About VON VON is the longest serving healthcare organization in Canada. We offer more than 75 different home care, personal support, and community services to enhance each client’s quality of life. We provide seniors with the supports needed to remain independent and in their own homes for as long as possible. In the Peterborough, Victoria and Haliburton Site we’ve been providing these services for over 75 years.

Our Service Stands Out • VON Peterborough, Victoria and Haliburton Site delivers care, 24 hours a day, 7 days a week, all year long. • VON delivers a variety of nursing care, specialized services, home support services, and Community Support Services. • VON offers personally designed care for the needs of our clients, caregivers and families. • As a charitable not-for-profit health care agency, donations are used to provide free or subsidized services to persons who cannot afford to pay. • For expert care and friendly, professional services, you can depend on VON’s carefully chosen health care team: Registered Nurses, Registered Practical Nurses, Health Care Aides, Personal Support Workers and Nurse Practitioners. • Experienced and specially trained staff provide a variety of in-home quality Nursing, PSW, and Community Support Services to people of all ages, backgrounds and needs. • Continuing education is a VON priority. VON staff attend specialized training and advanced courses in community health care to ensure they remain up to date with current practices and regulations. • For services to help you remain at home, actively and independently, VON offers Home Care and Community Support Services such as Adult Day Programs and SMART Exercise Programs. • VON Nurses link people with helpful community resources. • Every effort is made to speak with clients in their preferred language. “I am so blessed… there is no other word to describe it… I am just so blessed” Doris Gardner is 92 years old and currently takes care of and lives with her eldest daughter Ann; “Ann is going to be 70 her next birthday… we are both just shuffling along together,” Doris chuckled as she spoke of herself and her family. Ann attends VON’s Adult Day Program twice a week; she absolutely loves it!” Doris raised her family in Norwood, living on a farm that her husband, Allan, managed. “No sons, but our girls [four] worked just like sons - drove tractors, baled hay, did whatever.” Ann lived on the farm for holidays but spent most of her time at the Smiths Falls Hospital School; “Ann was always happy to get home and always happy to get back to school. Smiths Falls was a good school for kids like Ann who had trouble learning”. Doris was teaching at that time at the Norwood Public School; “I taught the perfect grade… grade 2; they like to come to school, they like their teacher, they are accustomed to being in school… they are the perfect grade.” Ann moved into the same apartment building as Doris in 1993; she lived on the third floor. Doris provided support - “when Ann needed something, I would run down the stairs - 5 floors ya know!” Later, when Ann moved in with Doris, they decided to become vegetarians for health reasons “Ann is a diabetic and since moving in with me her need for medication has been significantly reduced. We call ourselves flexatarians - not strict Community Health Guide

vegetarian; Ann will come home from the Adult Day Program and say ‘I had roast beef and potatoes for lunch.’ The Adult Day Program gives her the opportunity to eat outside the box - it’s very nice.” Ann’s sisters are all very good to her; “Ruth takes Ann and I to all of our appointments and takes Ann to her day program. I am so blessed by my family. Having the VON Adult Day program is added help for me. Ruth and I go grocery shopping or do errands - anything I can’t do myself. I don’t like to leave Ann alone for too long. ” Doris and Ann are a perfect example of how the VON Adult Day Programs provide valuable support to both the individual attending and their caregiver. Participants enjoy the therapeutic social and recreational activity of the day program while their caregivers get a much needed break to look after their own needs, knowing their loved ones are safe and well-cared for. Ann just loves the day program - “I like the staff, they do the cooking - I like meeting friends; the staff are all very helpful.” “My concern is for Ann… she is very aware of needing help and the staff are so good to her - the VON Adult Day Program is the perfect solution for us. Karen, her nurse, cuts Ann’s nails and keeps track of her health at the VON Centre. They are so compatible with Ann; she loves all the staff and knows all the other clients. In her stage of life right now, we don’t get out much. VON is our social life!”

Local VON Home Health Care Services VON, through the Peterborough, Victoria and Haliburton Site provides Registered Nurses and Registered Practical Nurses to go into clients homes to provide holistic client centred care which includes education and counselling to individuals and families. Our health care professionals provide:
Wound Care VON nurses care for patients with simple or complex wounds including those from surgery and trauma as well as chronic wounds.
Enterostomal Therapy After ostomy surgery, the VON nurse helps the individual learn to care for the ostomy and adjust to a new way of life with minimal disruption. VON enterostomal therapists are specialists in skin and wound care and act as consultants to other staff members at their site and throughout the community.
Palliative Care VON nurses deliver compassionate care and comfort to clients facing the end of life, as well as to their families and caregivers. The VON Peterborough Site also has nurses who are specialized in palliative care that goes further and takes care to an expert level.
Intravenous Care Many types of infusion therapy are being offered at home or in a clinic setting, whether people need fluids, medication, or even chemotherapy. VON nurses can provide a wide range of therapy including care of peripheral and central venous access devices.
Paediatric Services Trained staff offers paediatric care in the area of vascular access device management, wound management, medication administration, and diabetic care. Community Health Guide

