WATAUGA MEDICAL CENTER HOSPITAL MASTERPLAN

BRIDGING TO WELLNESS Boone, NC

ARCHITECTURE + HEALTH STUDIO 8950 GRADUATE COURSEWORK | CLEMSON UNIVERSITY FALL 2017 | GROUP PROJECT - WENZ TUTTLE & MARIAH GRESKO Watauga Medical Center is a regional hospital located in Boone, North Carolina, with a close proximity to Appalachian State University. It was originally a Hill-Burton facility that has been expanded upon throughout the years, creating a maze of pathways and spaces. Seeking a solution to their need for growth and desire to build a state-of-the-art facility in Boone while maintaining functionality, the Watauga Medical Center board of directors turned to the students at Clemson University in the Architecture + Health program for masterplanning insights. The Watauga Medical Center Campus Masterplan is designed to accommodate for the immediate need of an Ambulatory care center and future clinical needs. The system will serve as a model for promoting a healthy community and activating a medical campus to accommodate mixed-use facilities.

DROP-OFF APPROACH

The masterplan is designed to be implemented in a series of four phases, with the intent of fully replacing the existing facility on the site in the final phase. Furthermore, the concept also focuses on the experience of patients and staff as the primary driving factors in determining design guidelines. Phase one of the conceptual design masterplan includes the construction of the Ambulatory Care Facility to the south of the existing hospital, then the design of the adjacent replacement hospital with shared public amenities in phases two and three. The site is designed to engage the adjacent wellness district located in downtown Boone, with the intent of promoting walkability and healthy lifestyles for the residents. 9

“Abraham Maslow proposed that healthy human beings have a certain number of needs, and that these needs are arranged in a hierarchy, with some needs (such as physiological and safety needs) being more primitive or basic than others (such as social and ego needs).” -Psychology Today

Robin, 29

George, 68

Brian, 36 and Melissa, 46

After falling ill and losing her job as a cook, she spent nearly two years living in the woods of Orange County where she grew more ill, was nearly raped, and woke each day wondering how she would survive. She was cited for trespassing five times, and cycled in and out of the region’s emergency rooms so often she lost count.

George was drafted in the Vietnam War and returned to his hometown of Orlando after 7 months of military service from which he was dishonorably discharged. Sixteen years ago he was diagnosed with schizophrenia and has not been able to hold down a steady job since.

She cares deeply for her canine companion and always ensures he has food and water even when she finds it difficult to feed herself. Robin lives in a constant state of crisis not knowing what to do or with whom she can speak.

On good days, George speaks with other homeless individuals and hopes someone will give him food. He is occasionally able to bathe at a local shelter, but when he has to put on the same dirty clothes, he still feels dirty and wishes he had new or clean clothes to wear.

Brian met Melissa after he had been living on the streets for almost five years. He was suffering from depression, feeling lonely, and burned out. She was violently attacked by a gang once, when he left her to go and get them something to eat. He freaked out over the situation, and now never wants her out of his sight. Melissa fondly remembers that they met at a community food bank. Brian suggested that Melissa stay with him on the street. Being a couple gives them a sense of protection that they otherwise do not feel.

HEALTHY COMMUNITY PLANNING & DESIGN SITE 2 - ACROSS FROM HOSPITAL

ARCHITECTURE + HEALTH STUDIO 8960 GRADUATE COURSEWORK | CLEMSON UNIVERSITY FALL 2016 | INDIVIDUAL PROJECT WITH GROUP SITE ANALYSIS For our semester project, the studio was tasked to look at ways to design community that promotes health and wellness. Each project aims to demonstrate the complete development of the individual site and, sidewalks and building facade condition while keeping in mind the scale and culture of the community. Two- thirds of the semester was spent developing the wellness district in three teams of three people each, while the last two weeks was spent on developing the architecture of the individual site. Site two was particularly interesting since it has the possibility to connect directly to the hospital. To accomplish this, a skywalk added to the parking and retail centre that bridge both property that was both owned by Watauga Medical Center.

This two week deep-dive exercise informed the need for human-center and customer oriented design for a Preoperative and Postoperative spaces. Each team was assigned a room type and task with choosing the best workstation design for their room. For this specific space, the use of the an overhead boom allows for both flexibility and efficiency in an unusually layout. Three groups of people were identified to used this space and the design address the functions and needs associated with patient, caregiver and staff. The project exposes the ergonomic issues associated using workstation on wheels as well as the wall mounted stations. By researching existing booms on the market and incorporating our own design solution, the group was able to address issues related to maintenance, ergonomics, accessibility while maximizing floor space for a 140 SF room.

