There IS Help for Pain, Tingling, Swelling or Restlessness in Your Legs Venous disease is one of the most common conditions affecting our health. It is estimated that about 20% of adults may be suffering from venous disease. Although many people may have visible varicose veins, many others have no visible signs of the disease. Individuals with venous disease range from those with small vein disease (spider and reticular veins) to patients with large bulging varicose veins. When varicose veins are present, patients may experience swelling in the legs as well as skin changes such as discoloration, thickening of the skin, and in advanced cases, skin ulcers. Occasionally, large varicose veins may develop blood clots, a condition called superficial phlebitis. Patients without visible signs of venous disease may have symptoms such as leg pain (aching or cramping), leg heaviness or fatigue, swelling, throbbing, burning, itching or tenderness. Some people also experience restless legs.

Men and women of all ages can be affected by venous disease although women are affected more than men due to hormones. Venous disease has a very strong genetic component but there are certain risk factors that can also contribute to its development. These include obesity, pregnancy, prior trauma, surgery, lifestyle, and occupations that require prolonged standing. The treatment of venous disease has come very far in the past 10 years. Today we know much more about this medical problem and the diagnosis and treatment is far superior to that of times past. Venous disease can be easily diagnosed with the use of painless, noninvasive ultrasound performed in the office to determine if you have venous reflux. Venous reflux is the back flow of blood in the legs caused by weakened valves in the veins of the legs. If you experience any symptoms of the disease, you need to make sure that your physician is aware. Venous disease can be a serious medical condition, which left untreated, can lead to long standing complications. Treatment for venous disease may involve a procedure called EVLA which is a laser procedure done in the office to close down some of the larger superficial veins. Most patients also require sclerotherapy injections to shut down some of the smaller, more superficial veins of the legs. Venous disease occurs if these valves become damaged, allowing the backward flow of blood in the legs. When blood cannot be properly returned through the vein, it can pool in the legs, leading to a feeling of heaviness and fatigue—causing varicose veins and other skin changes. While usually not life-threatening, venous disorders still affect your circulatory system and can lead to more serious complications, such as ulcers, blood clots and/or stroke if left untreated. Spider veins are similar to varicose veins, but are usually fine and superficial. Spider veins are found closer to the skin’s surface and are often red or blue.

Where to Turn if you Develop Breast Cancer? By Lannette Johnston, MS,BSN,RN,iRNPA, CPST, CHC, Director of Outreach Programs of Tyrone Regional Health Network

Many people think that they should go to the Ivory Towers to seek the best advice when faced with a devastating diagnosis such as breast cancer. They look to some big name institution as if they have something more to offer. In reality, their experience will be limited primarily to the one individual provider with whom they have their initial encounter. This provider may or may not have much experience in dealing with breast cancer. Often, they are involved in some research pursuit to sustain their position at the institution and don’t have a wealth of every day clinical experience. At the Ivory Tower, your breast cancer case may be discussed at a tumor board. At this board, a brief synopsis of your medical condition will be presented by your provider. Your mammograms and any other imaging studies will be displayed. The pathology from your biopsies will be discussed. At the conclusion, your proposed therapy will be presented. Even if all members that are present do not agree with this therapy, your provider will still have the rubber stamp that proves your case was presented at a tumor board and therefore meets some higher standard of care. Sometimes, if your case does not meet the standard mold, suggestions will be made based on studies that have not been scientifically proven. Once again under the “authority” of the tumor board, novel therapies may be advised. Your provider will feel empowered by the board recommendations to promote these therapies. Unbeknownst to you, the patient, there may be no “proven” benefit. This is not to say that team work is not important in medicine. However, the most important part of the patient’s medical team is the leader of the team. In breast cancer treatment the leader of the team should be the initial provider whom you turned to. This leader should not abdicate the responsibility of your care. The leader should be fully versed on all aspects of breast cancer therapy, but more importantly, take the time to become thoroughly acquainted with your medical history and personal desires regarding therapy. Only when one provider takes responsibility as a leader in your care can you be sure that your best interest will be the primary goal. To put this in simple terms lets take the analogy of a crosswalk with pedestrian signals. You would think the walk and don’t walk indicators would make it safer. Unfortunately, because of these indicators it has been found that people often don’t look both ways and accidents January February 2015

