Patient Satisfaction:
Is the ‘Customer’ Always Right? By Scott L. Furney, MD
A
few days ago, a patient angrily expressed his dissatisfaction with the coordination of his care. His angry words still linger in my ears. The issue was that it took too long to complete a referral and have him seen by a specialist. There is some truth in what he was saying, of course. We have a responsibility to ensure the processes and outcomes of referrals are successful, even when we do not have them in our direct view and control. With my training in quality improvement, I can see the steps that need to be examined and the improvements to be made — but that is not the point of this letter. Patient satisfaction now is a fact of life for most physicians due to insurer mandates, and it is not without significant risk. When patients rate their medical care — directly impacting compensation— physicians and ACP’s will pay close attention. In theory, that is a good thing. It also can induce the risk of perverse incentive, in which patients’ desires for their care outweigh the best judgment of the physician. About a year ago, I saw one of my partner’s patients with a simple upper respiratory infection. I concluded it likely was viral and that symptomatic treatment, not antibiotics, was the best pathway. The patient clearly disagreed! “Do you mean to say I came in here for nothing?!” she exclaimed. I calmly replied that she came in for my expert assessment and advice on how to feel better quickly. She begrudgingly agreed, and I proceeded to take time to write out the over-the-counter medications she should find and instructions on how to use them for her symptoms. In these few minutes, I found something that could mitigate the risks of our worries about patient ratings of our care. If you are not convinced that patient satisfaction is a potentially risky thing to measure high-quality medical care, consider this. A national study of patient satisfaction published in the Archives of Internal Medicine in 2012 noted higher patient satisfaction was associated with: • 8 percent lower risk of ER visits. • 12 percent higher risk of inpatient admission. • 9 percent increased prescription drug and overall expenditures. • 26 percent increased mortality. While correlation does not prove causation, these results should give us pause on how we make our patients satisfied with their care. More is not always better, which is my interpretation of this study. Of the domains rated by patients in surveys, the one I choose to focus on for this letter is communication skills. We may be excellent diagnosticians and have elegant treatment plans, but if patients do not understand, they are unlikely to follow our advice. Of the many factors proven to improve communication ratings, the most successful are addressing patient agendas and basic communication techniques. The simple things often are overlooked. Introductions occur 50 percent of the time, eye contact 75 percent, and sitting down for discussion (not at the computer to document with your back to the patient!) is as low as 10 percent. One author commented, “When I sit down, patients sometimes get alarmed, thinking something is seriously wrong with them.” If the

basics are uncommon in practice, they are a good place to start. Back to my current conundrum. I am trying to take what I learned from the good recovery of patient satisfaction with the URI patient and similarly recover some of the momentum with the patient who is concerned about the referral process. Without blaming him, I reminded him I do care about his symptoms and the rapidity of the evaluation, but I cannot act on information I am not aware of. I asked him to notify me when there are delays, so I can act on them in a timely fashion. Engaging him in partnership and reminding him that we are a team seemed to mitigate some of the frustration. The fine line we walk is how to keep our patients satisfied while not giving in to the unnecessary antibiotic, tests and other demands they might believe are needed. Walking the fine line is easier if patients are on board with us, and engaged by our communication skills and empathy. We do not have to compromise our principles to have happy patients, but it will take time and skill to communicate. In humble service,

Started by the Mecklenburg County Medical Society in 2004, Physicians Reach Out is a network of more than 1,600 volunteer doctors, dentists and other healthcare professionals who see low-income, uninsured Mecklenburg County residents free of charge in their own offices. Other services, such as diagnostic testing, lab services and hospitalization are provided at a reduced cost. Through the generous support of our volunteer providers, more than 3,500 people received $17 million in donated care through Physicians Reach Out last year. The Smith Arthritis Fund was established in 1979 when Carolyn Kirkpatrick Smith donated $41,500 for arthritis research. This fund is administered through the Mecklenburg County Medical Society and is designed to support research projects as they relate to the needs of patients suffering from rheumatologic diseases. Each year, the committee awards about $1,000 per grant. There are no rigid restrictions. The Smith Arthritis Fund Committee of the Mecklenburg County Medical Society is now accepting grant applications from interested physicians through the end of June. Novant Health Becomes One of the First CMS Health Equity Award Recipients Novant Health has become one of the first healthcare systems to receive the Centers for Medicare and Medicaid Services (CMS) Health Equity Award. The national award, which was established in 2018, recognizes organizations that demonstrate a commitment to health equity by reducing disparities among racial and ethnic minorities, people with disabilities, sexual and gender minorities, and residents in rural areas. To receive this award, Novant Health had to demonstrate results in disparity