A bright future Research and Education WPHC contributes at an international level to the care of patients with chronic respiratory conditions. Our focus on translational research has empowered patients to improve mobility, health related quality of life, autonomy and community reintegration. We have influenced clinical practice and guidelines at a local and national level. We are accessing the new generation of apps and wearable technologies and are poised to work with AI to continue to advance service delivery. We lead the Provincial Long Term Ventilation strategy. Academic work in amputee rehabilitation focuses on understanding physical and behavioral approaches to enhance the recovery of amputees.
Education is a major focus. In addition to Canadian trainees we have fellows from Egypt, the UK, Iran, Singapore and Saudi Arabia, many of whom have created their own rehabilitation programs. Today’s research achievements provide the platform for future transformative clinical research and education to better science and comprehensive patient care. We are proud to contribute both to patient care and to academic productivity at the University of Toronto

Academic Productivity at West Park The clinical initiatives at West Park include rehabilitation, tuberculosis and complex continuing care. Research has informed optimal patient management. Our research is focused in three areas: Respiratory Medicine (Rehabilitation, Long-Term Ventilation and Tuberculosis), Amputee Rehabilitation and Spasticity.
We ask clinically relevant questions to advance clinical practice as reflected in valid reproducible outcome measures. Driving our work is a spirit of inquiry that permeates the clinical teams and includes our patients.

Respiratory Medicine: Respiratory Rehabilitation Research As a fully affiliated academic program in the rehabilitation and complex continuing care sector, research in respiratory rehabilitation has achieved national and international recognition. Clinical studies support our four rehabilitation programs (in-patient, outpatient, virtual and maintenance) and have yielded multiple publications in peer reviewed journals. In addition we have promoted the development of numerous programs across the country. We have been involved provincially through establishing guidelines for supplemental oxygen and (on-going) co-chairing Ontario Quality Standards for COPD. We are active in two Ontario Health Teams, guiding initiatives in respiratory population health. Our current focus is on promoting health equity for the underprivileged population.
We network with other respiratory rehabilitation programs across the country and in collaboration with the Lung Association initiated five national conferences on COPD (The COPD Alliance), subsequently founding the scientific program for the annual Canadian National Respiratory Conference. We participate in guideline creation for the American Thoracic Society and European Respiratory Society. Our commitment is to contribute to clinical care and academic productivity around patients with chronic respiratory conditions and to train the next generation of clinician scientists in respiratory rehabilitation research.

Pioneering the Evolution of Pulmonary Rehabilitation Since our foundational studies of PR (Lancet 1994, Lancet 1996), our group has focused on approaches to improve exercise function (assisting ventilation, helium- oxygen gas mixtures, muscle partitioning) as well as tackling co-existing conditions (chronic pain, poor nutrition, loneliness and osteoarthritis). We have evaluated sedentary behavior, repeat rehabilitation programs, rapid access rehabilitation and measures of walking speeds.
Our clinical research program includes; health literacy, telerehabilitation, wearable technologies, COPD apps and activity monitoring. We have been privileged to contribute to an advancing scientific field and have shared this experience with our trainees in medicine and rehabilitation sciences as well as our colleagues from around the world. Enclosed (page 27) is a list of publications and grants over the past 3 years.

Research Example: Transformative Impact of One-Legged Cycling on Exercise Function A novel approach to improve exercise function in those with severe ventilatory limitations has been the development of one-legged cycling, following our initial observation (Dolmage, Chest 2006) that patients with severe COPD increased endurance time when using one leg. We have reported the greater improvements in fitness (peak oxygen uptake) when compared with conventional cycling (Dolmage, Chest 2008).
Subsequently we have incorporated one-legged cycling as the principal exercise modality in pulmonary rehabilitation (Evans AJRCCM 2013, Evans Annals ATS 2015). More recently we (Dolmage Annals ATS 2020) have reported the effects of one legged cycling in patients with Interstitial Pulmonary Fibrosis and are midway through a randomized controlled trial for patients with IPF, with the aim of improving exercise capacity and quality of life (Dolmage Annals ATS 2020).

