The 2020 Community Needs Assessment Survey is Now Open Vaya Health is conducting our annual Community Needs Assessment Survey now through Oct. 30, 2020. We want to hear what you think about public mental health, substance use and intellectual and/or developmental disability services in western North Carolina. Your responses will be anonymous. Vaya will use the information you share with us to help us plan for future services and improve care in the 22 counties we serve. This short survey takes approximately 10 minutes to complete and is available on the Vaya website at www.vayahealth.com/cnas . Thank you for helping us support healthier communities throughout western North Carolina!
Medication Administration Record (MAR) Request Policy The individualized care plan is designed to help members meet their personal health and wellness goals and prevent both behavioral health and medical crises. Having up-to-date medication information, including complete and accurate Medical Administration Records (MARs) for each member, is critical for the development of a member’s care plan. When scheduling a member’s annual Health Risk Assessment (HRA) interview, a Vaya care manager will request the member’s MAR. The provider will have five business days from the receipt of the request to fax or securely email the requested MAR to Vaya. All Vaya network providers are required, per Section 2.14 of the standard Vaya Provider Contract, to make clinical, medical, financial and other records – including MARs – available upon request. If you have questions about Vaya’s MAR policy or an individual member’s MAR, please contact the member’s care manager. To further assist you, a short training module is available in our Provider Central Learning Lab.
