Welcome to the June issue of Cosmetic News. Last month saw the third annual Cosmetic News Expo take place at the Business Design Centre in Islington, and what a fantastic two days it was! With more than 800 visitors and a packed educational programme, the show has gone from strength to strength and we have had some phenomenal feedback from visitors and exhibitors alike. This guy is the most unlikely candidate for skincare you can imagine. A used car salesman, and avid sun worshiper, his idea of skincare is washing his ‘boat race’ in the shower. But as I knew he was worried about his eyes (and was even considering surgery) I thought he would be the perfect person to try it out, and I was not wrong! A few days after using the product I got a call to say he could not believe the difference and when I saw him a week later neither could I. Now three weeks in the difference is really noticeable. The skin around his eyes is smoother and less wrinkled and the bags have reduced. He is delighted! My second happy customer came after the Cosmetic News Expo when I procured some more of the product for a female friend of mine who is approaching 50 and is also a sun worshiper and regular sunbed user (tut tut). A few days later I get a text from her saying “OMG you need to see my face, I have been using that NeoStrata® sample you gave me’” I thought something was wrong but she replied saying it was “nothing short of miraculous” and she literally had ‘neo strata’ (new skin). With these kind of rave reviews I thought it would be pertinent to chose the product as my editor’s choice this month even though I have not used it myself.

Death of Baroness is tragic loss for industry

New Research Suggests PIP Implant Failures Significantly Higher Than Previously Thought New research published in the Journal of Plastic, Reconstructive and Aesthetic Surgery suggests that the failure rate of silicone breast implants manufactured by Poly Implant Prosthèse (PIP) could be as high as 33.8%. Patients who received PIP implants either for primary or secondary breast augmentation, between January 2000 and August 2005, were studied to determine implant failure rate and other complications. Results found that seven to 12 years after implantation the failure rate for PIP implants is in the region of 15.9–33.8%. “Previous studies have typically reported failure rates of between 2% and 5%and these estimates

The chair of the Treatments You Can Trust Governance Group, Baroness Ritchie of Brompton, has sadly passed away at the age of 66 after a private battle with illness.

 The Baroness, a formidable figure in local and national government, was a driving force behind the register which was set up to improve patient safety. She died on April 24.

A long-serving Kensington and Chelsea Councillor, Baroness Ritchie also masterminded David Cameron’s campaign for more women Conservative MPs and was chairman of the LGA’s Children and Young People Board for two years.

In this study, all participants were scanned by ultrasound, which provides conclusive indication of rupture, rather than clinical examination which is typically used. Most previous studies were based on multiple surgeon, multiple technique, with a relatively small number of patients and shorter follow up time span”, commented Mr Jan Stanek, lead author of the study. Concerns about the durability of PIP silicone breast implants have been expressed for several years prior to their formal withdrawal from the UK market in March 2010. Although precise details of what elements were at fault remain unclear, concerns have been raised about both the elastomer (the outer cover of the implant) and the filler gel. In this study PIP implants were used by the plastic surgeon for both primary and secondary breast augmentation. A database of patients was constructed of each patient offering a free consultation and referral for ultrasound scan. Chief outcome measures included secondary surgery, implant rupture rate and time to rupture. 453 patients with PIP devices were identified. Of this number, 30 had already undergone implant exchange for a variety of reasons. 180 (39.7%) could not be contacted and 19 had undergone implant removal elsewhere. Of those who could be contacted, 47 declined consultation as they had no concerns. 97 had neither clinical signs nor radiographic evidence of implant rupture and elected to remain under regular review. At the time of writing, 38 had undergone implant exchange after ultrasonographic indication of rupture. Based on these results, if the group of non-responders had no ruptures, the overall rupture rate would be 15.9%. However, it the non-responders had the same rupture rate as those examined in the study, the overall rupture rate would be 33.8%. Mr Stanek indicates that all PIP implants, due to the high rupture rate and uncertainty about the nature of the silicone gel, may need to be removed. Those patients with ruptured implants will have to have them removed; those with no evidence of rupture will need to be monitored on a regular basis. Further research into the nature of the elastomer and gel filler will determine whether all PIP implants should be explanted in the future.

Cosmetic News goes to Bootcamp Cosmetic News publisher Charlotte Body and editor Vicky Eldridge recently spent a week at the No 1 Bootcamp in Norfolk in a bid to shape up for the summer. The Bootcamp is a favourite with celebrities including the cast of The Only Way is Essex and model Sophie Anderton. Charlotte and Vicky were put through their paces with a greulling military style exercise regime and 1,200 calorie a day diet and lost 10lbs and 8lbs respectively.

PIAPA meeting, Manchester PIAPA’s latest meeting held at Etrop Grange Hotel in Manchester was yet another success. Dr Raj Acquilla and Miss Jonquille Chantrey presented a Practical Masterclass with a Whole Face Approach. Cadaver slides were reinforced by marking up the important anatomical markers on the live models before asystematic assessment of three models needs were discussed. The models where then treated with a range of Allergan products from Voluma to Hydrate. Finally Dr Acquilla met with members and took individual questions. PIAPA is a membership group for aesthetic practitioners across the North of England, which offers help, support, education and training to members. 10

