Everything You Need to Know

ABOUT REFLUX

Reflux – when babies bring up milk during or shortly after feeding – is pretty common. It isn’t usually a cause for concern and you don’t normally need to get medical advice if your baby seems otherwise happy and healthy, and is gaining weight appropriately. However, in some cases, babies suffer from extreme reflux – also known as GORD (Gastro-oesophageal reflux disease). This is a long-term form of reflux where the stomach contents rise up and irritate the child’s throat. It can be painful and is often misdiagnosed as colic.

WHAT CAUSES GORD? The muscle and valve that lead into the stomach are not working properly, which allows the feed, along with gastric acid, to be regurgitated causing the contents of the stomach to flow back up into the oesophagus. You can also have “silent reflux”, when the child is not actually sick.

Common symptoms DO NO include vomiting or spitting, T LAY YO arching the back, irritability, UR B A BY FLA frequent hiccups, gagging and T AFTER choking, raspy wheezy cough, very A FEED windy but problems burping, poor sleep patterns, excessive feeding or refusal to feed, bouts of inconsolable crying and white covering on the tongue (not oral thrush).

IS THERE A CURE?

No, but it can be controlled. Give your baby smaller, more frequent feeds. Keep baby’s body straight whilst feeding and hold your baby upright for a while after feeding - do not lay them flat on their backs after the feed. Prop the cot up whilst the baby is asleep. A feed thickener can be added to milk or expressed breast milk or a pre-thickened formula can be tried. Find out more about Reflux at www.nhs.uk/conditions/reflux-in-babies/

GETTING YOUR TODDLER TO

listen and cooperate Encouraging your toddler to listen and cooperate can sometimes be a challenge. So many parents tell me that their little ones have suddenly started saying “no” to everything, have difficulty sharing, have become picky eaters and throw tantrums when they don’t get their own way. It can be a tough time for parents, but these behaviours are developmentally normal and healthy. Toddlers begin to independently explore their own identities, separate from their parents. Pushing boundaries is their way of exploring how the world works. It’s also worth remembering that children of this age are going through a critical period of brain growth, with 90% per cent of the brain developing in the first three years of life and more than a million neural connections being produced each second. Additionally toddlers don’t yet have the ability to focus for long and need to practice these important attention skills. They are beginning to master language but their thinking is still very literal, making it hard for them to understand and process everything you say to them.

3 GIVE SIMPLE, CLEAR INSTRUCTIONS Use simple words, and keep instructions brief and to the point. For example, “Time to pack up your toys now.” Sometimes offer reminders in a single word, such as “teeth,” if you want them to brush their teeth. Pause for a few seconds, then ask them to repeat the instruction back to you to check their understanding. 4 USE POSITIVE LANGUAGE Instead of “Don’t run into the road,” use positive alternatives such as, “Walk on the pavement, please.” 5 USE WHEN/THEN PHRASING Make it clear what benefit or reward your toddler can expect in return for behaving and completing the task. For example, “When you’ve brushed your teeth, then I’ll read you a story.”

HERE ARE SOME TIPS FOR GETTING YOUR TODDLER TO LISTEN AND COOPERATE:

6 MAKE LISTENING FUN Put on silly voices, pretend to be their favourite superhero, or sing songs to encourage them to listen. Toddlers are more likely to cooperate if you change routine tasks, such as getting dressed, into familiar games that they have played before. For example, “Simon says, put your hands up,” (and then slip your child’s top on).

1 READ TO THEM Reading aloud is one of the best ways to improve your toddler’s listening skills and also helps strengthen your bond. Use silly voices or emphasise certain words or phrases in stories to grab your toddler’s attention.

7 PRAISE AND REWARD COOPERATION Show that you appreciate good listening skills. You could say, “I felt so pleased and happy to see you listening and getting quickly into your pyjamas. Thank you.”

2 GET THEIR FULL ATTENTION Minimise distractions by turning off the TV and putting your phone away. Get down to their level or pick them up so that you can look them in the eye. Speak in a calm, friendly tone of voice.

8 MODEL GOOD LISTENING SKILLS Toddlers learn a lot by observing you, so show that you are also listening to them by being attentive, making good eye contact and reflecting back what they say.

