Arthroscopy literally means visualizing the joint (arthros: joint, scope: to see).

In essence, artroscopy involves introduction of a thin telescope into the joint through small openings in the skin called portals, and directly visualizing the interior of the joint on a screen. In hand surgery arthroscopy, particularly that of the wrist has become a valuable and useful tool for the surgeon over past two decades. It allows diagnosis of pathologies within the joint and also allows the surgeon to carry out some therapeutic procedures (also referred to as minimally invasive surgery), without the need for a large open procedure, enhancing patient comfort and shortening the recovery time.

This procedure is commonly used in ligament injuries of the wrist and establishing the diagnosis in chronic wrist pain. Recently, with development of smaller scopes arthroscopy has also been extended to smaller joints in the thumb and fingers.

Dr Amitabha Lahiri passed his post-graduate surgical examination (FRCS Edinburgh) in 1998. He was trained in hand and microsurgery as a fellow at the Department of Hand and Reconstructive Microsurgery, National University Hospital Singapore, following which he joined the hand surgery service at the Department of Orthopaedic Surgery Tan Tock Seng Hospital as a registrar in 2004, and completed his Advanced Specialist Training in Hand Surgery in 2008. His areas of interest are microsurgery in hand reconstruction and wrist biomechanics. He has published research papers on peripheral nerve surgery and developed new models for microsurgical training.

ENDOSCOPIC CARPAL TUNNEL RELEASE SURGERY

Background Carpal tunnel syndrome refers to the entrapment neuropathy affecting the median nerve at the carpal tunnel. Figure 1

Patients present with pain and numbness in the median nerve distribution, with or without weakness of the thenar muscles.

Clinical Examination Examination reveals sensory changes affecting the radial three and a half digits, with or without thenar muscle weakness and atrophy. Nerve conduction studies to support the diagnosis may be performed.

Treatment Options

Traditionally, open carpal tunnel release involved a 3 to 4 cm incision in the palm. With the advent of endoscopic carpal tunnel release (See figure 2), only a 1cm incision at the proximal wrist crease is required. (See figure 3 & 4). This allowed earlier return to work and normal activities, as well as providing a better aesthetic appearance to the patient.

In our effort to meet the needs of patients who are unable to come during the day, several TTSH Departments have extended their clinic operating hours to run from 6pm - 9pm, by appointment only. Conservative treatment, comprising of splinting, activity modification and Vitamin B complex may be offered in the early, initial stages. However, in the recalcitrant, intermediate to advanced stages, that are not responsive to conservative management, surgery should be offered before irreversible nerve damage occurs. (Figure 1)

About Dr Tan Puay Ling Dr Tan Puay Ling graduated from the National University of Singapore in 1998. She completed her advanced specialist training in Hand Surgery in 2006 and subsequently went on to complete a fellowship in Microsurgery in the internationally renowned Chang Gung Memorial Hospital in Linkou, Taiwan. Dr Tan’s areas of interest include microsurgery, limb salvage and reconstruction as well as paediatric hand surgery. She is currently a visiting consultant in the Department of Orthopaedic Surgery, KKWCH. Dr Tan has also written numerous research papers in international journals and presented at various local and international scientific meetings. She is also actively involved in the teaching and mentoring of students in the undergraduate medical training programme.