Bocouture® 50 Abbreviated Prescribing Information Please refer to the Summary of Product Characteristics (SmPC). Presentation 50 LD50 units of Botulinum toxin type A (150 kD), free from complexing proteins as a powder for solution for injection. Indications Temporary improvement in the appearance of moderate to severe vertical lines between the eyebrows seen at frown (glabellar frown lines) in adults under 65 years of age when the severity of these lines has an important psychological impact for the patient. Dosage and administration Unit doses recommended for Bocouture are not interchangeable with those for other preparations of Botulinum toxin. Reconstitute with 0.9% sodium chloride. Intramuscular injection (50 units/1.25 ml). Standard dosing is 20 units; 0.1 ml (4 units): 2 injections in each corrugator muscle and 1x procerus muscle. May be increased to up to 30 units. Not recommended for use in patients over 65 years or under 18 years. Injections near the levator palpebrae superioris and into the cranial portion of the orbicularis oculi should be avoided. Contraindications Hypersensitivity to Botulinum neurotoxin type A or to any of the excipients. Generalised disorders of muscle activity (e.g. myasthenia gravis, Lambert-Eaton syndrome). Presence of infection or inflammation at the proposed injection site. Special warnings and precautions Should not be injected into a blood vessel. Not recommended for patients with a history of dysphagia and aspiration. Adrenaline and other medical aids for treating anaphylaxis should be available. Caution in patients receiving anticoagulant therapy or taking other substances in anticoagulant doses. Caution in patients suffering from amyotrophic lateral sclerosis or other diseases which result in peripheral neuromuscular dysfunction. Too frequent or too high dosing of Botulinum toxin type A may increase the risk of antibodies forming. Should not be used during pregnancy unless clearly necessary. Interactions Concomitant use with aminoglycosides or spectinomycin requires special care. Peripheral muscle relaxants should be used with caution. 4-aminoquinolines may reduce the effect. Undesirable effects Usually observed within the first week after treatment. Localised muscle weakness, blepharoptosis, localised pain, tenderness, itching, swelling and/or haematoma can occur in conjunction with the injection. Temporary vasovagal reactions associated with pre-injection anxiety, such as syncope, circulatory problems, nausea or tinnitus, may occur. Frequency defined as follows: very common (≥ 1/10); common (≥ 1/100, < 1/10); uncommon (≥ 1/1000, < 1/100); rare

(≥ 1/10,000, < 1/1000); very rare (< 1/10,000). Infections and infestations; Uncommon: bronchitis, nasopharyngitis, influenza infection. Psychiatric disorders; Uncommon: depression, insomnia. We celebrate the success of the third annual Cosmetic News Expo and give you a round up of what happened

April 29 and 30 saw the UK’s largest aesthetic meeting, the Cosmetic News Expo, take place at the Business Design Centre in Islington, London. The show was a huge success with more than 70 exhibitors and 800 visitors across the two days.

he Cosmetic News Expo is the UK’s only free of charge trade exhibition for the aesthetics industry, offering aesthetic professionals the chance to find out about the latest technologies and products on the market and network with key manufacturers and suppliers, all under one roof. Since much of Energist’s business is currently overseas, the Expo no doubt helped to generate interest from UK clinics.

FACE Ltd | Face Ltd is celebrating its 10th anniversary this year and was promoting its upcoming FACE meeting and Aesthetic Industry Summer Ball. This year’s FACE conference will take place at the Royal College of Physicians on Friday June 15-Sunday June 17. The Aesthetic Industry Ball will take place on the evening of Saturday June 16 and will be held at London’s Madame Tussauds.


Azzalure® Abbreviated Prescribing Information Presentation: Botulinum toxin type A (Clostridium botulinum toxin A haemagglutinin complex) 10 Speywood units/0.05ml of reconstituted solution (powder for solution for injection). Indications: Temporary improvement in appearance of moderate to severe glabellar lines seen at frown, in adult patients under 65 years, when severity of these lines has an important psychological impact on the patient. Dosage & Administration: Botulinum toxin units are different depending on the medicinal products. Speywood units are specific to this preparation and are not interchangeable with other botulinum toxins. Reconstitute prior to injection. Intramuscular injections should be performed at right angles to the skin using a sterile 29-30 gauge needle. Recommended dose is 50 Speywood units (0.25 ml of reconstituted solution) divided equally into 5 injection sites,: 2 injections into each corrugator muscle and one into the procerus muscle near the nasofrontal angle. (See summary of product characteristics for full technique). Treatment interval should not be more frequent than every three months. Not recommended for use in individuals under 18 years of age. Contraindications: In individuals with hypersensitivity to botulinum toxin A or to any of the excipients. In the presence of infection at the proposed injection sites, myasthenia gravis, Eaton Lambert Syndrome or Amyotrophic lateral sclerosis. Special warnings and precautions for use: Use with caution in patients with a risk of, or clinical evidence of, marked defective neuro-muscular transmission, in the presence of inflammation at the proposed injection site(s) or when the targeted muscle shows excessive weakness or atrophy. Patients treated with therapeutic doses may experience exaggerated muscle weakness. Not recommended in patients with history of dysphagia, aspiration or with prolonged bleeding time. Seek immediate medical care if swallowing, speech or respiratory difficulties arise. Facial asymmetry, ptosis, excessive dermatochalasis, scarring and any alterations to facial anatomy, as a result of previous surgical interventions should be taken into consideration prior to injection. Injections at more frequent intervals/higher doses can increase the risk of antibody formation. Avoid administering different botulinum neurotoxins during the course of treatment with Azzalure. To be used for one single patient treatment only during a single session. Interactions: Concomitant treatment with aminoglycosides or other agents interfering with neuromuscular transmission (e.g. curare-like agents) may potentiate effect of botulinum toxin. Pregnancy & Lactation: Not to be used during pregnancy or lactation. Side Effects: Most frequently occurring related reactions are headache and injection site reactions. Generally treatment/injection technique related reactions occur within first week following injection and are transient and of mild to moderate severity and reversible. Very Common (≥ 1/10): Headache, Injection site reactions (e.g. erythema, oedema, irritation, rash, pruritus, paraesthesia, pain, discomfort, stinging and bruising). Common (≥ 1/100 to < 1/10): Facial paresis (predominantly describes brow paresis), Asthenopia, Ptosis, Eyelid oedema, Lacrimation increase, Dry eye, Muscle twitching (twitching of muscles around the eyes). Uncommon (≥ 1/1,000 to <1/100): Dizziness, Visual disturbances, Vision blurred, Diplopia, Pruritus, Rash, Hypersensitivity. Rare (≥ 1/10,000 to < 1/1,000): Eye movement disorder, Urticaria. Adverse effects resulting from distribution of the effects of the toxin to sites remote from the site of injection have been very rarely reported with botulinum toxin (excessive muscle weakness, dysphagia, aspiration pneumonia with fatal outcome in some cases). fter a busy and successful first day at the Cosmetic News Expo 2012, speakers, exhibitors, delegates and VIP guests joined the Cosmetic News team and Hospitality Sponsors, BioTherapuetic, for the after show party.

