EDITOR’S LETTER With summer fast approaching, many of your clients’ minds will be turning to thoughts of how their bodies look on the beach and cellulite will be top of the list of body hang-ups for a large percentage of them. Despite the fact that 85% of women, no matter what their size and shape, suffer from the dreaded orange peel, many women are so self-conscious about it that they are embarrassed to show off their bodies. Cellulite has always been notoriously difficult to treat and, while there is still no miracle cure, aesthetic practitioners now have a variety of treatments that are effective in reducing the appearance of cellulite and giving women back some body confidence. In this month’s special feature, we examine some of the best methods of getting rid of cellulite (p23-28). Also in this issue, we talk to Ita Murphy about My Cells Platelet Rich Plasma Rejuvenation (p30-32) and look at how your business can sustain long term growth, even in hard economic times (p4244). COSMETIC CLAIMS ON THE RISE Hamilton Fraser has reported a dramatic increase of 126% in the number of claims notified by cosmetic practitioners during the past year. The specialist medical indemnity insurance provider says that the increase in claims is a warning to cosmetic practitioners of the importance of having adequate cover to protect them against any claims of malpractice or wrong doing in the practice of cosmetic treatments. Figures released show the breakdown of notifications by treatment type: 33% laser; 32% dermal fillers; 20% botulinim toxin; 11% laser lipolysis and 4% other. The value of each claim has also increased by a staggering 900% over the past two years, which Hamilton Fraser relates mainly to increased litigation costs. The highest value claim notified during the past 12 months related to a temporary dermal filler, and is expected to be settled shortly at a figure of around £40,000. Eddie Hooker, managing director of Hamilton Fraser said, “We attribute the rise in these statistics to the increasing costs of litigation and the increased popularity and availability of these treatments. There is no indication that practitioners are being trained to a lesser degree than before but as the treatments are predominately elective there is a higher expectation from the customer. However, whether the cosmetic practitioner is negligent or not, legal costs in defending the practitioner still need to be paid.” Practitioners working in aesthetic medicine are also advised that they should consider the effect of any claims on their general practice indemnity cover. Eddie continues. “The consequence of a claim can have a serious effect on practitioners both financially and professionally in terms of reputation. Whilst your defence organisation will support you in the event of a claim, practitioners may choose to protect their defence union reputation from their aesthetic medicine work. We always recommend practitioners to consider a separate commercial insurance policy for their aesthetic practice.”

Study Shows Efficacy of LLLT for Body Contouring and Spot Fat Reduction A study has proven the efficacy of Low-level laser therapy (LLLT) for body contouring and spot fat reduction. The study, published in Obesity Surgery, showed that that LLLT can achieve safe and significant and sustained girth loss over repeated treatments. The study examined the effectiveness and mechanism by which 635-680 nm LLLT acts as a non-invasive body contouring intervention method. 40 men and women aged between 18-65 with a BMI <30 kg/m(2) were randomised 1:1 to laser or control treatment. Subject’s waistlines were treated for 30 minutes twice a week for four weeks. Subjects were asked not to change their diet or exercise habits. Each treatment gave a 0.4-0.5 cm loss in waist girth. Cumulative girth loss after four weeks was -2.15 cm (-0.78 +/- 2.82 vs. 1.35 +/- 2.64 cm for the control group, p < 0.05). A blinded evaluation of standardised pictures also showed statistically significant cosmetic improvement after four weeks of laser treatment. In vitro studies suggested that laser treatment increases fat loss from adipocytes by release of triglycerides, without inducing lipolysis or cell lysis.
GPs are Routinely Advising On Skincare and Cosmetics, says doctors.net poll Dishing out advice on skincare and cosmetics is becoming commonplace among GPs, according to new research carried out by Doctors.net.uk. A poll by the site revealed more than half (57%) of doctors advise patients every month on skincare and cosmetic issues, with more than a third (37%) being asked about skincare as regularly as a few times every week. Following the survey, Simon Grime, head of healthcare for Doctors.net.uk, called on the healthcare, dietary and skincare industries to provide doctors with greater levels of relevant information and support. He said, “The frequency with which doctors find themselves consulting on skincare and cosmetic issues is naturally met with an implicit pressure to keep abreast of the latest developments in the field. Yet, with many of the contributing factors extending far beyond the pages of the traditional medical reference book, doctors are turning increasingly to trusted online resources for practical, up to date and relevant information. Through ensuring that such information is readily available online, the skincare and cosmetics industry can make a real contribution to the effective and efficient treatment of patients.”

