Changing times in medicine 2019 Dr. Pafford, geriatrician, opens new office in Bishop... he’s keeping his promise to our community By Christina Reed The Hired Pen
Eastern Sierra, CA-- “Our bodies change over time, and our health care needs to adapt with us. Geriatrics is the specialty focused on the high-quality, person-centered care we all need as we age. 'High-quality care' aims to improve health, independence, and quality of life for older people. 'Person-centered care' puts our personal values and preferences at the heart of our care decisions,� says the American Geriatrics Society (AGS). Geriatric medicine focuses on health care and medicine for elderly people, and it also promotes health by preventing and treating diseases and disabilities in older adults. Dr. Bennett Pafford, owner and doctor of Pafford Health, is one a handful of doctors, a geriatrician, whose specialty is health care adapted to aging minds and bodies. And, despite flatly untrue rumors circulating in the Eastern Sierra, Dr. Pafford is still in business, still making house calls to his patients, and is opening a new office in downtown Bishop, CA. Pafford Health's original business model is “Big City Medicine Small Town Compassion and Caring.� And, while the misinformation being given in Bishop is distressing to a trusting community, it has not prevented the important work Dr. Pafford and others who serve the veterans and elderly in our communities have come to expect to deliver to their patients and clients. A mix of Dr. Robert Denton (beloved Eastern Sierra country doctor), crossed with Dr. Nellie Doyle (one of California's first female doctors, who lived in the Eastern Sierra), Dr. Pafford has a diverse and very different path to his medical philosophy. Dr. Pafford is a son of a large cattle ranching family in Florida, and the rural life is a part of his heritage, and so are very active outdoor interests, like climbing, hiking, and running. Dr. Pafford is a retired Navy SEAL platoon commander (in 2000, following 20 years in the military), Navy Corpsman (a Marine medic, held in
high esteem in any branch of the military), and medical / public health specialist in remote war zones, such as Darfur, in the western region of the Sudan, in Africa. Pafford Health is a non-profit, 501 (C) (3) practice. Dr. Pafford is an internal medicine specialist with training at UC Davis, Sacramento, CA and residency training at George Washington University, Washington D. C.. He can treat all ages, but he is focuses on geriatrics (older adults) primarily, which Dr. Pafford says is “where there is the greatest need.� He adds, in remote regions, such as the Eastern Sierra, there is a greater urgency for “health literacy,� and using this knowledge base to prevent “avoidable deaths.� And, on the topic of pain management, Dr. Pafford says emphatically, he practices a “non-narcotic� based medicine, in regards to managing medical issues. He explains there are other medical approaches to pain management, which don't involve the community-destroying effects of prescription drug abuse. “The job as a physician is to not make you worse.� “We worked hard this last year, and invested a lot of time and money, building a clinic the community could be proud of. Finding out the building and owner was not going to honor the agreement to support our efforts was heartbreaking,� Dr. Pafford says. “It set us back, but we have a lot of people counting on us, and supporting us. One Doctor and Patient at a Time Keeping the geriatric patient in a one-doctor setting with appointments with the same consistent doctor, provides family, friends, and care givers the security of knowing one set of treatments or procedures, rather than the stress of multiple health care professionals' instructions, prescriptions, and procedures. There is a considerable amount of worry and stress involved when care givers must meet with multiple health care needs, along with multiple doctors, as well as maintaining their own health care needs. The American Association of Retired Persons', AARP's, article (February 19, 2019) talks about “When It's Time to See a Geriatrician.” The article's title includes, “And how they may keep you out of the hospital.” AARP, a US-based interest group / organization with 38 million members, continues, “About 30 percent of people over age 65 need one [geriatrician].” (Source: American Geriatrics Society) Geriatricians are experts at identifying and diagnosing signs of medical issues in our frail family members, and they are able to prescribe medications which might not be as risky to the elderly. AARP notes, “More than a third of adults over age 62 are taking at least five prescription drugs.” (Source: John Hopkins University School of Medicine study done in 2018, and published in the Journal of General Internal Medicine.) We will need more than 23,000 new geriatricians by 2030, says AGS, and regionally in rural counties (like Inyo, Mono, and Alpine Counties in the Eastern Sierra and western Nevada), with ever-increasing numbers of elders, we will need every geriatric specialist we can retain in the future. However, any good doctor needs the team work of physician assistants, pharmacists, nurses, veterans' services providers, social workers, public health and mental health providers. These are all in short supply in this region. That is why the community must insist that new providers are fully supported and welcomed. We cannot afford to lose even one. If it takes a village to raise a young person, it takes that same village to ensure the health and welfare of its elders....