Controversy Corner

MORE THAN JUST

Fussy Eating

By Stephanie Elliot, the author of the young adult novel, Sad Perfect

As a parent, you’ve most likely experienced some sort of picky eating in your child or children. Toddlers are known for – and prone to – refusing foods that don’t taste good, or look funny, or touch another food on their plate. Some kids are just naturally picky, refusing green vegetables or things that look weird to them. But when does it become more than just picky eating? When do you need to worry that maybe your child isn’t getting all the nutrients he needs? My daughter was an extremely picky eater as a baby and toddler, and we were not initially concerned. We chalked it up to just that – picky eating. But, sandwiched in between two brothers who never refused any foods and excitedly ate lima beans and broccoli, we began to notice that our daughter’s aversion to many foods was becoming problematic. She would cry and scream, sitting at the dinner table, shaking her head back and forth, adamant that she would not be eating what the rest of the family ate. Her main diet as a young child consisted of very basic, plain and white foods, heavy on the carbs, very low on the protein and vegetables. She ate plain bagels and bread, fish fingers, French fries, pizza with the cheese and sauce scraped off, cereal, and waffles (but refused butter and syrup). She ate all sorts of sweets, and on occasion, I could coax her to have a peeled apple slice or a few raw carrots. Her only protein source was peanut butter and Carnation Instant milk. Does this diet sound familiar to you?

16 January - March 2019 Toddle About

Our daughter had ARFID, which is Avoidant/ Restrictive Food Intake Disorder. ARFID was first defined in the DSM-5 in 2013, and is an eating disorder and a mental disorder. Children and adults with ARFID refuse a large variety of foods because they fear that eating may cause them to choke or to die. Their fight or flight instincts kick in and they believe that their survival depends on them not eating a particular food. Many people with ARFID have experienced some sort of trauma that ignited their ARFID. Some of these traumas may include a choking incident as a baby or a traumatic birth experience (cord wrapped around the neck, or difficult delivery). Since ARFID is still so very new in the medical world, there is still much to be learned. In my family’s personal experience, we noticed my daughter eating differently at a very early age. As I mentioned, we thought it was just picky eating. What makes ARFID different from picky eating is the impact it has on a person’s mental state. For our daughter, she became extremely anxious around food and at any social event that revolved around eating. If you think about it, that’s almost every event imaginable to a young child. Food is everywhere – at sleepovers, birthday parties, school, weddings, funerals. Food is a celebration, and when our daughter would become agitated and anxious anytime we were around food, we realised the matter of her ‘picky eating’ was more serious. If we went out to dinner as a family, we had to choose a place that would have a ‘safe’ food for our daughter, usually French fries or pizza. If we went to a restaurant where there was no food she would eat, I would pack her a peanut butter sandwich and some raw carrots so she would have something to eat. The

icon tells you which advertisers have a voucher in the centre pages

Our daughter began to withdraw, refusing to go to friends’ homes for fear that there would be a food that she didn’t like or she would be forced to try something. It was embarrassing for her to be at a social event and ‘pretend’ to eat what was available. For her, it was easier to avoid (there’s that word!) doing social things rather than be faced with an uncomfortable eating situation. There are characteristics of ARFID that are different than other eating disorders. ARFID is an eating disorder that is not based on body image or self-esteem. While some children with ARFID may eventually show deficiencies in weight gain and have severe health issues, that was not the case with my daughter. She reached every milestone tracked by her pediatrician. When I would share my concerns with her doctors (and I did – many, many times), I was told that she was fine, she was healthy, she was just a ‘picky eater.’ Back then, the word ARFID didn’t exist, so we didn’t have a diagnosis nor get treatment for our daughter’s problem until she was a teenager.

Through intensive therapy, family support, and a terrific and understanding doctor, we were able to get our daughter the treatment she needed so that she could feel comfortable around foods that were not originally safe to her. It was a long road, but recovery is possible!

IF YOU ARE CONCERNED THAT YOUR CHILD MAY HAVE ARFID, ASK THESE QUESTIONS: • Does he only eat a small variety of foods? • Does she have rituals or strange/unique habits when eating the foods she is comfortable with? • Is he anxious around mealtimes? • Does she make up excuses in order to not eat (not hungry, tired, have a tummy ache?) • Does he refuse invitations to parties, sleepovers, or visits with friends? • Is dinnertime a battle? • Do you find that you’re making something different every night to appease her tastes? • Is there stress in the family dynamic because of these food issues? • Does he complain that he’s hungry but then doesn’t know what he wants to/can eat? • Do you think she is depressed? • Is his social circle becoming smaller? • Does she gag or vomit when and if she tries a new food?

