Director’s Letter As we enter our fifth year of service in the communities of Southold Township, Central Elgin and St. Thomas, it is appropriate to say thanks to our clients, our volunteers, our wonderful Board of Directors, staff, partners and other community supporters. With this support, we have welcomed 2100 new clients, formerly without a health care provider, and provided programming for 800 residents. As I reflect, it has been an exciting time - building our centre, our staff and our services. We have been supported and helped by many people along the way. Our initiatives have been an example of true community development.

Over that same time, we have developed partnerships with other community agencies, in particular Public Health, CMHA, Ontario Works, YWCA, Destination Church, Northside Community HUB, and many others. In addition to primary care, we now offer a wide range of allied health services including chiropody, dietitian, physiotherapy, chiropractic, physical therapy, social work, system navigation and others. We would be remiss if we didn’t thank one of our most important partners, the Southwest LHIN. Central Community Health Centre (CCHC) Is a charitable organization whose purpose is the delivery of comprehensive primary care services to individuals and families in St. Thomas, Central Elgin and Southwold Township. CCHC’s services complement existing healthcare in these communities, and include health promotion, illness and injury prevention, and the diagnosis and treatment of illness and injuries. The Central Community Health Centre works in collaboration with the Southwest Local Health Integration Network along with various agencies and services within Elgin County. These partnerships enable us to provide our clients and the surrounding community with the care and programming they need.

Our Model of Care Comprehensive, accessible, client- and community-centered, inter-disciplinary, and community-governed are the hallmarks of the model of care which is grounded in the community development approach and is inclusive of the social determinants of health, including shelter, education, food, income, a stable eco-system, sustainable resources, antioppression, inclusion, social justice, equity and peace. The CCHC offers the residents of Central Elgin, Southwold Township and St. Thomas a range of comprehensive primary health care and health promotion programs. These are the five service areas: 1. Primary care 2. Illness prevention 3. Health promotion 4. Community capacity building 5. Service integration The services are designed to eliminate barriers such as poverty, geographic isolation, ethno- and cultural-centrism, racism, sexism, heterosexism, transphobia, language discrimination, ageism, ableism and other harmful forms of social exclusion, to accessing health-care. Barriers may include issues such as complex mental illness that can lead to an increased burden or risk of ill health.

The CCHC Model of Care is: Comprehensive The CCHC provides comprehensive, coordinated primary health care for its communities, encompassing primary care, illness prevention, and health promotion, in one-to-one service, personal development groups, and community-level interventions. Inter-professional • We build inter-professional teams who will work in collaborative practice. In these teams, salaried professionals will work together in a coordinated approach to address the health needs of their clients. The CCHC’s inter-professional teams will include physicians, registered practical nurses, nurse practitioners, a dietitian, social worker, health promoter, community outreach/system navigator and administrative staff. Integrated • We develop strong connections with the health care system and community partners to integrate CCHC service delivery with other health and social services. • We foster integration and efficiency by providing timely services, appropriate referrals, and the delivery of seamless care. Community-governed • We enhance the health of our diverse communities through the leadership of a volunteer community board and committees. • We respond to the needs of our community (St. Thomas, Central Elgin and Southwold Township). Who can become a registered client? If you do not have a family physician and live in St. Thomas, Central Elgin, or Southwold Township we welcome you to consider becoming a Registered Client. Once you are registered with our Centre you will be able to receive primary health care services from a team of Nurse Practitioners, Registered Practical Nurses, Family Physicians, Dietitian, Chiropodist, Chiropractor, Physiotherapist, Exercise Specialist and Social Worker. Please note: If you have a family physician, you can still access community programs, and the services of the Dietitian by pre-registering. Steps to becoming a registered client: • Complete an application form, available at the Centre. Our staff can help answer your questions. • Our staff will review your application, with priority given to those who are without a family physician and who may have difficulty obtaining health and social services.

• You’ll take part in an intake interview where you will tell us more about your health needs and what you’re looking for in a health care provider. • We’ll share with you the services that we offer and our partnership approach. • We’ll discuss if we can meet your needs. • Once you become a registered client, you’ll have an appointment for a physical exam. We’ll ask you to sign a consent form so that your past medical records can be sent to us, if possible, and so that we can share information within our team and with other health practitioners to whom we may refer you for additional services. Your privacy will be protected at all times. As a Registered Client you are: • Able to access a Nurse Practitioner or Physician for your health care needs • Able to access our on-site lab services • Welcome to take part in all of the programs and services offered at CCHC. Emergency Situations In emergencies, CALL 9-1-1 immediately or go directly to the nearest hospital emergency department. We are not equipped to deal with emergency or crisis health needs.

