I recall in the late 1990’s walking into Dan Nestlerode’s office and being introduced to one of the first portable digital music players The Rio, made by Diamond Multimedia. I wasn’t very impressed as it didn’t hold many songs, but it did portend the coming age of digital music. The age of Napster and Rio has passed but iTunes is still having an effect on the music industry. The first Apple music player, the iPod, was introduced in 2001 and Apple’s digital music store, iTunes, opened in 2003 and currently holds the title of biggest music vendor. Have you seen a stand-alone, bricks and mortar music store recently? They are few and far between. Album sales, which once were the backbone of the industry, are a thing of the past with singles leading the way. Now we have Pandora, the largest free internet customizable radio, and their many competitors providing free music streaming through the internet. With an iPhone, Sirius, Pandora and listeners’ other options, people can pick and choose the music they want to listen to and disregard those they don’t, which is even easier than turning a dial on the radio. With the introduction of the e-reader, a mobile electronic device to read digital e-books and periodicals, many pundits foretold the end of paper and hardback books. Although the first e-book was the Sony Librie, introduced in 2004, it was the Amazon Kindle that became a smash hit in 2007 selling out in five and a half hours. Amazon’s online bookstore and ebooks certainly put a large dent in bricks and mortar bookstores. Borders, an international book and music retailer, went Chapter 7 and ended its existence in 2011. Bookseller Barnes & Noble is also feeling the pinch from its electronic and internet competition. Its e-reader, the Nook, failed to compete with the existing e-readers and tablet computers, which run apps that allow for electronic reading. One area that is actually benefitting from electronics is audio books. Sales of audio books have moved up by double digits recently and audio books are a $1.2 Billion industry. In 2008, Amazon purchased Audible, an online digital audiobook retailer. Now, it has introduced Whispersync For Voice that permits a reader to switch not only among devices, but also from the audio version of a book to an e-book version and always leave off on either device where you stopped ‘reading.’ So you read a book in bed before you fall asleep and continue listening in the morning on the way to work in your car using your smartphone. The latest developments in entertainment can be seen in the stock price of Netflix (NFLX), with a price to earnings of 232 and a market price of $246.21 per share at the time of this writing. Netflix, Hulu and their competitors are changing the way viewers watch movies and TV. It started with DVRs (digital video recorders), which let people watch whatever program they like at whatever time they choose, without worrying about changing that VHS tape. I have to admit I don’t watch anything when it is actually on anymore and I recently subscribed to Netflix. I talked to a salesperson at an Apple store who had stopped his cable service and just used Netflix and Hulu for his TV viewing. Rather than be at the mercy of a broadcaster’s schedule, now people are viewing entire seasons of shows on weekends. This activity even has a name: binge viewing. Netflix took it one step further and created its own show with a big name star, House of Cards starring Kevin Spacey, with an entire season issued at once. Was it a success? House of Cards garnered nine Emmy nominations. As far as viewership, Netflix will not reveal the audience for their original shows. Will Netflix and internet viewing kill broadcast TV? Will paperbacks cease to exist? Or will it all just add more competition to a family’s downtime schedule? We know the latter is definite as far as what’s next for our entertainment dollars but whether paper books go the way of 8-tracks and cassettes is yet to be seen. What’s interesting is what will be next, reading Alice in Wonderland to your child while holographic images of the story are played out in front of you? It’s Time: Is Personal Care right for me? Standing Guard for Breast Cancer? Lymph nodes are often a topic of discussion when a cancer is discovered. They serve as a system of defense for our bodies from outside invaders. In the case of cancer, they can trap cells which can then be detected to help determine how aggressive the disease has become. The number of lymph nodes involved with cancer help determine the stage and prognosis for the patient. To understand lymph nodes you need to start with lymph. Lymph is a fluid which remains after the arteries deliver the nutrients to the tissues and the veins return the blood to the heart. At the tissue level lymph fluid is collected in tiny lymph vessels and through body motion is gradually propelled back to the larger lymph vessels. Eventually it is drained back into a large central vein where it is then circulated back in the blood stream. Since this lymph fluid is coming from the tissues, it has the potential to have bacteria and