Respiratory Care VON nurses provide ongoing care for clients with chronic obstructive lung disease, emphysema, cystic fibrosis and other respiratory conditions. The focus of respiratory care is to stabilize the client, shorten their hospital stays and decrease their overall visits to the hospital.
Dialysis In peritoneal dialysis, a solution removes the body’s waste products through the tissue that lines the abdomen. VON nurses care for clients with kidney disease and on Peritoneal Dialysis to support clients in their own homes.
Foot Care Clinics & In-Home Service VON nurses provide foot care for all ages from infants to seniors. Specially certified nurses provide assessment, care and advice for clients. Care includes specialized assessment, clipping nails, treating corns, calluses, ingrown nails and thickened nails and preventative care. Specialized diabetic foot care is also provided including monofilament testing.
Local VON Health Services Nurse Practitioner Services Nurse Practitioners are Registered Nurses with additional education in the provision of primary health care. They are able to assess, diagnose and treat a broad range of problems, and can order diagnostic tests and prescribe certain medications. VON Peterborough, Victoria and Haliburton site has Nurse Practitioner Clinics in Keene, Lindsay, Bethany and Kirkfield.

Local VON Community Support Services Community support services are offered through a variety of professional and volunteer programs. VON, through the Peterborough, Victoria & Haliburton Site offers:
Adult Day Programs VON Adult Day Programs are designed to offer engagement and relaxation in a secure and home-like setting for older adults who may be experiencing physical, social or cognitive challenges. While participants enjoy a day away from home, their caregivers benefit from a day of respite to spend as they wish -- taking time to rejuvenate their energies, go to work or attend to those items that are on hold. It has been recognized that attending an Adult Day Program better supports clients to remain living in their own homes and delays admissions to Long Term Care. Caregivers feel supported as VON staff work closely with family to better meet each individual’s needs. Our clients attend the Adult Day Program to participate in a variety of planned activities, supported by trained staff such as Recreation Therapists, Activationists, PSWs and Volunteers. These activities are designed according to the individual interests and abilities of our clients and include: • Baking, crafts, hobbies • Cards and board games • Discussion Groups, current events, reminiscence opportunities • Daily Exercise sessions to maintain strength and mobility • Music and entertainment • Seasonal events and celebrations • Home Cooked noon meal Community Health Guide

Currently VON operates eight Adult Day Programs across the region in communities such as Curve Lake, Lakefield, Peterborough (two locations), Havelock, Campbellford, Cobourg and Port Hope. Funding for this program is provided in part by the Central East Local Health Integration Network, community donations and a small daily client fee. The regular client fee for services is $19.00 per day which includes refreshments, a nourishing meal and all activities. A fee subsidy is available where financial need is indicated. Overnight Stay Program The VON Overnight Stay Program operates as an extension of the Woodland Adult Day Program located in the west end of Peterborough and is designed to help family caregivers get an overnight break from caregiving. Participants are individuals who are currently attending one of the eight VON Adult Day Programs and whose health care needs are stable and predictable. Overnight Stay guests are provided with a relaxed, friendly, safe and secure setting, each having their own bedroom with access to a walk-in shower. Trained staff are on-site at all times. This program welcomes four guests each weekend from Friday at 4:00pm to Sunday at 4:00pm.

During the daytime hours additional staff and day clients join the group to enjoy planned recreational activities. Funding for this program is provided in part by the Central East Local Health Integration Network, community donations and a daily client fee. The regular client fee for this service is $100 for a two night stay which includes refreshments, nourishing meals and all activities. A fee subsidy is available where financial need is indicated. In-Home Respite Program The VON In-Home Respite Program is designed to provide meaningful leisure visits for clients living with cognitive or physical health conditions who experience difficulty in accessing leisure opportunities in their community and whose caregivers could benefit from regular period of respite. While our clients receive a weekly visit from a trained Respite Worker, their family caregiver is able to enjoy those hours each week as they wish -- spending the time at home or out and about to catch up with friends, go to an appointment or run errands. After the Respite Service Supervisor meets both client and caregiver in their home, a care plan is developed detailing what the client would benefit from during the weekly In-Home Respite visits. These visits occur each week; same day, same time, same Respite Worker resulting in a success in building trust and friendship between client, caregiver and Respite Worker.
Keeping in mind the Care Plan, specific activities are implemented and may include: • Gentle exercise to maintain strength and mobility • Making lunch together • Reintroducing hobbies, games or music once enjoyed • Wonderful conversations Community Health Guide

• Social outings such as car rides, walks in the park or zoo. • Community activities or special events This program is currently offered to individuals living within the County of Peterborough. Funding for this program is provided in part by the Central East Local Health Integration Network, community donations and a small hourly client fee. The regular client fee for services is $5.00 per hour with fee subsidy available where financial need is indicated. Seniors Maintaining Active Roles Together® SMART Exercise Program The VON SMART Exercise Program is an exercise program designed for adults 55+ who may require a little extra support due to health issues, mobility restrictions, confidence, chronic disease or other reasons. There is no floor or mat work involved and all exercises are tailored to the participant’s ability level. Developed and implemented by VON, the SMART Program provides exercise opportunities for older adults keeping in mind their personal fitness level and ability. VON SMART Programs are led by adult volunteers who receive specialized training, including CPR and First Aid and ongoing education to deliver safe, appropriate, physical exercises designed for successful aging. This program is currently offered throughout Peterborough and surrounding area as well as Haliburton and Minden. These select exercises are offered weekly in both a group setting and one to one in a participant’s home for those individuals not able to attend a group class. Weekly classes are set up in 15 week sessions taking breaks between sessions. Funding for this 15 week program is provided in part by the Central East Local Health Integration Network, community donations and a small participant fee of $2.00 per class or $30.00 for the 15 week session.