Children with ARFID may show signs of depression and anxiety. They are depressed because they tend to isolate themselves from family, friends, and food. To them, it’s easier to stay away from the situations that cause them anxiety. This can become problematic in their school years when they are trying to make friends and establish relationships with other kids. In our situation, we felt we had to deal with our daughter’s anxiety and depression first and the food part could be fixed later.

The good thing today is that many clinicians, physicians, and psychologists are studying ARFID and learning how to treat it, whether it be through food exposure, therapy, nutrition education, or some other form of therapy. The more we talk about ARFID, the more familiar those around us will become with it, and it will no longer be a mysterious eating issue that your child has to suffer through. Each day, eating disorder specialists are learning more and more about ARFID and the techniques to treat it properly. If you suspect that your child may have ARFID, share your concerns with your doctor.G ge

CELEBRATE STORIES. LOVE READING. WORLD BOOK DAY - THURSDAY 7TH MARCH 2019 JOIN THE BIGGEST EVER BOOK CELEBRATION...

To celebrate World Book Day, we asked some Toddle About readers to review new releases of children’s books to find some amazing, and less well known gems that you might want to discover for yourself.

WHO’S HIDING?

BY ANA SEIXAS Reviewer: Rachel Manders This book is colourful, a good size for toddlers to hold, and has interactive sliding panels to reveal hidden animals. These are a lot more robust than cardboard flaps or doors, and my 18-month-old son has enjoyed engaging with them. I was disappointed that there was no story as such. It would have been nice if, alongside this use of repetition to reinforce language, there was a bit more of a story to tie it all together. Overall, though, it’s a nice book that my son has enjoyed.

FROGGY DAY

BY HEATHER PINDAR Reviewer: Alana Grocott Hop hop hurray for Froggy Day! What a delightfully funny book to start talking about the weather. Froggy Day takes children on a journey from the TV into a traffic jam, from the shops to the sea, and from the farm to the fairground. This book is full of colourful illustrations, as well as lots of fun sounds to make as you read together, and of course let’s not forget the frogs! Froggy Day is a must if your child is a fan of frogs, or the weather, or both.

20 January - March 2019 Toddle About

YOU MAKE ME HAPPY

BY SMRITI PRASADAMHALLS Reviewer: Alice McPherson You Make Me Happy is a powerfully simple book about love and sharing special times with the people you care about. Its simple but bright illustrations will appeal to babies as well as slightly older children. Older children will notice and appreciate the sentiment and adults reading it to younger little ones will enjoy passing the sentiment on! A beautifully told story full of possibilities for discussion. Embrace the love and share it with a special little someone!

WE EAT BANANAS BY KATIE ABEY

Reviewer: Felicity Harvey We Eat Bananas is a very colourful and engaging picture book! Great illustrations that help make it an interactive book for the child and parent to use together. This is more of an ‘eye-spy’ book rather than a story-telling book. But myself and my son had some giggles looking at the funny ways the animals ate their food! With a really obscure mixture of animals and foods, this is definitely something a bit different to your average book!

The

THE WONDROUS DINOSAURIUM BY JOHN CONDON

Reviewer: Gemma Callaghan The Wondrous Dinosaurium is a fantastically enjoyable read with brilliant illustrations and a unique storyline. Danny is determined to get himself a pet dinosaur, but he discovers that what you wish for and what you get are sometimes two different things. He takes us on a magical journey with a funny twist at the end. I would highly recommend this book for babies and toddlers the story is short and simple, keeping short attention spans entertained and in awe of the colourful drawings.

ALL ARE WELCOME

BY ALEXANDRA PENFOLD Reviewer: Tara Morris All Are Welcome is a book that promotes diversity and I would think it is aimed at school age children. The book talks about there being a place for everyone and that everyone is welcome. There are more pictures than words and it would be a useful tool in schools to facilitate group discussions as it easy to see and it creates a starting point to talk about diversity in our world. The use of words such as adversity and diversity would suggest to me that the book would need adult input to understand these words.

CELEBRATE STORIES. LOVE READING.

THINGS THAT GO

BY MATTHEW MORGAN Reviewer: Michelle Hayes The simple yet bold and colourful illustrations in this Things That Go book quickly engage young readers, however it is the sliding pages which really captivate - what starts as a single aeroplane becomes a full air display as you pull out the pages. The actual slide out pages are quite robust but it can sometimes be a bit tricky sliding them back in which has the potential to frustrate a little one. Each page has a different vehicle and the simple rhyming text make it suitable from a young age and the novelty will keep them entertained.