iiaa Introduces Remedial Camouflage Workshops The International Institute for Anti-Ageing (iiaa) is introducing new one-day training workshops this Summer, to meet demand from practitioners and aestheticians who are often faced with requests for camouflage to conceal pigmentation, vitiligo, tattoos, birthmarks, rosacea and scarring. The intensive Remedial Camouflage Workshop, which is based on skilful application of jane iredale mineral cosmetics, has been developed to provide the theoretical knowledge and practical experience needed to perform highly specialised concealment techniques. The session, which uses models with a range of skin conditions, includes the theory of the colour wheel and principles of colour correction. It features tutor demonstrations as well as giving students handson experience through which they learn useful techniques for layering and dealing with texture. study involving successful treatment of patients with severe burns scarring, using the Environ Medical Roll-CIT and needling technique. Dr Matthias Aust, from the Department of Plastic, Hand and Reconstructive Surgery at the Medical University of Hanover, Germany, won an award
of €10,000 in recognition of his excellent work in treating severe and deep burn scars, which will be used to fund further studies. The award was made by Cicatrix, a leading German association which provides education and information to self-help associations, individuals, doctors, therapists and other institutions that are concerned with burns and scars. The association’s aim
Home-Use Body Device Delivers Moderate Results Says Study A study, published in the European Journal of Dermatology, has shown that the TriPollar RF home-use device for body treatments can deliver ‘discrete circumference reduction’ and ‘moderate laxity improvement’. The objective of the study was to evaluate the TriPollar home-use device for circumference reduction, cellulite improvement and skin tightening using objective and subjective methods. An ex-vivo human skin model was used for histological and biochemical evaluations of the TriPollar clinical effect. Additionally, 24 subjects used the new device on the abdomen and thigh areas and the circumference reduction was measured. Exvivo models indicated a significant increase of 82% in hypodermal glycerol release. Histology revealed a 34% alteration in adipocyte appearance. Collagen synthesis increased by 31% following TriPollar treatment. A significant average reduction of 2.4 cm was measured on the treated thighs. On the control thighs a lesser, non-significant reduction was found. Average abdominal laxity was reduced from 1.4 at baseline to 0.8 following treatments. A certain reduction was measured in the abdomen circumferences, although it was not significant. The reported results demonstrate the safety and efficacy of the new TriPollar home-use device for body contouring and skin tightening.
Less Frequent Doses Of Botox® Result In Reduced Wrinkles after two years Patients can decrease the frequency of Botox® injections after approximately two years and still receive most of the same wrinkle-smoothing cosmetic benefits, according to new research at Oregon Health and Science University. The research, which was sponsored by an unrestricted educational grant from Allergen and carried out by Roger A Dailey, studied 50 women ages 30 to 50, who received regular Botox® injections for two years. “We found that after the patient receives Botox® injections every four months for two years, the frequency of the injections can be changed to every six months and still achieve good results,” Dailey said. “This demonstrates patients have the ability to achieve good results with broader treatment schedules and ultimately at a lower overall treatment cost.”

The medical needling entry by Dr Aust met all the requirements of the award. Rather than relying simply on before and after images, which can be questioned, Dr Aust was able to quantify and qualify his findings in the laboratory. He demonstrated that this is the first operation in a living organism that is able to regenerate the skin, allowing healing and scarforming without increasing the risk of pigmentation. The pilot study led by Dr Aust involved 16 patients evaluated percutaneous collagen induction (PCI) in post-burn scarring. On average, patients rated their improvement as a mean of 80% better than before treatment. Linda Hutchinson, director of registration at the Care Quality Commission, talks about what registration means for cosmetic surgery providers
The Care Quality Commission (CQC) is introducing a new system of registration, which brings the NHS, independent healthcare and adult social care under a single set of essential standards of quality and safety for the first time.
