ANSWER: Hospitalists are specialists in inpatient medicine. Hospitalists typically have no outpatient responsibilities, and spend the entire day in the hospital taking care of admitted patients only. Because they are constantly in the hospital, they usually can see an admitted patient more expeditiously than a doctor with outpatient responsibilities, and they tend to become expert and efficient at managing serious illness requiring hospitalization. Most studies comparing hospitalists with doctors who do both outpatient and inpatient medicine have shown that hospitalists facilitate decreased lengths of stay

in the hospital and at least as good medical outcomes. The theory is that by specializing in just inpatient medicine, one can become expert as well as very efficient at using the resources available at a particular hospital. The potential downside of hospitalists is that the personal knowledge about a particular patient isn’t as high. Your PCP knows you — hopefully very well, if he or she has been taking care of you for a long time. Although your records may be available to the hospitalists, it isn’t the same as personal knowledge. That being said, with good communication (both ways) between your doctor and the hospitalists, the quality of care can be excellent. There is no doubt that the hospitalist model is increasing in prevalence

Underwater loans put consumers at risk A growing trend known as negative equity financing is putting consumers in a risky financial position. Put simply, it is when a buyer owes more than the vehicle is worth. A recent report by the Financial Consumer

Agency of Canada suggests new vehicles are typically purchased every three to four years and the average length of an auto loan now exceeds six year, demonstrating a growing trend toward negative equity financing.

How does this happen? Typically, consumers will trade-in a vehicle they still owe money on, and roll the debt forward into the financing of a new vehicle, adding their old debt to new debt. At a glance, advertise-

ments offering low rates, longer terms and low monthly payments make vehicles seem attractive and affordable, but it is important to know financing a vehicle this way may make it difficult to pay off the loan.

One example of consumer risk is if a vehicle is destroyed. Depending on the policy, the insurance company will generally pay what the vehicle was worth, not what the consumer owed, meaning the consumer may be on the hook for a large debt and have nothing to show for it. Tips to avoid negative equity: Pay off existing vehicle loans. Avoid rolling negative equity forward into new purchases. Don’t focus on a low monthly payment. Look at the total cost of the loan. Buy within your budget and stick to it. Consider a shorter

term loan. Longer term loans are typically more expensive. Snacks can help children get the nutrients needed to grow and maintain a healthy weight. For younger children, prepare single-serving snacks to help them get just enough to satisfy their hunger. Let older kids make their own snacks by keeping healthy foods in the kitchen. Here are some ideas: Save time by slicing veggies Store sliced vegetables in the refrigerator and serve with dips like hummus or low-calorie dressing. Top half a wholewheat English muffin with spaghetti sauce, chopped vegetables and low-fat shredded mozzarella and melt in the microwave.

Mix it up For older school-age kids, mix dried fruit, unsalted nuts and popcorn in a snack-size bag for a quick trail mix. Blend plain fat-free or low-fat yogurt with 100 percent fruit juice and frozen peaches for a tasty smoothie.

curiosity about space. Decorate a plain, smooth lampshade you already have, or purchase an inexpensive version to create your night-sky theme. With just a few supplies from around the house, you

and your kids can make an out-of-this-world shade to brighten a bedroom or family room. Here’s what you’ll need: —Plain, standard smooth white lampshade —Sheets of tissue paper in several colors (we used navy blue and shades of purple) —Household white glue diluted with a few drops of water to create a milkshakelike consistency —Paintbrush —1-inch glow-in-thedark adhesive-backed stars —Darning needle —Sturdy craft wire —Medium and large buttons, beads, starshaped charms and bau-

bles with holes Here’s the fun: 1. Tear tissue paper into 1-inch-by-1-inch pieces. This is a fun job for preschoolers. Older children might prefer cutting straight-sided squares or rectangles with scissors. 2. Set the lampshade on a newspaper-covered work surface. Paint an outside section with the glue-andwater mixture over the top of the tissue paper and smooth each piece out with your fingers. When the shade is covered, finish it off with a coating of glue and water. Let dry. 3. Attach glow-in-the dark stars here and there on the tissue-paper-cov-

ered exterior, then use the darning needle to poke about 6-8 holes evenly around the top edge of the shade. 4. Now it’s time to put planets and stars jutting outward in space, circling the shade. Start with a 10-inch length of wire. Poke and twist in place one end through the holes of a large flat button to represent a planet or the sun. Loop the opposite end of the wire through one of the holes in the shade. Bend the wire outward so that it looks like it is suspended in space around the shade. Continue attaching a single charm, bead or button to additional pieces of cut wire to represent