foreign substances which we do not wish to have in our blood stream. The lymph node has special cells, which to put it simply, filter and destroy these invaders by making antibodies and other special substances to protect us. This is not, however, the whole story with cancer cells. Let’s take the example of breast cancer and how lymph nodes become involved. In the average human axilla (arm pit) there are 55 lymph nodes. Each node receives a lymph vessel from a particular place in the body. As far as the axilla is concerned, this may be the hand, arm, shoulder, upper chest and the breast. There is an average of 15 to 20 nodes filter fluid which comes from the breast. The sentinel lymph node biopsy is a special procedure used to find the node which drains a specific site on the breast to begin the evaluation for cancer spread. The name sentinel, meaning guard, was chosen as if that node was defending the body from a particular place in the breast. It is found by injecting a radioactive protein or a dye around the cancer and then finding the node that lights up in the arm pit. If the node is negative then the axilla is considered clinically clear of cancer. If the node has cancer cells, depending of the size of the cancer in the sentinel node, additional lymph nodes may need to be evaluated. It is the number of nodes involved which determines the cancer stage. Unfortunately, the lymph node is not just acting like a filter in the case of cancer. The breast cancer cells are actually growing in the node as new nests of tumor. A single cancer cell in a lymph node is not considered positive and the body may eventually kill it. The nests of growing cancer cells are what present the concern. These nests mean that the particular cancer has cells which have special characteristics which let them grow with autonomy. The patient’s body does not recognize them as foreign and they can develop their own blood supply permitting them to grow and spread. So the sentinel node really is not much of a guard for the body with cancer. It is just one of the first places a cancer that has bad acting cells can set up shop. When found, these tumor nests in lymph nodes let us know how aggressive a cancer is to help plan therapy. The more lymph node disease present, the greater the chance of distant organ spread. This is unsurprising, as the study also identified several reasons why women continually experience such uncertainty about their bra size. A Fun Event for a Serious Cause the event that they wanted to hold one in Altoona, with proceeds going to the Joyce Murtha Breast Cancer Center. I went that first year… and I was hooked, I talked with the co-chairs; Juls Bratton and Dana Chestney, and volunteered to be on their committee for the next year. These incredible women work tirelessly, year round, for this signature event.
By Dianne M. Crust Researchers say that over 240,000 women in the United States will be diagnosed with breast cancer this year. That’s an alarming amount. Many women, when asked, say that it is one of their greatest fears. In my family alone, my mother and my oldest sister, Mom’s sister and her oldest daughter faced that fear, and have been diagnosed and treated for breast cancer. In 2008, after a routine annual exam, my doctor’s office called with the message “your doctor suggests that you see the breast cancer specialist, Dianna Craig at the Joyce Murtha Breast Cancer Center in Windber. You have a questionable mammoand she wants a specialist to see you and get your family history. If you stay on the line, we can make that appointment for you”… within 5 minutes, I had an appointment for the following week.

Successful Rehabilitation Relies on a Team Effort A few months ago, Centre County resident Gerry Santoro sustained multiple injuries from a motorcycle accident when a car pulled out in front of him in Pine Grove Mills. Broken bones included his left humerus, left wrist, right thumb, left ankle and left knee cap. After surgeries, and a stay at Altoona Regional Trauma Center, Gerry chose HealthSouth Nittany Valley Rehabilitation Hospital for inpatient rehabilitation. As a faculty member in Penn State's College of Information Sciences and Technology, Gerry’s main focus is on network and cyber security. He has an amateur observatory and a small organic farmette. Gerry also studies martial arts, and enjoys playing guitar and riding his HarleyDavidson. Gerry’s wife, Sue, describes him as “someone who likes to go above and beyond in life.” She adds, “He was determined to go to HealthSouth because he knew that he would get more therapy.” THREE HOURS OF THERAPY A DAY For over three weeks, Gerry worked hard in physical and occupational therapy. At HealthSouth, three hours of therapy a day are required. “In physical therapy, we worked on gaining strength and flexibility in my lower body,” says Gerry. Occupational Therapy Assistant Darla Dickson adds, “When Gerry came to HealthSouth, he only had the use of his right shoulder and his left hand. We focused on teaching him to use adaptive equipment so that he could accomplish goals like dressing himself.”