The party was hosted at the Pitcher and Piano in Islington and had an Olympic theme to mark the 2012 Olympics coming to London this year. The trendy location is just a stones throw from the Business Design Centre, where the two-day Cosmetic News Expo took place. Guests were greeted with a complementary cocktail, bubbly, wine and beer before tucking into a delicious selection of British themed food including mini fish and chips and mini bangers and mash. More than 300 people attended the party, which is always a highlight of the event and a fantastic opportunity to network and for everyone to let their hair down after the show. One person who was not there to enjoy the fun was Cosmetic News editor Vicky Eldridge who was stuck in bed at the hotel with food poisoning. She said, “I am a sorry I didn’t get a chance to celebrate with everyone at the party. It is always one of the highlights of the year for me as it gives me a chance to catch up and socialize with everyone after all the hard work that goes into the Expo. I was very upset to miss it.”

T H E HA I R AN D N OW Hair removal is big business for aesthetic clinics. We examine the latest treatment options

Unwanted hair is a huge problem for many men and women and as such hair removal has become on of the top things on the agenda of clients visiting beauty salons and aesthetics clinics. Excess hair may be the result of medical conditions, but there is also a huge social pressure, particularly for women to be ‘hair free’. The psychological trauma caused by excessive or unexpected hair growth is often strong enough to motivate patients, especially women, to seek treatment. With many people dissatisfied with traditional methods of treating unwanted hair such as shaving, waxing and tweezing which can not only be costly over time but are also cumbersome and have a short term effect, the prospect of more permanent treatments, such as laser or IPL hair removal and electrolysis has been an attractive one. L ight based hair removal Offering quick, effective and permanent hair ‘reduction’ on a wide range of body areas including the back, cheeks, chin, upper lip, shoulder, under arms, legs and arms, light-based treatments have become an increasingly popular choice with consumers in recent years. When it comes to laser and light based hair removal treatments today it is all about providing less painful or even ‘pain free’ hair removal in a shorter amount of sessions with longer lasting and more permanent results. The laser and IPL market is seeing innovations all the time with more and more systems now also being approved for use on darker skin types. However, light based hair removal is still most effective when the hair colour being targeted is darker than the skin colour, due to the absorption of light by the melanin or pigmentation, which acts as the chromophore or target of the light. Treatment is therefore most effective on patients who have black or brown hair and pale skin with darker skin types as well as blonde, white and red hair colours more difficult to treat and in some cases contraindicated. There is no such thing as completely permanent hair removal with any machine or any type of treatment however an ever-increasing number of published studies have confirmed the long-term efficacy of laser and IPL treatments. Each person’s hair growth pattern is different and is affected by many known and unknown factors, mostly hormonal influences. Laser and IPL will cause a dramatic and permanent hair reduction, with remaining hair re-growth becoming finer and lighter. Most people who complete a course of treatments will find they get an extremely effective hair reduction during the course and very little

hair re-growth following. Some people will actually get complete hair reduction with no re-growth and others will require regular maintenance treatments once a year or so.

E lectrolysis Despite the advances in laser and IPL hair removal, electrolysis is still the only hair removal treatment that can claim to be ‘permanent’. Electrolysis removes unwanted hair through the means of a probe or needle being inserted into the hair follicle. There are three different methods available on the market: galvanic electrolysis; thermolysis/short wave diathermy electrolysis and blend electrolysis. The aim of all the methods is to damage the base of the growing hair in order to cut off the blood supply and nutrition to the hair, preventing any new growth from developing. The technique is selective and no minimal damage should occur to surrounding tissues. Although electrolysis does not offer the speed associated with LASER and IPL treatments, it can be used for many indications that LASER and IPL cannot, such as lighter hairs or on darker skin types and, as such, it is increasingly being used in conjunction with LASER and IPL treatments to enhance treatment outcome. Elaine Stoddart from Sterex explains, “LASER and IPL with their capability of treating large areas has resulted in electrolysis now being utilised to complete these treatments and ‘finish the job’. Often white hairs remain after LASER or IPL and electrologists can successfully remove these where LASER and IPL cannot. Electrolysis and LASER or IPL therefore work well together for the benefit of the client, offering every skin and hair type and colour the ‘holy grail’ of permanent hair removal.” On the following pages we round up 10 of the best laser and IPL hair removal devices on the market...

T he ‘ O lympic ’ G old S tandard L aser – M otif ( S yneron C andela ) The latest in ‘pain-free’ hair removal, the Motif (eLase) from Syneron Candela is so effective that it has the accolade of sponsoring the GBR synchronised swimming team for the London 2012 Olympics. The new laser combines radio-frequency with laser diode light and is powered by the company’s state-of-the art elos® technology to offer deep dermal penetration and longer lasting results. Because Motif harnesses RF to boost the light energy, it is highly effective, extremely fast and very comfortable compared to traditional laser hair removal. It is also safer as it uses less optical energy. Although RF is non-selective it has two advantages, according to James Bartholomeuz, VP of Global product design at Syneron Candela. “RF non-selectively heats tissue volume quickly and effectively gives accumulated heat”, he explains. “RF is more predictable than light with 100% flowing into tissue. The RF energy can also be used to monitor the impedance (resistance) in skin and so allow us to give a good feedback about the delivery. RF can be measured in j/cm3 so typically with Motif 50j\cm3 is delivered equivalent to 40j/cm2/second – this is on top of the light energy delivered.” This use of RF also makes the treatment less painful as it builds up an accumulative heat and more directly heats the hair follicle rather than the tissue surrounding it. The Motif can be used to treat all skin types and all hair colours, including fair facial hair, in as little as four treatments and, as it also utilises a fast acting Contact Cooling Device, the treatment is virtually pain free.