This new system brings with it significant changes for independent healthcare providers, including those providing cosmetic surgery. Although such providers are currently registered under the Care Standards Act 2000, there is no automatic transfer of registration to the new system, and providers will need to apply to be registered under new legislation. The Health and Social Care Act 2008 outlines the scope of registration. The Department of Health has set out the new regulations in the Health and Social Care Act 2008 (Regulated Activities) Regulations 2010 and Care Quality Commission (Registration) Regulations 2009. Previously, different types of services were regulated under different Acts, with different regulations and standards. The new registration system makes sure that essential standards of quality and safety apply across the whole care sector, creating a level playing field for providers. It means that people can expect services to meet the same essential standards across the care sector and that they are protecting their safety and respecting their dignity and rights. After initial registration, we will continuously monitor whether providers are meeting essential standards, as part of a new, more dynamic system of regulation that places the views and experiences of people who use services at its centre. We have recently published the guidance about compliance: Essential standards of quality and safety, that is focused on outcomes - the experiences people have as a result of the care they receive. The guidance makes clear to providers the outcomes we expect people to experience if providers are compliant with the new regulations. It describes quality and safety from the perspective of people who use services. The essential standards have a legal status. We have a wider range of strong new enforcement powers including imposing conditions on registration, fines, and in extreme cases, cancellation of registration. We will use these to ensure registered providers comply with the regulations. The key differences between the new registration system and the old one are as follows: •

While chirality is actually nothing new to science and the pharmaceutical drug industry, it is a relatively new concept to skincare. But what do chirally correct ingredients offer the skin and can they provide real benefits otherwise unmatched? In 1847, renowned French chemist Louis Pasteur first discovered the concept of chirality as a graduate student while studying two acids in wine. He observed that while the chemical composition of tartrate and paratartrate were identical, their crystals polarised differently. This observation led him to understand that many molecules have two ‘mirror-image’ parts – that is, they are the same in all ways physically and chemically with one exception: how these individual isomers, or single enantiomers, link together and react with other molecules in the body. Chirality is often described as the ‘handedness’ of molecules. The isomers are typically referenced by either an ‘L’ or ‘D’ notation designating the two parts. To illustrate, think of your left and right hand. They are mirror images of each other, but could you fit your left hand inside a right-handed glove? The same holds true for chiral isomers, which do not link perfectly with one another and are not superimposable but are, rather, mirror images of each other. ver the past year or so, we have run a series of articles looking at the role of marketing in the running of a successful aesthetic medical practice. These articles should have given you a good insight into how marketing works (particularly within the aesthetic medical sector), how it can help your practice, and what you need to do to promote your practice.
Now it’s time to take a more detailed look at the four core elements of marketing: Price, Promotion, Place and Product (the so-called four P’s). These are worth looking at in depth simply because they are so important to any marketing strategy. If you don’t take them into account before embarking on your marketing, it is unlikely that your marketing will have maximum effect. Before taking a look at the first in the series, price, it’s worth giving a quick resume of what marketing is all about. The Chartered Institute of Marketing defines marketing as: “The management process responsible for identifying, anticipating and satisfying customer requirements profitably.” This highlights a crucial issue – the customer is at the heart of any marketing
you carry out. Your goals are important too, but if you don’t meet the needs of your clients then you are unlikely to meet any of your own. That said, if you are about to start marketing your clinic, the first step is to decide what you are aiming for. It is, after all, your business and your career, and you need to be happy with the direction in which they are going. You need to be sure that you know what your business goals are, both tangible (e.g. anticipated turnover and profit) and intangible (growth in reputation, job satisfaction, etc). You also need to know what sort of patients you want to attract. In the medical aesthetics sector, choices would include those at the very top end of the market; younger, trendier image conscious people; or perhaps those
who are genuinely distressed about a particular aspect of their looks and will take whatever measures are necessary to ease that distress. Of course, the two do need to be congruent. For example, you cannot hope to charge premium prices and reap maximum profits if you want to help those who are in distress but do not have huge disposable incomes. Once you have your answers, you can then use the four Ps to optimise your marketing. They can help you to establish what your target clients want and how you can meet their needs. They can also help you to provide an attractive, consistent and congruent image to your chosen patient group, and ultimately to help you to have a successful and profitable practice. So, onto the first of the four P’s, price, or in other words, your fees.
