UPDATED RETAINER PAYMENT RATE/REQUEST FORM One temporary flexibility permitted by CMS under the DHHS Appendix K Emergency Preparedness and Response Waiver allows Vaya Health to provide retainer payments to Innovations Waiver service providers and Employers of Record (EORs) for their direct support professionals who normally provide habilitation and personal care services to a Vaya plan member. Vaya previously provided retainer payment reimbursement at 60% of the service rate. We will now provide this reimbursement at 100% of the service rate, retroactive to the start date of the Appendix K retainer payment program. We will not issue new approval letters to network providers. Learn more: COVID-19 Appendix K Retainer Payment Request Form * *The updated form is for new submissions only. When completing this form, please note that both North Carolina’s statewide Stay At Home order and local (county) stay-at-home orders are considered to meet the definition for “sequester” set forth in Appendix K.