measures related to, but not limited to, CMS Quality Strategy priorities over the past calendar year. These priorities include: • Making care safer by reducing harm. • Strengthening personal and family engagement. • Promoting effective communication and coordination of care. • Promoting effective prevention and treatment of chronic disease. • Working with communities to promote best practices of healthy living. • Making care affordable. Led by Tanya S. Blackmon, Novant Health’s executive vice president and chief diversity and inclusion officer, the healthcare system organized a comprehensive team to focus on health equity. Through its research, the team identified a disparity in the hospital readmission rate of African-American patients who were discharged with a diagnosis of pneumonia. They had a 4 percent higher readmission rate than other patient populations. The goal was to decrease avoidable readmissions, while also improving the patient experience. Through a variety of system changes, Novant Health closed the gap in readmission rates by 50 percent among its African-American patient population diagnosed with pneumonia. This project created a framework and blueprint that is being used for other health equity initiatives within the system and in the communities that Novant Health serves. “This honor recognizes the exceptional work being accomplished across Novant Health. Our organization has a strong focus on health equity and ensuring that patients, families, n
communities and team members receive high quality care and services when and where they need them, regardless of their ability to pay,” says Blackmon. “We’re confident that, in partnership with other organizations, we can make a significant impact on healthcare disparities, ensuring everyone has access to the excellent care they deserve.” Vincent J. Pompili, MD, Named Service Line Lead for Heart and Vascular Care Novant Health has appointed a new physician service line leader, Vincent J. Pompili, MD, as it continues to expand its heart and vascular capabilities. Dr. Pompili will lead heart and vascular services across Novant Health’s footprint, including cardiac, Vincent J. Pompili, MD thoracic and vascular surgery. Pompili will serve as physician executive director of the Novant Health Heart & Vascular Institute after an already distinguished career in leadership roles that includes some of the country’s most prestigious academic medical centers. He also will perform clinical duties in interventional cardiology and structural heart disease at Novant Health Presbyterian Medical Center in Charlotte and Novant Health Forsyth Medical Center in Winston-Salem. n
Scott M. Schneider, MD, Accredited for Center of Excellence in Minimally Invasive Gynecology Scott M. Schneider, MD, has been honored as a provider in North Carolina accredited for the Center of Excellence in Minimally Invasive Gynecology (COEMIG) by the Surgical Review Corporation. Scott M. Schneider, MD SRC develops and administers best-in-class accreditation programs for surgeons, hospitals and freestanding outpatient facilities throughout the world. The COEMIG program recognizes hospitals and surgeons around the world who provide exceptional minimally invasive gynecologic surgical care and are dedicated to continuously improving healthcare quality and patient safety. Dr. Schneider, a physician with Novant Health Bradford Clinic OB/GYN, has completed more than 125 minimally invasive surgical cases (more than 35 annually), and also receives more than 24 hours of additional training per year in his field. n

Charlotte Independence Extends Partnership With Novant Health Novant Health has re-signed as presenting sponsor of the Charlotte Independence soccer team through the 2020 season. Under the agreement, Novant Health will remain the exclusive provider of medical care to Charlotte Independence players, provide an athletic trainer, team primary care and sports medicine physicians (Kyle Pope, DO, and Karan Shukla, MD), team orthopedist (James Romanowski, MD) and onsite medical services on game days. Additionally, a team performance coach will be provided by Novant Health Sports and Performance Medicine powered by EXOS. “Novant Health is proud to expand its partnership with the Charlotte Independence,” says Jason Bernd, vice president of Charlotte Orthopedic Hospital. “It’s been an honor to support the team since they first kicked off their program in 2014. Girls on the Run Launches New Camp Program With Novant Health For more than 21 years, Girls on the Run Charlotte has provided life-changing after-school programming to girls ages 8-13. As a current sponsor of Girls on the Run Charlotte’s fall and spring program and their end of season 5K series, Novant Health is excited to further confirm its commitment to empowering local girls by signing on as the first sponsor of Camp GOTR. Launching this summer, the camp will provide an opportunity for girls to develop selfconfidence and learn life skills. Led by caring and qualified Girls on the Run coaches, this week-long camp is designed to address physical, emotional and social development in girls grades 3-5 through fun and engaging lessons. “As both a pediatrician and a parent, I have seen first hand how Girls on the Run positively impacts girls and families in our community,” says Catherine Ohmstede, MD, pediatrician at Novant Health Dilworth Pediatrics and pediatric service line leader for Novant Health in the greater Charlotte market. “I am thrilled Novant Health is working with our local Girls on the Run to launch this fun and engaging summer camp program, helping our girls develop selfconfidence and learn life skills even when school isn’t in session.” n

Have a Migraine? There’s an App for That Are you one of the millions of people who suffer through recurring migraines? Russ Bodner, MD, an Atrium Health neurologist, discusses how patients can manage their migraines using the MigrnX™ app to better understand their frequency patterns, the triggers behind their symptoms, medication effectiveness and the severity of their migraines. n