occur. They put their trust in his signal instead of taking responsibility themselves. These crosswalks are just like medical teams without a leader. The members of the team all have some reliance on the others to make sure the therapy is provided correctly. No one member has the responsibility to make sure fine details and patient’s personal desires are addressed. A team without a leader is an accident waiting to happen. To maximize your health outcomes, find somebody you trust with a good reputation and the clinical experience to back it up. A leader for your cause who will make certain all aspects of your care are addressed including your personal desires. You may still be referred to other specialists for specific therapies but once all is said and done, you will have one medical leader to turn to if questions arise. There is no cookbook for breast cancer therapy because every patient has a unique set of problems and desires that a cookbook cannot provide. Unfortunately, today many providers are relying on cookbooks because they don’t have the experience to develop the fund of knowledge needed to make sound medical decisions. Or, they may have the knowledge but it is just easier to turn to the cookbook (traffic signal) and relinquish their responsibility. Three Sources of Income By Judy Loy, ChFC®, RICP® and CEO of Nestlerode & Loy, Inc. When planning for retirement, it is important to consider all sources of income. There are three sources of income throughout life: Social Capital, Human Capital and Financial Capital. Human Capital is our capacity to create wealth through our human endeavors—namely working for a salary. This is used primarily in our working years but many retirees choose to work for extra income, social interaction and a feeling of being involved and useful. Social Capital is typically used by retirees through Social Security, Medicare and other Federal ‘old-age benefits.’ Financial Capital is what we save that creates our Net Worth. Human Capital can be part of retirement income but our capability and often our desire to work lessens as we age. Sickness many times can take this away from us earlier than expected-either though our ill health or the illness of a loved one that needs our care. Since this is an unknown factor, it is important to plan the other two sources for retirement for maximum impact. Financial Capital is where a majority of my expertise lies. It is finding the best ways to save, the best investments to use and the ways to maximize the amount that people save or can pull in retirement. According to a survey by Vanguard in March 2014, the impact of appropriate diversification, pulling wealth from taxable and qualified accounts in the appropriate order and using total return investing can add 3 percentage points a year to investors’ returns and allow them to enjoy their wealth throughout retirement. Therefore, it is important to plan these decisions.

In planning out the third phase of your life (retirement), it is vital to make conscious decisions in each area of capital. The choice to work two more years, thus saving two more years and delaying social security two years, can sometimes make all the difference to allow for a comfortable retirement. Security has three dates that are very important: The age at which you are first eligible (currently 62), your full retirement age and age 70, which is the latest age you would want to claim your benefits. A majority of people are pulling social security at age 62, which can decrease their monthly checks for the rest of their lives. It is very important to try to maximize this benefit given your potential longevity and the lifetime of income it provides. To look at your social security benefits, go to their website at ssa.gov. Work with an advisor (many advisors, including myself, have analyzers that will help illustrate social security choices) or take the time yourself to understand the impact of different claiming ages and strategies.

GET TO KNOW YOUR BREASTS By Renee Quarterman, MD, FACS, Susquehanna Health Breast Health Center cycle. Examining just after your period is often easiest because your breasts are not as dense. If you don’t have a cycle, pick the same time each month. With routine exams you’ll learn what is normal for you. Promptly report these conditions to your healthcare provider: ■

Changes in the breast, such as lumps or masses, typically trigger cancer concerns. But did you know that only about 20 percent of lumps discovered turn out to be cancerous? Knowing your breasts well makes you an active participant in your breast health and guides your provider as they examine and evaluate you. Your own assessment can guide therapies and make your treatment plan easier. The breast is a complex structure made up of lobules, connective tissue, fat, lobes, ducts and lymph nodes. Breast self-exam (a regular, five minute visual and physical inspection) helps you learn your breasts’ unique makeup and characteristics both of which change cyclically and over time. A young woman typically has denser breasts with more hormonally active tissue than an older woman who has gone through menopause. Beginning breast selfexam in your 20s is ideal, but any time is a good time! If you have a cycle, examine your breasts at the same time in each

