Robert Tompkins, MD Program Director Rural Family Medicine ResidencyI am so pleased that you have chosen Pittsburg, Texas, for your new home (at least for the next three years). I know this has been a long journey, and we are excited to take this next step with you! Our program is super excited that you are joining the Rural Family Medicine Residency program in Pittsburg. We congratulate you on this professional and personal milestone in your life.
You will receive notice from the Office of Graduate Medical Education to complete several tasks soon. However, there are many different forms within this document and several items of documentation that are required prior to attending orientation week. Also, plan to provide proof of your current certifications prior to beginning orientation. If you do not have certain credentials completed prior to onboarding, you will complete these during your two-week onboarding and orientation, along with many other items.
Certifications and Training Onboarding Documents
Several certifications are required before you can begin your training. Please review the list below, and let us know if you are still in need of one or more of the certification training requirements below. Please note that residents will have an opportunity to complete any outstanding certifications once arrived during orientation week.

Orientation Schedule and Maps
Your Epic training will primarily occur at CHRISTUS Trinity Mother Frances Hospital because they will not accept an attestation from our trainers at UT Health that states that you have completed training. After completing your CHRISTUS training, you will test out of Epic at UT Health East Texas so that you do not have to complete the training twice.
ORIENTATION SCHEDULE
Plan on a very busy first two weeks of orientation and training. You will be completing ALS, BLS, PALS, and NRP certificates, Symplr Learning modules and possibly other items after-hours. If you are already certified in any of these credentials, please send me your certifications so that you do not have to complete it during orientation.
Orientation is scheduled to begin June 13-28 from 7 a.m. – 5 p.m. each day. Please monitor your inbox for an email regarding the official orientation schedule. During orientation, you will complete your Net Learning modules via Symplr Learning.

Clinical Rotations
TYLER FAMILY CIRCLE OF CARE ROTATION
All residents will be credentialed for the entire program. All documents are needed ASAP for processing.
The following documents are needed from the PGY-1 Residents:
• Copy of CV
• Copy of Life Support Certification
• Copy of Valid Driver’s License
• Copy of University of Texas Medical School Certification Form
• Copy of All Current Immunization Records
• UT Proof of HIPAA Training (will be in Symplr Learning Assignments when in orientation; once completed, we need proof of completion)
Letter of Good Standing:
• UT Liability Coverage (COI) (you will sign this when you complete Institutional Orientation)
• Proof of COVID-19 Immunization or Exemption Form (complete and return if exemption)
• Texas Medical License (Physician in Training Permit) (we will receive this)
• DEA (Institutional) (we will provide this)
• The Letter of Good Standing from UT Tyler Health Science Center must state that all immunizations, background check, drug screen and program status are satisfactory as in previous years (RFM Residency Program will provide this)
Tyler Family Circle of Care Documents
• Fitness for Duty (complete and return)
• Practitioner Data Record (complete and return)
• Program Type Application (complete and return)
• Resident Information Sheet (complete and return)
CHRISTUS TRINITY MOTHER FRANCES ROTATION
The following documents are needed from the PGY-1 Residents:
• Copy of CV
• Copy of PIT Permit or Medical License (we will receive this)
• Copy of Medical School Diploma
• Copy of ECFMG Certificate (if applicable)
Letter of Good Standing:
• Copy of Immunizations Record and Proof of COVID-19 Immunization or Medical / Religious Exemption Form
• Copy of ACLS/BLS Certifications
• Institutional DEA (we will provide this)
• Proof of Malpractice Insurance COI (you will sign during Institutional Orientation)
• Letter from home program – must include verification of the following: program start and end dates, good standing, and that a background check was completed (RFM Residency program will provide this)
CTMF Documents
• CTMF Onboarding Documentation (complete and return)
• CTMF Visiting Resident Application (complete and return)
IMPORTANT REMINDERS
• As soon as you receive your medical school graduation diploma, submit for your PIT.
• Federal Policy: You must bring the original copy of your current driver’s license, social security card or passport if you have it within the first three days of orientation.
