Spring is finally here and so is tick season, meaning Lyme disease becomes a concern for all of us. The symptoms of Lyme disease can show up with a bang or progress very slowly, innocuously, and cause a great deal of confusion. Commonly the initial symptoms are flu like with fever, headache, nausea, muscle ache and a stiff neck. Many do not get these early symptoms and months later their first signs may be tingling/numbness, muscle weakness, tremors/tics, optic disturbances, memory deficiency, psychiatric presentations, or other neurologic disorders. In the nymph stage the ticks are as small as the period at the end of the sentence and very hard to detect. Therefore, many individuals who are infected write it off as flu unless they get the very classic bull’s-eye rash in an easy to find area. But the bull’s-eye rash only occurs or is found in about one third of the diagnosed cases. Many also believe that the tick needs to be attached for 1 to 2 days for the bacteria to be transmitted. However, as a person scratches, brushes up against things, or rolls over in sleep the tick can be compressed, causing it to expel its contents into the victim in less time. When Lyme disease is not identified or properly treated in the early stages symptoms may creep into one’s life over months, or even years. The symptoms may wax and wane even go into remission only to come out years later when the individual is under a great deal of metabolic stress. Negative lab results mean very little for some lab tests are extremely unreliable in the diagnosis of Lyme disease and other coinfections. Lyme patients are often finally diagnosed because of chronic symptoms such as arthritis, autoimmune disorders, fibromyalgia and chronic fatigue syndrome. Individuals may also suffer with thyroid symptoms that are not responding well to thyroid medication. Consider the number one cause of thyroid problems in the US today; Hashimoto’s Thyroiditis. It is an autoimmune condition occuring when your immune system attacks and destroys healthy tissue. A chronic infection such as Lyme disease is a common reason for the immune system to become overactive and attack the thyroid and other tissues; the list of associated disorders from this is nearly endless. Lyme disease detection is extremely difficult even in chronic cases. Commonly used antibody tests can miss up to two thirds of positively infected individuals. The ELISA test is useless in the first month after a tick bite, and may not detect past infections. The commonly relied upon Western blot test is nearly as ineffective, with only 44% sensitivity. In one study of children known to have Lyme disease the Western blot test was only able to identify 3 out of 10 cases. Personally, as a doctor, I take a very broad view of what Lyme disease actually is. It is traditionally defined as an acute infectious illness caused by the bacteria Borrelia burgdorferi, however, in its chronic form it involves so much more. In chronic Lyme disease other factors take on more significance, such as immune dysfunction, co-infections, metabolic and hormonal imbalances and auto immune responses such as Hashimoto’s thyroiditis. This is what makes the disorder so difficult for the general practitioner to understand and properly treat. Undiagnosed co-infections commonly involve the microscopic protozoan parasite Babesia or the Bartonella bacteria, both transmitted by biting insects, especially ticks. Other tick-borne diseases infecting both animals and humans are Anaplasmosis and Ehrlichiosis. It is important for individuals suffering with chronic pain, fatigue and neurological symptoms to see a doctor who has an in-depth understanding of chronic Lyme disease. These other infective agents should be considered and possibly tested for by utilizing laboratories specialized in their diagnosis. Want to know more, attend my FREE SEMINAR! On May 7, 1896, Dr. H.H. Holmes, one of America’s first well-known serialkillers, is hanged to death in Philadelphia. Although not as well known as Jack the Ripper, authorities discovered the remains of more than 200 victims on Holmes’ property. On May 6, 1937, the airship Hindenburg, the largest dirigible ever built and the pride of Nazi Germany, bursts into flames upon touching its mooring mast in Lakehurst, N.J., killing 36 passengers and crewmembers. The lighter-than-air craft was lifted by highly flammable hydrogen gas. On May 5, 1945, in Lakeview, Ore., Mrs. Elsie Mitchell and five neighborhood children are killed while attempting to drag a Japanese balloon out of the woods. Unbeknownst to Mitchell and the children, the balloon was armed, and it exploded soon after they began tampering with it.

