One Out of Every Ten Dogs and Cats Will Visit the ER This Year! Veterinary Emergency Care in the 21st Century By Steven Wolchinsky, DVM
O
ne of the most dramatic changes in veterinary medicine over the past 20 years has been in veterinary emergency medical hospitals. Hospitals like Rocky Gorge Animal Hospital, are transforming into 24 hour total care facilities. They provide the comfort and convenience of being there for you 24 hours a day, 7 days a week, from preventative care to sick visits to anytime emergencies. It will be just like the old time veterinarians that you loved, except with 21st century technology.
There are things you can do to prevent an emergency visit but pet owners should have an emergency plan. Know where the closest emergency facility is and how to get there and have their information readily available. Some indicators that a pet may need emergency care include difficulty breathing or walking, collapses, seizures, difficulty urinating, weakness, lack of appetite or vomiting and diarrhea.
Emergency hospitals have become very sophisticated high tech facilities with state of the art diagnostic equipment and treatment options. Veterinary emergency centers now feature all of the equipment of an emergency intensive care unit such as an ICU patient cage with temperature, humidity and oxygen controls, blood pressure and EKG monitoring, full in house blood analysis capabilities, blood transfusion capabilities, state of the art surgical suites and some even have full imaging equipment including radiographs, ultrasounds and Cat Scan or MRI units. This allows us to deal with almost any critical care emergency case, including heart failure, lacerations, toxicities, seizures, trauma, bloats and the list goes on and on.
The 24 hour availability and all the advances in emergency veterinary care can cost a lot more than a regular office visit. Keep your pet’s vaccines and vet visits current instead of waiting until your pet is in distress. Cost is the biggest obstacle for good emergency care. Pet insurance can help cover the cost of emergencies, but you need to carefully read the coverage limitations and what is considered pre-existing and genetic conditions for your pet’s breed. Choosing the Right Breed of Dog for Your Lifestyle

ith hundreds of breeds of dogs to choose from how do you determine the dog that is right for you. The average person spends more time and thought in purchasing a car than in choosing a dog who will be a member of the family for the next 10 or 12 years. Before choosing a dog you should research the breed of the dog to see if it matches your lifestyle. Most pure breed dogs were bred for a specific purpose. By understanding the purpose we can get valuable information as to the dogs instinctive behavior. The working dog group for example includes: Akita, Boxers, Doberman Pinschers and St. Bernard’s. These dogs were bred to pull carts, guard property and perform search and rescue missions. A dog bred to pull a cart will need plenty of exercise to burn off high energy levels. Do you
have a large fenced in yard? Are you prepared to take long walks after a long day at work? Dogs bred as watch dogs are protective and need to be well trained and obedient. A warm hello to all! Summer is here and that can mean only one thing– fun in the sun. It’s time to get out with family and pets and enjoy the outdoors or hit the road for a fun vacation. We all love long, sunny days of summer outdoors. However, pet owners should be cautious about overdoing summer activities with their pets. High temperatures can lead to sunburn, dehydration and heat stroke. Clearly, the summer heat and your pet’s safety is something to be taken very seriously. Here are a Karl Kessinger with Kallie Publisher few tips to help you through. Never leave your pet in a car when you travel or do errands. During warm weather, the inside of your car can reach 120 degrees in a matter of minutes, even if you’re parked in the shade. Keep plenty of fresh water available for your pet, make sure your pet has a shady place to escape the sun when the temperature is very high. Get your pet groomed and ready for summer. Another summertime threat is fleas and ticks. Use only flea and tick treatments recommended by your veterinarian. A visit to the veterinarian for an early season check-up is a must. Make sure your pet is up-to-date on all necessary vaccinations. If your pet seems to be having any problem get help immediately. Summer is also the time for many fun social events. Look out for events that you and your pet can enjoy. Don’t forget to look at our “Upcoming Events” section. There is something for everyone - from happy hours, to patio parties, to workshops to help you and your pet, to online auctions to help other homeless animals. Check out events happening in your community. Get out and enjoy the outdoors and make some good memorable memories with your furry friends. Some breeds are better with children than others. Do you live in an apartment? Apartments that do accept dogs will usually only accept dogs under 20 lbs. If this is the case you should probably be thinking of a toy breed or a lap dog. Is the breed you are thinking of a shedder? How long is his coat? Will the dog require monthly trips to the groomer? Those breeds will require more time and money. Do you work long hours? Maybe you should have two dogs to keep each other company. Some breeds of dogs are natural barkers. Don’t expect the breeder to tell you about the negative traits of the dog. Before purchasing a dog research the bread. Talk with dog trainers and veterinarians about the breed, even better contact a rescue group for the breed of dog you are considering. Since many of the dogs they rescue come from
homes where they were unwanted, they are intimately familiar with what are the right lifestyles for that breed. You may want to consider adopting a rescue dog since their personalities and demeanor are more developed than a puppy. Once you have researched the breed you must decide on a puppy or adoption from a rescue group or shelter. If you do choose a puppy you should choose a reputable breeder. Take time to look into the background of the breeder and the stock that they use to breed from. Many purebred dogs have been over bred causing genetic flaws. Determine the sex of the dog you want and go meet the breeder. Good luck and have fun choosing your new family member! Also remember to be a responsible dog owner and have your dog spayed/neutered! x