B reast practice We explore the recent decision by Q-Med to discontinue the breast indication for Macrolane™ and answer some of your concerns about why

On April 25, the MHRA issued a Medical Device Alert with regards to Q-Med’s dermal filler for breast augmentation, Macrolane™ Volume Restoration Factor (VRF). The alert followed a letter that was sent to practitioners by Q-Med (a Galderma Division) on April 17 recommending that Macrolane VRF20 and Macrolane VRF30 were no longer used for injection into the breast because it could make diagnosis of breast cancer more difficult, particularly during mammography. The decision was taken due to the lack of consensus amongst radiologists regarding best practices in the examination of breasts treated with Macrolane. After reviewing the current situation, and in consultation with the relevant authorities, Q-Med, decided to stop promoting the use of Macrolane in breast enhancement until consensus has been reached regarding best practices in breast radiology examination following Macrolane treatment. In response to this the MHRA issued the following advice and called for action to be taken by breast surgeons, cosmetic surgeons, GPs, mammographers, plastic surgeons, radiographers and radiologists: • •

Do not use Macrolane for breast augmentation. Have procedures in place for radiographers to check with patients presenting for breast examination for any history of breast injection. Staff should request the card that the patient was given at the time of injection, identifying Macrolane as the product used, and pass it on to the examining radiologist. Women may present with concerns that the injected Macrolane has affected mammogram results. GPs may wish to consider referral for repeat imaging.

But what does this mean for practitioners who have been using their product and their patients? What is Macrolane? Macrolane is a safe and well-tolerated injectable product used to augment body contour and correct soft tissue defects. Introduced in 2008, it has improved the quality of life of thousands of patients in indications ranging from HIV-related lipoatrophy to breast augmentation. Macrolane is the first hyaluronic acid–based product for body contouring. It provides instant and long lasting results without the need for open surgery and therefore avoids some of the consequences such as scarring. It is injected subcutaneously for body contouring. The Macrolane gel is based on Q-Med’s unique and

patented NASHA™ technology. NASHA gels for facial aesthetic treatments have been used in over 15 million treatments in over 70 countries worldwide. The safety and effectiveness of NASHA gels have been clinically proven and well documented in facial aesthetics for over 15 years. Macrolane is available in two different formulations, or volume restoration factors, Macrolane VRF20 and Macrolane VRF30. Macrolane VRF20 is suitable for areas with less tissue support and cover, while Macrolane VRF30 is a thicker gel suitable for areas with substantial tissue support and cover.

Does the decision to discontinue the breast indication for Macrolane mean that the product is not safe? No. There are no safety concerns with Macrolane as a product. This decision has been taken due to the lack of consensus amongst radiologists regarding examination of breasts treated with injectable products. Macrolane is a high-quality product manufactured to CE Mark standards. Macrolane is a safe and effective product providing a high level of satisfaction for thousands of patients who have received the product in conjunction with a body contouring or breast augmentation procedure in many different countries in the world. A safety reporting system has been in place since launch in 2008 and no safety concerns have been identified. Macrolane will remain available for use in body contouring procedures. Will Q-Med withdraw Macrolane from the market completely? No. Macrolane is not being recalled or withdrawn from the market. Macrolane will remain available for body contouring procedures. How does Macrolane interfere with the reading of mammograms? A safety reporting system has been in place since launch and no safety concerns have been identified however, all breast augmentation procedures can

interfere in the reading of mammograms. This is well-recognised and is typically addressed by using additional standard assessment techniques or radiology investigations. To use ultrasound as an adjunct to mammography is not unusual, particularly in women with dense breast for whom the sensitivity of screening mammography maybe reduced. Macrolane can interfere with reading of mammography. The current Macrolane product information provides very clear information about this. Q-Med supports international recommendations on the importance of baseline examination prior to breast augmentation. After breast enhancement, digital mammography is the preferred method of obtaining adequate information for screening purposes. Supplemental ultrasound may also be employed. This sequence of investigations has been proposed by expert radiologists. A 24-month follow-up study was recently conducted in Sweden and France to evaluate any potential difficulties with the interpretation of digital mammography image (s) or ultras onography and the value of this combination of radiology investigations. The preliminary results of this study suggest that an adequate examination can be performed using those techniques. Q-Med will make the 24-month follow-up data available as soon as possible to facilitate optimal clinical follow-up of women treated with Macrolane. Do I need to contact my patients who have received Macrolane? No. There are no safety concerns with Macrolane as a product and there is no need to contact your patients who have received it. Women who have undergone breast augmentation by injection of Macrolane do not need to take any action and are not subject to specific medical follow-up other than as prescribed by their practitioner following the procedure. They should continue to attend scheduled follow-up consultations. How shall I address concerns raised by my patients who have received Macrolane? Inform patients that Macrolane is a safe product consisting of hyaluronic acid and water and that there are no safety concerns with Macrolane. Reassure patients that because Macrolane is comprised of natural ingredients that their body will break down typically between 12 and 24 months after injection, there is no cause for alarm and that Macrolane does not need to be removed. Additionally, Q-Med is preparing a patient information brochure regarding the discontinuation of the Macrolane breast indication that will be distributed to you in the near future.

An old anti-ageing method re-invented

Dr Penelope Tympanidis gives her View On skin needling treatments Skin needling or micro-needling is a form of Collagen Induction Therapy that is intended to smooth wrinkles, improve depressed acne scarring as well as reduce the appearance of stretch marks. It is one of the easiest and most effective skin treatments available. Skin ageing and wrinkles are due to the fact that our skin produces less collagen and elastin than the ones it breaks down. It is the increase of these key ingredients, which help the skin to appear rejuvenated.

