Saying Goodbye By Stephen J. Ezzo, MD
I
t is seldom a good thing to enter an exam room and unexpectedly find everyone crying. You are rendered somewhat discombobulated, as your mind is faced with several options. Part of you wants to hold them and give reassurance, even though you don’t know what has brought on the tears. Part of you wants to cry along with them. And part of you — let’s be honest here — wishes a wormhole miraculously would open up so you can slip into it and return later at a less emotional time. So imagine my confusion and worry a few months back when one of my long-time patients was scheduled for a well-exam and I had not even said, “Good morning,” when her mother sobbed as she hugged me. “It’s our last visit with you,” she explained. “Why? Are you moving? What’s wrong?” My head was spinning. “She’s 18 and now has to find another doctor. We don’t want that.” Relieved, I took a deep breath, let out a small laugh and reassured her that was not the case, and I would be there for them until she turned 21. Most likely, what I said was something along the lines of, “Sorry. But you’re stuck with me for another three years.” The visit improved greatly after that, but in reality it only delayed the inevitable. Three years from now I will have to say good-bye, and that will not be easy. It is an aspect that is unique to my specialty. All my patients at some point are going to leave, and often many of them will be healthy and not want to go. Nor do I want to send them off. Early in my career it was not so difficult, as I had cared for most of them only a few years. But now, the ones who are aging out have been mine from the start. Often I held them in my hands on the first day of life. Seeing them depart is not sweet sorrow, just sorrow. From a purely clinical standpoint, I know it is the right thing to do. I have not cared for an adult patient in more than 30 years, and I am fortunate to have so many excellent family and internal medicine colleagues who will provide a seamless transition of care. Still, I am never in a hurry to make the hand-off. I never liked the idea of 18 years as a transition point. This is a time of upheaval as teens are either off to college or to the work force (or both). With all the changes, responsibilities and independence I thought it unfair to require a new physician as well. When you come home sick on college break the last thing you need is a new, strange place to receive care. Moving out to 21 allowed my patients to have another safe, comfortable place to come home to. For me, the rewards of caring for this age group are immeasurable. I do not deceive myself into thinking the care I provide is better than anyone else. No, the benefits I reap are mainly selfish. While I find joy in all my patients at any age, every now and then it is nice to have an adult conversation with someone other than the parents. I learn so much as I listen to them tell me about their classes, their work, their hobbies. If I am lucky, at times I can relate my own similar experiences, which I believe allows my patients to see me less as a doctor and more as a person. For them to hear
I struggled with physics, calculus, or Beowulf in Middle English breaks down a lot of barriers. So do I bend the rules? You bet I do. Sometimes it can take a while to get established with a new physician, so I let them know I will be available during the transition phase. At times, one of my 21-year-olds is close to finishing college, so I will hang on to them until they graduate. For many of my patients in college, I have found that upon graduation they are ready to move on. As you can see, I am somewhat of a pushover whose use of the word “no” is rather limited (which is one of the reasons I sit here writing this as MCMS president for the last two years). It’s difficult to square this fact with the paisano tough-guy image I strive to present to the world. Maybe the way to look at this is not so much refusal to let go as being protective and not wishy-washy. To my patients as they enter adulthood: Thank you. Thank you for the privilege of caring for you. It has been my pleasure. Thank you for putting up with my bad jokes, my eccentricities and my other duties that at times limited my availability. Thank you for teaching me about subjects I had little concept of. Thank you for realizing I am human and would not get everything right. I hope that you take some small part of me with you as you leave. I am proud of each and every one of you. Remember the words of Dr. Benjamin Spock, “Trust yourself. You know more than you think you do.” To those former patients who now have children I care for: I cannot adequately express what it means that you have put your trust in me. It is both flattering and humbling. Having this honor allows me to think I did some things right. I truly enjoy getting to know you again — this time as a parent and friend. What a great journey we continue on. While this internal debate will persist in me, I am intrigued by recent studies that indicate the human brain does not fully finish developing until the mid- to late-20s. Maybe the AAP will revise its age recommendation? “The pediatrician…is the legitimate adviser to the judge and jury, and a seat for the physician in the councils of the republic is what the people have a right to demand.” — Abraham Jacobi, MD

