About Concordia

Who we are Concordia Hospital and Concordia Place are integral partners within the Winnipeg Regional Health Authority, working with health care agencies and other providers to serve the community of northeast Winnipeg and surrounding areas. Concordia is accredited by the Canadian Council on Health Services Accreditation. Our team consists of over 1100 staff, over 125 physicians and over 350 volunteers. We rely on our patients and their families as part of our team and are very grateful for everyone’s participation in their health journey with us.

Welcome Welcome to Concordia Hospital, your 186-bed community hospital and Concordia Place providing a personal home like care to 140 residents. We hope this Patient Services Directory will help to inform you and your family of the services here and where to find them. At Concordia our everyday heroes -- our staff, leaders, physicians and surgeons strive to provide compassionate and respectful quality care to our patients and their families. As a Mennonite faith-based organization, we are here to serve as we partner with our Primary Care Community Clinics, North East Winnipeg Community Services, our faith community, and the Winnipeg Regional Health Authority (WRHA). While we know that change is coming to the services offered throughout the WRHA, our Emergency Departments and Intensive Care Units will remain fully functional until late spring of 2018. We are pleased that this past year, Concordia Hospital has improved wait times for clients in our Emergency Department. We are pleased that Concordia Hospital will be able to continue its role as a community leader offering quality, compassionate care in a faith-based environment to all patients we see. We excel as a provincial leader in hip and knee surgery, and our care for the elderly, for our patients, and for our community has been recognized and applauded. Concordia’s History Concordia Hospital was started in 1928 by a group of Mennonites who had immigrated to Canada from Russia. Seeing the need for health care in the growing immigrant community, they started their first project, which was a four-bed maternity hospital on 290 Machray Avenue, with two volunteer nurses and one volunteer obstetrician. Over the past 90 years, with God’s blessing and the dedication and perseverance of many, Concordia Hospital has grown to include an acute care hospital and a long term care facility, serving 320 patients and residents with an extensive campus of services. Despite the many changes and the growth, the mission of Concordia Hospital remains the same, “To provide outstanding health services, in a way that reflects Mennonite Christian values and demonstrates our passion to serve our community.”

Patient Relations

Concordia Hospital participates in the Winnipeg Regional Health Authority (WRHA) Patient Experience Surveys. The surveys are sent out to a random selection of people who have been admitted to our hospital or have needed to use the Emergency Department’s services. If a person is randomly selected for a survey, it is sent in the mail usually two to three months after their visit. The surveys have been developed by the National Research Council of Canada and ask a variety of multiple choice questions (with room for comments) regarding the care and services received at the hospital. Concordia Hospital then uses the survey feedback that has been submitted to help inform our care teams about what is done really well with the care we provide, and areas where there is opportunity to improve. The feedback is then used to make further improvements to our patients’ experience to ensure we are providing the best care possible.

Parking Patients and visitor parking can be found in the parking lots in front of Concordia Hospital and Concordia Place.

Accessible parking Parking for persons with a disability is available near the Emergency Department and the Rehab Services area (back of the building - enter off Kimberley Ave.). The appropriate parking decal must be displayed on the rearview mirror.

Hourly parking Parking is available on an hourly basis in front of both Concordia Hospital and Concordia Place.

Frequent visitor parking If you need to make many or long visits to Concordia within a short period of time, a long term parking pass is a convenient and affordable option. The price of a seven day pass is $40 while the price of a 30 day pass is $80. GST is included in these rates. The pass will work in the South East lot located in front of Concordia Place. To purchase a pass, contact the Security Services office located in the front lobby of Concordia Hospital or call 204-661-7229.

Meter parking Metered parking spaces are available on the front driveway of Concordia Hospital.

Transit Bus Information Printed Transit schedules are available at the Front Lobby Ambassador’s kiosk located next to Information in the front lobby. For information on bus routes and schedules, please visit the Winnipeg Transit website http://winnipegtransit.com/en

Handi-Transit Information Handi-Transit provides public transportation to individuals with disabilities who are unable to use regular Transit service. Door-to-door service is provided, using specially equipped vehicles and taxi cabs. Registration is required to use Handi Transit. Certain eligibility requirements apply. Taxi Information Taxi pickup may also be arranged by using the taxi phones (buttons are located at the bottom of payphone keypads) at our Emergency Department and the hospital lobby. Taxis may pick up and drop off fares at any of our entrances. Please do not order a taxi pickup until you are ready to leave Concordia.

Drop Off and Pick Up Visitors to Concordia can be dropped off or picked up at any of our major entrances. However, all entrances have a 15-minute loading zone restriction; vehicles cannot be left unattended. Please plan for this when making your arrangements to be picked up or dropped off. Please be courteous and do not block the way of other drivers, ambulances, healthcare staff and please adhere to the fire lane restrictions.