Most people spend roughly a third of their lives sleeping. Except when they have a hard time sleeping, and many do. Sleep disorders are common, affecting 70 million Americans. The most often mentioned ones are insomnia, sleep apnea, restless leg syndrome and narcolepsy. For now we will address sleep apnea, one of the most common and under diagnosed sleep disorders. Sleep Apnea is a condition in which the tongue and other muscles of the airway relax and cause the airway to collapse. In some cases this can happen over 50 times per hour. Often, individuals are not even aware of the apnea occurring as they sleep. It may be the spouse that witnesses the snoring and gasping for air, nudging their partner to wake them concerned that they may die of suffocation. The chance of this happening is very remote. The brain recognizes when the airway collapses and arouses the person enough to open the airway and get the air they need. However, the effect of this repeated airway closure can eventually take its toll. Despite spending an adequate amount of time in bed, people with sleep apnea often feel tired. This is due to the repeated arousals that occur to keep the airway open during the night. The condition can eventually affect the heart and brain. People who have sleep apnea are at higher risk for cardiovascular diseases such as hypertension, atrial fibrillation, heart attack and heart failure. There is also a higher incidence of stroke and diabetes in the presence of sleep apnea. Chronic excessive daytime sleepiness can lead to problems with memory and concentration affecting your ability to make decisions. It can also impact relationships. When a person is constantly tired and needing to take naps, they do not have the time and

On May 15, 1963, Gordon Cooper is launched into space aboard Faith 7 on the longest American space mission to date. Cooper completed 22 orbits of the Earth and spent 34 hours in space. On May 13, 1981, Pope John Paul II is shot and wounded at St. Peter’s Square in Rome. Turkish terrorist Mehmet Ali Agca, an escaped fugitive already convicted of a previous murder, fired several shots at the religious leader. Agca was immediately captured. After a two-day trial, he was sentenced to life in prison. On May 10, 1990, the government of the People’s Republic of China announces that it is releasing 211 people arrested during the massive protests held in Tiananmen Square in Beijing in June 1989. Most observers viewed the prisoner release as an attempt by the communist government of China to dispel much of the terrible publicity it received for its brutal suppression of the 1989 protests. Many people think they could not sleep in a lab setting so they avoid going into a sleep clinic. That is why some clinics are now offering an at home sleep study option. Sleep specialists provide detailed instruction on how to use the device. The patient is then able to be studied in the familiar environment of their own bedroom. The home sleep study device primarily screens for sleep apnea and may not be adequate when other conditions are present, depending on disease severity.

Most people are able to adapt to CPAP and do fine with it. But it is important to get the specialized care necessary to help with adjustment to CPAP. Says Dr. Gensmer, board certified sleep physician at Northwind Lung Specialists & Sleep Center in Coon Rapids, “We are a small clinic and can give personal care that the large health centers simply are not set up to provide.” The communication within the specialized clinic is uniquely cohesive as the sleep health providers, sleep center and equipment department are all under one roof. “We really get to know the patients and their family since we are seeing them so often and we meet their individual needs.” Says Julie Dahl, Nurse Practitioner at Northwind. The primary objective for treatment of sleep apnea is keeping the airway open. Continuous positive airway pressure (CPAP) is the gold standard for this as it treats mild, moderate and severe sleep apnea. CPAP is a machine about the size of a shoebox with a hose that extends to a mask that is worn over the nose and pressurized air is blown into the airway preventing it from collapse. CPAP is pressurized air, not oxygen.

Tidbits is taking a look at some familiar folks born during the month of May. Follow along as we take a brief glimpse into their lives. • Sally Ride, the first American woman to go up in space, saw an ad in the newspaper seeking applicants for the space program and along with 8,000 others, answered the inquiry. At age 28, this young woman with a Ph.D. in physics joined NASA. In 1983, her first memorable flight was taken aboard the Space Shuttle Challenger. A second flight followed in 1984, and she was preparing for her third when the Challenger disaster occurred. She left NASA in 1987 and is currently on leave from her professorship of physics at the University of California and directorship of the California Space Institute. • May baby Priscilla Beaulieu (born Priscilla Wagner) was just 14 years old in 1959 when she was introduced to Elvis Presley in Germany. Her stepfather, a U.S. Air Force officer, was stationed there, as was Presley, and a young Air Force recruit introduced the pair. Elvis returned to the states the following spring but stayed in contact with Priscilla via phone for the next two years. When she turned 21, he married her at the Aladdin Hotel in Las Vegas. Six and a half years later, they were divorced.