The Number One Disease in Dogs and Cats Is?
By Animal Place Veterinary Hospital oes your pet have bad breath? Red gums? Is the surface of their teeth covered with tarter and plaque? Periodontal Disease is the number one disease of cats and dogs! They don’t brush their teeth twice a day like we do, so the potential for dental disease is, unfortunately, great. When you look inside your pet’s mouth, what you see is only a portion of the problem. Lurking under the gums may be a cavity, infection, or bad tooth. And guess what? Most pets will show no signs of discomfort! You or I would run to the dentist right away, but your pet will most likely just chew their food a little differently or swallow their food whole.

Looking inside of your pet’s mouth is just a part of discovering if your pet has dental disease. Dental radiographs (x-rays) are now able to show us much more. For years, veterinarians have been performing dental cleanings with scaling and polishing as your dentist performs on you. The philosophy has been that if there is a loose tooth or doubt about the health of the tooth, pull it. If the tooth looks good, leave it alone. But now, more and more veterinarians are recommending full mouth x-rays for each pet while they are anesthetized for their dental cleaning- again, the same thing that is done when you or I go see our dentist. By doing this, we are able to save teeth that we previously would have pulled, and discover
problems that we previously would not have known were there! What sort of problems? Well, sometimes there are teeth that we’ve assumed are missing, but are actually there under the surface, causing pain to the surrounding tissue as well as crowding other teeth. A perfectly good looking tooth may have damage to the root. These are the painful things that your pet isn’t telling you about! Periodontal Disease can cause diseases and damage to the heart, kidneys, and liver. Our pets age about seven years for every one year of our life. Keeping our pets healthy is very important and oral care is just as important as check-ups and vaccinations. What’s Causing My Dog’s Lameness???
I
t’s all too common, a dog suddenly comes back from the park limping, or even holding up the hind leg after chasing a Frisbee, stepping into holes, or from strenuous exercise following a long winter of inactivity. More often than not you will end up seeing your veterinarian. Injuries to the CCL of the stifle (knee) are common in dogs. These injuries may be sudden or may develop slowly over months with gradual breakdown of the ligament or may be brought on by some sudden traumatic event. This type of injury may cause other damage in the joint area with the dog refusing to bear weight on it. While some problems may occur as the result of injury, 75% of cases are thought to be due to genetic predisposition or degeneration in the stifle. Obesity is a huge factor! Some breeds like the Labrador retriever, Golden Retriever, Rottweiler and small dogs like Westies are frequenly seen with this type of injury.
Numerous surgical techniques have been developed over the past 30 years. The most recently developed techniques for restoring normal limb use involve tibial osteotomy (bone cutting).The osteotomy techniques alter the weight bearing forces acting at the stifle. Instead of repairing the CCL, these techniques focus on eliminating the need for the CCL. Tibial tuberosity advancement (TTA) and tibial plateau leveling osteotomy (TPLO) are the two most commonly performed procedures. TTA is a surgical treatment already commonly performed in Europe (with over 18,000 cases performed),and is gaining popularity in the U.S. The long term prognosis for excellent limb use following TTA is comparable to TPLO.TTA is far less invasive, results in less swelling and pain following surgery. Strict rest is required following surgery to allow for proper healing of the tibia. Initially, there should be no running, jumping, or playing. After the first month of strict rest, a gradual introduction of controlled activity is performed until 3 months following surgery. Physical therapy is also performed to fasten healing. Post surgery, x-rays are taken to assess healing of the tibia. After the healing is complete, it is rare for problems to develop.

ACL stands for Anterior Cruciate Ligament; in proper veterinary terminology this ligament is the Cranial Cruciate Ligament, or CCL. We often hear of ACL injuries occurring in football players, but this happens in dogs and cats too. In fact, cruciate ruptures are the most common orthopedic injury seen in veterinary medicine. Due to the degenerative nature of cruciate disease, between 30-40% of dogs will go on to eventually injure the other knee. This is due to increased weight bearing on the ‘good’ knee. The majority of pets need to have surgical correction to provide return to normal function of the leg.

Boarding School for Dogs!

