A COLLABORATIVE PARTNERSHIP FOR BUILDING STRONG, HEALTHY AND RESILIENT FAMILIES Conducted by Tan Tock Seng Hospital’s healthcare professionals and community partners, these community health engagement programmes aim to motivate and empower families to take charge of their health, well-being and family life.

OUR VISION We Engage. We engage our patients and families through platforms such as CareConnect and CareConnect Volunteer Committee to understand their needs and feelings and to build familial relationships.

We Empower. We bring the community and healthcare partners together to co-design learning platforms and resources, tailored to the needs and health-seeking behaviours of patients and their families.

We Voice. We bridge conversations and build active partnerships with the healthcare community to improve the care experience of our patients and families.

These programmes are under our Families for Care initiative, an initiative that is a partnership between Families for Life Council and Tan Tock Seng Hospital.

We Inspire. We share inspirational stories and experiences of patients and caregivers to motivate others to make a positive impact within their families and the community.

ENGAGE IN LIFE (EIL) PROGRAMME

Be Your Own Health Manager is a programme developed by Stanford University, USA. It is a six-week workshop that helps people with chronic health conditions gain confidence in better managing their symptoms and their daily life.

EIL originated from the Passion for Life programme in Jonkoping, Sweden. It is a self-empowerment programme that aims to empower older people (aged 40 and above) to live a healthier and more independent lifestyle in their senior years. These interactive workshops encourage participants to try out new techniques and skills which they can incorporate in their everyday lives.

Tan Tock Seng Hospital Nutrition and Dietetics Department provides practical advice on health related nutrition information to help others make appropriate dietary choices for better health. Nutritional needs change over time according to the different stages in life and varies from individuals. A wide range of topics may be discussed, such as: • Relationship between Food and Health • Practical Tips on Healthy Eating • Nutrition and Management of Various Medical Conditions

Advance Care Planning (ACP) is a discussion on future healthcare preferences, suitable for all ages and health status. During hospitalisation, an ACP can help healthcare professionals care for you in a way that is aligned with your goals and preferences, and ease the burden of decision making upon your family. TTSH Advance Care Planning team will share more about: • What is ACP? • Why is ACP Important? • What are the Types of ACP? • Common Misconceptions of ACP • Difference between ACP and AMD • Who is in Involved in an ACP Discussion? • What is a Nominated Healthcare Spokesperson (NHS)? • Where and How to Complete an ACP Discussion?

Stroke affects everybody differently. It is normal to feel angry, anxious or depressed after a stroke. You may feel worried about work, money and relationships, and the tiredness caused by the stroke can make things worse. Designed for both stroke survivors and their caregivers, Re-Learn and Enjoy Active Living (R.E.A.L.) is a framework of wellness programmes that help both stroke survivors and caregivers navigate and manage the post-stroke recovery journey. S3 R.E.A.L. Recovery focuses on getting stroke survivors re-integrated into society, which involves taking an active approach to ensure that life goes on.

Central to Tan Tock Seng Hospital’s Family for Care (FFC) programmes is the hospital’s patient experience centre, CareConnect and a volunteer committee, CareConnect Volunteer Committee. CareConnect is a one-stop information hub that provides patients and caregivers access to resources within and beyond the hospital. Through CareConnect, FFC’s suite of programmes will focus on physical, mental and social health and promote volunteerism. Hospital
