Lifestyle Change: Expanding Our rEach A program to study and treat prediabetes
Metamorphosis — noun: A profound change in form from one stage to the next in the life history of an organism, as from the caterpillar to the butterfly.*
Since its inception in 2010, Rush Oak Park Hospital’s Project Lifestyle Change has transformed hundreds of lives. In addition to providing free prediabetes screenings to nearly 800 people, the education and prevention program has diagnosed and enrolled hundreds of people with prediabetes. All have made a metamorphosis: “Once I got over the shock, I really appreciated having the information about my health that would allow me to do something about it.” “Just getting together with other people that are in the same boat was very helpful.” “My A1C was 5.8. Now it’s 5. I lost 12 pounds. I’m happy.” “The program is like here it is: This is what can happen and this is how you can prevent it. I thought it was excellent.” Because of this tremendous success, the program continues to spread its wings to screen, educate and change lives, now in partnership with the West Cook YMCA. Published annually by the Forest Park Review and the Chamber of Commerce and Development, this guide contains all the information you need to learn everything you need to know about Forest Park. Forest Park has long been known for its small-town charm and community spirit. As you explore Forest Park and talk with community leaders, merchants and residents, you’ll find a cooperative spirit and an enthusiasm that is hard to resist. That, along with the dedication and hard work of many, is just part of what makes Forest Park the perfect place to live, work, shop, dine and play. Charming antique stores, specialty shops and unique boutiques, along with an endless choice of casual- and finedining options make up our vibrant downtown. From Lemonheads, fine wine, and amazing ice sculptures; to LAURIE KOKENES gourmet chocolate, vintage décor, and an old-fashioned community bank, Chamber of Commerce and Development Director Forest Park has something for everyone. Have your dog groomed, get a family portrait, enjoy a massage, or browse the record stores for buried treasurers. Find what you need for any project at our old-fashioned hardware store, or buy a fabulous pair of shoes at one of our unique boutiques. After shopping, enjoy dining at one of Forest Park’s many eateries. For dessert, there’s delicious homemade ice cream in a traditional ice cream parlor. The ‘best in the west’ aquatic center, and community events like the Wine Walk & Shop, Holiday Walk & Live Window Display, Groovin’ in the Grove and Ribfest make Forest Park a perfect spot for entertainment and recreation. There is also an abundance of history here. The Haymarket Martyr’s Monument, Michael Todd’s grave, and Showmen’s Rest mark just a few of the historically significant gravesites in Forest Park’s many cemeteries. The nearby Checkerboard Air Field (now Miller Meadow) was regularly used for mail runs, and it brought Charles Lindbergh to Forest Park. Parichy’s Bloomer Girls, featured in the movie A League of Their Own, played here too. Don’t miss our fabulous 3rd Annual Casket Races October 25, 2014. Good clean fun as local teams try to beat each other in their wheeled coffin-creations. Forest Park is also just 10 miles west of downtown Chicago and is easily accessible from Interstate-290 and the multiple public-transportation options. The Chamber is a partnership of local business people, civic leaders and community-minded individuals working together to promote business and a sense of community. New Residents Guide Driver’s License To receive a driver’s license or ID card, you must provide a minimum of three acceptable forms of identification to prove name, date of birth, Illinois residency, Social Security number, and a signature comparison. One of these forms should be a certified birth certificate. The nearest Secretary of State driver licensing facility is located in Melrose Crossing, at the corner of Mannheim Road and North Avenue. School Registration
Parking Permits
Students should be registered at the public schools prior to the first day of class, preferably during the dates assigned each year for each of the neighborhood schools. Proof of residency — including tax bills, mortgage statements or leases, as well as driver’s license, insurance papers or utility bills — is required. All new registrants must provide birth certificates. For complete information, call the District 91 administrative office at 366-5700.
On-street parking is prohibited in the village of Forest Park from 2 a.m. to 5 a.m. Permits are available for $25 per month for night-only parking, and $40 per month for 24hour parking. There are several lots throughout the village and spots are first-come, firstserved. Permits may be obtained through village hall. For more information call 3662323. Residents with overnight guests must call in plates to 366-7272 to avoid being ticketed.

