Damage to the stern hull and fendering system of the tug Mark E Kuebler, from the Nisalah propeller. NTSB photos. 2023
The same type of incidents are reported year after year. Machinery breakdown and failure, collision, fire and explosion, grounding, mis-declared cargo, cargo shift, and liquefaction are consistently at the top of the list, but lessons identified through lengthy investigations often point to human error and seldom appear to be learned or corrective measures implemented.
Further, emphasis on human error can overlook fundamental flaws in safety culture. This points to systemic failure to implement mechanisms and solutions to manage known risks.

A job hazard analysis (JHA) or job safety analysis (JSA), offers several benefits:
• Risk Reduction: JHAs help identify potential hazards and risks associated with specific tasks or jobs. By analyzing these risks, organizations can implement preventive measures to reduce incidents and injuries.
• Improved Safety Culture: When employees actively participate in JHAs, it fosters a safety-conscious culture. Workers become more aware of safety protocols, leading to better compliance and overall safety awareness.
• Training and Skill Enhancement: JHAs provide an opportunity for training and skill development. Employees learn about safe work practices, emergency procedures, and proper use of equipment.
• Efficiency and Productivity: By identifying potential hazards, JHAs allow organizations to streamline processes and eliminate unnecessary risks. This leads to improved efficiency and productivity.
• Cost Savings: Preventing incidents and injuries reduces medical costs, worker compensation claims, and downtime. JHAs contribute to long-term cost savings for the organization.
