Your Safety While in Hospital Health Care-Associated Infections (HAIs) Leamington District Memorial Hospital is dedicated to protecting and promoting the health of patients and health care providers. LDMH performs an active surveillance system to detect health-care associated infections (HAIs) within our facility. We incorporate the recommended practices of the Provincial Infectious Disease Advisory Committee on Infection Prevention and Control (PIDAC-IPC) to improve quality of care and patient safety. PIDAC-IPC’s best practices are a combination of the best available scientific evidence and expert opinion of professionals from the fields of infectious diseases, infection prevention and control, public health and epidemiology. Health care-associated infections (HAIs) are infections that a patient develops while in hospital and were not present before being admitted to the hospital. Some examples of HAIs include bloodstream, post surgical, urinary, respiratory, gastrointestinal, Clostridium difficile infection (CDI), skin and soft tissue infections. Factors that increase the risk to patients for developing an HAI include advanced age; underlying illness; complex treatment regiments; the emergence of new infectious agents; emergence of community-associated communicable diseases; prevalence of antibiotic-resistant organisms (AROs), such as Methicillin Resistant Staphylococcus aureus (MRSA), Vancomycin Resistant Enterococci (VRE), Carbapenemase-Producing Enterobacteriaceae (CPE) and Extended-Spectrum Beta-Lactamase (ESBL); and international travel. Patients with an HAI may remain in the hospital longer on average than patients without an infection. Additional treatment and or readmission to the hospital may be required.

How to Prevent (HAIs) One of the single most important practices you can do is wash your hands well. Germs that have the potential to make you sick are everywhere – at home, work, in the community and at the hospital. Good hand hygiene technique is an effective measure to prevent HAIs and limit the spread of germs, such as MRSA, VRE, ESBL & CPE and Clostridium difficile. It is important to clean your hands often with either alcohol based hand rub or soap and water before handling or eating food; before you drink; after using the bathroom; after blowing your nose; after coughing or sneezing; and before touching your face (e.g. eyes, nose or mouth). Ensure you have rubbed your hands for at least 15 seconds to kill or remove the germs on your hands. If you are unable to clean your hands, please ask your health care provider or family member for assistance.
Keep Two Simple Rules in Mind Rule 1: When your hands are visibly dirty or they feel dirty, clean them with soap and water. Rub all areas of your hands; including the back and front; between your fingers; your fingertips and cuticles. Dry them thoroughly by patting them with a paper towel. Rule 2: If you can’t get to a sink and for routine cleaning as noted above, use an alcohol based hand rub. Apply 1-2 pumps of the hand rub to kill the germs on your hand. Rub all areas of your hands; including the back and front; between your fingers; your fingertips and cuticles. Continue to rub your hands until they are completely dry. It is important to remember that the alcohol based hand rub is flammable and can cause a fire or burn yourself. Family members, visitors and health care providers also have an important role in reducing HAIs. It is important that they also clean their hands. Family members and visitors should clean their hands when they enter and exit the facility AND before they enter and exit your room. All health care providers should clean their hands when indicated by “Your 4 Moments for Hand Hygiene”.

“Your 4 Moments of Hand Hygiene� Moment 1: Before initial patient or patient environment contact, the health care provider cleans their hands when entering your room and before touching you or touching any object or furniture in your room. Moment 2: Before aseptic procedure, the health care provider cleans their hands before starting an IV, changing a dressing, or inserting a catheter, etc. Moment 3: After body fluid exposure risk, the health care provider cleans their hands after emptying your catheter, after starting your IV, after changing your dressing or after performing any other procedure where there was exposure to any of your body fluids and after removing their gloves. Moment 4: After patient or patient environment contact, the health care provider cleans their hands when leaving your room after they have finished providing care to you or after touching any object or furniture in your room.
Respiratory Etiquette Respiratory infections can easily be spread to others. It is important to cover your mouth and nose when you are coughing or sneezing. Sneeze or cough into your sleeve or a tissue; not into your hands. Immediately dispose of the used tissue into a trash can and clean your hands. If you have to leave your room for tests, your health care provider will ask you to clean your hands and put on a mask. To prevent the spread to others, persons who are sick or have been around anyone who has been sick please do not visit the hospital unless you are seeking treatment.

