This form must be updated every time there is an alteration to parents booked hours. If you are not able to book actual times care will be considered non-booked (casual care). Educator cannot be guaranteed to be available to meet all requests for casual care. School aged children are booked on a school term basis. If care is required during holidays an additional booking form is required and holding fees for cancellations will be charged. For termination or decrease in hours of care, either parent or FDC Educator must give 7 days notice to Windermere. Any late notice will result in regular hours being charged for the period of 7 days. We agree to follow the conditions overleaf:

CHILD CARE BENEFIT (CCB) is a payment made by the Commonwealth Government to assist families with the cost of quality care. To register for Child Care Benefit you need to contact the Family Assistance office on 136 150. ABSENCES are charged at the normal rate and CCB is paid for 42 allowable absences, across all services per financial year (which includes public holidays, illness, curriculum day, etc). CCB cannot be claimed for absences taken for more than 6 consecutive weeks. When a family goes on leave, fees need to be paid and timesheets must be signed in advance. Educators must be notified if your child is not going to attend care for any reason. If your Educator is unable to care for your child fees will not be charged. Please notify the Early Childhood Education and Care Services office 24 hours prior so that alternative care can be arranged. Parents will supply meals/snacks? YES NO

All families are required to pay for their care through the Hub Debit system, via a bank account or credit card. I agree to pay my fees on HubWorks via: Direct Debit Self Select