On paper, it makes sense that professional membership associations (across all industries) have been in decline over recent decades. Online access has opened up endless ways to find information and training opportunities; social media has allowed convenient ways for networking and made it easier to locate colleagues or groups of interest. Formal annual meetings and educational events attended in person can seem like too much effort in our digital 2017 lives. However, the current landscape of healthcare is changing rapidly for physicians and the patients we serve. As hardworking doctors in offices all over this county find they can barely make it through the ever increasing demands of their day, it has never been more important to find the time and energy to connect, unify, and find a neutral ground where all of our local colleagues can collaborate on how to advocate for ourselves, our patients and our community, regardless of hospital affiliation, work environment or specialty. This is why I am asking all physicians and physician groups in Mecklenburg County to join or renew membership in our local society. At MCMS, requests or ideas can be discussed and acted upon by receptive physician leadership that represents diverse voices in the community in which we live and work. Expertise ranges from non-invasive cardiology, to electrophysiology, interventional cardiology and vascular surgery. n
Non-Invasive Cardiology Tesfa Michael, MD, comes to Novant Health from the University of South Florida, where he completed training in advanced cardiac imaging. Dr. Michael received his medical degree from Gondar College of Medical Sciences in Gondar, Ethiopia, and completed his residency in Tesfa Michael, MD internal medicine at the University of Texas Southwestern Medical Center in Dallas. He then completed a fellowship in cardiovascular disease at Oregon Health & Science University Hospital in Portland. Michael practices at Novant Health Heart & Vascular Institute – Monroe. Sameer Chaudhari, MD, received his medical degree from Lokmanya Tilak Municipal Medical College in Mumbai, India, and completed his residency in internal medicine at Johns Hopkins in Baltimore. Param Vidwan, MD, received his medical degree from the University of North Carolina School of Medicine and completed a residency in internal medicine and general cardiology at UNC Hospitals. Vascular Surgery Richard W. Bock, MD, comes to Novant Health from Mission Health System in Asheville, where he founded its vascular surgery program 22 years ago. He founded Carolina Vascular and the Mission Richard W. Bock, MD Health Vascular Center and built its group of five vascular surgeons. He performed the first FDA-approved endo AAA repair in the U.S. and soon after became a physician trainer for Guidant, teaching endovascular AAA techniques to visiting surgeons from across the country. He completed seven years of general surgery residency at the University of Louisville under Hiram C. Polk, Jr., including one year of research in carotid artery disease in Sydney, Australia. He then served a two-year vascular surgery fellowship and faculty appointment at UCLA. He is a board-certified vascular and endovascular surgeon. Dr. Bock practices at Novant Health Heart & Vascular Institute – Surgery in Charlotte. Martin J. Carignan, MD, has been with Novant Health Vascular Specialists of Salisbury for two years and is now expanding his scope of care to Charlotte. Dr. Carignan is a board-certified surgeon with a fellowship in vascular/ Martin J. Carignan, MD endovascular surgery. He offers expert specialized care for patients with venous and/or vascular disorders, such as peripheral artery disease, aneurysms, renal artery disease, peripheral venous disease, varicose veins, blood clots in the veins and lymphedema. CHS and UNC Health Care Now Sharing Patient Data Carolinas HealthCare System and UNC Health Care have partnered to make patient information easily accessible for clinicians at both organizations by connecting CHS CareConnect to Epic@UNC, the health information exchange (HIE) at UNC Health Care. Our goal is that this collaboration will improve patient care by offering providers at both health systems a more complete picture of their patients’ health information. “Our patients’ health issues don’t know city, county or state boundaries, and neither should the exchange of their medical records electronically,” says Craig Richardville, senior vice president and chief information and analytics officer for Carolinas HealthCare System. What is happening? • This connection will allow clinicians to pull in data from more than 4 million UNC Health Care patients. • Similarly, UNC Health Care users will be able to view data on the more than 6 million CHS patients. Is this a result of the new organization announced August 31 by Carolinas HealthCare System and UNC Health Care? No, the technical teams at Carolinas HealthCare System and UNC Health Care already were working together before the August 31 announcement to make it easier to share patient data across the two organizations. n
Levine Cancer Institute Stands Ready to Address Ocular Melanoma Concerns Although ocular melanoma typically strikes five in one million people, nearly two dozen cases have been reported in Huntersville and the surrounding area over the past five years. In addition to being extremely rare, ocular melanoma usually affects males over age 60, making this cluster of largely young female victims a mystery that continues to stump local experts. The state has put up $100,000 for environmental testing and genetic counseling, but no specific cause has been found. n