HOP LITTLE BUNNIES

BY MARTHA MUMFORD Reviewer: Suzanne Gaines At first sight the book made me smile, bringing back memories of playgroups past with the little ones singing this lovely sweet tune. The story is good for toddlers and when they are a little older, and they begin to read on their own. The story makes great use of all little animals children love and know. The drawings are fun, lively and vibrant with good sturdy flaps. It is a good size and weight to sit down with comfortably, and the children can use up any extra energy by jumping before going to bed! A great read for all the family to enjoy.

INTO THE FOREST

BY LAURA BAKER Reviewer: Lauren Cowley The minute my little boy laid eyes on this book all he wanted to do was read it. He really enjoyed pointing at the animals in the book, shouting their names and touching each different texture throughout. The rhymes in the book were appealing and short and sweet for little ears to hear and learn. The different page shapes enable you to see which animals are featured throughout the book with added frogs and butterflies along the way. It’s a lovely introduction to wild animals and their habitats.

HOW RUDE!

BY CLARE HELEN WELSH Reviewer: Ria Burch An appealing looking book which teaches children in a simple but effective way about manners and friendship. I would have liked more polite behaviour at the end but I did find the illustrations adorable! The facial expressions of the characters brilliantly captures the intended emotions, meaning a child can easily understand what is happening. My daughter particularly loved shouting, ‘How rude!’ This is a great book to share with a pre-schooler, for guaranteed giggles and conversation starters.

NOT YET A YETI

BY LOU TRELEAVEN & TONY NEAL Reviewer: Chloe Langley This book is beautifully illustrated and the wording on the blurb grabbed me straight away; “A story of courage, acceptance and finding yourself.” Upon opening the book, it is dedicated “To you, whoever you decide to be,” which I think is a lovely message to put across. The story itself is simple yet effective, with short and understandable sentences which were perfect for my nearly 2-year old daughter. She also loved the illustrations and I like that they became more colourful when George found himself and accepted that he was different.

NURI MEANS LIGHT BY JOY E TRICHE

Reviewer: Katie Skelton Nuri Means Light is a beautiful book about a little girl on her first day of preschool who learns about the diverse names of children in her class and the meanings behind them, finally discovering the meaning behind her own name. My four year old son really enjoyed this book. The pictures are bright and clear and the story itself is a nice length for preschool age children. It was refreshing to read a story with a diverse range of characters. It encouraged us to look up our own names and the meaning behind them which we really enjoyed.

HUNGRY BABIES

MARIE

BY ISABEL SANCHEZ VEGARA Reviewer: Sarah Hopkins We have all enjoyed reading this inspiring little biography about Marie Curie. It’s a wonderfully informative book, with simple text accompanied by beautiful illustrations, telling the wonderful story of Marie and her dream to become a scientist. I feel the book is best suited for 4-7 year olds, nonetheless my 2 ½ year old enjoyed the story. It’s a fantastic story to help our little ones learn about important people in history and inspire our scientists and engineers of the future. I hope there are more in this series.

BY FEARNE COTTON Reviewer: Vanessa Gjeta When I saw that the author of Hungry Babies was Fearne Cotton, I was intrigued. The first thing I liked about the book was the fact it is hard back with a very catchy, bright front cover. The illustrations are amazing and it is nice that the author has used children’s names from different nationalities. Everything flowed nicely with the words rhyming on each page. The story is about babies in all different eating scenarios and reminded me so much of my every day life with my son it made me smile reading it to my son.

ADORA BULL BY ALISON DONALD

Reviewer: Emily Short With my children, Elena (4 years) and Blake and Beatrix (nearly 2 years ), we gathered at bedtime to read this book and we loved it. It’s a lovely demonstration of friendship and a bull’s hi-jinks. Alfred the bull unfortunately thinks he’s losing his best friend Tom now he’s at school so Alfred does his very best to be adorable, but Tom has a bit of a surprise for Alfred. The pictures are lovely and detailed and allowed us to chat and elaborate this lovely little story that demonstrates a strong bond.