A
Board and Train program consists of your dog living at the training school for a set number of days while learning a predetermined set of skills. A specific number of owner lessons are included to insure a smooth transition from school to home. Answering the following questions will help you choose the right program: • What you want your dog to learn? • How you want it taught? • How much you are willing to spend? • How long you are willing to be away from your dog? • After training support, yes or no? A good Board and Train program: The simplest, no frills programs teach (introduce) and then train (practice) • Walking on a loose leash (heel) • Sit, Come, Place command (optional)
It takes both dogs and humans a minimum of 21 days of practice before a newly introduced skill set moves from shortterm temporary memory to long-term temporary memory. The more inclusive programs add off leash training to the above as well as adding: • Sit/stay • Down and down/stay • Stand and stand/stay • Door/gate etiquette • Move out of the way when told • Go up/down stairs safely • Enter/exit a vehicle safely • Socially appropriate behavior with other dogs Additional skills sets require longer programs.

Lower cost programs should raise some red flags; however, it does not follow that the highest priced programs will always be the best programs. When “afford to” is different from “willing to”, the deciding spending factor should be value. After training support, yes or no? There is an old trainers saying; “you will lose 50% of the training the day the dog goes home.” To make Board and Train worthwhile you need a program that offers one to three handling lessons and offers you a choice of follow-up classes for your learning enrichment. The best programs make available a broad range of advanced training rather than limiting you to a few simple basics.

Pets Get Diabetes Too!
octor, my pet is thirsty all the time and now he’s been having accidents in the house! “He’s been eating okay, but he seems thinner and acts a little tired.” These are common observations made by the owner of a diabetic pet. Diabetes Mellitus is a common disease of both dogs and cats due to a deficiency of insulin, a hormone produced by the pancreas. Insulin is necessary to transport glucose (sugar) from the bloodstream to the cells of the body. Without insulin, the body thinks it is starving, and starts to break down muscle and fat for energy needs. While the rest of the body can use other sources of energy, the brain can only use glucose to function, so adequate insulin is essential. Often pet owners don’t realize changes in their pet’s habits until the disease has progressed. Changes are usually subtle at first. Common clinical signs include: weight loss, excessive drinking, and urination, muscle weakness, cataracts, lethargy, recurrent urinary
tract infections, and even pets licking up their “sweet” urine. As metabolic imbalances become more severe, stupor, coma, seizures and even death are possible. Diabetes can affect any pet but is seen most commonly in older, overweight female dogs and male cats. Diagnosis can be made by a simple blood test revealing high sugar levels. Effective treatment protocols are available but require consistent dedicated effort on the part of the owner and veterinarian. Treatments range from special diets, natural supplements, exercise and most commonly use of insulin by daily injections. The initial control of diabetes can be challenging. The selection of insulin type, dosage, frequency of administration, dietary changes are all determined by the veterinarian during the first week of treatment and require periodic monitoring. Fortunately, most pets respond well to treatment and live normal, happy lives. They guide our blind, assist the hearing impaired and disabled, search for drugs and contraband, help our fire fighters. They get us outside, lower our blood pressure, and help us socialize. What to Do When You Find Cats Outdoors
A
lley Cat Allies is the only national organization dedicated to the protection and humane treatment of cats. One of the most common questions people ask when calling Alley Cat Allies’ National Cat Help Desk is what they can do to help cats they have found outdoors. When you find a cat outdoors, the first thing you need to do is figure out whether have you found a stray cat or a feral cat. Stray and feral cats have different needs, so in order to do what’s best for them, it’s important to know the difference. A stray cat has been socialized to people at some point in her life, but has lost her home. These cats are often out during the day and may approach you, meow, or make eye contact. They may appear disheveled or ungroomed and they will likely be alone. A feral cat has never had any contact with humans and can’t be adopted as a pet. Feral cats are more often out at night. They are unlikely to approach you and will probably hide, crawl, or crouch to avoid you. Feral cats might be more well-groomed and often live in social groups called colonies. Often times, peoples’ first instinct is to call animal control or take the cat to an animal shelter, but this is the wrong
move. Since they can’t be adopted, feral cats brought to animal shelters are almost always killed. When in a stressful environment like a trap or a shelter, stray cats often behave fearfully and are mistaken for feral cats. In fact, the number one documented cause of death for cats in America is being killed in a shelter. For this reason, never call animal control about a cat you see outdoors. Instead, if you’ve found a stray cat, your next steps are to humanely trap the cat and find her a good adoptive home. Feral cats, on the other hand, live healthy lives outdoors and can’t be adopted. But there are still great things you can do to help them! Trap-Neuter-Return (TNR) is the humane, effective way to improve feral cats’ lives. With Trap-Neuter-Return, feral cats are trapped, neutered, vaccinated for rabies, and returned to their homes outdoors. Stray cats and kittens are adopted. TNR improves the cats’ health and their relationship with the community by ending mating behaviors like yowling, fighting, and spraying. The Breeder’s Edge
he Olney-Sandy Spring Veterinary Hospital (OSSVH) has developed a progressive reproductive management program called “The Breeder’s Edge.” Breeding stock selection, by reputable breeders, is based on such factors as conformation, temperament, and lack of hereditary defects. The American Kennel Club allows registration of litters bred by artificial insemination using fresh, fresh-chilled, or frozen semen. This allows the breeding of dogs that are geographically separated, but without the cost and risk involved in shipping them. It also provides more information on the quality of the semen and/or the state of the uterus, and can be physically safer. Breeders and veterinarians can now focus more on predicting accurate timing for breeding. Starting with pre-breeding exams,
the veterinarian ensures the female dog’s overall health. Ovulation timing, which is based on vaginal cytology and serum progesterone levels, aids in predicting the best time for conception. Various artificial insemination methods are available, including vaginal, transcervical, and surgical. Ultrasound is used to verify pregnancy and to help diagnose reproductive problems. Caesarean sections are planned for females at high risk for birthing complications. Male reproductive services include breeding exams and collection and evaluation of semen. The semen can then be used for insemination in the hospital, or chilled and shipped to another veterinarian. OSSVH is affiliated with C.L.O.N.E. and collects, freezes, stores, and ships frozen semen for future use.x