Infection Prevention and Control Measures Screening for AROs All admitted patients are screened for Methicillin Resistant Staphylococcus aureus (MRSA) and Vancomycin Resistant Enterococci (VRE). Depending on risk factors, you may also be screened for other AROs, such as, Carbapenemase-Producing Enterobacteriaceae (CPE) and ExtendedSpectrum Beta-Lactamase (ESBL). Your health care provider will take a swab of your nose, rectum, and any wounds you may have. We appreciate your cooperation in being tested as this is a necessary step in protecting patients. For more information on Antibiotic-Resistant Organisms (AROs) or Hand Hygiene, please ask your health care provider for the individual pamphlets.
Surgical Site Infection (SSI) WHAT ARE SURGICAL SITE INFECTIONS (SSIs)? Surgical site infections occur when harmful germs enter your body through the surgical site (any cut the surgeon makes in the skin to perform the operation). Infections happen because germs are everywhere – on your skin, in the air and on things you touch. Most infections are caused by germs found on and in your body.
WHAT ARE THE SYMPTOMS OF SSIs? • Increased soreness, pain or tenderness at the surgical site • A red streak, increased redness, warm/hot, or puffiness/swelling near the incision • Greenish-yellow or foul-smelling discharge from the incision • Fever of 101 degrees Fahrenheit or higher (or greater than 38 degrees Celsius) • The incision separates/opens up on its own Symptoms can appear at any time from hours to weeks after surgery. Implants such as an artificial knee or hip can become infected a year or more after the operation. If you experience any of the above symptoms after surgery, please contact your healthcare provider or visit the emergency department as soon as possible.

WHAT ARE THE FACTORS FOR SSIs? The risk of acquiring a surgical site infection is higher if you: • Are an older adult • Have a weakened immune system or other serious health problems such as diabetes • Smoke • Are malnourished (don’t eat enough healthy foods) • Are overweight

Speech Attendant Do’s and Don’t’s • Speak normally and at a regular pace. Your Admission Day Admission All personal information will be considered strictly confidential. It is important that you make staff aware if you have a Health Care Directive or if you have been in a hospital out-of-province during the past year. Hospital wrist identification bracelets are used for all inpatients.

Personal Items You should bring only necessary personal articles such as a toothbrush, toothpaste, comb and brush, shaving items, or cosmetics (No scented products please). Although LDMH provides patient gowns, you may want your own pajamas or nightgown together with a bathrobe and nonslip/nonskid slippers.
Medications Bring all current prescriptions and non-prescription medications, in their original containers. Some medications are not supplied by the Hospital and you may be required to supply your own. Any medications required during your stay will be prescribed by your doctor and will be brought to you by your nurse. All other home medications will be sent home with family. Please tell your doctor and nurse if you are allergic to any medications or suffer from any other allergies. If you have any questions about your medications, your doctor, nurse, or Hospital pharmacist will assist you.

What Not to Bring Please do not bring: valuable papers or documents, scented personal care products, items of sentimental value, jewelry or expensive clothing, large amounts of cash, alcoholic beverages, TVs, DVD players, radios, or plugin appliances unless otherwise approved by staff. The hospital will not accept responsibility for patients’ personal items and valuables.