Scientists: Respiratory Rehabilitation Research Dr. Roger Goldstein Dr. Goldstein is a Senior Scientist at West Park and the Head of Respiratory Medicine. He is a specialist in Respiratory Medicine who trained in the UK, Toronto and San Francisco. He was recruited by West Park and the University of Toronto to establish an academic respiratory rehabilitation program. He is the Respirology Division Head at West Park as well as the Chair of the Research Education Council. Dr. Goldstein is a Professor of Medicine and Physical Therapy at the University of Toronto and the Founding NSA Chair in Respiratory Rehabilitation Research. He is cross appointed as a Professor of Rehabilitation Sciences at McMaster University. He has extensive experience in the clinical management and investigation of patients with chronic respiratory conditions. He has numerous publications and has made many invited presentations at local, national and international meetings. In partnership with Dr. Dina Brooks, he co- directs the post-graduate respiratory rehabilitation research group at West Park, which includes both Canadian and International trainees.
Dr. Dina Brooks Dr. Brooks is a Senior Scientist at West Park. She is also a Professor in the School of Rehabilitation Science and is the Vice-Dean (Health Sciences) and Executive Director, School of Rehabilitation Science at McMaster University. Dr. Brooks is recognized as a leader in cardiorespiratory rehabilitation and is a fellow of the Canadian Academy of Health Sciences. She held a Canada Research Chair in Rehabilitation and COPD and presently holds the National Sanitarium Association Chair in Respiratory Rehabilitation Research. Dr. Brooks is an alumna of University of Toronto, where she completed a Bachelor of Science degree in Physiotherapy, a Master of Science degree in Physiology and a PhD in Medical Sciences. She holds a status appointment as Professor in the Departments of Physical Therapy and Medicine and Rehabilitation Science Institute at University of Toronto.
Tom Dolmage Mr. Tom Dolmage is a Scientist at West Park Healthcare Centre where he has been the Special Procedures Technologist for many years. He graduated from the University of Waterloo with a Bachelor of Science in Kinesiology and Exercise Science degree followed by a Master of Science in Exercise Physiology from York University. Tom is an experienced physiologist, innovator and researcher in new approaches to rehabilitation of patients with lung disease. His research is widely recognized and he is a Centre resource in statistics and methodology.

Physician Leads: Respiratory Rehabilitation Research Dr. Robert Varadi Dr. Robert Varadi is a respirologist with the Respiratory Rehabilitation and Long term Ventilation services at West Park. He also practices general respirology at West Park and the Toronto Western Hospital. His clinical interests include rehabilitation in chronic respiratory conditions, chronic respiratory failure, and respiratory impact of neurologic/neuromuscular diseases. He is the West Park lead on a multi-center CIHR funded project on remote monitoring in ventilator dependent individuals.
Dr. Deborah Casey Dr. Deborah Ksan Casey is a Respirologist at West Park Healthcare Centre and Sunnybrook Health Sciences Centre. Her expertise lies in managing chronic respiratory conditions, respiratory failure, and the respiratory aspects of neurologic/neuromuscular diseases. Dr. Casey is the respiratory lead in quality improvement and is currently evaluating pressure ulcers in long term ventilation.

Research Example: From Breath to Balance Integrating Balance Training into PR Patients with COPD experience falls more frequently than the general population. The observation that impairments in balance tests could discriminate fallers from non-fallers in COPD (Beauchamp, Respir Med 2009) and that conventional PR did not impact tests of balance (Beauchamp, Archives Phys Med 2010) and that the deficiency was apparent in all the subcomponents of balance testing (Beauchamp, Chest 2012), resulted in a randomized controlled trial of balancetraining integrated into pulmonary rehabilitation that observed higher balance scores in the study group (Beauchamp Chest 2013). A subsequent knowledge translation study (Harrison, BMC Pulmonary Medicine 2015) integrated balance training into clinical PR. Guidelines on staffing and training frequency (O’Hoski Physiotherapy Canada 2021), set the stage for the ongoing CIHR funded international, randomized controlled trial evaluating the effect of balance training on falls in individuals with COPD.
Navigating New Digital Frontiers: Innovations in Pulmonary Rehabilitation Through a series of studies, we are examining the capabilities of commercially available COPD apps (PhD applicant Quatch) and wearable devices (PhD applicant Michaelchuk) investigating the realm of virtual pulmonary rehabilitation (PhD applicant Verweel). In this digital era, we recognize the power of technology to reshape healthcare experiences to enhance clinical outcomes and access to care.

A Legacy of Mentorship Dr. Dina Brooks (PhD) and Dr. Roger Goldstein, lead the West Park program in Respiratory Rehabilitation Research. There have been numerous doctoral and postdoctoral trainees associated with the Centre. Many have continued to lead in their faculties at other Universities. Recent examples of trainee success includes:
Dr. Marla Beauchamp PhD in Balance Assessment and Training in COPD at the University of Toronto (2007 – 2012) and completed postdoctoral studies in Outcome Measurement at Harvard University (2012 – 2014). She is currently an Associate Professor in the School of Rehabilitation Science at McMaster University and holds a Tier 2 Canada Research Chair in Mobility, Aging, and Chronic Disease.
Dr. Tania Janaudis-Ferreira Came from Umea University (Sweden) to undertake postdoctoral studies in Upper Extremity Training in COPD (2010 – 2012).She is currently an Associate Professor in the School of Occupational and Physical Therapy at McGill University.She is a Scientist at the Research Institute of McGill University, holds 2 CIHR-IMHA Inclusive Research Excellence Prizes and is a co-founder of the Canadian Network for Rehabilitation and Exercise for Solid Organ Transplant Optimal Recovery.
Dr. Ana Oliveira Completed her PhD at the University of Porto (Portugal) and undertook post-doctoral studies in Interstitial Lung Disease and Pulmonary Rehabilitation (2019 – 2022). She has co-authored 45 papers and 4 book chapters and is an Adjunct Professor in the School of Health Sciences at the University of Aveiro (Portugal).

Respiratory Medicine: Tuberculosis Research Our team is recognized nationally as a leader in the management of complex tuberculosis (TB) cases, including multidrug resistant TB (MDR-TB).
The team has also described the costs of managing TB, including MDR-TB, along with 2 other Canadian centres (Emerging Infectious Diseases 2022).
We published the largest case series of MDR-TB in Canada in 2015 (Canadian Respiratory Journal 2015), and subsequently participated in several large and high-impact individual patient meta-analyses on MDR-TB management (Lancet 2018, European Respiratory Journal 2020, Lancet Respiratory Medicine 2020), which directly influenced WHO treatment guidelines.
We co-led the writing of the DrugResistant TB chapter of the Canadian TB Standards, 8th Edition (Canadian Journal of Respiratory and Critical Care and Sleep Medicine 2022).
We published our unique experience on use of lower-dose amikacin in MDRTB (BMC Infectious Diseases 2021), our experience managing TB in patients with chronic liver disease (J Assoc Med Microbiol Infect Dis Can 2023).
Since 2016, we have trained 10 fellows in Mycobacterial disease (TB and nontuberculous mycobacterial disease), most of whom have gone on to obtain academic positions as mycobacterial disease specialists in their home cities and countries.

Scientists & Physician Leads: Tuberculosis Research Dr. Sarah Brode Dr. Brode is an Assistant Professor of Medicine with a specialized focus on Tuberculosis (TB) and Non Tuberculous Mycobacteria (NTM). She is the Physician Lead for the Tuberculosis Service at our provincially designated treatment centre for difficult-to- treat cases and she is appointed to UHN (Toronto Western Hospital) as the medical director of the TB clinic and the education director of the NTM program.
Dr. Natalie Sabur Dr. Sabur is appointed to the Tuberculosis Service at West Park Healthcare Centre and at St. Michael’s hospital. She trained in Toronto and Calgary before completing her Masters in Public Health at Johns Hopkins Bloomberg School of Public Health after which she underwent further training in tuberculosis at Groote Schuur Hospital (University of Cape Town) in South Africa.

Respiratory Medicine: Long-Term Ventilation Centre of Excellence A Comprehensive Approach to Long-Term Ventilation: Combining Clinical Expertise, Research, and System-wide Integration Our initiative in supporting ventilator assisted individuals (VAI) outside of the ICU began many years ago with two patients transferred from the ICU at TGH. Building on the need to free ICU space and to facilitate a better quality of life we now host the largest (69 bed) long term ventilation (LTV) unit in Canada providing assessments, transition to the community and a safe environment for VAI. As the designated Provincial Centre of Excellence for LTV our mission is to promote LTV care throughout Ontario. In the GTA we work closely with our partners (Michael
Garron and Toronto Grace) using a single co-ordinated pull strategy to facilitate accommodating VAI at the most suitable location. Our interests include non-invasive (Brooks CRJ 2002) and invasive (Wijkstra Chest 2003) ventilation, user perspectives (Brooks CRJ 2004), caregiver experience (Evans CRJ 2012) and transitioning VAI from acute care to the community (Rose CJRCCM 2018). We participate in national studies on Home Ventilation (Rose, Respiratory Care 2015) Healthcare Utilization in Neuromuscular Disease (Rose, ERJ 2018) and maintenance of Lung Hygiene by Mechanical Insufflation (Rose, Respiratory Care 2021). Research in this field taps into the wealth of experiences found in one of the largest adult amputee rehabilitation services in Canada. Our amputee rehabilitation research examines components and models of care with the goal of maximizing functional independence. The addition of an amputee scientist (2019) has advanced our academic productivity. With a history of recognition through professional society awards and multiple peer-reviewed papers we have integrated research into the program's core.

Grassroots Innovation: Revolutionizing Prosthetics Pioneering the future of prosthetics, West Park has become a hub for the real-world implementation of 3D printing. This innovative digital pathway offers a seamless and efficient experience for individuals living with lower limb amputation. In collaboration with front-line clinicians, Dr. Crystal MacKay has led qualitative evaluations of the barriers and enablers of using 3D printing with patients and providers resulting in numerous presentations and publications in Prosthetics and Orthotics International and the Journal of Disability and Rehabilitation. Our research group has started to explore other dimensions of 3D printing, including socket geometry, sensor technology, and software applications
CIHR-Funded Innovation: Promoting Physical Activity Recovery Following Dysvascular Amputation Dr. Crystal MacKay has secured a noteworthy achievement in healthcare research with a substantial Canadian Institute of HealthResearch Project Grant. This grant, totaling $300,000 over two years, is for her project "A Behaviour Change Intervention Promoting Physical Activity Following Dysvascular Amputation: A Pilot Study." This research focuses on practical behavior change interventions that encourage physical activity, aiming to empower individuals and enhance their overall well-being.

Our Scientists: Amputee Research Dr. Crystal MacKay, PhD, MScPT, MSc Dr. MacKay is a Scientist at West Park Healthcare Centre and Assistant Professor(status) in the Department of Physical Therapy, University of Toronto with a cross appointment at the Rehabilitation Sciences Institute. Following her PhD (Institute of Medical Science) and postdoctoral training at Toronto Rehabilitation Institute her research interests include health needs, outcomes and service use of people living with chronic conditions, focusing on patient-centred complex health interventions.
Dr. Steven Dilkas Dr. Dilkas plays a multifaceted role at West Park, serving as a staff physician in the Amputee Rehabilitation service and extending his expertise to high-performance athletes, including both able-bodied and Paralympic athletes. Beyond his contributions to West Park, he holds the prestigious position of Chief Medical Officer at Canadian Sport Institute Ontario, where he influences the health and performance of elite athletes. In addition to his clinical and sports medicine commitments, Dr. Dilkas contributes to academia as an Assistant Professor in the Division of Physical Medicine and Rehabilitation within the Department of Medicine at the University of Toronto.

Sensor research aims to improve life for amputees As an engineer, assistant professor, and amputee, Dr. Fae Azhari is participating in research that could revolutionize the fit and comfort of lower limb prosthetics, improving the lives of amputees. Azhari's research at the University of Toronto focused on the monitoring of structures such as bridges and buildings; however, after losing her leg in an accident in 2018, she developed a passion for the engineering of prosthetics. She aims to develop an affordable and convenient method for monitoring patients' gaits using inexpensive sensors attached to a shoe cover. This technology could help clinicians better understand each patient's unique gait and adjust prosthetics accordingly. With her familiarity with using sensors to monitor engineering structures, Azhari took the first step toward research at West Park that would combine her expertise with her own lived experience to hopefully, one day, help other amputees. Azhari, West Park scientist Dr. Crystal MacKay, and several graduate students have successfully developed a prototype adjustable sensor system costing less than $200. It is adjustable and goes over the shoe rather than an in-sole, negating hygiene issues. “This research is exciting because the novelty in what Dr. Azhari is doing is using cheap sensors that can provide valuable information to clinicians quickly, with minimal training required,” - Dr. Crystal MacKay

Fostering Empathy through Digital Care: Navigating Compassion in Rehabilitation The findings from Dr. MacKay’s study on how individuals experience compassionate care in rehabilitation will help us understand how to improve care in a digital environment. The pandemic has provided the impetus for an unprecedented rate of adoption of virtual care in rehabilitation. Dr. MacKay’s team has been mindful of the importance of maintaining compassion and empathy even while using digital tools.
Walking Stronger: Revolutionizing Physical Activity for People with Lower Limb Amputations Dr. MacKay and her team led a national consensus in 2020 to develop a physical activity action plan for people with lower limb amputations. The resulting behaviour change intervention: Improving Physical Activity through Coaching and Technology following Lower Limb Loss (IMPACT-L3) has been implemented and is the subject of the CIHR funded randomized controlled trial of effectiveness for those with dysvascular lower limb amputations.

The Spasticity Research Team has been dedicated to driving transformative research at the convergence of botulinum toxin and spasticity management. Their pioneering publications are revolutionizing neurorehabilitation by illuminating the nuanced dynamics of this intersection. The team’s systematic reviews offer novel insights into the effects of botulinum toxin on clonus, guiding clinicians in refining spasticity management. Beyond clinical applications, their work acknowledges the holistic patient experience. The team's study "Why Should Physical Therapists Care about Their Patients' Diet?" underscores comprehensive care's significance in spasticity management.
Furthermore, the team’s collaborative research unveils practical implications for individuals dealing with spasticity. Their investigations into post-stroke spasticity and botulinum toxin effects on standing balance reveal the intricate interplay between medical interventions and functional recovery. The Spasticity Research Team's dedicated exploration of botulinum toxin in spasticity management is redefining neurorehabilitation. Their contributions resonate through clinical practice, research, and patientcentered care, igniting innovative strategies to optimize spasticity treatment to improve the lives of West Park’s patients.

Spasticity Scientists and Physician Leads Dr. Chris Boulias Dr. Chris Boulias is a Physiatrist and Clinical Researcher at West Park and an Assistant Professor in the Department of Medicine at the University of Toronto. He has been instrumental in the development of the clinical, education and research arms of the Spasticity Management Program and is passionate about enabling neurorehabilitation patients to live life to their fullest potential. Dr. Boulias is a leader of the Canadian Spasticity Group Residency Course and a founder of the Canadian Botulinum Toxin Conference. He has more than 50 publications and several studies ongoing.
Dr. Farooq Ismail Dr. Farooq Ismail is a Physiatrist and Clinical Researcher at West Park. He is Assistant Professor in the Department of Medicine who specializes in research, teaching and education, in the field of spasticity. He has been instrumental in the development of the clinical education and research arms of the Spasticity Management Program at West Park. Dr. Ismail heads the West Park Spasticity Management Clinic, the largest adult spasticity program in the country. He is involved in clinical research and practice of Botulinum toxin injection. He runs the Northern Ontario clinic for diagnosis and treatment of patients with spasticity.

The Triumphant Journey of a Spasticity Patient Jen Schuringa embarked on her four year dream to conquer the arduous pilgrimage of Camino de Santiago in Spain after surviving a traumatic car accident in 2013 that left her unable to move or speak at will, forcing her to communicate only through blinks. She has a debilitating neurological condition called spasticity, an involuntary and often extreme tightening of the muscles. With treatments from West Park’s Spasticity Management Clinic and unyielding family support, Jen has endured gradual, yet significant, improvements over a decade. She relearned how to eat and even mastered pedaling a bike, eventually covering a third of the pilgrimage by cycling. “The functional recovery that Jen has achieved is unique and attributable to her strength of mind and the support of her family. The comprehensive adult spasticity management program at West Park and our team are inspired and thankful to be a small part of Jen’s accomplishment,” said Dr. Farooq Ismail, one of two physicians managing the spasticity clinic. Jen and her team - made up of family and supporters dressed in pink in her honour - garnered applause from hundreds of fellow pilgrims at the end of their destination in Santiago de Compostela’s main square and cathedral. The entire experience serves as a testament to the human ability to reclaim life, joy and community.