Why You Shouldn’t Clean Your Ears with a Cotton Swab Are you one of those people who like to use cotton swabs each morning to remove excess wax and debris? Anyone in the medical field can tell you, they have seen many catastrophes resulting from the use of cotton swabs. From punctured ear drums to super impacted wax, there are many negative consequences associated with "do-it-yourself" ear cleaning. There are a number of items that people use to clean their ears. Some of the most interesting seen in the doctor’s office are: Cotton Swabs (Q-Tips), Hair Pins, Tweezers, Pens and Pencils, Paper clips! (CAUTION!!! It is important to understand that NOTHING should be placed inside the ear to remove dirt and debris. This is a dangerous practice that could leave you with hearing loss.) The ear canal has specialized cells that produce ear wax. For some people, ear wax accumulates much faster than others. This can lead to wax build-up that causes decreased ability to hear and in some instances, pain and ringing in the ears. Some people resort to using swabs to remove excess wax to avoid seeing the physician. While this may seem like an excellent alternative to waiting in a doctor’s waiting room, using a cotton swab may do more harm than good. The ear drum is easily reached with a swab. Because the ear drum is so delicate, it can be easily ruptured by using even the gentlest of pressure when using a swab. Ask anyone who has experienced a punctured ear drum - it isn’t a pleasant experience. While a punctured ear drum will heal, it typically takes awhile and can even lead to a conductive hearing loss. In most cases, the ear canal does not need to be cleaned. During hair washing or showers, enough water enters into the ear canal to loosen the wax that has accumulated.
Additionally, the skin in your ear canal naturally grows in an outward, spiral pattern. Most of the time, the wax will loosen and fall out on its own while you are asleep. For those that have heavy wax build-up, a trip to the doctor may be needed. Doctors can easily remove with specialized tools or an irrigator that injects water into the ear. The process is virtually painless and is very effective in removing impacted wax. Never stick anything into your ear canal, including your own fingers. As a good rule of thumb, if you aren’t sure if what you are doing is safe, contact a health professional. If you or someone you know has hearing loss, visit your local audiologist to see what can be done for you or your loved one.

And That’s Why I Should Have Let Him Keep the House! When you get divorced, deciding what to do with the marital home can be an emotional issue. Keeping that house may not be a victory. Don’t let it become so. You want to keep the marital home. It is agreed you will have custody of the kids and you have the admirable goal of trying to give them a stable environment. After all, the divorce is hard enough on them. Why also burden them with a stressful move and uprooting their lives? You can afford the mortgage as long as you receive financial support from him through child and/or spousal support. Makes sense right? Not really. First, your house is an illiquid asset with no cash flow. Even if it has positive value, it may take years to realize if the house is not sold immediately. You may be better off taking liquid assets instead. Second, if you have a joint mortgage with your husband, it will not matter to your creditor who is responsible for the payment. If your ex-husband decides to withhold child and/or spousal support, or simply can’t afford it, the creditor can still come after both of you and your credit is impacted if the spouse responsible for the payment does not pay. Also, having your name on that debt may prevent you from getting a new mortgage. If you keep you may need to refinance to avoid the above situation and remove the other spouse from the mortgage debt. Refinancing will be done on your credit score. When you consider keeping the house, don’t forget to factor in the expense of fluctuating mortgage payments,
increases in property taxes, maintenance costs, replacing old systems (HVAC, hot water heaters, etc.), and utilities. This house may become a financial burden in a very short period of time. Consider selling or letting your ex keep the home and getting an alternative home that is less expensive. Consider whether this new home that has less maintenance. Kids do not necessarily have to change schools and all their friends can still live close by. Be pragmatic. Make sure you know whether you would be in better financial shape if you walk away from the marital home.

Seasonal Changes As the hot days of summer fade into fall it appears that we have experienced one of the best real estate markets in several years. It also appears that we have finally returned to a traditional market after a decade of false highs and the ensuing crash. That means that the bulk of real estate occurs between Easter and Labor Day. Buying and selling will continue throughout the year but the volume has already begun to taper off and will continue to do so until next spring. The year started off with a buyer’s market, then quickly went to a seller’s market during the “hot flash” of this late spring and summer as demand increased dramatically. It now appears to have returned to more of a buyer’s market. Demand has dropped so those properties that remain will have to compete for what is available. So what does this mean for buyers and sellers? Interest rates are still incredibly low despite a gradual increase over the past few months. This trend should continue for a while so it is still a good time to buy. Prices too have started to rebound, but once again we are at a very good level for buyers and it is a good time to purchase. For sellers there are fewer buyers in the market so you have to make sure your property is properly priced and move-in ready. The properties that will sell this fall will have something that sets them apart from the competition. Location cannot be changed but sellers will need to present their properties in the best conditions possible. This could
include features that improve the operating costs (i.e. more efficient appliances) or offers a “wow” factor (i.e. good quality floors and counters). First impressions will continue to be important so the appearance of the exterior and entrance can pull the buyer from a similar property that is not as well kept. Real estate continues to be an excellent financial investment, if you are prudent in your decisions. To improve the chances of success it is important to have accurate and current information. As always, having the support of a knowledgeable local Realtor is an important part of the equation when calculating whether it is the right time for you to buy or sell.
