved and did not withdraw) for Summer Camp, ActivKids, or Preschool because participant’s vacation, illness, or any other absence for returned (new or used) uniforms or costumes for Enrollment, Administration, Registration, or Annual Fees; nor for Tickets for Punch Passes (these passes have no expiration date) includes Pool, ActivKids, Open Gymnastics, Open Baton for “Private” Recreational Packages: Personal Fitness Packages, Customized/Private Swim Lesson Packages, Private Voice, Private Gymnastics Lessons for trips – no refund unless customer provides a replacement. If replacement is found, Service Fee will still be assessed to the original participant for Adult Athletic Leagues –if a team chooses to drop out Additional Notes: No prorated registration, enrollment, or administration fee for late registrants.
Smoking Policy
The District’s philosophy is to promote health and a clean air environment. The use of any and all tobacco products is prohibited in all park district buildings and facilities and at all parks and trails. Carol Stream Park District joins other park districts in Illinois with a tobacco-free policy. To read the full policy, go to csparks.org.

Meet Your Park Board of Commissioners The Carol Stream Park District Board of Commissioners consists of seven individuals who are elected by residents to represent the community’s best interests. The charge of the Board is to set policies that guide the Park District. Board members serve a four-year term and receive no compensation for the time they give. They come from a variety of backgrounds and share one important trait ― a commitment to serving the park and recreational needs of our community.
Board Meetings
Regularly scheduled meetings of the Board are held on the second and/or the fourth Monday of each month according to a schedule adopted by the Board yearly. Please check our website for our current meeting schedule. Regular Board Meetings begin at 7pm at Fountain View Recreation Center, 910 N Gary Ave, Carol Stream.