For Your Safety At Leamington District Memorial Hospital, the safety of our patients, staff and visitors is our number one priority. Regardless of our roles within the hospital, we all must contribute to making a safe environment - safety is everyone’s responsibility. Sometimes however, things can go wrong for many reasons. When this happens, it may result in unintended injuries, complications such as infections, falls, medication errors, or other incidents affecting patient care.
What we do at Leamington Hospital to Improve Patient Safety: • Increase awareness and promote the partnership between patients/ families and staff in all aspects of patient safety. • Make recommendations and implement new policies and procedures to improve patient safety. • Evaluate and measure implemented changes made to improve patient safety. • Keep informed on ‘best practices’ by other hospitals, studies and reports, and new ideas for patient safety. • Ensure equipment is safe and up-to-date for use. • Ensure our staff is properly trained for their work. • Identify, report and follow-up in a timely manner on safety issues.
Ask questions! Don’t be afraid to ask questions. For the general public, medical language is hard to understand. We know that but sometimes we forget. We need to know if you don’t understand so we can explain it in a different way. It is very important that you fully understand the information you are given so you can make informed decisions about your care and treatment. Hand washing is a very important safety practice. Don’t ever be afraid to ask any staff member or Doctor, “Have you washed your hands?” And be sure to remind your family members and visitors to wash their hands too!

Balloon Policy In an effort to minimize unnecessary exposure to latex and to safeguard the health and safety of staff, patients and visitors, there will be no latex balloons permitted within the Hospital.
Electrical Appliances You are welcome to provide your own electrical appliances such as razors and hair dryers, which can be safely used in the electrical outlets available in the patient bathrooms. All electrical equipment must be approved and checked by the hospital maintenance staff before use. Battery operated appliances are encouraged. Radios, iPods, MP3’s or DVD players cannot be accommodated without headphones for volume control.
Fire Safety When you hear a fire alarm, stay in your room or area; do not move to another place unless a member of the Fire Department or hospital staff directs you. If you are in the Hospital Cafeteria, please stay there until the all clear signal is given. Elevators must not be used when there is a fire or a fire drill. Fire exits are clearly marked throughout the buildings. During a fire alarm, elevators are shut down. All fire doors automatically close until there is an “All Clear” announcement on public speakers. Those entering the Hospital at the time of an alarm will be asked to remain at their point of entrance until the “All Clear” is given.
Hospital Security Hospital staff and volunteers are easily identified by their photo identification badge. Please notify a staff member immediately if you observe any suspicious activity or individuals, have any security concern, or if you have lost a personal item.

Infection Control WHEN THERE ARE INFECTIONS IN THE COMMUNITY In the event of any community or hospital spread of infection, visiting will be restricted as appropriate to the situation. Infection Prevention and Control At Leamington District Memorial Hospital, we are committed to protecting the health and safety of our patients, visitors and staff. Patients who are sick are not able to fight off infections as easily as healthy people. If your loved one is not feeling well please ask them to postpone their visit. Hand washing is the best way to decrease infection. Please wash your hands frequently during your stay or use alcohol hand wash available throughout the hospital.
Infection Control Guidelines for Visitors Visitors must read and follow the infection control guidelines posted at all entrances. If an isolation sign is posted on a door all visitors must report directly to the Nursing Station where they will be provided with instructions.
To protect against the spread of infection, Visitors are asked not to: • Use the washrooms in the patient’s rooms – there are public washrooms throughout the hospital. • Sit on the patient’s bed during your visit. • Use the patient’s telephone. • Share the patient’s food or use any of their utensils, glasses or cups. 22
Patient Information How to get information as a family member or a patient Identify to the staff and your doctor one person you want to share information about your condition with and to act as your advocate if you need one. The staff will call that person at any time at your request. We will identify that person on your record. Please ask other friends and family not to call the Hospital for condition reports. Not all family doctors visit their patients in the Hospital. If your doctor does not or if you don’t have a doctor; you will be assigned to a hospital based doctor (Hospitalist). Keep track of what you want to ask the doctor, they make rounds daily. If you feel you need to communicate with the doctor in charge of your care between visits; communicate this to the nurse assigned to your care. We are proud of our Hospital and the compassionate care close to home we provide. As a patient or a family member – do not hesitate to bring any questions or concern you may have to our attention while you are in hospital. Communicate with the person assigned to your care or with the doctor. If you are not satisfied, ask to speak to a manager. A manager is on call 24/7 to help staff work through any issues patients may have with their care and services.
Scent Policy The Hospital has a “no scents make sense” policy for the protection of people with sensitivities or allergies to perfumes or scented products. Please refrain from wearing or bringing any scented products, including flowers such as lilacs and lilies into the Hospital.
Smoking Policy As provincially regulated, LDMH is a non-smoking facility. Smoking is not permitted within 9 meters of any entrance. There are designated smoking areas outside. On or before 2018 the hospital will be smoke-free as legislated by the Ontario Government. Ask your physician/nurse about our ‘smoking cessation’ products available while in hospital.