A headache isn’t always a migraine So, what is a migraine? “A migraine is a type of headache that is disabling in some fashion — it inhibits someone’s level of functioning,” says Dr. Bodner. The most common symptoms include one-sided throbbing pain, light and sound sensitivity, and nausea. In certain cases, an estimated 15 percent of migraine sufferers experience a migraine with an aura. People who experience this type of migraine associate the aura with visual disturbances, such as seeing sparkles or experiencing tunnel vision. To combat these symptoms, consult your doctor about medication options. Medications used to treat migraines fall into two broad categories: pain-relieving medications (also known as acute or abortive treatment) and preventative medications. “Typically, patients would want to use an abortive agent — a medicine they would take when the migraine is coming on,” says Bodner. “If people are having lots [of migraines], we look at preventatives.” Introducing MigrnX™ The good news for migraine sufferers? The MigrnX™ app is here to help. MigrnX™, released by SensorRX in 2017, is the first automated migraine management tool that directly impacts patients’ care. More than 500 patients at Atrium Health already are using the app as an electronic version of a migraine diary, which provides clinicians with the data necessary for better treatment. “Atrium Health and SensorRX share the largest clinically-integrated electronic
database (clinical integration is key to treatment), which is increasing by thousands of headache reports each month,” says George McLendon, PhD, founder of SensorRX and vice president of therapeutic research and development at Atrium Health. In just a few clicks, patients are able to electronically record when they are having a migraine, the symptoms they are experiencing and the severity. They are then prompted to check off the medication they are using and how it affected their migraine. The app also allows patients to record other possible triggers, like if they are menstruating, are not getting enough sleep or have eaten something outside their usual diet. Even further, the app records environmental triggers that may affect migraine patterns. “The app automatically is recording the barometric pressure, the weather, the light in the room, and sound, to detect environmental triggers,” says Bodner. But that’s not all. Every three months, the app automatically prompts users to participate in a routine migraine disability assessment (MIDAS) test to determine the extent to which migraines are affecting everyday life, such as asking whether or not they have missed work/school days or are lacking in productivity. The history of past assessments are stored within the app and users are given the option to share the results with their physician by either printing a PDF report or sending their records via email. How do I get the app? “Right now, all Atrium Health neurology staff can give the app to patients they feel have an appropriate need,” says Bodner. Patients can download MigrnX™ for free from the App Store or Google Play, but they need to consult their physician for an access code in order to log their information. From there, patients can record their migraine information in just four clicks. MigrnX™ then stores all recorded information, allowing users to view their headache archive on a calendar to help them better identify headache frequency and associated patterns. “An ongoing collaboration between SensorRX engineers and Atrium Health has created an integration where patients may share data with their provider directly into the electronic medical records (EMR), ensuring the best diagnosis and care,” says McLendon.
First Septic Shock Treatment Saves Child’s Life This February, Atrium Health’s Levine Children’s Hospital became the first clinical trial site in the world to test a new medicine for children that can stabilize their blood pressure in life-threatening situations. For 8-year-old Lucas Judy, the timing couldn’t have been more perfect. Just weeks after LCH was approved to participate in the trial, Lucas was clinically diagnosed with the flu. With an uncontrollable cough and fever, and increasingly shallow breathing, he was admitted to Levine Children’s Hospital. Within 24 hours, he was transferred to the pediatric intensive care unit. A potentially deadly combination of influenza, pneumonia and a bacterial superinfection had caused Lucas to develop septic shock. He was put on a ventilator and given two special medications (vasopressors) to maintain his blood pressure, which had fallen to dangerously low levels. Enter LJPC-501. Developed by La Jolla Pharmaceutical Company, LJPC-501 was approved last December to increase blood pressure in adults with shock who have low blood pressure despite receiving current standard-of-care therapy. But it had not yet been tested in children. Because Lucas’s condition fit the requirements for this investigational drug, LCH doctors thoroughly explained the study to his family, and they consented to his participation. Within three hours of taking LJPC-501, Lucas’s condition had stabilized, and he was able to come off all vasopressor support. “Lucas was an extremely rare scenario,” says Dwight Bailey, DO, specialty medical director of pediatric critical care at LCH, who treated Lucas. “But even though there are relatively few patients who don’t respond to the standard of care, we believe we can save lots of lives with this therapy.” As the study lead for the trial, Dr. Bailey is hoping to enlist 10 additional pediatric intensive care units across the country to enroll about 30 more patients. LJPC-501 has yet to be approved for children outside of the study. The first child to receive this medication for his condition, Lucas is expected to make a full recovery. n