A distinctly different area of the breast A lump or thickening in the breast that does not change with your cycle A change in the size or shape of the breast A change in the texture or appearance of skin or nipple A discharge from the nipple

Imaging, biopsy, and/or monitoring are used to further evaluate changes in the breast. Many benign conditions require no treatment at all. These include: Cysts—fluid filled collections in the breast that may be deep or closer to the skin surface. Cysts may have the consistency of a water balloon or be a bit firmer. They often don’t require treatment but may be aspirated or drained if they become too large or painful. Breast cysts typically go away after menopause unless you are taking hormones Fibroadenoma—solid, noncancerous tumors that are most common in women under the age of 30. The tumor may change in size or even go away. They can be monitored, biopsied or completely removed depending on the patient’s symptoms and the doctor’s impression

Many lifestyle choices that promote general good health also benefit breast health. Choose a diet high in fresh fruits, vegetables, and fiber and low in fat and sugar. Exercise at least 20 minutes daily and maintain a healthy weight. Women who are overweight or obese have a higher risk of developing breast cancer as do women who smoke and/or drink more than four to five glasses of wine per week. Mammograms, starting at age 40, are an excellent screening tool that can detect small changes in your breasts before you can see or feel them. You are the best advocate for your breast health. Learning how your breasts change, understanding your risks for breast cancer, living a healthy lifestyle and receiving annual mammograms give you the best opportunity for detecting and treating any breast disease at its earliest stage. Successful women not only know they are not in the world alone, but also know the great value of offering appreciation and gratitude for those who have helped us. The gesture could be a quick note, a longer card, or a small token gift. How we express it isn’t as important as the fact that we take the time to appreciate others. Resolutions: January & February and Beyond By Lois Grose, President and Owner, Molly Maid of Centre, Mifflin and Juniata Counties Now that all the parties and family dinners are a distant memory. We tend to make New Year’s resolutions like be a better person, lose weight, declutter. At least they are my usual. This year, I am determined to declutter my office, home and life. So declutter the home is where we start. Closets are overflowing, pantries are low on staples (darn holidays!!!). Get out your To Do List and lets get started. Nothing feels better than checking off something on the list that is completed. Start with the pantry. You need the staples like tomato sauce, pastas (whole grain of course remember your other resolution to lose weight), flour, sugar, canned veggies etc. You can throw together a simple meal without having to stop at the grocery store. Remember if you start small, you will be successful. Check pantry off your list. Then move to the hall closet where all the winter coats are kept. If you don’t have hangers that match, get them. Really helps to keep things neat and they aren’t very expensive.

Continue your way through your home, one closet, one drawer at a time. My personal worst is the sock drawer. I always think I am going to find that one sock that is missing. Forget about it; pitch. So you are thinking, if I attempt to carry out my resolutions, how can I possible still clean my home? Well that is where the “Mollies” come in. Seniors Making a Splash: Healing Benefits of a Therapy Pool Aquatic therapy benefits individuals living with arthritis, Parkinson’s, after-effects from a stroke, depression, scoliosis, diabetes, and memory issues. Aquatic therapy helps stave off chronic diseases such as rheumatoid arthritis and fibromyalgia and is best accomplished in a specially designed pool, a therapy pool. (CDC) Swimming pools can be for more than just fun. If they are therapy pools they can provide healing and health benefits. Water therapy is an exercise program that is performed in the water. Aquatic therapy benefits individuals living with arthritis, Parkinson’s, after effects from a stroke, depression, scoliosis, diabetes, asthma and memory issues. It helps stave off chronic diseases such as rheumatoid arthritis and fibromyalgia. Water therapy can help a variety of people deal with limitations and improve quality of life. In waist deep water, body weight is reduced by approximately 50%, in neck deep water that increases to 90%. The buoyancy of water makes it much easier for painful arthritic older adults to move about. It reduces stress on joints and connective tissues. Therapy pools are warmer than regular swimming pools. Because heat enables muscles to relax and move better, it promotes an increased range of motion. Higher temperature combined with ease of movement will improve blood flow to painful and injured areas. This can be particularly helpful for seniors suffering from muscle spasms and back pain. The warm moist air helps those seniors who suffer from exercise induced asthma. In addition to buoyancy and warmth, the water also gives firm resistance to any object moving through it. Water offers 12-14% more resistance than exercising on land. This translates into a very efficient form of strengthening arms, legs and core muscles. Finally, hydrostatic pressure and jets promote circulation, resulting in decreased swelling in extremities January February 2015