Understanding and treating cellulite A lot of misinformation is circulating about cellulite, a condition that affects around 90 percent of women at some point in their lives. Separating the cellulite fact from fiction can help those interested in finding out what causes it and how to make it less noticeable. Cellulite is easy to recognize. It often forms as dimpled skin or bumpy areas on the thighs and buttocks. Sometimes it is described as looking like cottage cheese or an orange peel. Try something exotic at your next BBQ When it comes to grilling, anything goes. Many people stick to popular staples like hamburgers and hot dogs, but just about anything can be whipped up over an open flame. Those who want to try their hand at something more exotic than traditional backyard barbecue fare can try this Spanish recipe for “Pinchos de Pollo (Grilled Chicken Skewers with Harissa Couscous,” which comes courtesy of James Campbell Caruso’s “España: Exploring the Flavors of Spain” (Gibbs Smith). PINCHOS DE POLLO (GRILLED CHICKEN SKEWERS WITH

refrigerator for 4 hours (up to 24 hours is ideal). Thread chicken evenly onto 6 wooden or metal skewers. Prepare a grill on medium-high heat and grill the skewers for about 5 minutes per side, until chicken is cooked through.

and cumin in a small skillet over medium-high heat until aromatic, about 30 seconds. Transfer to a food processor. Cook garlic in the same skillet, covered, over medium-low heat until tender, about 10 minutes, turning occasionally. Let garlic cool, then peel it and add to processor. Char bell peppers over a gas flame or in a broiler until blackened on all sides. Enclose in a paper bag; let stand 10 minutes. Peel, seed and coarsely chop peppers. Add peppers, oil, sugar, and crushed red pepper to processor. Puree. Season with salt and pepper. (Can be made 1 day ahead. Cover and refrigerate.)

Green tomatoes not just for the frying pan Perhaps in part due to the popular 1991 film “Fried G r e e n Tomatoes,” many people are familiar with the Southern U n i t e d States side dish of the same name. But as proven by the following recipe for “Grilled Green Tomato ‘Sandwiches’ with Herbed Cream Cheese” from Karen Adler and Judith Fertig’s “The Gardener & The Grill” (Running Press), green tomatoes can be even more delicious when grilled than they are when fried. GRILLED GREEN TOMATO “SANDWICHES” WITH HERBED CREAM CHEESE Serves 6 Herbed Cream Cheese •1 8-ounce package cream cheese at room temperature •1 garlic clove, minced •2 tablespoons chopped fresh basil •1 tablespoon snipped fresh chives Tomatoes •4 large green tomatoes (about 1-1⁄2 pounds), sliced 3⁄4-inch thick (to make 12 slices) • Olive oil, for brushing •2 teaspoons Seasoning Salt (see below) or kosher

salt • Ground black pepper Prepare a mediumhot fire in your grill. Place a welloiled perforated grill rack over direct heat. In a bowl, blend the cream cheese, garlic, basil, and chives together until smooth. Set aside. Brush the tomato slices with olive oil on both sides and season with seasoning salt and pepper. Place the slices on a baking sheet and bring out to the grill with the bowl of Herbed Cream Cheese and a knife for spreading. Grill all of the tomatoes on one side for about 3 minutes with the lid open, then flip and grill on the other side for 3 minutes more, or until the tomatoes have good grill marks. Remove the tomato slices from the grill and allow to cool slightly on the baking sheet. Spread Herbed Cream Cheese on half of the slices, top with a second slice and set the sandwiches on a platter. Serve the sandwiches hot, with oozing cream cheese filling. Variation: Grill all of the tomato slices as above and top each grilled tomato with a dollop of the cream cheese and serve open-faced.