Family members are part of the care team as they are integral to the life and healing of a patient/resident. Family members provide support, comfort, and important information during their health care stay regardless of time of day, day of week, or department. Families are welcome 24-hours a day according to patient preference. The number of people welcomed at the bedside at any one time will be determined in collaboration with the patient and family. In situations, where there are shared rooms, this negotiation will include the other patient, his or her family, and other partners in care. Children supervised by an adult are welcome.

Visitor Information

Visiting Hours

If you are visiting a patient in the Intensive Care Unit (ICU), please report to the ICU waiting area to request permission to enter the unit. Visitors are welcome from 8 a.m. to 8 p.m. or from 11 a.m. to 8 p.m. for surgical patients.

Doors locked at 9 p.m. All entrances to Concordia are locked at 9 p.m. - except the entrance to Emergency, which is open 24 hours. To access the hospital after 9 p.m., enter and exit through the Emergency Department.

Who May Visit The support of family and friends is important to a person in hospital. In most cases, visitors are appreciated and welcome. Visitors under 18 years of age must be accompanied by an adult. A patient, family or health care team may decide to restrict visitors. For example, hospital staff may ask visitors to leave or limit the number of visitors in a room. Please respect that this measure is taken to protect the patient’s best interests. Pets are generally not allowed to visit. Special consideration is made for Service Dogs and for Pet Therapy. These arrangements must be made in advance with the Manager of Patient Care on the unit.

Things to Consider • •

Please do not visit if you are not feeling well. Be thoughtful of the rights of all patients and hospital staff. Be sure to treat them with courtesy and respect, and please assist with the control of noise and the number of visitors. Lounges are available on most floors. Flowers and balloons may impact individuals in the immediate environment who suffer from allergies. Suggestions for gifts include books, magazines, puzzle books, a new nightgown or pajama’s, slippers, sending a Well Wish (see Well Wishes for Patients for more information), etc. Please do not wear scented products as some individuals may suffer from allergies/sensitivities. Visitors must respect our patients’ right to privacy. Visitors must leave the patient room or care area when asked to do so by staff. Sleeping in a patient’s room may be considered following consultation with the patient and the care providers. For safety reasons, adult supervision of young children is required.

If you need help finding a unit Staff at our information desk can provide you with a map highlighting your route and destination. Also, Concordia volunteers are available to help you find your way. You can recognize our volunteers by the blue vests/jackets they wear. You may also ask any of our staff to give you directions. Each member of our hospital team is required to wear an identification badge. Our staff can answer questions of where departments are located, or refer you to someone who can help you find your way.

Flower deliveries To keep our patients safe from infections, flowers are not permitted in some areas of the hospital. Please check with unit staff before sending or bringing flowers to a patient.

Respecting Privacy and Comfort Most rooms at Concordia are not private rooms. When you come to visit a friend or family member, you may see and hear things related to other patients. Please respect everyone’s right to privacy. It’s greatly appreciated when you minimize your impact on other patients and their visitors. Please remember that people’s private information is confidential by law. Do not discuss what you see and hear in the hospital and please refrain from videotaping.

Visitor Lounges / Waiting areas Lounges and waiting areas are provided for patients and visitors at various locations throughout Concordia. Ask your nurse to assist you or your family in locating the lounge closest to the unit you are visiting. Complimentary television is provided in most waiting lounges.

Public washrooms Public washroom facilities are located on the Main and Ground floors. A wheelchair accessible washroom with diaper changing station is located on the Ground floor. Note: patient washrooms are intended to be used by patients only.

Smoke-free In accordance with City of Winnipeg Outdoor Smoking By-Law No. 62/2011 and WRHA policy, Concordia Hospital is a smoke free facility. Smoking is not permitted anywhere on the property of Concordia Hospital, including all entryways, grounds and parking areas. This includes the use of e-cigarettes.

Alcohol Alcoholic beverages are not allowed within Concordia.

Cell phones and other wireless communications devices The use of cellular phones and other wireless devices is permitted in Concordia Hospital, except where indicated.

Postal Service Volunteers deliver in-hospital mail to all departments and can be asked to read cards and letters received by a patient. They are also available to write letters. A Canada Post mail box is located outside the main hospital entrance.

Scent Free Concordia Hospital is a scent free facility. Perfumes, colognes, and scented toiletry products may not be used in the facility.

Hospital Services

The Concordia Café The Concordia Café is located on the ground floor of the Hospital just outside the public elevators.