Dr Penelope Tympanidis Dr Penelope Tympanidis is a consultant dermatologist and the owner of the Dermaperfect clinic on Harley Street. Recognised by the European Board of Dermatology, she is one of London’s leading dermatologists and has been a pioneer in minimally invasive anti-aging techniques for the last 13 years, having been featured in Tatler magazine, the Telegraph, Vogue, the Financial Times Weekend supplement as well as the Channel 4 programme, Make Me Perfect. She has gained first-hand experience in general dermatology, photo-dermatology, allergy and patch testing, pediatric dermatology, skin cancer clinics, laser treatment sessions, and teaching students and General Practitioners. She now trains junior plastic surgeons, on minimally invasive cosmetic procedures, and junior dermatologists on cutaneous surgery.

The microscopic punctures (which are often just the opening of the pores of your skin) stimulate collagen production in the papillary dermis layer. The trauma caused triggers the fibroblasts (scar formation and healing cells) for collagen and elastin formation.

The tattoo gun technique evolved to ‘manual’ needling, which has been widely applied till present. The technique involves application of local anesthetic prior the treatment. This will allow the dermatologist to become more “invasive “ on key areas which will actually act as facelift.

Skin needling is not a new anti-ageing method. Already in the late eighties, skin specialists have published scientific papers on the results of needling on old depressed scars, wrinkles as well as stretchmarks.

However in recent years there have also been developed the ‘dermarollers’. A dermaroller is a device which caries nearly 200 minute needles on it. As it rolls on the skin surface produces a mild trauma. There have been also introduced dermarollers for home application. My advice on this is, to take your dermatologists’ advice before you apply this. At home application of dermaroller might result, infection, discoloration and other unwanted results. Skin needling with a skin roller produces hundreds of tiny dermal injuries, similar to the ones described above. Each dermal injury triggers the body’s natural wound healing response within the skin, producing and depositing new collagen in the treatment area. Each successive treatment triggers the production and deposition of new collagen that progressively fills in depressed scars and deep lines.

Dermatologists also noticed that hypochromic (depigmented) skin, repigmented through the transplantation of melanocytes (pigment producing cells) from normal skin into the hypochromic (depigmented) skin during the needling procedure. They also reported flattening of hypertrophic (overgrown) scars. The first needling procedures thirty years back, were performed using a high speed tattoo gun (without pigment obviously), under local anaesthesia and treated each scar to pinpoint bleeding. The procedure was every two to eight weeks and no side effects or complications were reported. Skin needling can be safely performed on all skin colours and types. There is no risk of postinflammatory hyperpigmentation (pigmentation of the skin as a result of skin trauma) as the melanocytes remain, like the dermis, intact during skin needling. This is the major distinguishing safety feature when comparing skin needling and other invasive procedures that are used to treat deep lines and depressed scars, ie laser resurfacing, deep chemical peels and dermabrasion.

which are able to produce new collagen. The epidermis (the outer layer of the skin ) varies in depth from .05mm on the eyelids to 1.5mm on the soles of the feet. The epidermis of the face (other than the eyelids ) varies from 0.3mm to 1mm in depth and therefore a 0.75mm to 2mm needle length is more than adequate to reach the intermediate reticular dermis. In my experience, needles more than 2mm in length have a tendency to catch and tear the skin. Having said all the above I still finish the needling procedure with manual work. Needling is a great technique when treating wrinkles, acne scars and skin pigmentation, but it is also an art in its own right. Manual needling still remains the most sophisticated technique for the best results, as it is a tailor made procedure to each patients’ need. It is more precise, as the skin has not the same quality inch by inch. Manual needling is also what makes the difference of a specialists’ Dermatologists’ work, to a non-specialist physicians work as no one knows the biology of the skin as a specialist dermatologist.

This infilling process can continue for up to 12 months after a treatment has been performed. There are now a number of skin rollers available and they come in many different needle lengths, needle diameters and needle numbers, which can make it very confusing for their users. In my own practice, I have tried many different needle lengths and needle diameters, in an attempt to determine the best combination for treating scars and rejuvenating the skin. The number of needles on a roller is the least important feature, as repeated rolling causes numerous dermal injuries. Needle diameter is very important as we are seeking to maximise the dermal injury without creating a new scar. In my experience, 0.25mm needle diameter is the maximum that can be used without causing a new scar in the skin. Smaller diameter needle skin rollers can be used but do not maximise the dermal injury and therefore will be slower to produce results. Needle length is also a critical issue. The target when we needle the dermis is a layer in the upper dermis called the intermediate reticular dermis. This dermal layer contains the highest number of stem cells,

The most cutting edge technique with ‘face-lift’ results, is the combination of needling with hyaluronic acid injections. It is slightly different than the technique described above, however, it works out miracles. You might wonder what the secret of celebrities is who declare they had no facelift done: it is the combo needling with hyaluronic acid injections which tightens the skin, while at the same time, sculpturing of the face is performed. Results of needling also vary from patient to patient. Results depend on many factors, mainly on each individuals response to trauma. Improvement may vary from 20% to 90%. Skin needling is now well established as a treatment option for depressed acne scarring and wrinkles. It is a far more cost effective option than fractional laser therapy and delivers very similar results with repeated treatments.I would confidently affirm that needling performed appropriately may postpone a face-lift for a decade. As the skin has a memory and will seek to return to its previous state, I recommend repeated skin needling treatments over a period of one to two years. I also recommend that my patients minimise all factors that may compromise the results such as sun exposure and alcohol consumption.