Assisted Living Services The VON Assisted Living Services for Seniors provides services to eligible seniors living within designated geographic areas including the City of Peterborough and the communities of Campbellford, Cobourg, Lakefield,Havelock and Curve Lake. Services include personal support assistance such as dressing, personal hygiene, assisting and monitoring medication use and other routine activities of living; homemaking services including shopping, housecleaning and meal preparation; security checks or reassurance services including visits to ensure client health or safety; and care coordination. Funded by the Central East LHIN, services are flexible to the needs of the client and available 24 hours a day, 7 days a week on both a scheduled and unscheduled basis. Eligible seniors include those at high risk of requiring institutional care without this flexible and client-centred style of enhanced support. The VON Assisted Living Services allow these individuals to remain in their own homes with the services and support they need to maintain their health and safety. Access to this support service is through referral by the Central East LHIN – Home & Community Care.

Adult Day Program Assist VON staff in delivering a program of activities to clients attending the Adult Day Program. Volunteers with this program, which operates at five different sites throughout the City and County of Peterborough and three in Northumberland, enjoy socializing with clients and fellow volunteers while working with staff in one of our Adult Day Centres. VON Adult Day Centres provide a safe and supportive environment for seniors and people with long term illnesses such as Alzheimer Disease, Parkinson’s or living with the effects of a stroke.

Community Corporation Provide leadership at the local site level of VON. People interested in this level of volunteering choose to share their expertise and knowledge to enhance organizational objectives and support the goals of advocacy, strategic development and fundraising to enable VON charitable programs to be delivered locally. The Board and Committee members generally meet in the evenings on a regular basis. Administrative Support Administrative volunteers assist during regular business hours at the VON office or other off site locations. Volunteers assist with filing, photocopying, faxing, bookkeeping, data entry etc. Foot Care Foot Care clinic volunteers greet clients and assist with administrative functions of the program. This position incorporates socializing and bookkeeping and is done in one of our many Foot Care Clinic locations throughout the three counties we service. Volunteers keep the clinics running smoothly while specially trained nurses provide foot care to our clients. Special Events Throughout the year, the Peterborough, Victoria & Haliburton Site holds fundraising events in the community. Volunteer assistance is most welcome to make these events a success. SMART Group Program SMART Exercise Program volunteers receive extensive training through the VON SMART Program Coordinator in order to be qualified Seniors’ Fitness Instructors. The Program Coordinator at VON mentors volunteers during this process until they are comfortable in leading the weekly exercise groups on their own. VON provides all the training at no charge to the volunteer. Other volunteers play a supporting role in the SMART Program by welcoming participants and helping to keep our groups running smoothly.

SMART In-Home Program SMART In-Home Volunteers receive a one-day training session in carefully designed exercises to assist older housebound adults to maintain or increase their level of fitness. Volunteers then visit a participant’s home on a weekly basis to lead them through this series of exercises and encourage them toward their goal of improved mobility. The SMART Program Coordinator mentors the volunteers and VON provides all the training at no cost to the volunteer.
How to Become a Service Volunteer with VON It is as easy as calling the VON office and expressing your interest in becoming a VON volunteer. VON will ask that you: • Complete a confidential application form • Attend an interview with the Coordinator of Volunteer Services and successfully complete our screening process. • Participate in training and orientation appropriate to your Volunteer position After completing these steps, you will become involved with a program that suits your interests, skills and experience.

How to Become a Community Board Volunteer with VON • Contact the VON office and express your interest in joining the Community Board. • Submit a written resume. • Attend an interview with the Community Board Chair, the Chair of the Board Development Committee and VON staff representative. • Participate in Board and/or Committee Member training and orientation.

Community Corporation VON Peterborough, Victoria and Haliburton Community Corporation is the VON equivalent to a volunteer Board of Directors or Foundation; responsible for raising funds to support local programs and services. They also participate in a variety of activities promoting community based health care, and advocate on behalf of seniors and caregivers in the region.
Did You Know??? Provincial Governments are responsible for healthcare in Canada; and to address the ever-increasing need, fundraised dollars are also required to help cover costs. The money raised by our VON Community Corporation comes from a variety of sources and is used to: provide client subsidies for those unable to pay, purchase specialized medical equipment, and to provide ongoing training and education to our professional staff and volunteers. In 2018 we need to raise $175,000 from the community. Please help us reach our goal by making a donation or organizing a special event.