Cardiology for Dogs and Cats T By Chesapeake Veterinary Cardiology Associates
he family pet is now considered much more ‘family’ than ‘pet’. More and more families insist the best food, supplies and health care for their loyal companions. Specialty veterinary medicine has evolved and is now widely available to meet this need. Board-certified veterinary cardiology specialists now collaborate with primary care veterinarians to form a comprehensive health care team for pets just as they do in human medicine. In order to specialize they must complete three to five years of postdoctoral, residency training and pass rigorous board exams. There are many types of heart disease in dogs and cats. Some are acquired diseases, occurring later in life, and some are congenital diseases, meaning present from birth. Fortunately, although pets don’t typically have coronary artery disease like people do, they can have many other types of heart disease. Examples of these include conditions such as disruptions in the electrical activity of the heart, abnormalities in the structure such as malformed heart valves, chambers or blood vessels, or changes in the ability for the heart muscle to serve adequately as a pump. Pets often have no symptoms of heart disease, and it is only during a wellness exam that a veterinarian hears an abnormal heart sound (murmur) or detects an abnormality in the electrical activity of the heart (arrhythmia). Other pets may show signs such as coughing, lack of energy, or difficulty breathing. The primary care veterinarian will
typically refer the patient to a cardiologist immediately upon suspicion of heart disease for several reasons. First, as in human medicine, patients with cardiac disease show greatly improved quality of life and longevity when managed by both a primary care veterinarian and a cardiologist, and the earlier this happens in the disease process, the better. Most cardiac conditions can be diagnosed utilizing techniques such as electrocardiogram (ECG) or a cardiac ultrasound (echocardiography) by a veterinary cardiologist. Echocardiography is the gold standard to identify the source of any cardiac abnormality or disease. Fortunately, these tests are non-painful, noninvasive and performed while the pet remains awake and with the family by their side. These diagnostics give the cardiologist a great deal of information regarding the structure and function of the heart. Once the condition is properly diagnosed, treatment can begin. The vast majority of veterinary cardiac patients are treated on an outpatient basis with medication. Most pets tolerate cardiac medications remarkably well with minimal to no side effects. Cardiologists utilize numerous human and veterinary licensed medications as well as nutraceutical and nutritional support in their treatment of heart disease. Cardiologists create a treatment plan individually tailored to the pet with the goal of enhancing the pet’s quality of life and longevity. With a team of doctors and a loving family, most pets still live healthy, happy lives even with a heart condition. x

For your basic broth ice cubes, mix equal parts water and broth, (about ½ cup each), and pour into an ice cube tray. Freeze until solid and serve. Homemade cold dog treats will last about 2 months in the freezer. Once you know your dog thoroughly enjoys the plain broth ice cubes, start mixing in all sorts of yummy treats. For example: Chicken or Beef: Small pieces of plain cooked chicken or beef can be added to each ice cube cup. What a delicious surprise once the broth is licked away! Other Meats: Don’t think you are limited to chicken or beef. You can also add tuna. Blend together ½ cup of low sodium broth (chicken or beef) to a 6 ounce can of tuna in water. Add one tablespoon of water at a time if needed to help the blender combine the ingredients. Pour into two ice cube trays and freeze until solid. Veggies: Your dogs’ favorite veggie like broccoli, green beans or carrots can be added. Or, add a couple to each cup. x
