Vaya Health Authorization Guidelines

Medicaid 1915(b) Research-Based Behavioral Health Treatment Services

for Autism Spectrum Disorder

Services must be in the provider’s contract with Vaya Health (Vaya). Acronyms are defined below. For services that do not require authorization, a concurrent request must be submitted if the maximum (if it exists) is exhausted. Guardianship paperwork is required as applicable for all services. Benefit limits may be exceeded for Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) requests.

Behavior Identification Assessment (CCP 8F)

• Initial: SAR; SO; clinical documentation by MD, DO, or psychologist validating diagnosis of ASD using scientifically validated tool(s)

• Concurrent: SAR; treatment plan; SO

• Note: 32 units required prior to starting RB-BHT for six months

• Initial: 32 units every six months

• Concurrent: maximum of 32 units every six months

Observational Behavioral Assessment and Follow-Up (CCP 8F)

Initial and concurrent: SAR; FBA; treatment plan; SO (annually)

Initial and concurrent: 16 units per month for six months or until end of plan year

Direct Intervention by Paraprofessional (CCP 8F)

Initial and concurrent: SAR; FBA; treatment plan; SO

Initial and concurrent:

• 32 units per day, 160 units per week, 690 units per month

• 160 units per week, combined with 97154, for six months or until end of plan year

• 16 units per day; 32 units per week; 136 units per month