Executive Director Forest Park Chamber of Commerce How long have you lived/ worked in Forest Park? My work life in Forest Park began in 1975 when I joined my father at the Forest Park Review. I did everything from typesetting, ad sales and design, and layout, to writing, billing and janitorial chores. Eventually my dad honored me with the title of General Manager. I grew up in Westchester, Illinois and graduated from Proviso West, but at the ripe old age of 18 when I realized I knew more than my parents, I moved to an apartment on Troost Avenue in Forest Park with a friend. What brought you here?

Although I reside in Brookfield, Forest Park feels like home to me. My dad’s love of this village attached itself to me and I’ve never been able to shake it What gets you fired up in the mornings? Long-time friendships and
new relationships being built every day, plus the opportunity to contribute a small part in Forest Park’s continued growth gets me up in the morning and keeps me coming back. What keeps you up at night? I can worry about anything from the simplest, to the most serious of issues that someone I love is going through. Oh, and money, too. If the Chamber will have me, I’ll probably be here until I’m 80! What do you love about Forest Park I think the thing I love most about Forest Park is accessibility. I’m not referring to its easy access by car and public transportation -- but the ability to actually ‘reach out and touch someone’ when you need to. Whether it’s the mayor, park or school superintendent, police or fire chief or a local merchant they’re pretty much just a phone call or email away.

What brought me here? Sister Mary Fearon, who taught at St. Bernardine School and eventually directed the Religious Education Program called me in the spring of 1987 when she knew she was leaving the parish and asked if I would consider coming to St. Bernardine’s. After an interview with the pastor I was invited to join the staff. What gets me fired up in the mornings? I am definitely a people person and my position gives me many opportunities to meet, work with and support a variety of wonderful people. My favorite morning of the week is Sunday - the day we have the Religious Education
program because our devoted catechists (teachers) bring a contagious spirit with them into the classroom. They truly live what they teach and their care and concern for the students is genuine. What keeps me awake at night? A late Sox game!!! And “Words With Friends.” I am truly a night owl. What do I love about Forest Park? Through the years I have met many wonderful people both in St. Bernardine’s parish as well as in the larger Forest Park community. Since I live alone I have come to value all of them as my extended family! And for this I say “thank you” from the bottom of my heart.

Mohr Community Center, named in honor of the late state senator and Forest Park mayor, serves as a hub of both entertainment and social services. From supervising children before and after school to sponsoring weekly senior citizen trips to checking up on the elderly, disabled and shutins, the community center meets many needs. Adult and Youth Job Corps Businesses and individuals who are in need of help can contact the center with a list of available jobs, such as cleaning, babysitting, etc. Applications are available at the center for individuals who are seeking employment.

When people come into the library, especially downstairs in the youth area, I love seeing the parents who recognize kids who attend school with their kids, or the ones who recognize kids they attend church with or who are sometimes at their homes visiting their children. It reminds me that outside of these walls, there are people who interact with each other and know one another and actively work towards a feeling of community.

Pace Bus Passes Pace bus passes are available to adults at the rate of 11 passes for the price of 10. In addition, reduced fare rates for the passes are available to seniors and students.
Parent and Tot Parents and children up to 4 years old are invited to enjoy two hours of fun Wed., 9-11 a.m. A snack is included. There are both indoor and outdoor activities. Come and enjoy your children’s playtime as well as the company of other parents. Registration is not required. Cost is $2 per child per week.
Rules of the Road Review Class Offered periodically, the purpose of the course is to help citizens pass the Illinois drivers license renewal examination. Call for dates and times.
Senior Activities and Trips The center offers a number of activities and services for seniors including Wed. trips to a variety of venues (theater, sports, games of chance, special exhibits.) Informational and personal safety seminars are also scheduled throughout the year.

Senior Citizens Club Meets Tues. 9 a.m.-1 p.m. at the Community Center. Offers social activities for seniors including arts and crafts, card and board games, visiting with old friends and making new ones. Business meetings are held the first Tues. of the month, Sept.-June. The club welcomes members from surrounding communities. Transportation is available to Forest Park residents by appointment.
Senior Income Tax Preparation The American Association of Retired Persons will be at the center on Tues. mornings to assist senior citizens with income tax preparation Feb.April. This free service is by appointment only. Transportation is available by appointment.