When Conor McGregor and Floyd Mayweather square off the night of Aug. 26, it’s already a foregone conclusion that they’ve both won. McGregor stands to make something north of $60 million and Mayweather $100 million. Mayweather, of course, has the most to lose — his legacy — but it’d be hard to shed too many tears for the guy if he were to lose. Perhaps the biggest winner in all of the prefight spectacle is Paulie Malignaggi. Malignaggi,

even more relevant today. When you don’t knock many people out, you have to rely on the scorecards in boxing. How are the scorecards determined? Nobody has a clue. Aside from the “10 point must system”, nobody knows who the boxing judges are, nobody knows how the fight will be judged. Imagine going to any other sporting event and never knowing the score? Imagine watching four quarters of a football game, seeing seven touchdowns but when your friend asks the score you say “not a clue” every time. Or check in on the baseball game and just let random people in the stands judge whether or not runs should count that day. It was Malignaggi’s take

on Conor McGregor that got the boxing world laughing along with him. His analogy was that golfers don’t suddenly become hockey players because a driver looks similar to a hockey stick. While that knock earned him an invite to spar with McGregor leading up to the fight, the analogy doesn’t quite work. People like to talk about the “death of boxing” and always compare the “sweet science” to mixed martial arts, but it doesn’t have to be so. They can co-exist, and to say that McGregor isn’t a fighter is absolutely absurd. But as we saw from watching Ronda Rousey try to outbox a boxer and lose badly, and knowing enough about Floyd Mayweather’s worldclass defense, McGregor is still a massive underdog

with a puncher’s chance. Even if he wins, it doesn’t mean that boxing is a lesser sport, it just means that Mayweather lost a fight. We’re getting closer to finding out the answers, but boxing will, no matter the outcome, still have plenty to answer for. It’s time to fix the scoring system or else these r

How to streamline household cleaning Travel down the cleaning aisle of a neighborhood store and you will be met with product after product designed for specialized cleaning. There are sprays for windows, cleansers for electronics, carpet-cleaning foams, and scouring solutions for tubs and showers. Although many of these are effective, the average person can rely on just one cleaning aid for a wide variety of tasks: vinegar. Vinegar is a versatile product that can be used in everything from cooking to pickling to preserving foods to household cleaning. Vinegar is made by two distinct biological processes, offers The Vinegar Institute. Via fermentation, yeast will first change the natural sugars in the vinegarto-be material solution into alcohol. The second process involves a group of bac-

teria called “acetobacter,” which convert the alcohol into an acid. Vinegar can be made from any fruit or natural material containing sugar. For the purpose of generalized cleaning, white vinegar is the most common type used and is typically made from corn.Utilizing vinegar around the house is quite simple, and can be used in various ways around the house. • Remove lime buildup. Vinegar is good for cleaning faucets and showerheads. Make a paste of one teaspoon of vinegar and two tablespoons of salt. Apply to sink fixtures and rub with a cloth. Fill a plastic baggie with vinegar and attach to the showerhead with a rubber band. Let sit a few hours to overnight and then rinse away the mineral deposits.

• Brighten laundry. Add 1⁄2 cup white vinegar to the wash cycle. It will help loosen stains and brighten clothing. An added benefit is the acid will reduce static cling. • Disinfect cutting boards. Give cutting boards an extra cleaning by spraying them with straight vinegar, then rinsing to clean. • Clean the disposal. Make sure food residue doesn’t clog garbage disposals and lead to odor. Mix vinegar and baking soda in the drain and allow the bubbling action to remove built-up residue. Use a scrub brush for extra scouring. • Get streak-free windows. Mix a 50-50 solution of vinegar and water in a spray bottle. Use to clean windows and other glass surfaces. Wipe away with a microfi-

ber cleaning cloth so no lint is left behind. • Eradicate pet odor. Pour a little bit of vinegar into an empty litter box. Let stand for around 20 minutes, then rinse with cold water. Vinegar also can be used to blot up pet stains on the carpet. Just test color-fastness in an inconspicuous area first. • Banish water stains. Fix water stains and rings on wood furniture by mixing equal parts vinegar and olive oil. Use a soft cleaning cloth moving in the direction of the wood grain. Polish using a clean cloth. Vinegar can be used all around the house — these were just a few of the dozens of ways to rely on vinegar. Just steer clear of using it on natural stone and marble, as the acid could cause damage.

