Know your diabetes

I am having diabetes since 2005. My blood sugar was under control when I did the blood test in January 2009. Fasting blood sugar was 106 mgs and postprandial was 182 mgs. I do regular walking and follow strict diet plan every day. When I checked yesterday, my blood sugar level increased to fasting of 190 and postprandial of 256 mgs. This the common problem encountered by many patients. Diabetes is a chronic (long term) disease. Mostly diabetes is progressive disease, sometimes it regresses too. Since diabetes is a progressive disease, blood sugar also rises over a period of time even though diabetes patient regularly follows diet and medications. And every day we have different variety of foods and different type of physical activities, blood sugar varies according to that also. So that only, we advocate diabetes patients to do regular blood sugar checkups once a month and HbA1c once in three month. If one does regular blood checkups, we can detect the increase in blood sugar values early and treat it appropriately. It is always advisable to follow regular diet at regular timings along with regular medicines to avoid fluctuations in sugar levels and avoid complications. And behind a differently-abled person who is confident and completely independent? His orthopaedician, in all probability! Vadapalani Times on a chat with Dr. Jayakumar, who works w i t h d i ff e r e n t l y - a b l e d people and was given a State Award for his work. Doctor, could you tell us about the State award that was recently presented to you? I work at the Govt. Institute of Rehabilitation Medicine, K.K.Nagar. I was given a State award for special services to the handicapped people. What are the major causes for the disabilities? Well, we still have polio affected people who fell talented bassist (Loyola victims to it many years College) and Bharath, a ago. Presently, we see a lot self-taught keyboardist of spinal cord injury and ((Rajalakshmi Engineering cerebral palsy which results in disabilities. Road College). victims to the hospital within the 'golden hour' help prevent them from being permanently disabled? Definitely. It can greatly increase their odds in favour of escaping without a permanent disability. For example, when a finger or hand is severed, the patient has to be brought to the hospital within 6 hours. In case of a brain stroke, a clot is formed in the blood vessels. There should be no delay in bringing the patient to the hospital. Some people think they could take the victim to the hospital as the first thing on the next day. That will not help. Only if they are given medical attention within 6 hours, the clots can be dissolved and the one-side paralysis can be prevented. How adequate the technology that we are currently using for rehabilitating the disabled persons is? I would say we are still using slightly primitive techniques when it comes to artificial limbs. By studying the gait patterns of patients, we can give them appropriate appliances. We have requested the government for a 'Gait Lab' to do this. Artificial limbs can also be made more light-weight than they are now. Also, stem cell research is very essential in treating spinal cord injuries. R&D definitely needs to be improved.

What is the personal goal that you are working towards in dealing w i t h d i ff e r e n t l y - a b l e d persons? For me, it is about making them self-dependent. Job opportunities for them have to be increased. Private companies should be motivated to employ them. If a person has lost his legs, he can still be an efficient worker if only his hands have to be used in doing something. We have to convince employers of this. Coordinating with NGO's, patients can be trained to handle their disabilities better and also more job opportunities can be created. For a person who suddenly finds himself with a d i s a b i l i t y, i t c a n b e extremely difficult to accept it. How do you make people overcome this? To motivate patients and help them believe that they can still be independent, we have psychological counsellors at our institute. We once had a patient who was a mridangam artist. Unfortunately, he lost one of his hands. However, by giving him an artificial limb and the psychological support that he needed, he was encouraged to try playing the mridangam again. Today, he does just that. Psychological counselling can create a huge positive influence on the patient's response to treatments.