State of Illinois Mobile Drivers Service Available at the center, this service issues state IDs, renews driver licenses needing eye and/or written tests only, for individuals who are within a year of expiration. Also changes information such as new address. Issues license plate renewal stickers (must have renewal form.) Road testing is not available. Current fees apply. The program is open to anyone younger than 75. Those older than 75 must go to a secretary
of state facility.
Summer Clubhouse The Community Center offers nine weeks of summer daycare for children entering grades K-6. Creative projects, free play, weekly field trips (additional cost), swimming, structured games, indoor and outdoor sports. This program provides summer adventures that promote a healthy mind, body and spirit. Call for dates and fee information.

3. Belle Gunness (Forest Home) [1859-?]. Landlady, NorwegianAmerican serial killer. Thirtyfour men, and their cash, disappeared soon after arriving on Gunness’s farm and later human remains were found buried in the hog pen. She is also said to have killed a number of children, including her own.
orest Park has over 680,000 dead and 15,000 living, giving it the Guinness World Record for having the greatest ratio of dead to living. Here is a list of some of the more notable persons interred in the village’s vast cemeteries. They include capitalists, artistic types, anarchists and disaster victims.
1. Haymarket Martyrs (Forest Home Section N) Executed: November 11, 1887. Adolph Fischer, George Engel, Louis Lingg, Albert Parsons and August Spies were falsely convicted of conspiracy in connection with the deaths of seven police officers at Haymarket Square.
6. Eastland Victims (Concordia) July 24, 1915 844 passengers and crew died when the Eastland capsized in the Chicago River. Sixty of the victims are interred in Concordia cemetery.
9. Emma Goldman (Lot 1044, Section N) 1869- 1940 Goldman was an anarchist lecturer, leading feminist and an early advocate for birth control. Her activism led to her being deported from the US in 1919. She remained a radical to the end, requesting to be buried on Radical Row.

5. Abraham J. Balaban (Jewish Waldheim) 1889- 1962 His firm built Chicago’s most famous movie palaces, including the Riviera, Uptown and Chicago theaters which hosted the Marx Brothers, Ginger Rogers and John Philip Sousa. Family mausoleum resembles an Egyptian temple.
8. Dr. Clarence E. Hemingway (Forest Home) 1871- 1928 Dr. Hemingway was father of novelist Ernest Hemingway. Practiced medicine in Oak Park, where his famous son was born on July 21, 1899. Suffering from diabetes and depression, Clarence shot himself with his father’s Civil War pistol.
11. Billy Sunday (Forest Home Section 32) 1862-1935 Made immortal by Frank Sinatra, National League outfielder Billy Sunday became the first US mega-preacher -- storming against the dangers of alcohol and sin.
10. Philander Barclay (Forest Home) 1878-1940 Barclay wasn’t famous during his lifetime but over 100,000 photographs of the local area the 1880’s and 1902 are priceless.
4. Showmen’s Rest (Woodlawn Cemetery) 56 Victims of Train Wreck – June 22, 1918 Elephants stand watch over the victims of a circus rail accident. The Haggenbeck-Wallace Circus train was rear-ended by a runaway troop train. Victims, many unidentified, were buried in a mass grave.
7. Arie Crown (Jewish Waldheim) 1861- 1937 The patriarch of the Crown family, Arie emigrated from Lithuania and worked in a sweatshop. Ancestor of the 11th-wealthiest family in the U.S., the Arie Crown Theater at McCormick Place bears his name.
2. Michael Todd (Avrom Hirsch Goldbogen) (Jewish Waldheim) 1909-1958 Elizabeth Taylor’s third husband produced Oscar winning Around the World in 80 Days. His plane The Lucky Liz crashed in the mountains of New Mexico.

Neighborhood Watch Neighborhood Watch is a national anti-crime program in which residents contribute to the effort of keeping their neighborhood safe. Organized on a block-by-block basis, the program encourages citizens to become alert to and report suspicious activity in their area. Overnight Parking Parking is prohibited on village streets from 2 a.m.-5 a.m. Residents who have overnight guests who must park on the street should call the police department at 366-7272 to prevent the car from being ticketed.
Parking Permits Parking permits for space in village parking lots are available. Permits can be obtained for day parking, night parking or 24-hour parking. The cost is $25 per month for night-only parking and $40 per month for 24-hour parking. Call the village clerk’s office at 366-2323 for more information. Tree Replacement The village will replace trees on public parkways in front of residences at no cost. Seasonal brochures list these activities and special events and are distributed to each resident. Brochures also are posted on the website. Various events and classes are publicized in the park district brochure and Forest Park Review. Early registration is recommended to prevent cancellations. The Park includes 17 acres with six lighted tennis courts, three baseball/softball fields, a multipurpose synthetic turf soccer/ football/lacrosse field, an in-line skating rink, bocce ball courts, two batting cages, a skate park, a playground, a pavillion and a distance-marked walking path.