By slanting two walls in the room, circulation space around the patient bed increased and sightline for the patient, caregiver and staff improved. The care partner has an uninterrupted zone while the workstation allows fluid moment for the staff around the bed. The maneuverability of the boom means that there will be no wasted space caused by stationary workstation.

AMBULATORY SURGERY PAVILION MUSC MUSCULOSKELETAL AMBULATORY SURGERY CENTER AND MEDICAL OFFICE BUILDING ARCHITECTURE + HEALTH STUDIO 8950 GRADUATE COURSEWORK | CLEMSON UNIVERSITY FALL 2016 | INDIVIDUAL PROJECT | SEMESTER PROJECT The semester long Ambulatory Service and Medical Office Building project is based off of project needs for the Medical University of South Carolina’s (MUSC’s) Musculoskeletal Institute and their future Orthopedic Ambulatory Surgery Center. Approximately 40,000 SF is allotted to the surgery center and another 40,000SF is given to the MOB. In addition, the 80,000 sf project also includes spaces for the imaging department, medical offices, staff amenity spaces and a cafe. The design takes in consideration the future expansion if the operating room and clinics. The operating room will eventually expand from 2 O.Rs to 6 O.Rs. The O.R.s will have windows to bring in natural light. The use of daylight was crucial to the design of this building. This outpatient facility is focused on providing quality patient-center care while given healthcare staff a functional and health work environment. The building uses the cabin-concept for preop and post-op area, an European work core for the O.Rs. and the pod concept for the clinics.

The goal of the project is to address MUSC’s space program needs while using a holistic approach to understanding what a healing environment should be. To do this, the program addresses the integration of medical innovations and deal with the internal and external influences that define a healing environment. The project also has an larger impact on the community as a whole and should address issues of topography, vegetation while paying homage to the importance of this specific site and its contextual significance in West Ashley area of Charleston, S.C. For this exercise, the continuous surface of a Mรถbius Strip was stretched and manipulated to create a form that is then reimagined as a built project. By using the Rhino software, I was able to see the form in wireframe, slice it into sections and rejoining the sections onto solid curving beams. The end result of the exercise is a large pavilion space which allows the user to sit and contemplate their thoughts.

To establish a high-end clinical brand of service for the Department of Gynecology and Obstetrics within the second level of Nelson Harvey Building on the East Baltimore Campus. This includes the consolidation of various programs in the department under Johns Hopkins Medicine. The primary focus of the design is to provide patient comfort. The interior design of the clinical spaces works to suppress feelings of anxiety often associated with patients who visit our specialty clinics. One of the biggest changes to our standard flooring specifications is the use of resilient sheet goods in replacement of PVC flooring and carpet. This standard is now incorporated into a campus-wide initiative to replace all carpeted spaces with environmentally considerate materials.

The beauty of blooming cherry blossom trees symbolizes renewal, hope and birth in the spring time. It was chosen as the theme of this floor and corresponds with the feng shui atmosphere of the space. The color palettes were chosen to compliment the theme. Specially designed window films were printed and installed on each unit’s storefront windows. These graphics also serve as a wayfinding tool.

Final Fiberglass Panels Installed in the Main Nelson/Harvey Lobby

45

Originally constructed in 1977, the Nelson/Harvey building went a thorough 24 month renovation to redesign both the interior space and exterior facade to match the modern standard set by the Zayed/ Bloomberg Building. The existing doublebed patient rooms were converted into 136 single rooms to provide more privacy and comfort for the patients. The goal of this project was to envision, design, purchase and install healthcare appropriate artwork to promote an enjoyable environment for patients and staff. The theme of “Baltimore and Mid-Atlantic Gardens was used to accomplish this design objective. The project is divided into two phases: Framed photographic artwork inside patient units and ATI LuxCore Fiberglass reinforced plastic in public corridors. It is made out of chemically abrasion resistant material with Class-C fire rating.

In the last year of undergraduate study, the design of an architecture firm’s satellite office design was chosen as the project program. The existing building is a 28,500 sq. ft. structure with large windows and a 30’x30’ interior. This space is a direct reflection of the more dynamic design approach that architectural firms are taking in their office design & construction. The atmosphere of the dynamic spaces and useful furniture designs is chosen by their function and comfort. Lobby Pendent Light 54

In this project, the spaces are designed to drive motion toward one specific direction. This was accomplished by applying the intense red color of the brand to architectural features so the eyes will naturally follow the lines of extension. Circulation is one of the most important features in the space, because without openfloor accessibility and mobility, the concept of pathfinding could not be accomplished.

Level 06 Reflected Ceiling Plan

The lowered red ceiling plates were very important to the application of the concept. It is the overhead acoustic panels that draws the eye towards the direction of the proposed path. Since most people will naturally move towards the right upon entering a space, the boldest lighting and ceiling applications are placed on the right side.