We summarise some of the most recent studies you may have missed Title: Validation of a Lip Fullness Scale for Assessment of Lip Augmentation Author: Kane, Michael, Lorenc, Z Paul, Lin, Xiaoming, Smith, Stacy R Published: Plastic and Reconstructive Surgery, May 2012, Volume 129, Issue 5, p822e–828e Abstract Background: Given the growing use of dermal fillers for cosmetic lip augmentation, a validated instrument with which to measure lip fullness is desirable in the clinic and as an efficacy endpoint in clinical studies. The authors developed and conducted a validation study of a Medicis-developed lip fullness scale. Methods: The Medicis Lip Fullness Scale consists of separate five-point scales for the upper and lower lips, with three photographs exemplifying each grade. Five board-certified dermatologists or plastic surgeons assessed 85 test photographs for each lip on two separate occasions for the first round of validation (photograph versus photograph). Three of the evaluators also graded lip fullness in 39 live subjects, followed 2 weeks later by scoring of the same subjects’ photographs for the second round of validation (live versus photographic). Results: Within-observer agreement between the two sequential photographic evaluations was almost perfect (weighted κ = 0.81). Study Design/Materials and Methods: This randomised, controlled, bilaterally paired withinpatient, double-blind study compared low level fluence 810 nm (15 Joules/cm2) laser diode therapy to nontreated shaved control areas on long-term hair

reduction. Seventeen patients with skin type II–V were treated four times at 1 month intervals. Hair count was assessed monthly over 10 months using an objective computerised method. Safety and tolerability were assessed by adverse reactions monitoring. Results: Statistically significant differences in hair count between treated and control sites were observed at each follow-up visit. The majority of patients reported a slight but bearable sensation of heat during the laser treatment, and transient erythema posttreatment. Conclusions: This study showed that laser hair removal with the F1 Diode Laser™ system was generally well tolerated, safe, and efficacious in this small sample study of patients with various skin types. After just four treatments, permanent hair reduction following one complete hair cycle has been shown. Controlled studies on larger groups of patients within each skin phototype are needed to confirm these promising results.

Objective: The aim of this study was to evaluate the efficacy and safety of nanosome vitamin C iontophoresis and to compare the therapeutic effects of nanosome vitamin C iontophoresis vs. glycolic acid peel 70% in the treatment of melasma in Egyptian women. Methods: This study included 14 patients of melasma with skin type IV–V taken for a right–left comparison study of six sessions. Glycolic acid 70% peel was applied on the right side, whereas nanosome vitamin C was applied by iontophoresis on the other side. The results are evaluated using the melasma area and severity index score and with photographs at baseline and after six sessions. Also the photographs were evaluated by two single-blinded physicians before and after sessions. Results: Both sides were improved, but the side treated with nanosome vitamin C showed better results. Side effects were few and transient. Conclusion: We concluded that nanosome vitamin C is a new, safe and effective, easy and painless method in the treatment of melasma.

Abstract A new range of hyaluronic acid (HA) dermal fillers has been designed using Optimal Balance Technology™, which centers around three main parameters: the degree of cross-linking, the size of gel calibration and the HA concentration. The five different products in the range (HA E Touch, HA E Classic, HA E Lips, HA E Deep and HA E Volume) have the same concentration of HA (20 mg/mL) and various degrees of cross-linking and gel calibration, in order to have distinctive physical properties adapted to their specific indications. HA E Classic, HA E Deep, HA E Lips and HA E Volume are available in two different formulations either with or without lidocaine. The efficacy, safety and patient satisfaction of the HA E range in various indications have been assessed in several clissical studies. In this study, the rheological measurements of HA E Deep being the firmest gel within the range. Addition of lidocaine did not change the rheological properties of the HA E fillers. HA E fillers have three different degrees of gel calibration, with HA E Touch, HA E Classic and HA E Lips having the same smallest gel calibration and HA E Volume having the largest gel calibration within the range. Injection of all HA E fillers was smooth, regular, and required low extrusion force when using the Ultra Thin Wall (UTW)needle provided for each product. Traditionally, soft tissue lifting and redraping have constituted the cornerstone of most facial rejuvenation procedures. Changes in the facial skeleton that occur with aging and their impact on facial appearance have not been well appreciated. Accordingly, failure to address changes in the skeletal foundation of the face may limit the potential benefit of any rejuvenation procedure. Correction of the skeletal framework is increasingly viewed as the new frontier in facial rejuvenation. It currently is clear that certain areas of the facial skeleton undergo resorption with aging. Areas with a strong predisposition to resorption include the midface skeleton, particularly the maxilla including the pyriform region of the nose, the superomedial and inferolateral aspects of the orbital rim, and the prejowl area of the mandible. These areas resorb in a specific and predictable manner with aging. The resultant deficiencies of the skeletal foundation contribute to the stigmata of the aging face. In patients with a congenitally weak skeletal structure, the skeleton may be the primary cause for the manifestations of premature aging. These areas should be specifically examined in patients undergoing facial rejuvenation and addressed to obtain superior aesthetic results.

Cosmetic News: Karen, you are arguably one of the most successful businesswomen in the UK cosmetic medical market. How did you get here? Karen Hill: I started in the pharmaceutical industry with Boehringer and ended up running a national team of brilliant sales people. Together we not only stopped the decline of our cash cow but actually increased the turnover by 50%! After a MSc and a spell with Ansbacher, a private Bank, I took the plunge with the Healthxchange. The attraction was an intuitive, innovative senior management team with a ‘never say die’ attitude. It’s been an amazing experience and I have made so many great friends in the industry.

Businesses are afraid to invest for fear of over committing. For many clinics this is irrational as revenues remain strong and borrowing interest rates are low. It is a perfect time to invest. The switched on companies understand this and have taken action e.g. SmartMed, marketing the hugely successful INTRAcel, have introduced a scheme whereby they will cover the first six lease payments for the clinic. In the intervening six months, even with low to moderate uptake, the clinic will earn enough to cover the whole cost of purchase. It is effectively getting a free INTRAcel.

