• In the early days of the industrial era, there was no such thing as standardization. Each machinist turning out nuts, bolts, and screws made them in whatever size they wanted, and none of the designs ever matched up with the othersʼ. In 1841, an Englishman named Sir Joseph Whitworth introduced a new precision-made screw. It was recognized for its superior quality and within a short time it was in use throughout the world. Whitworth also proposed that all screws have a set number of threads per inch for various diameters, a suggestion which was adopted throughout Britain. The Whitworth screw was the first step towards standardization of screws.

Sellers’ new screw threads sloped at a 60 degree angle, the exact angle of an equilateral triangle, and a far easier angle for the machinist to achieve. • Whitworth’s screw threads had rounded tops and rounded valleys, but Sellers’ had flat tops and flat valleys, which made them easier to catch in the threads of a bolt. The Whitworth screw required three kinds of cutters and two kinds of lathes to manufacture, while a Sellers screw needed just a single cutter and only one lathe. The Sellers screw was clearly superior, and Sellers also understood the need for standardization. He was determined to ensure that his screw became the standard. • Members of the Franklin Institute were sold on the Sellers screw, but other manufacturers knew standardization would put them out of business. Sellers set out to overcome that opposition. First he approached four of the largest machine shops in the eastern U.S. and convinced them to begin using his screw design. He then convinced the U.S. Navy to adopt it, and next the Pennsylvania Railroad, due in no small part to his membership on their board.

Frosted Light Bulbs • Marvin Pipkin went to work for General Electric in 1919. Only 30 years old at the time, he had already invented the charcoal filter for gas masks during his service in World War I.

Neighbors Helping Neighbors with Social Security Issues Q:

I have a young 30-something neighbor who stopped working a few years ago to raise a family. She told me that she got a letter from the government telling her that if she doesn’t return to work soon, she will lose all the Social Security credits she earned while she was working. Because I am getting Social Security, she thinks I am an expert on such matters. But I haven’t a clue what to tell her. Can you help?

qualify for such benefits just because you have a disability. Instead, you are eligible if you have a disabling condition that keeps you from working. Because Paul is working, he is not legally disabled. Even if he wasn’t working, the fact that he is over age 66 means he can’t get disability benefits. Once you reach that age milestone, you can only get retirement benefits. But that brings up another possibility. Paul might be eligible for Social Security retirement benefits IF he has been paying Social Security taxes. You said he’s worked for the feds for about 33 years. Anyone hired by the federal government after Jan. 1, 1984 has been covered by Social Security. If he was hired before that date, he is probably paying into the Civil Service Retirement System, not Social Security. If he is covered by Social Security, he could sign up for retirement benefits tomorrow. Once you are over age 66, you can get Social Security checks even if you are working full time. But if he is a CSRS employee, he won’t be able to sign up for his federal pension until he retires. You asked if they can get SSI. The answer is no. Supplemental Security Income is a federal welfare program that pays a small monthly stipend to the elderly poor (and to indigent disabled people). As a general rule, if an older couple already has income over about $1,100 per month, they won’t qualify. I’m not sure what Mary’s Social Security check is. But as a GS-3 (GS is the pay scale for federal employees), Paul is probably making about $24,000 per year. So that is well above the SSI income threshold.

Time to Replace Heart Valve Again? DEAR DR. ROACH: I had mitral valve prolapse for many years. In 2005, I got an infection, and it ended up as endocarditis. My mitral valve was not able to be repaired. I got a porcine valve. I am starting to have a little problem with my heart. Have you heard of a doctor doing a second replacement? My cardiologist says it is a really big deal. -- J.L.F. ANSWER: Endocarditis is an exceedingly serious infection of the heart valves. It is much more likely in people who have damaged heart valves, such as from rheumatic fever, but it also can happen in people with normal heart valves. Endocarditis is treated with antibiotics, almost always intravenously to begin with, but complications such as an abscess or heart failure may require surgery, either repair or a valve replacement. I have seen many cases of replacement of a damaged prosthetic valve. As your cardiologist suggests, the risk of the operation is high. However, if the valve is very damaged, you don’t really have any other options. It is clear that the decision to replace a prosthetic valve is difficult and should be undertaken only if other treatments have failed. An experienced cardiac surgeon, hospital and entire team is critical. You didn’t say what the new problem with your heart is. A recurrence of infection is fairly common, since artificial valves, both bioprosthetic (like your porcine valve) and mechanical, are at higher risk of becoming infected. Consultation with an infectious disease expert certainly would be appropriate, if that were the case.