Patient Services Accommodation Admission criteria are specific to each patient care program. Your plan of care will be established with you and the LDMH health care team. People with valid health cards are covered by the Ontario Health Insurance Plan (OHIP) which includes admission to a four-bed room for inpatient care. Some of the costs NOT covered by OHIP include: ambulances, crutches, walking casts, etc.
You may want to upgrade your Room to: • semi-private with 2 patient beds in one room • private with 1 patient bed in a room Please check your insurance coverage and present your insurance cards at the time of admission. OHIP does not cover extra costs such as upgraded rooms, telephone or television. Your insurance company may cover part of the costs of upgraded accommodations, if not, you are responsible for those costs.

ATM Machine A 24-hour ATM machine is located on Basement Floor across from the Gift Shop.
Cafeteria The Cafeteria is open daily from 9:00 a.m. to 1:00 p.m. Monday to Friday, closed weekends and holidays. Refreshment and confectionary vending machines are available after hours. The Cafeteria is located on the first floor directly across from the main elevator. Vending machines are located outside the cafeteria and in the main entrance lobby.
Crisis Intervention Counselors are available to help you, or your family, deal with a crisis situation.
Discharge Discharge time for inpatient units is 11:00 a.m. To avoid additional charges, please make sure you leave your room by discharge time and allow it to be prepared for the next patient. If you require transportation home, the hospital will not be responsible for payment.
Patient Advocate The Patient Advocate works to support you the patient and your family during your stay or visit to the Leamington District Memorial Hospital. The Patient Advocate assists patients and families who are experiencing difficulty, or who may have questions, concerns or even a compliment about their care or service at LDMH. If a concern arises during hospitalization, we want to know about it. The Patient Advocate is available Monday to Friday, 8:00 a.m. to 4:00 p.m. at 326-2373, ext. 4118

Finding a Patient Staff are permitted to provide confirmation as to whether or not a patient is at the Hospital, provided that the patient has not requested otherwise. Staff cannot provide any information on the condition of the patient. • 519-326-2373 and Dial “0” or say switchboard where an attendant will be glad to direct your call.
Flowers/Parcels/Mail Flowers, parcels and mail will be delivered to your room. Please note: flowers are prohibited in ICU.
Food Services Patient meals at LDMH are an essential part of your care. LDMH takes great pride in our food services. Patient meals are prepared fresh daily by our dietary staff in consultation with your health care team both as a comfort and to support recovery. The hospital cafeteria is located on the 1st floor - directly across from the main elevators.

Internet and Wireless Guest Network Leamington District Memorial Hospital is pleased to offer guest wireless internet access. This wireless network is available, at no charge, for patients and their guests to use as long as the person is an inpatient at LDMH. Once admitted, patients may use their own laptops to login using their MRN (Medical Records Number) and their Account Number, which are available on their medical bracelets. The login is valid for 24 hours after which patients will have to go through the initial login process again. It is important to note that while LDMH is providing wireless internet access, we do not provide wireless laptops or other wireless devices to take advantage of this access nor will we provide printers. If you have problems accessing the internet over the LDMH Guest Wireless Network, hospital staff cannot assist in making changes to network settings or perform any troubleshooting on your device. You will need to refer to your owner’s manual or seek other support for assistance. The LDMH Guest Wireless Network is not secure so you should take the usual prudent precautions regarding the transmission of confidential personal, credit card or banking information. LDMH assumes no responsibility for any personal information that is compromised nor any damage to your hardware or software due to electrical surges, security issues or consequences caused by viruses or hacking or any other causes. As part of LDMH’s normal network management processes, your transmissions will be monitored and it is our expectation that you will use our wireless network in a responsible manner and in compliance with all Provincial and Federal laws.