She is your clinic’s public face and is a living representation of the private philosophy of the practice. Most patients will make their mind up what they think of the practice in the first 45 seconds, long before you ever get to see them! Training is important. Have protocols for answering the phone, handling complaints,

meeting and greeting patients, keeping them informed and comfortable if there is a delay and the subtle art of making settling the bill a pleasure. Pay attention to the detail. Never work alone. Get a great nurse colleague to support and work with you. Your injecting and treating time is valuable and an expert nurse can be arranging consents, discussing charges, applying anaesthetic and cross marketing other treatments when you are seeing the next patient. Surround yourself with colleagues that you can call upon to discuss difficult cases and refer to. Don’t forget the support you can get from your sales reps. They can help organise an open evening, advise on marketing campaigns and its free! Cover your costs with horizontal earnings. Most injectors, surgeons and doctors earn fees on an ‘item for service basis’. The harder you work the more you make. This can sometimes seem like being on a treadmill. I have even heard some professionals say they don’t like to take time off for holidays as all they can think of are the mounting costs while they are away from the clinic. Horizontal earnings are called that because they can make you money whilst lying on a bed or sun lounger. Sounds ideal doesn’t it! The easiest way to do this is to introduce a cosmeceutical range into the clinic, such as Obagi. In the US some clinics add 50% onto their total revenues with very little added effort or cost. Look to make money from your assets e.g. rent rooms out when you are not using them, choose professionals offering related services e.g. a plastic surgeon, or train your nurse to perform hair

removal, skincare consultations and laser work all of which will turn a cost into a revenue source. Target your marketing. Many clinics have a strategy of throwing out a wide net to draw in patients. This is expensive and you have no control over the type of consultation such a strategy will bring in. The return on investment is often low and hard to measure. A much more effective way to market is directly to your database. These are patients who already know about your services and are often advocates for your clinic. Collect email addresses, ask consent to send information and ensure your reception staff update information every time a patient attends the surgery. In a very short time you will have a ‘clean email contact list’. Be careful that you send balanced, interesting information about your services. One e-shot can drive as much as 30% additional business for up to three months and it costs very little to do. The Healthxchange is developing an electronic magazine which can be bespoke for individual clinics specifically aiming at the electronic marketing market. No ads just great information! Hold your nerve, stay true to your values and beliefs, I believe you get what you pay for, its better to decide am I a quality professional and do I deserve to be paid for that or do I want to adopt a stack ‘em high flog ‘em cheap option? My view? Stay away from cheap! And short term tricks like Quidsin and Groupon. It is often a false economy, cannibalises your own market, drives down your own prices and does not smack of quality professionalism you can rely on.

According to BUPA’s 2011 Health Pulse Report 70% of people in the UK use the internet to access health information, and with British interest in cosmetic surgery at an all time high clinics have no choice but to create an online presence to help attract new customers. However, the rapidly evolving nature of how consumers access the internet means that even the most recently designed clinic websites could be causing unexpected problems for potential patients. A simple check to make sure a clinic’s website works properly on the most common mobile phones and tablets could make the difference between gaining new customers or losing them to a savvier rival. The Internet On The Go There is a growing body of evidence to show that mobile and tablet internet usage is expanding at break-neck speed in the UK. According to the Office for National Statistics in 2011, an extra six million people used their mobile phone to access the internet compared to 2010 (1) and now for the first time more people in the UK say they use a smartphone (89%) than a laptop or desktop computer (75%) (2) to go online. Google also conducted research in January 2012 to compare smartphone usage in the US, the UK, Germany, France and Japan and found that the UK had the highest penetration of smartphone ownership at 45% of the population. It also found that 54% of smartphone users were accessing the internet on their phone on a daily basis. Growing smartphone ownership and usage is translating into traffic for health related websites too. For example, WhatClinic.com, a consumer search engine for medical services and facilities, has seen mobile phone traffic increase by 23% in the last month alone (3), and by a staggering 339% comparing April 2012 to April 2011. Mobile and tablet traffic to the website now accounts for more than 23% of the site’s total traffic. In the past, the web browsers on most smartphones were not able to handle all the technological features of modern websites, requiring website owners to build mobile specific versions of their sites. Thankfully phone browsers have come on in leaps and bounds recently but there are still some potential pitfalls that clinic owners can take some simple steps to avoid.

For example, if you allow Team GB supporters time off to watch a team event, you should apply the same policy to non-GB nationals who want to watch their own national team. Likewise, do not assume that only men will be interested in the games and remember that those who are not interested in them should still be treated fairly and not excluded from the festivities. Publicising the equal opportunities policy and any non-harassment/bullying policy to ensure employees are familiar with their obligations and understand that any offensive behaviour will not be overlooked and will be dealt with through a disciplinary process.

When breakfast is done I put in an appearance at the gym around 7.am I never thought I’d be a gym enthusiast less so one at 7am in the morning! but never say never. Clocking in at the gym daily gives me the mental clarity I need to get through the day, I am physically and mentally much better off for having made the changes in my life recently that I have. My drive to work takes me ten minutes Medizen is located in Mere Green, Sutton Coldfield, in Birmingham. I listen to Radio 2 Chris Evans, simply because I think it has the right mix of music and irreverent chat. I arrive in the morning usually around 9am Once in I think one of the first things I do is check the diary and see how the day is shaped out for

me, I then meet with our clinic manager to discuss any clinic issues such as particular patient enquiries, staff sickness, staff training, stock levels, bonus schemes for staff those kinds of day to day issues. Once this is tidily dealt with, it’s my coffee time where I chat with my lovely receptionists and aestheticians who fill me in on what’s been going on front of house, they’re a lively bunch but an essential part of the operation and without each of them performing their roles Medzien would not be the place that it is. I start my morning clinic by seeing a series of patients for Botox, fillers, laser resurfacing, facial sculpting and cosmetic consultations. Medizen has a broad spectrum of patients and clients but we cater predominantly to 30-50 year old mums, professional women and men, with some footballers wives and girlfriends and local celebrities. I can see anything from six to ten patients myself in one morning clinic. I break for lunch at the end of the morning clinic; these days lunch for me usually consists of a healthy salad or sandwich at my desk, but if I’m lucky meet my wife for lunch. Annie, my wife is an Aesthetic Nurse Practitioner at MediZen, we’ve been together over 20 years and as a result have two wonderful kids Laura,17 Nick, 16 Since graduating from Birmingham University in 1987, to be a general practitioner of medicine I have always held a deep-rooted special interest in dermatology which has spanned over 20 years. I believe I made the right career choice as my profession allows me to utilise my artistic and practical skills, plus I meet lots of lovely people both as clients and colleagues. Some of the ‘not so great’ aspects of this job is having to break the news to clients who have unrealistic expectations in terms of what they are ‘aesthetically’ looking to achieve, but I believe honesty and transparency is the best approach and the safest for the patient. A typical afternoon will be the much the same as the morning, though I do a lot of clinical