While LCI is not directly involved in the study, several of the individuals diagnosed with ocular melanoma are being treated with chemotherapy at LCI. In addition, LCI stands ready to provide follow-up care for those already diagnosed, as well as address concerns by those who are worried about developing ocular melanoma. Of course, local ophthalmologists and optometrists are aware of the situation and are being vigilant in looking for signs and symptoms of ocular melanoma among their patients. Those diagnosed with ocular melanoma may be referred to Carolinas HealthCare System NorthEast or Carolinas Medical Center for free genetic counseling and free testing available to those within the cancer cluster study. Be a Sepsis Hero Michael Barringer, MD, a physician leader at Carolinas HealthCare System Cleveland, knows more about sepsis than he ever intended to. After years of treating patients with sepsis, in 2016 his daughter contracted the lifethreatening condition. Julie Johnson had just had her third child two weeks prior. Everyone assumed her lethargy and related symptoms were due to sleep deprivation. But her OB/GYN signaled her case may be more severe and sent her to Carolinas HealthCare System University, where the maternity unit immediately flagged her as highrisk for sepsis and activated the Rapid Response Team. Dr. Barringer then watched his daughter battle toxic shock sepsis in the ICU. Despite slim chances of survival, Johnson’s care team provided timely, coordinated care — delivering a “home run” case. Sepsis is a complication caused by the body’s overwhelming and life-threatening response to infection. It can lead to tissue damage, organ failure and death — and it can happen to anyone. It can be difficult to diagnose because it happens quickly and can be confused with other conditions. This medical emergency is deadly when it’s not quickly recognized and treated. n
In Loving Memory: Charlie Rich, MD It is with a heavy heart that we share the news that Charlie Rich, MD, passed away. Our thoughts and prayers are with his family and friends. Dr. Rich was a member of the Mecklenburg Medical Group for more than 25 years and was n

a true leader, working across many departments in our organization, helping to lead many innovative system programs while impacting and influencing countless teammates. Because of his widespread reach, we felt it important to share this news with you. Beloved by his patients and colleagues, Rich was a devoted internist who cared for patients at Mecklenburg Medical Group. His patients routinely shared their positive comments about him — and trusting, caring and kind were just a few of the many words they frequently used to describe him as a caregiver. In fact, he was often voted by his patients as “top” or “best” doctor in local and national publications. Always quick to share a warm smile and listening ear, Rich was known as a diplomatic statesman who was devoted to his colleagues and staff. He was a respected leader who served on many committees over the years, helping to shape our medical group as a “founding father” of sorts, given his tenure and many contributions. He also found time to serve as a mentor to aspiring doctors through several volunteer programs. Rich also was passionate about teaching and always made time for learners. He spent countless hours with medical students, residents and ACPs, passing on his knowledge and wisdom to the next generation of providers. Rich was a loyal friend and colleague to many in this organization and community. Many of us are grieving because of this unexpected loss, so please know there are services available for those who need support. Independent Physicians of the Carolinas
Independent Physicians of the Carolinas is a nonprofit 501(c)(6) membership organization whose mission is to create public awareness of medical doctors not employed by a network or hospital system and to provide educational programs and resources to physician members and their administration. Visit us at IndependentPhysicians.org. Independent Physicians of the Carolinas (IPC) recently celebrated its ninth anniversary. Colleen Dey, IPC Board treasurer, and Dino Kanelos, MD, founded the organization in the summer of 2008 and were soon joined by Sherie Bradshaw, Board secretary. The first meeting was held September 9, 2008. Review of the minutes reveals 12 doctors present, of whom seven remain independent. Two doctors joined hospital systems, two retired and one left the state. Eighteen doctors attended the second meeting and 14 remain independent. Over the ensuing nine years, physician membership has ranged between 68 and 103. The concern of many at that time was that independent practices would be taken over within a few years, however the state of independent physicians is not as grim as anticipated. We can only continue to grow with the joining of all Independent Physicians big or small, conservative or liberal, cash pay or managed care, north or south of Charlotte and across the Carolinas, newer or older, traditional or non, primary care or specialty, minority or non, local or across the states or seas. Do not let competitiveness with other independents’ services be a factor in coming together. Clinical services are not of discussion, and we do not view competition as among one another, but to simply be compounded in the IPC and exert competitiveness in the state’s healthcare market for a lower cost of care than what is documented at the hospital-owned practices. Going forward, our goals are restructuring for incorporating the organization to primarily make changes in legislation to make health care great again. It was a great success with more than 100 golfers and about 150 at the dinner. Charlie Petrizzo, Project2Heal founder says, “This event could only happen because of the charity and generosity of people like you. The sponsored pups were there to meet the golfers as they came to the event and were seen throughout the course, visiting the golfers as they played. They were a big hit.” Oncology Specialists of Charlotte (OSC) has partnered with DJL Clinical Research, a local investigative site network founded by two
independent physicians who participated in medical research and served patients in Charlotte and in Union County for more than 35 years. Currently, OSC is conducting two breast cancer trials.