Hours of Operation: • •

Monday to Friday, 7 a.m. to 8 p.m. Saturday, Sunday and Statutory Holidays, 8 a.m. to 3:30 p.m.

Tim Hortons and Grab and Go The Concordia Café features Tim Hortons which offer a variety of hot and cold beverages with a large variety of pastry choices. Concordia Café also offers an excellent selection of grab and go food choices that you can enjoy ranging from our assorted salads, deli sandwiches, wraps, bun-wiches, and many other cold food options.

Vending For your convenience, 24-hour vending machines are located in the back seating area of the Café as well as outside the Café, in the Emergency Department and Main Lobby.

Automated Teller Machine An automated teller machine is located across from the public elevators on the main floor. The $1.50 service charge for your transaction is a donation to the hospital.

Crutches, Canes and Walkers If walking aides are required, patients will be given information on how to rent or purchase them.

Hair Care Beauty Salon The unisex salon located in Concordia Place is open to patients of Concordia Hospital and offers cuts, colours, highlights, styling and perms. All services are provided at a reasonable rate.

Hours of Operation: Monday to Friday, 9 a.m. to 3 p.m. To book an appointment call 204-661-7207 (external) or 7207 (internal).

Telephone and Television Rentals Telephone and television rentals are provided by Hospitality Network. Weekday Activations: From the patient bedside telephone, dial extension 7515 and a Hospitality Network Representative will assist you during the hours of 8 a.m. to 7 p.m., excluding statutory holidays. Weekend Activations: All requests must be directed to the Hospitality Network Representative who is on site between the hours of 4 p.m. to 7 p.m. The representative can be reached at 7515.

To activate outside of the hospital: Please call 1-866-223-3686 during the hours of 8 a.m. to 7 p.m. Monday to Friday, and 9:30 a.m. to 7 p.m. Saturday and Sunday, excluding statutory holidays. Note: All rental services (including special rates for long term renters) are nonrefundable and non-transferable to another patient.

Concordia Community Pharmacy The Concordia Community Pharmacy is conveniently located inside Concordia Hospital on the ground floor across from the occupational therapy/physiotherapy waiting area.

Lost and Found Please see Security Services. To enquire about a lost item, please call 204-661-7229 or visit the Security Office.

Security Services For your safety, cameras are located throughout Concordia and Security personnel patrol the buildings and grounds 24 hours per day. Phone: 204-661-7229, 24 hours a day

Staxis and Wheelchairs Staxi Chairs Staxis are transport chairs available for outpatients and visitors who need assistance travelling to and from appointments at Concordia. They are for adult use only (age 12 and up). The chairs require another adult to push you. Please have a family member or friend accompany you if you need to use a Staxi. Please return the Staxi chair to the front lobby when leaving the hospital.

Wheelchairs Wheelchairs are available for use while you are in the hospital, and to get you to the front entrance when you are discharged. Please do not remove wheelchairs from Concordia. If you think you may need a wheelchair once you are home, your health care team can make those arrangements with you as part of your discharge plan.

Well Wishes to Patients Sending your love to patients at Concordia Hospital The Concordia Hospital “Well Wishes” program gives you the opportunity to send your special thoughts and best wishes to patients at our facility. Well wishes from family and friends bring a bright ray of sunshine to recipients, and are a wonderful way to show how much you care.

How it Works Concordia Hospital Volunteers deliver “Well Wishes” to patients on a daily basis (excluding weekends and holidays). The process is simple. Go to the Well Wishes link on the Concordia Hospital website www.concordiahospital.mb.ca, compose and submit your message. Our volunteers will receive the printed email and hand deliver it to the patient. If the patient wishes, the volunteer will read the message aloud. All messages received are handled confidentially. However, a Concordia Hospital Volunteer Services staff member will need to read these messages in order to ensure content is appropriate. As well, patients may choose to have the message read aloud. The system is designed for well wishes only. We cannot send outgoing replies from patients. E-mails intended to conduct personal business, containing questionable content or business/vendor solicitations will be deleted from the system. If the patient has been discharged, or cannot be located in our facility (some patients request anonymity), the email will not be delivered. Send your love through the Concordia Hospital “Well Wishes” program, and make today special for someone close to you in our care. Please be aware that you are unable to insert pictures or send any form of attachment(s).

Spiritual Care Spiritual Care at Concordia is reflective of our core value of compassion and respect. Our staff are trained to accompany and support our patients and their loved ones during a time of illness while being respectful of their beliefs, values, and religious or spiritual background.