Mono County resident dies of Hantavirus infection A Mono County resident recently died of hantavirus infection (North American hantavirus is also called Sin Nombre Virus). This is the first fatality caused by hantavirus this year in California, and the third confirmed hantavirus case in Mono County in 2019. This tragic loss is a reminder of the risk of hantavirus in the Sierra, a known region of exposure in the United States. People may catch hantavirus by inhaling virus that is shed in mouse waste (urine and feces) of infected deer mice. About one in three people with hantavirus infection die. One cannot catch this virus from another person. Exposure to hantavirus typically occurs indoors where mouse waste may have accumulated. Opening up seasonally closed cabins, homes, and outbuildings during late spring and summer poses our most common risk of infection, however, this case and a non-fatal one which occurred in January of this year demonstrate that infection can occur all year around. State and county health department experts have investigated the casepatient’s residence and place of employment, finding evidence of mice in and around the home. Exposure at the workplace in Inyo County was found to be unlikely because minimal signs of mice were found. Hantavirus illness begins with a fever and flu-like symptoms, such as headache and body aches, typically one to five weeks after inhaling the virus. Gastrointestinal symptoms such as vomiting, diarrhea and abdominal pain are common in the early part of the illness. Respiratory symptoms, including cough and feeling short of breath, are not present at first, but may develop after a few days, signaling increased chance of deadly respiratory and heart failure. When people start having trouble breathing their condition may rapidly worsen to become critical. There is no specific treatment for hantavirus infection, but high-level intensive care has allowed many people with life-threatening illness to survive. Diagnosis and timely transfer to higher level hospitals is challenging as the disease can progress rapidly before diagnostic testing can be completed. Most people who become ill with hantavirus report some exposure to rodents in the preceding weeks, typically while cleaning a room or a shed that has been closed-up for some time with mice living there. To decrease risk of hantavirus infection open windows and doors of a potentially contaminated area and allow it to air out for at least 30 minutes before cleaning. Sweeping, vacuuming or other activities that stir up dust and dirt that may contain mouse waste should be avoided. Spray rodent carcasses, nests, droppings, and other potentially contaminated items and surfaces with a 10% bleach solution (1 part bleach with 10 parts water) or a disinfectant made to kill viruses (check the label). Allow the solution to sit on the material for at least 5 minutes before wiping. Mice incursion in vehicles may also pose some hantavirus risk, especially if mice infest the heating and air conditioning system. Inspect vehicles for rodents. It is prudent to minimize mouse entry points at home and at work, although it may be challenging to completely mouse-proof some buildings. Mice may enter through very small gaps under doors or around windows and where conduits and vents pass through walls. Heating and air conditions ducts should be periodically inspected for holes. 5th Annual Winter Adventure Series in Mammoth Lakes announced
Eastern Sierra Interpretive Association (ESIA) is hosting the fifth annual Winter Adventure Series in Mammoth Lakes, January through February 2020. The Winter Adventure Series seeks to educate, motivate, and inspire attendees to explore the Eastern Sierra through presentations and films by professional athletes, photographers, filmmakers, scientists, and outdoor adventurists. The series is supported by The Town of Mammoth Lakes and Mammoth Lakes Recreation, and funding from Measure U. This year features a diverse collection of speakers, including a Sage-grouse photographer, local professional climbers, and exciting outdoor cinematographers. The series runs every Thursday night, January 16 through February 20, 2020 in the U.S.F.S. Auditorium at the Mammoth Lakes Visitor Center.

The Programming of Your Day Life is not like a crap game where you roll the dice and have to deal with the results of the roll. We determine how to respond to life’s quirks and turns long before we ever encounter them. We may have picked up those reactions from observing how our parents responded or some other example we registered, however we received it, we have adopted or adapted it to our response pattern, and it is a pattern. Some of those response patterns can be very destructive to the things you may hold dear. Each individual can change that pattern if they desire. I’ve known some people that have very little good to say about new or innovative ideas, events or things in general. They weren’t the best company to keep. On the other hand, I’ve known others that greeted new ideas and innovation with enthusiasm and encouragement. These people are a pleasure to be around. We are capable of programming our life. Actually, you are programming the subconscious mind. The subconscious already has your reaction pattern down pat. What we can do is supersede the old reaction pattern with a new and rejuvenating pattern. We open our consciousness to a whole different world of growth and wonder. This is spiritual programming, which is setting you in tune with the Divine Plan of the universe. We feed into our subconscious mind the thoughts and emotions that will result in a greater understanding of you and an acceptance of your capabilities. The whole idea is to live with wisdom and joy. In Religious Science we call this programming spiritual treatment. Others have called it Scientific Prayer. In treatment you state clearly what your desires are and visualize and accept them as already done. The following is a treatment written by Raymond Charles Barker for the purpose of setting his mind in concert with the Divine Plan for his life and experience. The action of the Infinite Mind is Life, and my life is that Life now. My consciousness is this action. My body is this action. All my affairs are this action, God’s action in me, through me, and as me is total action. It is always perfect and complete. It is always vital and productive. It is always healthy and abundant.