and less muscle soreness from exercise. When you are almost completely submerged in water, blood circulation improves dramatically. If you are an adult-child caregiver to aging parents and loved ones or you are facing that possibility, help is out there! ACAPCOMMUNITY - what is it? ACAPcommunity is a non-profit organization that provides monthly educational programs to adult-child caregivers of aging parents. Founded in 2012 in Hickory NC, their goal is to provide information, resources, support and community for attendees. Exploration is underway on the possibility of establishing an ACAPcommunity chapter here in State College and your input is appreciated.

IS THERE A NEED? Adult-child caregivers are one of the most significant, yet most overlooked populations in our nation. There are an estimated 15-24 million adultchild caregivers in the United States today. If family caregivers were paid, the value of their services would be more than $450 billion annually. While two-thirds of the adult-child caregivers are daughters, onethird are sons. On average, adult children spend approximately 20 hours per week in care giving activities for aging parents. Those hours double if a parent has Alzheimer’s or some other type of dementia. Since the majority of adult children are employed, care giving means the January February 2015

adult-child works a “regular” job and then spends time as if she/he held an additional part-time job or possibly full-time job in caring for parents or in-laws. Without question, adult-child care giving exacts a toll on the caregiver and family, jeopardizing health, relationships and economic wellbeing.

TELL ME MORE ACAP programs are facilitated by local experts. The monthly program is approximately 90 minutes in length and includes time for the presentation, question and answer period, with fellowship at the end. The day and time of the meetings will be determined by the majority of interested attendees. All communication with and contact information of ACAP participants are confidential and will not be shared with others.

In a recent survey of ACAPHickory participants, 100% of caregivers tell us they have learned new information from attending an ACAP program. More than 90% indicated that they felt more confident about care giving, and 85% said they felt less anxious. In addition, 64% said they felt they were doing a better job of balancing work life with home responsibilities after attending ACAP. Caring for an aging parent is a wonderful way to say “thank you” to Mom and Dad. Accompanying our parents as they age can be a privilege and joy, but doing so sometimes brings challenges. The relationship between the aging parent and adult-child, honed over decades, is like no other, and the dynamics between the two are filled with history, memories and emotions as a result of typical parent-child interactions.

Are You Ready for RESPA? In 1974 the Real Estate Settlement Procedure Act (RESPA) was enacted to standardize the costs and procedures related to the sale of real estate. This was done to increase transparency and consistency in how transactions were done. It established standards that have driven the associated professions for decades. The administration and enforcement of RESPA was initially the responsibility of the Department of Housing and Urban Development. In 2011 the oversight of RESPA was transferred to the Consumer Financial Protection Bureau (CFPB) to address the need for further oversight as a result of the problems that led to the collapse of the housing market a few years ago. In 2015 there will be some changes in the process and settlement statement. The essence of buying a property and securing a mortgage will remain the same. What will change is the formatting of the settlement statement as well as the timelines involved in securing a mortgage. Expect to have to provide documentation of your financial status sooner, and you will receive the final HUD statement at least 3 days prior to settlement for review. If there needs to be changes in the HUD the timetable will reset, which can significantly delay a closing. The bottom line in 2015 is that you will need to have