Central Community Health Centre Client Rights & Responsibilities AS A CLIENT OF CCHC I AGREE: • T o be committed to maintaining and/or improving my health and to participate in my health care plan • To treat others courteously, with respect and fairness and without discrimination • To be accountable for my actions and to accept the consequences of my behaviour, which may include being refused service by the Centre • To be on time for appointments and to call and cancel appointments (with at least 24 hours notice) if I am not able to attend • To inform the Centre of any changes in my contact information • To be honest in my interactions with the Centre

Primary Care Team Services Our Primary care team consists of Family Physicians and Nurse Practitioners
supported

Registered Practical Nurses. Our health care teams work in an interdisciplinary environment
collaborative that
includes
specialists, and other health care professionals. Clients in Central Elgin, St. Thomas, and Southwold Township can access these services by appointment. The centre provides diagnostic services such as a twice daily lab courier, ECG, Spirometry, and Ambulatory Blood Pressure monitoring. We also offer consultants such as a Chiropodist through self-referral. We believe in the importance education both for our staff and for upcoming medical professionals. We provide learning opportunities for students: Residents and Medical students, Nurse Practitioner, Dietitians, and Social Workers.
Family Physicians and Nurse Practitioners Family Physicians are trained in all areas of medicine and can diagnose and treat a broad array of health care problems. They are at the centre of your care, coordinating
and
guiding
you through your health care options. They are experts

in prevention, treating most acute illnesses and helping people manage their chronic diseases. To ensure your best treatment options our physicians work with Allied Health services, specialists and other health care professionals. CCHC has several family physicians and on occasion medical resident physicians who can address your primary care needs. If your usual doctor is not available you will be seen by another member of the team. Please inform specialists and hospitals of your doctor’s name. This will enable us to receive copies of your reports so that they may be included in your chart. Primary Care Nurse Practitioners are advanced practice nurses RN (EC) registered with the College of Nurses. They have the independent authority to provide wellness care, monitor infant development, screen and monitor chronic disease, diagnose and treat common illnesses and injuries and prescribe most medications and lab tests. They work collaboratively with other members of health care team.
Registered Practical Nurses Registered Practical Nurses direct or work within a team of health professionals in providing patient care and preventing illness. They educate patients on disease and how to lead a healthy lifestyle. They triage and monitor patients, carry out blood work, immunizations and perform clinical and clerical duties. They administer medication and treatments prescribed by nurse practitioners and physicians and help patients rehabilitate from illness or injuries.

Medical Secretary The Medical Secretary is a support position for both Health Care Practitioners and Administration. The duties include greeting all clients and other visitors who come to the CCHC or call by telephone; listening to their needs and requests and directing them to the appropriate team member. The Medical Secretary also schedules clients for specialists and diagnostic appointments.
Administration Director, Finance and Administration The

Administration is responsible for the high level management of finance, building maintenance and human resource practices for the Central Community Health Centre (CCHC). The Director works directly with the Executive Director to ensure the development, maintenance and auditing of a comprehensive and confidential system of corporate, human resource, financial and client records as it pertains to relevant legislation. The Director, Finance and Administration serves as bookkeeper/comptroller for the CCHC, and is responsible for all bookkeeping and financial records. Executive Assistant The Administrative team is a support for Administration at the Central Community Health Centre (CCHC). The duties include Human Resources, developing documents letters, and reports and providing an array of support to staff as required. Bookkeeper The Bookkeeper ensures that the general accounting of the Centre is recorded and that appropriate financial reports and documents are produced.

Health Promotion and Community Development The Community Development Team is made up of a Health Promoter, Dietitian, Volunteer Coordinator Outreach
and
Community
Worker/System
Navigator. Working with our community partners they develop and coordinate individual and group programs that promote illness prevention and health and well being. The team believes that health and wellbeing are shaped by a number of factors such as housing, income, education, employment, and social support networks. These are often referred to as the social determinants of health and the Community Development Team works with other community agencies on committees and initiatives to advocate for programs,services and changes that will positively influence people’s health. Our programs are free and open to all individuals , regardless of whether or not they have their physician at Central Community Health Centre. Our programs are listed at the back of this guide. Social Workers Social Workers registered with the Ontario College of Social Workers assist people in managing their daily lives, coping with issues, navigating relationships, and managing personal and family problems. They help individuals to understand and adapt to illness, disability such as chronic pain and loss. They work with other social services such as government assistance

