Connections Case Management, through leadership and advocacy, shall promote and support quality comprehensive community services for persons with developmental disabilities and their families in Jasper County. The Jasper County Sheltered Facilities Board felt it necessary to offer consumers and families in Jasper County the opportunity to select county or state case management. As a result, on November 12, 1998 the Jasper County Sheltered Facilities Board entered into a contract with the Missouri Division of Mental Retardation and Developmental Disabilities to provide county-based case management. On December 7, 1998, Connections Case Management opened their doors and began providing Targeted Case Management to 149 persons. Since that day, we have grown to employ over 26 case managers. Each case manager has a caseload of 30-35 consumers. Connections Case Management operates as a team to provide the highest level of quality of case management services possible.