Interpreters – Language Service Associates LDMH offers Interpreter services. Many people with limited Englishspeaking skills are hesitant or unable to access health care because of the language barrier. LDMH uses Language Services Associates which is an over the phone interpretation service providing interpretation in various languages. This service provides our staff access to interpreters 24 hours a day, each and every day. These interpreters are highly trained and certified in the medical field. Ask any LDMH staff to assist you in getting connected to an interpreter. The LDMH staff will be happy to assist!
Newspapers The Southpoint Sun is available in the ground floor main elevator lobby near the gift shop.
Parking PAY AS YOU GO Public Parking is available on the north and east side of the Hospital, accessible by gates off Fader and Armstrong Streets. There are also a limited number of free 15 minute parking spaces to the south east of the facility off Fader Street. There are accessible parking spaces in both areas. Parking fee is $5.00 per day. Monthly Parking Passes are $25 per month and are available for purchase from the Business Office or Registration after hours with a $10 deposit, which is refunded when the card is returned. NOTE: Emergency area, patient discharge area, 15 minute zone and handicap areas are for designated use only. All others will be ticketed and towed with fines payable to the Municipality of Leamington.

Programs & Services Inpatient Programs Our Inpatient Programs including Maternal-Newborn Obstetrical Unit, Intensive Care/Step Down Unit, Medical-Surgical Unit and Complex Continuing Care/Rehab Unit which are all staffed by a collaborative team of healthcare professionals including physicians, nurse practitioners, registered nurses, registered practical nurses, physio therapists, occupational therapists, respiratory therapists, speech pathologists and dieticians who work in collaboration with patients, families or caregivers, other health care providers and the community to deliver safe and efficient care that is focused on our patients.
Emergency & Outpatient Programs The Emergency Department operates 24 hours per day with a Physician onsite. As part of our commitment to provide service close to home, the Hospital has developed specialty clinics for outpatient services. The following Outpatient Clinics are currently offered to residents in our community: • Anticoagulation Clinic • General Surgery Clinics • Gynecology Clinic • Medical Day Care • Mental Health Program • Ontario Breast Screening Program • Pediatric Clinic • Pre Admission Surgery Clinic • Preanesthesia Clinic • VideoCare Consultations - these are supporting patients to access specialists who are at a distance. An example might be a follow-up visit by videoconference with Sick Children’s Hospital in Toronto. We continue to explore other programs and services as opportunities arise to keep our commitment of providing ‘compassionate care - close to home.’

Diagnostic & Therapeutic Services The Hospital operates a comprehensive Diagnostic Imaging Service that includes full Radiology Services, Ultrasound, Mammography, CT Scan and Nuclear Medicine. The Mammography program has expanded to include Tomosynthesis (3D Breast Imaging) – one of only a few hospitals in the province that currently offers this technology. LDMH hosts the Ontario Breast Screening Program (OBSP) and is a certified Breast Assessment Center. The Cardiopulmonary Department performs Pulmonary Function Tests, Respiratory Therapy, ECG’s, Stress Testing, Holter Monitoring and Echocardiography. Clinical Pharmacy services include drug utilization, counseling and pain control and management. The laboratory provides essential laboratory testing for in-patients and the Emergency Department. Outpatient testing is available at one of the local private laboratories.