and legal aid and are involved in social policy development. Social workers develop, implement, and assess programs to address social issues such as domestic violence, poverty, addictions, and mental health. Individual, couple, and family counselling is available. To obtain an appointment talk to your health care professional.
Chiropody Each foot contains 26 bones, 33 joints, and a network of more than 100 tendons, muscles and ligaments and a vast network of blood vessels and nerves. Such an intricate mechanism deserves specialized attention through Chiropody, the treatment and prevention of diseases or disorders of the foot. Chiropodist can use therapeutic, surgical, orthotic and palliative means to treat foot problems. Chiropodists /Podiatrists are the only health care professionals trained exclusively to assess and treat foot problems. Other health professionals may provide some foot care but only those who are licensed by the College of Chiropodists of Ontario may use the title Chiropodist (pronounced KER-RAH-PO-DIST) or Podiatrist (pronounced PO-DIE-AH-TRIST). This service is available through self- referral or talk to you primary care provider.
Registered Dietitian Dietitians have to meet national standards for education and training. They are also required to be registered with the College of Dietitians in whatever province they work. The Dietitian at CCHC provides individual and family counselling on a variety of food and nutrition issues such as high cholesterol, high blood pressure, bowel concerns, picky eating, unintentional weight loss and a non-dieting approach to weight management. If you have diabetes, you will see the dietitian in the diabetes education program. Our Dietitians accept referrals from health care providers and self-referrals from individuals within our catchment area. They also do group programming the community.

Volunteer Services Coordinator Volunteers are an important part of what we do at the
Central
Community
Health Centre. Our team of enthusiastic and dedicated volunteers support our staff and clients in a number of different ways including: • Preparing and serving food at our summer Community Barbecues; • Directing and helping clients at our mobile foot care, flu shot and blood pressure clinics; • Driving clients to their medical appointments • Leading community programs; • Typing, copying and other administrative tasks. The Volunteer Services Coordinator is responsible for making sure every aspect of our volunteer program runs smoothly. She works closely with staff members to identify and design new opportunities and programs for volunteers that help to meet the needs of our clients. Each new position gives volunteers the chance to make a difference and to contribute in the great work that the Centre does in their own unique way. Once positions have been identified,

interviews and screens each person to ensure that not only are they right for the position but that the position is right for them. Upon acceptance, new volunteers receive an orientation to the Centre, and training that is specific to their position. Finally, she is there to provide each volunteer with the coaching, assistance, support and recognition that they need to reach their potential. The Volunteer Services Coordinator also works with the staff members at the Centre to help them to provide the best possible experience for both themselves and the volunteers they work with. She puts on training workshops to teach new skills on how to work with volunteers, and offers assistance, coaching and support to help them over any rough spots that might occur. Finally, you will see our Volunteer Services Coordinator out in the community, networking and establishing relationships with other organizations and community partners. These efforts not only help to build and enhance the profile of the Centre and our volunteer program, but also promote volunteerism and the importance of volunteers in creating a healthy community. Central Community Health Centre is pleased to now be offering programs at the new Northside Neighbourhood Hub located at 114 Confederation St. The HUB is an initiative that came out of a proposal from the Elgin Children’s Network to the Doable Neighbourhood Projects. St. Thomas was chosen as one of four communities in Ontario to establish programs that would promote healthy, connected and safe neighbourhoods. To date there have been several initiatives including way finding signs, a neighbourhood ice rink, and the opening of the Northside Community HUB. There are many other programs and activities underway at the Northside Neighbourhood Hub. Feel free to drop by any time and get a tour and see what’s going on. Client Visitor Guidelines No Smoking Policy Central Community Health Centre is a smoke free clinic and is committed to providing a healthy environment for clients, staff, and visitors. Smoking is not permitted in doorways and must take place 9 metres (30 feet) from CCHC property. Scent Free Policy Exposures to strong scents and fragrances in the environment can cause strong discomfort as well as directly impact to the health of some individuals. For the safety and protection of clients, staff, and visitors please do not use scented and/ or fragrant products, perfumes or after shave. Parking Clients and visitors can park behind 359 Talbot St. at the Curtis Street parking lot located between East and Mary Street. Parking is free for 2 hours.