Volunteer Services Giving Back Concordia Hospital is a community hospital, and we have always relied on community support. Volunteers have been the cornerstone of Concordia Hospital since its inception more than 89 years ago. Volunteers contribute their unique talents, skills, and knowledge to our facility and our community to provide personalized attention to our patients, to enable employees to concentrate on the work for which they are trained and to foster good relations between the hospital and the public which it serves. Over 350 volunteers provide over 27,000 hours of service to Concordia Hospital and Concordia Place. Are you interested in getting involved? We offer a wide variety of volunteer opportunities. For example, volunteers assist as ambassadors throughout the hospital, provide visits to patients, and deliver newspapers and fresh water to inpatient units. Volunteer positions are available 7 days a week, in morning, afternoon and evening shifts. Meal and Leisure Companion Program The one-to-one Meal and Leisure Companion Program is a volunteer assisted program, where volunteers visit and assist patients who require meal set up, cueing, encouragement to eat and/or who may benefit from enhanced social interaction. The program is offered on all in-patient units (excluding ICU, Emergency and Observation) for all meals, daily (dependent on volunteer availability).

At risk for falls? Falls are a leading cause of injury related to hospitalizations in the elderly population. Many people suffer serious injuries as the result of falls. Concordia Hospital is committed to reducing falls in the hospital setting. Tell the nurse if there are any issues that relate to you or your family member so that we can plan ways to reduce the risk of falls. Concordia Hospital has a Falls Prevention Program to alert staff when someone may be at risk for falls. A falling man symbol placed at the head of the bed indicates that a patient may be at risk for falls. Staff will implement appropriate safety measures in order to reduce the chances of sustaining a fall while in hospital. It is important for family members to inform staff when they are about to leave. This will allow staff to reset the bed or chair alarm, if one is being used. While no system is fail proof, communication with staff is essential to ensure all safety measures in place work to their fullest potential.

Language Services It is important that you and your family can communicate clearly with your health care team. People specially trained to interpret health information in dozens of languages can assist with many languages and dialects. If you would like to have an interpreter, please talk to your nurse to make arrangements. The Personal Health Information Act (PHIA) is a provincial law that governs the collection, use and disclosure of personal health information within the health care system. In accordance with the Act, all personal health information is collected and used in a confidential manner.

Personal health information includes: • • •

Name, age, address and personal health identification number (PHIN) Details about health, history and the care received Details about payment for the health care received

We only share personal health information with those who need to know it, in order to provide care. This may include some or all of the following: • • • •

Health care providers at Concordia Hospital or in the community who are directly providing, monitoring or evaluating health and health care Students who are members of the health care team Those who require such information to conduct health or illness-related research, or to plan and evaluate care in Manitoba Contact information will be shared with Spiritual Care providers in the community and with the Concordia Hospital Foundation, unless it is requested that it not be shared

Rights related to personal health information include: • • •

Examining your personal health information Receiving a copy of your personal health information upon request (photocopying costs may apply) Requesting your personal health information be corrected

My Health Teams bring together different health care providers close to home to provide the best care for you. Most teams are made up of doctors, nurses, nurse practitioners and others who work together to provide high-quality care. Each team member uses their expert knowledge and skills to provide the very best care possible. Some My Health Team providers will be based in the same clinic, health centre and/or community. Other providers will be in different locations. But they all will work together to plan and deliver care.

My Health Team will help to: • •

Link you with other health services and community resources depending on your needs. Connect you with an appropriate My Health Team provider. For example, a nurse may see you regarding diet concerns, or a mental health counselor may provide some short term counselling and link you with other resources to support your needs. Work with you and team members to manage and track your care in a coordinated manner. Reduce the time you spend repeating your medical history to different health care providers. Help you avoid unnecessary trips to the emergency room or urgent care.

My Health Team

If your regular doctor or health care provider joins a My Health Team, you will be able to access the services of that team. No individual registration is needed. Ask your doctor if he/she is part of a My Health Team.

My Right Care Emergency Departments are for health emergencies. Conditions that require emergency treatment include: • • • • • • • • • •

heart attack stroke major trauma severe head injury loss of limb in an accident severe difficulty breathing (due to an allergic reaction or another cause) sexual assault domestic violence severe bleeding or any time a person is unconscious

Concordia Place Concordia Place is a 140-bed Personal Care Home which strives to provide compassionate care and services in a home-like environment, guided by Concordia’s Mission, Vision and Christian Values. Concordia Place supports an “age in place” philosophy. Each room is single occupancy, that has its own washroom (toilet and sink) and is furnished with a bed, a nightstand, a dresser, and a chair. Residents have the ability to personalize their room as able. In addition to medical services, Concordia Place also provides services such as, but not limited to: Clinical Dietitians, Therapeutic Recreation, Spiritual Care, Hair Care & Esthetic Services, Finance, Mail and Laundry Services.