of weight loss. Look for rodents in the henhouse, as infected mice or other small rodents may transmit salmonella as well. Chickens also need safe, roomy clean conditions to remain healthy and content. According to the resource MyPetChicken, a diet of whole grains and seeds also may be associated with decreased salmonella colonies. Some experts warn against washing eggs as a preventative method. According to a report written by Diane Schivera, an organic livestock specialist for the Maine Organic Farmers and Gardeners Association, thoroughly cleaning egg shells can remove a protective “bloom” that prevents

bacteria from entering eggs. Eggs shouldn’t be scrubbed, but some suggest a warm water rinse that will push dirt away from the shell’s pores.

Old eggs are more susceptible to bacteria penetration. Storing eggs at room temperature may cause them to degrade faster. Once eggs are gathered, individuals should wash their hands and make sure the eggs are chilled.

Salmonella can be prevented in backyard chicken coops. Plus, it’s important to note that risk of infection is very small. The American Egg Board’s Egg Safety reference says an average consumer might encounter a contaminated egg once every 84 years.

Grill the hot dogs until the bacon is cooked through and the hot dogs are browned on all sides, about 2 minutes per side. To toast the buns, put them cut-sides down directly over the fire for the last minute of cooking. Serve the hot dogs on the buns.

Finding relief from dairy disorders Dairy is sometimes misunderstood. While dairy has nutritional value, it may cause various health problems for those who have a dairy intolerance or allergy. Using less energy when weather is warm The temperature is rising, and that can be a good thing for those people who love long days of outdoor recreational activities. Sometimes, though, there can be too much of a good thing, and consistently hot temperatures may become uncomfortable and even dangerous. Finding ways to keep cool without spending a fortune or using a lot of energy is challenging but possible. 1. Find your comfort zone Determine an indoor temperature that works best for the household. Keep the thermostat set as high as possible to maintain that comfort level. If you will be out most of the day, set the programmable thermostat at a higher temperature, and then lower the temperature about an hour before you plan to return home. This reduces the number of hours the air conditioner is running. 2. Keep curtains closed Draw the blinds and close curtains on south- and west-facing windows during the hottest times of the day. This prevents the interior of the home from heating up. 3. Maintain air circulation

Invest in ceiling fans, which can move air through the home. This will help prevent pockets of hot air in certain spaces. According to the U.S. Department of Energy, even in homes with air conditioning, ceiling fans enable homeowners to raise the thermostats on their AC units by about 4 F with no reduction in comfort. Attic fans can draw heat out of a home through vents, also helping to reduce indoor air temperatures. These devices can reduce reliance on air conditioners, especially on moderate days or cool nights. 4. Cook outdoors Put that backyard grill to good use. Plan meals around outdoor cooking so the ambient heat will stay outside and you will not have to turn on the oven or the stovetop. Include side dishes, such as salads, fresh vegetables and fruits or gazpacho, that can be served cold. If you must heat something indoors, try doing so in the microwave. 5. Run appliances at night Wait until the washing machine or the dishwasher is full before turning on the cycle. Temperatures are cooler at night, and you may not notice

the extra heat in the home generated by such appliances. Consult your energy bill as well. Sometimes energy rates are lower in the evening than during the peak hours of the day. 6. Line dry clothes Maximize that fresh air and warm sun by hanging freshly washed clothes to dry outdoors. This also works for quickly drying swimsuits and towels used at the pool or the beach. 7. Rely on natural light In the summertime, the sun stays out longer, so you’ll be less reliant on electric energy to keep things illuminated. Spend more time outdoors so you can read, play or chat in the sunlight while keeping indoor lights off. 8. Determine pool filter efficiency Experiment with reducing the amount of water filtration time until you come up with an amount of time that still maintains water sanitation. If you think your filter and pump are no longer working efficiently, consult with a pool supply retailer. When the heat is on, learn strategies to keep homes cool and use less energy in the process.