HOW TO IN T ROD U C E YO UR BABY T O

F Written by Sally Rowden, Nursery Nurse and Jo Fletcher, Clinical Team Lead,

0-19s Children’s Universal Services, Northants

Getting the correct advice before introducing solid food to your baby will not only help your little one grow up to be healthy, but it will also help you to enjoy this special time.

you w ill learn Responsive introduction to to how m gether solid food means following u how o ch and your baby’s lead when your ften yo ur baby is around 6 months old. baby w ants Just like feeding with milk, you to eat will learn together how much and how often your baby wants to eat. For some babies this will be a slow process of having little bits of finger foods to finally eating 3 meals a day, and for other babies they may really enjoy joining in with every family mealtime very quickly. Whether slow or fast to enjoy solids, by responding to your baby and enjoying the process together you start ith are helping them to enjoy their baby w y r u food, make healthy choices and o v a s food control their appetite. finger We suggest starting baby with savoury finger food that babies are able to pick up and put in their own mouths. This is a really great opportunity for your baby to practice their developing skills and to have fun. Your baby will learn skills in choosing the foods that you want them to continue to eat as they get older. Mealtimes are great opportunities to come together as a family, so building that sociable, relaxed time into baby’s day will also help her to enjoy those healthy foods – a chance for her to see, feel and taste new flavours and textures. And don’t forget - now baby is having solid food, it is important to offer water from a free flowing cup with meals to help her digest the food.

The most up-to-date research tells us that babies need only breast or formula milk up to the age of 6 months. During the following few months, you can relax and really enjoy the experience of introducing solids, safe in the knowledge that baby’s nutritional needs are still met by their milk intake. You may recall health visitors and feeding specialists talking e about ‘responsive’ feeding iv s n Respo eans - the style of feeding m g in feed by promoted by our teams a b e h that t od and around the world to red fo e f f o is get children off to the best y are e h t n whe start. Responsive feeding y, hungr means that the baby is offered food when they are hungry, and that they join in with the experience of controlling their appetite. The benefits of this are huge - your baby will feed the right amount to gain weight according to their own individual needs; they will learn to feed to the capacity of their tummy; and they’ll enjoy feeding times as baby and carer tune into one another. Just like adults’ your b aby will fe appetites vary from person ed the right a to person, so do babies’. m to gain ount Grownups simply cannot weigh a ccordin t understand how much or g own in to their how frequently our little dividu ones want to eat without needs al their help.

Sorry

Seems to be the Hardest Word…

How many times do we ask our children to say they are sorry? How many times do we ‘make’ children apologise for something they have said or done?

But what about adults?

I’m a parent – we’re only human! We have bad days when we are less tolerant than usual or even a tad grumpy! However, if we can hold our hands up and say sorry to our children, then they can learn from us.

As a parent and a teacher, I always ‘ask’ children to say sorry, although I insist on taking it a step further. I ask children to be specific. Saying sorry is easy and doesn’t really mean anything if it’s not linked to what you are sorry for.

Your apology might sound a bit like this:

When children are specific with their apology, they are actually thinking about what they have done and whether it was a good choice.

“I’m sorry, I didn’t mean to sound grumpy, I’m not feeling well today and I’m having a bad day. I shouldn’t have reacted that way towards you. I’m sorry.”

So, for example, “I’m sorry for hitting Billy with my pencil case.”

When we say sorry for our behaviour and can admit that we are wrong, children learn empathy, it builds a bond of trust and respect within the relationship and it also helps to de-escalate the situation. When we apologise, our children see this as us being fair and that we are trying to understand and listen to them.

But what about adults? Should we apologise to our children when we get it wrong? It’s funny how hard it can be to say sorry when you’re the parent. As adults, we often feel that our children should do as they’re told – without question. And we really don’t like to admit when we’re wrong. We might feel that backing down and compromising will send the wrong message to our children - it might reduce our authority, or let them feel that “they have got away with it”.

Children also learn that when they have moments where their emotions are ‘out of control,’ that it is normal and, in fact, adults experience similar issues and have learnt how to manage their emotions. They see that they can do the same. As an adult, saying sorry benefits everyone. It helps to develop children’s social development and enhances their ability to self-regulate their behaviour. It demonstrates the need to consider other people’s feelings.

However, I believe we should ALWAYS be prepared to apologise for our behaviour. And we should say sorry specifically for something we have said or done, in exactly the same way our children should. We know that children learn from their experiences and from the people around them. So, if what they see around them in their world are good role models who are saying sorry and admitting when they are wrong, then that is the behaviour that they will emulate.

So, next time you find yourself in conflict with your child, think,

“Am I in the wrong?”, And remember the power of saying sorry…