your resources and documents in order. You will need to work more closely with your Realtor and lender from the beginning of the process. This will make for a smoother settlement by reducing last minutes adjustments. As 2014 winds down the statistics for Centre County Association of Realtors indicate that it was a solid year. The overall number of sales dropped a little for residential properties, but the average sale price increased slightly. The number of days on the market dropped, while the sale-to-list price ratio was above 95%*. Indicators for 2015 are that it will continue to be an improving real estate market in central PA, with prices and interest rates rising slightly by the middle of the year. Lewisburg Lewisburg is the county seat of Union County, home to a population of 5,733 last year. In the past it was a commercial center for a fertile grain and general farming region. A beautiful historic town, it is on the West Branch of the Susquehanna River and home to Bucknell University. Its 19th-century downtown was recently placed on the National Register of Historic Places. Be sure to check out the authentic single-screen 1921 movie theatre, The Campus Theatre. Take a self-guided tours of Lewisburg’s National Register Historic District, preferably as the streetlights turn on at dusk. Deaf Women Helps Others with Independent Living & Service Dogs

ALTOONA – Leslie Kelly lost part of her hearing at age 7, with total deafness at age 34. She has dedicated her life to helping others – as the Deaf and Hard-of-hearing Specialist at the Center for Independent Living of South Central PA and as the founder of Service Paws of Central Pennsylvania. At CIL, she helps those with hearing disabilities to achieve personal independence. Service Paws honors her past and present Hearing Dogs, Sara and Nokie, by raising money to help defray veterinary costs for others with service dogs from Susquehanna Service Dogs in Harrisburg. A service dog costs $5,000 from SSD to cover the initial training. The waiting list is six months to two years. Leslie also served as an advocate for Gov. Corbett, who signed House Bill 165 imposing a criminal penalty upon those who abuse service dogs.

A Change of Seasons The snow blew past my salon windows sideways while my heaters worked overtime to maintain a cozy temperature. All but one client had canceled. And while I was tempted to close, I thought if she needed her hair done bad enough to come out in this I would be available. Finally headlights pulled into my parking lot. When she opened the door the wind blew her in with a spray of wet snow. “I can’t make it until spring” she gasped as she took off her gloves. “The holidays are over and there’s not even so much as a birthday until spring. I’m going crazy, I need a diversion.” Well, tweaking your image is a great diversion for this time of year. Sit down, make yourself comfortable and we’ll talk, I said. As the outside grew colder and darker, inside, her hair became warmer and lighter. That evening we advanced her regular color formula to a multi-dimensional design. We added a sprinkle of strategic highlights while wrapping her shoulders with deep warm lowlights. Her cut simply needed to be readjusted to her head shape, and presto… winter blahs gone. As my client’s new look took shape she

began planning the rest of her transformation. The next day called for a make-up adjustment. Then a more colorful winter coat and scarf. Is Your Hearing Loss Associated With Other Health Issues? Much research has taken place recently to look at the relationship between hearing loss and other health issues. Studies have linked hearing loss with Cardiovascular Disease, Chronic Kidney Disease, Cognitive Decline and Dementia, Depression, Diabetes and Falling. A 2011 study found an association between Cardiovascular Disease and poorer hearing sensitivity. A 2010 study found that Chronic Kidney Disease also known as Chronic Renal Disease was associated with a 43% increased risk of hearing loss. A 2011 and 2013 study found that hearing loss was associated with accelerated cognitive decline and cognitive impairment in older adults. Hearing loss was associated with developing dementia. A 2014 study found that hearing loss was associated with depression, particularly in older women with moderate hearing loss. A 2012 study found that hearing loss was twice as common among people who had diabetes compared to those who did not. A 2012 study found for every 10dB increase in hearing loss, the risk of falling increased one and a half times. A 25dB hearing loss was associated with a nearly threefold increased risk of falling.

Stop Badmouthing Your Ex Sure, he’s a jerk, and he is making your life miserable, but badmouthing your ex will only cause problems and no good can come from it.

One of the factors a court in Pennsylvania is to consider is “the attempts of a parent to turn the child against the other parent.” If a court weighs this factor against you, you may very well lose primary physical custody of your children. Finally, just know that if he is badmouthing you, then something is probably going wrong in his life. It is very natural for a person to want to tear down someone else in order for themselves to feel better. If he sees that you are not badmouthing him, then that will bother him more since you are getting on with January February 2015