Emergency Department Program What we do The Leamington District Memorial Hospital (LDMH) Emergency Department (ED) is part of a regional network in Windsor Essex County. The LDMH Emergency Program provides a range of services to meet emergency health care needs in the South Essex communities. The LDMH ED provides emergency care for patients who are seriously ill or injured, or who are suffering from a life-threatening condition and who are unable to wait to be seen by their family physician. People with troublesome chest pains, signs of stroke or severe breathing difficulties should call 911 for immediate care. At LDMH ED we see approximately 30,000 patients per year with a variety of illnesses, injuries and complaints.
Transfer of Care It is important to know that we are able to initially assess the needs of all patients that arrive through our doors. Should a patient require care beyond the abilities of our Department & Hospital, the appropriate consultation process will be followed and the transfer of patient care will occur. This may occur in a number of different ways; a follow up appointment with a specialist or fracture clinic or an ambulance transfer to the most appropriate hospital where the receiving specialist will take over care.
Emergency Department Team at LDMH Our Emergency Department is staffed 24 hours/day 365 days/year with an arrangement of physicians, nurse practitioners, nursing staff and unit clerks. This group of health care professionals is specially trained in the delivery of emergency care. The Nurse Practitioners (NP) run a “fast track clinic” that sees patients who would normally be seen in a doctor’s office, available 7 days a week during business hours.
What can you expect when in the Emergency Department When you arrive in the Emergency Department, you will be welcomed by a triage nurse. The triage process uses Ontario-wide guidelines to decide how quickly you need treatment. These guidelines are based on a history of your illness and a short physical assessment. Due to the unpredictable needs of our patients, we cannot give you an accurate predicted time for when you will be seen. This is because the ED 31
does not work on a first-come, first-serve basis; patients who are sicker are seen first. This is why you will sometimes see someone come in after you and go into the ED before you.
Religious & Spiritual Care Non-denominational services are provided for our patients each week. Pastoral Care services are available on an on-call basis. Please ask your nurse for more information.
Telephones Family and friends may call you by dialing 519-322-2501 + your extension from a TOUCH TONE phone or 519-326-2373 (ask for your extension). To call OUTSIDE the hospital, dial ‘9 ’, then the telephone number. For LONG DISTANCE, dial ‘0’, and the operator will connect you to a Bell operator. Calls are charged to your home phone, calling card, or you may call collect.
Television Information pamphlets/order forms for television rentals are available at each nurse’s station. Personal TV’s are not permitted.
Valuables You should send all articles of value home. If you are unable to send your valuables home, the nursing staff will put your belongings in a “safe keeping” envelope which will be stored in the Business Office safe. Please be advised that the Hospital cannot be responsible for loss or damage of personal belongings.
Visitors Visitors may be asked to leave a patient’s room while care or treatment is in progress. In some situations, family or friends may actually become involved in patient care (bathing, walking, feeding etc.). If this is the case, please discuss this with your nurse. Visiting hours may be extended to family members participating in planning and/or providing care. Children visiting must be monitored by the parent. Remember to respect the privacy of other patients. 32
Visiting Hours Visiting Hours are from 8:00 am to 8:00 pm. ALL visitors MUST wash their hands upon entering and exiting the hospital. Young children are strongly encouraged not to visit. Please consider the following: • Don’t visit when you are ill • Don’t bring children to visit (maternity is an exception) • Wash your hands at the entrance and when you exit • Limit the number of visitors to a minimum • Respect the privacy of a roommate – control the noise Ask for instructions about visiting before you enter when you see an INFECTION CONTROL SIGN on the door of a patient’s room. It is your responsibility to protect yourself, the patient and the community from spreading infection. A staff member may ask you to leave to perform a treatment or if visiting is creating a disturbance. In general 2 visitors at any one time is the maximum - take turns visiting to keep the numbers down and ask the patient who should visit. Additional visitors may be asked to wait in the lounge.
Volunteer Services Volunteer Services provides many programs for adults and students to volunteer in our patient care and non patient care areas at Leamington Hospital, Leamington and Area Family Health Team and at the Erie Shores Hospice Residential Home. At Leamington Hospital volunteers have the opportunity to provide care to our patients and support our staff in the Operating Room/Recovery Area, Emergency Room, Clinics and other patient care areas. Hospital volunteers can also choose to give their time as greeters in our registration area or volunteer in our Inspirations Gift Shop providing retail therapy for our staff, patients and visitors.