Skate Park 366-7500 (Park District) The skate park is unsupervised, though the park district recommends the use of helmets, elbow pads and knee pads. No smoking, eating or drinking is allowed, and skaters cannot bring their own
ramps or other skating obstacles. Bicycles are not allowed. Hours: Open until dusk, weather permitting.
West Suburban Special Recreation Association The Park District of Forest Park, through West Suburban Special Recreation Association (WSSRA), offers more than 60 programs for Park District of Forest Park residents with disabilities. All ages and all ability levels are served in these therapeutic recreational activities. Many programs are similar to traditional park district classes. The association’s swim
lessons, fitness classes, scouting programs and day camp are some examples. At these programs, a recreation specialist will adapt how skills are taught, how much personal assistance will be given and if any equipment accommodation needs are to be arranged. Communication boards, wheelchair use, behavior management plans and other tools are used. Some programs require one-on-one staffing for a participant to be successful. Others are designed so that the competition is at a fair level, as in Special Olympics. Preparation for IEPs, grading papers, and taking care of other responsibilities after my daughter goes to sleep tend to keep me up as well. McElligott: Thinking about the different needs of my students, and how I will best meet them. What do you love about Forest Park?
Dudlo: Forest Park is a beautiful and tight knit community with friendly residents, a small town feel, and of course, great businesses and establishments. I enjoy being a part of a great staff at FieldStevenson School as well. McElligott: Forest Park is very family oriented. Forest Park is a tight-knit community that supports/gets behind their local community, whether it is the schools, Park District, local shops etc.

Race to the graveyard Second Annual Casket Race comes to Forest Park

leven wheeled caskets in a deadman’s derby propelled down Beloit Avenue Saturday, Oct. 26 as Forest Parkers gathered to witness the second annual Forest Park casket races. Pushed by costumed racers, they sprinted two-by-two “Bone Hur” style -- ending at a pile of hay bales set up to prevent a gruesome crash. The prize? A ghoulish tombstoneshaped trophy.
The Park District of Forest Park -- last year’s champions -- beat out the Svengoolies by a little over a second at 16:62 seconds to 17:87. Grumbling in the crowd implied the “casket” used by the Parks team was so aerodynamic it would only fit a cadaver shaped like a cheese-wedge. But coffinbuilder Ryan Russ defended his design. “We followed the specs, we cut last

year’s design down by a foot so it was two-feet wide,” Russ said. Like last year’s casket, this year’s model had no bottom, to lighten the weight, Russ said. Eddie Fielden of Nobs Towing played the cadaver reclining on his truck bed in a silver casket his sister drags out for Halloween parties. Nobs didn’t race, just handed out candy. Back from the Chicago Marathon, runner Molly Hanrahan joined her teammates wearing Chicago Blackhawk gear for a Stanley Cup-shaped casket splashed with RIP graffiti for the Bruins, Redwings and Kings. Returning to this year’s race was the “Napoleonic Cassion” model debuted last year by Centuries & Sleuths Bookstore. This year, Historical Society of Forest Park Executive Director Diane Hanson Grah manned the chariot, pushed by Jill and
Ned Wagner, and Paul and Meghan Roach. A new entry this year was the Stencib casket with rider Linda Cibula pushed by Joe and John Cibula and Nancy Goggin. The Forest Insurance Agency returned with last year’s Forest Freaky Fiend
mobile. Team participants were Brian Sullivan, Dan Browne, Bobbie Cox, Mary Sullivan, Gina Robbins and Nichole Hoppe with rider Maria Lopez. The Brown Cow Ice Cream Parlor competed in a model of their famous ice cream “trough” complete with Neapolitan scoops of ice cream. Also returning this year were the “bluesy horror punk band” Shock Theater from Lombard and their “Mr. Grim” casket. This year’s coupe was improved with dry ice, a chrome rib-cage and matching tibia/fibula grille. Zach Hauger, in spooky skull makeup tux and cane, rode the casket, pushed by Steve Connelly, Scott Hall, brother John Hauger and Rebecca Marshall. They won the “creepiest” award.
