WELCOME Thank you for your interest in the Sixth Form at The Hazeley Academy Our vision is to equip students with the most valuable qualifications whilst developing their Character, Confidence and Creativity; we frequently refer to this as Q3C for short. By achieving this vision we believe our young people are prepared to take their place in the increasingly dynamic world in which we live and will have the tools to enable them to adapt and change in response to challenges and opportunities. Partnerships are at the heart of all we do. Partnerships between staff, students and home are the bedrock on which we work. Open and honest communication is fundamental to our success. We have high expectations for which we do not apologise, but we maintain these expectations in a supportive environment where we aim for success. This is very much the Hazeley way. The KS5 teachers are highly skilled specialists, who have access to the best support and training. This high level of excellence is also reflected in our KS5 pastoral team who are experts, not only in supporting students to choose the most appropriate path through the Academy, but also beyond into further study or work. DESTINATIONS Sixth Form students develop the confidence to go onto University and Further Education, securing places on the best courses at top universities throughout the UK and overseas. Hazeley students are placed well above the national average in achieving Russell Group university places and accessing more of their first choice universities. The Hazeley Academy has been hugely successful with competitive undergraduate applications to Medicine, in addition to courses as creatively diverse as Aerospace Engineering, Podiatry, Architecture, Sound Technology, Professional Musicianship, Occupational Therapy, Primary Teaching, Animal Science, Pharmacy, Computer Science, Music Production, Graphic Design, International Fashion Management, Games Software, Law, Hispanic Studies, Sport Science … the list really does go on and on. Importantly, a significant proportion of our students also leave The Hazeley Sixth Form equipped with the character for the world of work and apprenticeships.

SIXTH FORM TEAM Sixth Formers at The Hazeley Academy are well supported in their transition into sixth form life. All students will be allocated to a tutor group; they will meet with their tutor every morning who will be instrumental in supporting the students over the two years. Working in conjunction with the tutors is the Sixth Form Executive Team which consists of a Progress Leader for each Year group, Sixth Form Centre Manager and Q Zone student support, as well as the Head of Sixth Form. An even more important leadership team within the Sixth Form is our Senior Students, each tutor group is represented by a Senior Student and they are led by our Principal and Deputy Principal Girl and Boy. The senior students lead not only within the Sixth Form but also in conjunction with the whole Academy Student Voice. Alongside the Senior Students are the House Captains, these students are instrumental in organising and supporting the younger years in their house events and supporting the Heads of House.

WHAT MAKES OUR SIXTH FORM UNIQUE? Student Voice is very important at The Hazeley Sixth Form. Hazeley students develop the character, confidence and creativity to lead important aspects of sixth form life especially through our Principal Girl and Boy as well as our Senior Students and specialist student led committees. At The Hazeley Sixth Form we are proud of the care and support we provide for our students. The pastoral team looks after our students ensuring that they feel safe in a dynamic learning environment. Every day our students participate in our Life Skills programme which includes elements of personal, social, health and economic education as well as spiritual, moral and cultural information. We also offer a wide range of extra-curricular and sporting activities as part of The Hazeley Creativity Action Service (HCAS) programme. This includes the Young Enterprise Award, Duke of Edinburgh Award, World Challenge, Community Sports’ Leaders, Business Challenge, volunteer opportunities and a range of inter-house sporting activities. Recently renovated Sixth Form facilities include different learning and study zones. The Hub is an area for group study; here students also have access to the new ‘Pronto’ café where studying can be accompanied with a fresh coffee and hot or cold snack. The Q Zone is an area for independent silent study; students work in individual study booths and have access to Sixth Form specific books and journals. Here they also have access to computers and laptops, and can also use their own devices as both the Q Zone and Hub have WIFi.

HOW TO APPLY? Applications are available on the Post 16 area of our website. Hazeley students must ensure their current teachers approve their subject choices. To study the A Level route offered, students will need to achieve 8 A*-C at GCSE as well as the individual subject entry requirements. The Academy also offers two vocational routes which require 5 A*-C at GCSE. Applications must be submitted by 4pm on Friday 29th January 2016. All received applications will be acknowledged by email.

Transition Meetings Applicants who meet the application deadline may be invited for a transition meeting. Interviews will take place for Hazeley students before the February half term and for external applications, prior to the Easter break. The transition meeting is a key part of the application process as it is an opportunity to discuss subject interests and future plans, to ensure that the correct subjects have been chosen.

Offer of a Conditional Place Following the transition meeting, many students will receive a conditional offer based on the published grades being achieved in their chosen subjects. In order to accept this conditional place, the acceptance form must be returned.

Induction All students in receipt of a conditional offer are expected to attend the Sixth Form Induction on Monday 27th and Tuesday 28th June 2016. Students will attend taster lessons in each of their chosen subjects to get an idea of what it is like to be a student at The Hazeley Academy Sixth Form.

Enrolment Student enrolment will take place on GCSE results day, Thursday 25th August 2016. If students do not achieve the GCSE grades expected, options can be discussed with a member of the Sixth Form Team. Settling In Whether students are new to The Hazeley Academy or have been here for a number of years, the daily routine in the Sixth Form will be very different from what you are used to. The Sixth Form Team is available to support students throughout their time with us. Students will be part of a form group and will be required to meet expectations on attendance and commitment to their studies. They benefit from greater independence and an increased responsibility for their own learning and development. Our induction days on Monday 27th and Tuesday 28th June 2016 will give applicants the opportunity to experience chosen subjects and to start the transition into Sixth Form. During the two days students will take part in Key Stage 5 lessons, as well as having the opportunity to integrate with other sixth formers to learn about how else The Hazeley Academy will develop them as individuals. There will be opportunities to develop leadership skills by working with younger students, taking part in various extra curricula activities and being involved in Senior Student roles. Students will need to commit to achieving and contributing to the best of their ability and the staff will be here to support, encourage and advise them at every step, and to help celebrate each success and achievement along the way.