trial work and am currently running studies on lip enhancement, facial volumising, acne and botulinum toxin. I will usually meet a colleague or company rep a couple of times a month over a longer lunch. At the moment I am spending time on developing my training and media skills in preparation for more travel and lecture tours abroad. Another great project I am working on is Human fibroblast transplantation for acne scarring, there are some great developments occurring in this area. I do very much love my work and am inspired by the work of Tim Flynn, Koen de Boulle, Mauricio de Maio. Much like those guys I take great pride in my work and I think one of my proudest moments was being invited t South Africa to train fellow colleagues on toxin techniques. There is much scope for improvement in our industry and personally I would like to see some positive changes transpire such as a stop to non-medical personnel injecting toxins and fillers, I believe a speciality such as cosmetic medicine should only be accessible by trained nurses, doctors and dentists. This industry does need tighter regulation and I advocate the ban of remote prescribing - it is unethical. My average working week is around 50 hours, but I think you tend to notice long working days less if you love your work and enjoy positive results. My day unwinds by retreating to the sanctuary of home to fine food and wine, cooking, spending time with family and friends, skiing, travelling and spending time on my motor yacht, ‘BoatOx’. My reflection for today? Take time out to smell the coffee.

BOTOX® (botulinum toxin type A) Abbreviated Prescribing Information Presentation: Botulinum toxin type A (from clostridium botulinum), 50 or 100 or 200 Allergan Units/vial. Indications: Temporary improvement in the appearance of moderate to severe vertical lines between the eyebrows seen at frown (glabellar lines), in adults <65 years, when the severity of these lines has an important psychological impact for the patient. Dosage and Administration: See Summary of Product Characteristics for full information. Do not inject into blood vessels. Doses of botulinum toxin are not interchangeable between products. Not recommended for patients <18 or >65 years. Use for one patient treatment only during a single session. Reconstitute vial with 1.25ml of 0.9% preservative free sodium chloride for injection (4U/0.1ml). The recommended injection volume per muscle site is 0.1ml (4U). Five injection sites: 2 in each corrugator muscle and 1 in the procerus muscle: total dose 20U. Contraindications: Known hypersensitivity to any constituent. Infection at proposed injection site(s). Warnings/Precautions: Relevant anatomy and changes due to prior surgical procedures must be understood prior to administration. Do not exceed recommended dosages and frequency of administration. Adrenaline and other anti-anaphylactic measures should be available. Reports of side effects related to spread of toxin distant from injection site, sometimes resulting in death. Therapeutic doses may cause exaggerated muscle weakness. Caution in patients with underlying neurological disorder and history of dysphagia and aspiration. Patients should seek medical help if swallowing, speech or respiratory disorders arise. Clinical fluctuations may occur during repeated use. Too frequent or excessive dosing can lead to antibody formation and treatment resistance. The previously sedentary patient should resume activities gradually. Caution in the presence of inflammation at injection site(s) or when excessive weakness/ atrophy is present in target muscle. Caution when used for treatment of patients with peripheral motor neuropathic disease. Use with extreme caution and close supervision in patients with defective neuromuscular transmission (myasthenia gravis, Eaton Lambert Syndrome). Contains human serum albumin. Procedure related injury could occur. Interactions No interaction studies have been performed. No interactions of clinical significance have been reported. Theoretically, the effect may be potentiated by aminoglycoside antibiotics or other drugs that interfere with neuromuscular transmission. Effects of administering different botulinum toxin stereotypes simultaneously, or within several months of each other, is unknown and may cause exacerbation of excessive neuromuscular weakness. Pregnancy: BOTOX® should not be used during preganancy unless clearly necessary. Lactation: use during lactation cannot be recommended. Adverse Effects: See Summary of Product Characteristics for full information on side effects. Based on controlled clinical trial data, the proportion of patients that would be expected to experience an adverse reaction after treatment is 23.5% (placebo: 19.2%). In general, reactions occur within the first few days following injection and are transient. Pain/burning/stinging, oedema and/or bruising may be associated with the

injection. Frequency By Indication: Defined as follows: Very Common (> 1/10); Common (>1/100 to <1/10); Uncommon (>1/1,000 to <1/100); Rare (>1/10,000 to <1/1,000); Very Rare (<1/10,000). Infections and infestations. Uncommon: Infection. Psychiatric disorders. Uncommon: Anxiety. Nervous system disorders. Common: Headache. Uncommon: Paresthesia, dizziness. Eye disorders. Common: Eyelid ptosis. Uncommon: Blepharitis, eye pain, visual disturbance. Gastrointestinal disorders. Uncommon: Nausea, oral dryness. Skin and subcutaneous tissue disorders. Common: Erythema, Uncommon: Skin tightness, oedema (face, eyelid, periorbital), photosensitivity reaction, pruritus, dry skin. Musculoskeletal and connective tissue disorders. Common: Localised muscle weakness, Uncommon: Muscle twitching. General disorders and administration site conditions. Common: Face pain, Uncommon: Flu syndrome, asthenia, fever. The following other adverse events have been reported since the drug has been marketed: dysarthria; abdominal pain; vision blurred; pyrexia; focal facial paralysis; hypoaesthesia; malaise; myalgia; pruritus; hyperhidrosis; diarrhoea; anorexia; hypoacusis; tinnitus; radiculopathy; syncope; myasthenia gravis; erythema multiforme; dermatitis psoriasiform; vomiting and brachial plexopathy; rash; psoriasiform eruption; anaphylactic reaction (angiodema, bronchospasm); alopecia and madarosis. Adverse reactions possibly related to spread of toxin distant from injection site have been reported very rarely (exaggerated muscle weakness, dysphagia, or aspiration pneumonia which can be fatal).