Bipolar I disorder is diagnosed when there are manic episodes. A manic episode is defined medically as an abnormal, persistently elevated mood with increased activity or energy, lasting at least a week. Feelings of increased self-esteem, decreased need for sleep, being unusually talkative, having racing ideas and distractibility are common. Spending sprees, sexual indiscretions and foolish investments sometimes happen during these episodes. In bipolar II, true manic episodes are not present. Depression always is, as is at least one episode of hypomania, which is similar to manic episodes but less severe or long-lasting. Bipolar II is probably underdiagnosed, partly because hypomania can be hard to recognize. It is important to make the diagnosis, since treatment is different, and it is worth reconsidering the diagnosis if treatment is ineffective.

Physician Ambassadors The Department of Veterans Affairs has a new program: the Physician Ambassador Program, which pairs volunteer medical personnel with VA medical centers and clinics. Since they will be volunteers, they’ll be working for free. These won’t be the nice ladies who bring around a cart of magazines. These will be licensed and trained physicians and clinicians for mental health, cardiac care, emergency medicine, primary care, urgent care, rural care and more. They’ll work alongside the health teams in any of the 1,700 VA facilities. And did I mention that they’ll work for free? I have questions: --Who is going to check their credentials? --In the event of a dispute, who does the veteran patient turn to if the medical person in question isn’t part of the VA? --Who has the authority to hire/fire a volunteer? Each volunteer will have the credentials, but we know that doesn’t actually mean anything when it comes to the level of care given. Since they’ll be volunteers, will they answer to the Voluntary Services Office, the same department that provides the magazine cart lady? --How is this new program different from the one already in place? For example, a partial list of ambassadors needed at a few of the VA medical centers includes taking lab and blood samples, working in the nursing unit, assisting in the dialysis unit, helping in the ER, assisting with patient care and pharmacy support. --If the plan is to use volunteers in lieu of hired (paid) staff, how will it impact the level of care if volunteers don’t show up? Also included with the mark are the words “East Germany -- Handmalerei.” The height of the bowl is over 13 inches, and the diameter is almost 10 inches. It is reticulated to make it look like a basket and decorated with raised pastel flowers. On the base are a boy and a girl on a seesaw, along with raised flowers. It is in mint condition. I hope you can provide information on its histor y and value.

The 1905 steamship ocean liner restaurant menu in this photo belonged to my great-grandmother. It was found in an antique chest that has been passed down in our family. According to family histor y, she would have been around 12 years old when they were traveling home from China on a North German Lloyd ship. Featured in color on the menu are illustrations of the Haghia Sophia and an obelisk in Istanbul. It was the lunch menu for Dec. Vintage ocean liner menus 12, 1905. He tinkered for weeks on his project until he finally made one that would actually work. • Peter also worked part-time at the local Sears store, so he took his new wrench invention to show his boss. His boss showed it to his boss, who showed it to all the company executives. A year went by and Peter never heard back. • Then a lawyer came to visit Peter. He said Sears was prepared to offer him two cents for every one of his ratchet wrenches they sold, with the stipulation that they would not owe him any royalties after he reached the $10,000 mark. The attorney said that sometimes inventors get far more money for their inventions than they are really worth. He said it was going to be difficult and time-consuming for Sears to manufacture the wrenches, and that it might take Peter years to collect the maximum amount of the royalties owed him. The lawyer then offered Peter a flat $10,000 payment in return for him signing the patent over to Sears. Not knowing any better, young Peter agreed. • Peter then joined the Air Force. While stationed in England, he saw a Sears catalog featuring a full page ad for his rachet wrench. Suspecting that he had been taken, Peter hired a lawyer, who took the matter to court. The lawyer argued that Sears was guilty of fraud by leading Peter to believe that they would only be able to sell a few wrenches per year, when in reality they were selling millions of them. He also pointed out that Peter had been a minor when he signed over the patent contract. • Sears' lawyers fought back, claiming no wrongdoing in the matter. Litigation continued for more than twenty years until a federal jury ultimately found that Sears had fraudulently obtained the rachet wrench patent. Sears was ordered to pay Peter $5 million in damages, an award that was later increased to a settlement of $8.2 million. �
