Battle on the Rock Registration Package This registration and information package contains all of the documents necessary to participate in the Battle on the Rock Football Tournament. If any information is incorrect, please contact the Battle on the Rock tournament immediately. Please be advised, once a participant is registered for the tournament, there will not be any refunds issued to participates who decide to drop out of the tournament. Upon registration, the Battle on the Rock orders event shirts, and makes arrangements for your team’s participation in the tournament. Team registration fee is $200.00 and is due by August 17th 2015. The first four teams in each division that pays the registration fee will be entered into the tournament. Additional team registrations will be placed on a waiting list. Teams on the waiting list will receive a full refund if their team does not participate in the Battle on the Rock tournament. A player participation fee of $25.00 is required for each player participating in the tournament. The player registration fee is due on August 24th, 2015 along with an initial list of players participating in the tournament. Teams will be required to submit their final team roster by August 28th; this will allow teams to make roster changes. Changes to the team roster will not be allowed after the final team roster is submitted. Teams will be responsible for player participating fees for additions to their team roster.

No Roster changes or additions will be made after August 28th, regardless of the circumstances. NO EXCEPTIONS! Tournament brackets for each division will be selected on Tuesday September 1st. A conference call number will be provided to each team to allow for each team to be present for the bracket selection. Specific times will be determined for each division on September 1st. The purpose of the conference call is to answer any last minute question/concerns and to select the brackets. 1st round games on Saturday September 5th will be played at 9:00, 11:00, 1:00, & 3:00. The Battle on the Rock tournament committee will consider special request to accommodate team’s travel plans on a first come, first serve basis. Special request for 1st round game times are NOT GUARANTEED. Team check-in will begin 1.5 hour prior to your first scheduled game. A team coach will present their team book at the tournament check-in booth at their assigned time. The team check-in process will be busy so please be present will all required documents at your assigned times. Please have all required documents at check-in. Birth certificates are required for all players. Please have birth certificates in alphabetical order with player’s name and birth date high-lighted to ensure an efficient check-in process. ONLY ONE COACH FROM EACH TEAM IS REQUIRED TO BE PRESENT FOR TEAM CHECK-IN. Please ensure all players have the necessary equipment to compete. All players MUST have a mouth piece which attaches to the helmet facemask. Failure to have the proper equipment will result in a player being ineligible to compete in the tournament. Steel tipped cleats are NOT ALLOWED.


Any money due by the team must be paid by Cash, Money Order, Traveler’s Check or Credit Card. NO PERSONAL CHECKS WILL BE ACCEPTED.

League Name: __________________________________________________ Team Name: ___________________________________________________ Battle on the Rock Tournament Rule Book This book contains the forms, rules and procedures which must be adhered to for teams to participating in the 2015 Battle on the Rock youth football tournament (hereafter referred to as BOTR tournament). The tournament will be played in accordance with the rules as set forth under the National Federation of State High School Associations (NFHS). NFHS and BOTR tournament rules are for game competition. Your league may have rules regarding postseason competition which may or may not be superseded by NFHS or BOTR tournament Rules. The Head Coach of each team participating in the BOTR tournament is ultimately responsible for all records keeping, team finances and conduct of all participants (including parents, spectators, etc.) attending the tournament. We expect over 400 participants in this year’s tournament. Due to this large number we must limit the authority to contact the BOTR tournament to the Head Coach or the League President. Please ensure all documents requiring to be notarized are notarized prior to submitting. This includes you final tournament roster. Failure to have these documents notarized will prevent your team from participating in the BOTR tournament. All team documents th except for the final team roster must be submitted by August 17 2015. The final team roster must be by August 28th, 2015.

Acceptance of the Battle on the Rock Tournament Rules, Terms, and Conditions I have read and understand the rules and terms of the BOTR regarding my team’s participation in the BOTR Tournament. I further understand that failure to arrive in sufficient time to allow the tournament administrators to verify player’s ages, weights, and team documentation may result in the team’s forfeiture of games. I understand that the tournament begins on September 5th and ends on September 6th. You agree that your team will participate in all scheduled tournament games. I have discussed the rules and expectations of proper conduct with all Tournament participants, including Coaches, Players, Parents, and other spectators who may be attending the tournament as representatives of my team.


Battle on the Rock-BOTR Player Waiver/Release AMATEUR ATHLETIC WAIVER AND RELEASE OF LIABLILITY

READ BEFORE SIGNING In consideration of being allowed to participate in any way in the 2014 Battle on the Rock Tournament (BOTR) athletic sports program, related events and activities, the undersigned acknowledges, appreciates and agrees that: 1. The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment and personal discipline may reduce this risk, the risk of serious injury does exist; and, 2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and, 3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and, 4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS, Oak Harbor Football & Cheer League, Battle on the Rock , their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASES”), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE. 5. I understand that if ANY games are cancelled by the Battle on the Rock committee due to an Act of God (i.e.: rain, flood, lightning etc) I will not hold OHYFCL or “BOTR” liable for any cost associated with the loss of games. I further understand that there are no refunds whatsoever. 6. I authorize onsite Emergency Medical personnel to provide medical services to my player in the event of a medical emergency. I, have read this release of liability and assumption of risk agreement, fully understand its terms, understand that I have given up substantial rights, by signing it, and sign it freely and voluntarily without any inducement. This is to certify that I, as parent/guardian with legal responsibility for this participant, do consent and agree to his/her release as provided above, of all the Releasees, and for myself, my heirs, assigns, and next of kin, I release and agree to indemnify and hold harmless the Releasees from any and all liabilities incidents to my minor child’s involvement or participation in these programs as provided above, EVEN IF ARISING FROM THEIR NEGLIGENCE

As the authorized representative of the football team participating in the Battle on the Rock Tournament (hereafter referred to as “BOTR Tournament”), I do hereby accept full legal responsibility for all minor associated with my football team, while being transported to, from or in attendance at any and all BPTR tournament events. I certify that I have obtained parental or legal guardian permission for each minor travelling with the football team to any 2014 BOTR tournament events. I certified that all parents have been informed that the BOTR tournament have not established minimum play requirements for participants in the BOTR tournament. The coaching staff for each team will determine minimum play requirements for their football players in accordance with the rules and guidelines set forth by their respected league. As the authorized representative of my team, I hereby state that all persons attending any BOTR tournament event have been made aware of the risks and dangers of competing in full contact football. By attending any BOTR tournament event, all persons affiliated with my team agree to accept these risks and dangers and agree to hold harmless the Battle on the Rock Tournament, it’s directors, officers, employees, officials, judges, sponsors, medical representatives and tournament contracted hotels, Oak Harbor School District, the City of Oak Harbor plus their officials, agents, service contractors, and employees acting in the scope of their duties relating to the Battle on the Rock Tournament for any injury or illness to any football player, parent, or other adult associated with this football team attending any BOTD tournament event.
