DURING THE DECADE THE ORGANIZATION HAS BEEN ACTIVE IN TEN KEY AREAS/FIELDS:
FOREWORD HAVING STARTED ITS ACTIVITIES IN LITHUANIA IN NOVEMBER 2000, THE PUBLIC INSTITUTION “GLOBAL INITIATIVE ON PSYCHIATRY” MARKED ITS 10YEAR ANNIVERSARY AT THE END OF 2010. ESTABLISHED AS A REGIONAL OFFICE OF THE INTERNATIONAL ORGANIZATION “GLOBAL INITIATIVE ON PSYCHIATRY” AND WORKING INDEPENDENTLY IN LITHUANIA AND IN THE COUNTRIES OF THE FORMER EAST AND CENTRAL EUROPEAN REGION, THE ORGANIZATION HAS COME A LONG WAY SINCE ITS INCEPTION, PROMOTING HUMANE MENTAL HEALTH CARE AND OPEN SOCIETY IDEAS BASED ON RESPECT OF HUMAN DIGNITY.
1. DEVELOPMENT OF NEW (COMMUNITY) MENTAL HEALTH SERVICES AND . SPECIALIST TRAININGS. Efforts have been taken to create a coherent chain of mental health services by implementing and further developing new mental health services in the city of Vilnius, as well as on the national level and in the neighbouring countries. Implementing innovative services necessitates a change in the attitudes of professionals and an upgrade in their professional knowledge and practical skills. To this end, the “Global Initiative on Psychiatry” regularly organizes trainings for police officers, emergency medical aid staff, medical nurses, journalists, social workers – job coaches, correctional system officers, judges, prosecutors, all of which are aimed at insuring the rights of people with mental problems. 2. IMPACT ON POLICY IN REFORMING THE . MENTAL HEALTH CARE SYSTEM. The organization actively participates in the processes of the on-going mental
health services reform: it has contributed to the development of mental health strategy for the city of Vilnius, it participated in designing the National Mental Health Strategy and the plans for its implementation as well as in preparing laws and executive acts relating to mental health care and patient rights. 3. DEVELOPMENT OF CHILD AND ADOLESCENT MENTAL HEALTH SERVICES AND IMPLEMENTATION OF PREVENTION . P RO G R A M M E S . During the ten years of its activities the “Global Initiative on Psychiatry” has been active in developing child and adolescent mental health services both in and outside Lithuania. . 4. HUMAN RIGHTS PROTECTION. The “Global Initiative on Psychiatry” was the first non-governmental organization to carry out, in collaboration with other non-governmental organizations, human rights monitoring in mental health care establishments for adults (2004-2006). Also, in collaboration with the Children's 4
rights ombudsman institution, human rights monitoring was conducted at child care and education establishments (2006). Seeking to ensure the rights of persons with mental disorders, the organization has raised issues of incapacity and legal regulation of care and conducted an analysis of incapacity system in Lithuania. 5. PATIENT'S PERSON OF TRUST PROGRAMME. Adopted for use from the Netherlands, the person of trust programme was started in Lithuania in 2005. Since its inclusion into the mental health strategy of the city of Vilnius, the services provided under the programme in mental health care centres have been financed by the municipality. Other municipalities of Lithuania were also familiarized with the person of trust programme, the programme was introduced and launched in Kaliningrad. 6. I M P L E M E N T A T I O N O F M O D E R N FORENSIC PSYCHIATR Y SER VICES. . In collaboration with the Rokiškis mental hospital, interdisciplinary team work, a modern patient database and instruments for repeat offences risk assessment (HCR-20) were implemented. Since 2007 activities aimed at implementation of modern forensic psychiatry services have been implemented in Kaliningrad
7. PRISON MENTAL HEALTH AND RE. SOCIALIZATION OF OFFENDERS. The “Global Initiative on Psychiatry” has been active in improving offenders' mental health and in launching resocialization programmes, among which cognitive / behaviour corrective programmes for adults ('One to one'), for sex offenders (SENAT ) and for juveniles (EQUIP) are worth noting as well as performance evaluation studies, continuous supervision of the execution of programmes. The organization initiated the preparation of the description of the implementation of correction programmes in Lithuania. 8. O V E R C O M I N G T H E S T I G M A A N D CHANGING SOCIET Y'S ATTITUDES (CAMPAIGNS AND ACTIONS). . During the ten years of its activity the organization carried out various actions and campaigns, which aimed at changing the prevailing negative attitude of society toward persons with mental disorders. Among the noteworthy are: the 2005 action “„Statome save” ('Building ourselves'), the 2006 campaign „Kvaila bijoti atsisukusio varžtelio” ('It's silly to be afraid of a loose screw'), the 2007 tolerance week “ Griauname sienas – ugdome toleranciją” ('Pulling down walls and learning
tolerance'), the 2007 campaign promoting society's awareness „ Čia gali būti ir tavo vaikas” ('It could be your child, too'), and the 2008 social action „Puodeliai” ('Cups'). 9. USER, CARER AND NGO EMPOWERMENT IN MENTAL HEALTH. . Search for employment mechanisms adjusted to persons with mental disorders and intellectual disabilities, their self-representation. I mplementation, both in Lithuania and in other countries, of projects directed toward consolidating and strengthening organizations representing the interests of persons with mental health problems was priority area within the organization's work. 10. DEVELOPMENT COOPERATION. . Since 2008 Lithuania has been actively participating in the development cooperation programme, implementing projects in Georgia, operatiing in Tajikistan and Belarus. Also, development education activity was carried out in Lithuania with a view to including mental health issues into the development cooperation context.

The work done in 2008-2010
In summary, the work done and the results achieved in 2008-2010 show that the organization has remained faithful to its mission and has been active in the fields of reducing social exclusion of mental health service users (2 projects put into action), protection of human rights (5 projects). Child and adolescent mental health care (2 projects) remained important within the organization's agenda, as well as mental health of prisoners and their re-socialization (9 projects) has not declined in importance during the recent three years. In 2009 the “Global Initiative on Psychiatry” launched a new activity aimed at improving the quality of life of people living with double or triple stigma. HIV/AIDS and mental health has become one of the fields where the organization develops its competencies and passes on the accumulated knowledge. During the recent three years “Global Initiative on Psychiatry” remained actively involved in forming mental health care system and monitoring its realization (2 projects). Specialists of the organization participated in various working groups set up by public authorities, where issues of mental health strategy implementation, mental health rehabilitation, reform of the system of social care institutions, ensuring protection of human rights of persons with mental and intellectual disorders issues were discussed and the relevant documents and recommendations prepared. Special attention was devoted to the preparation for the ratification in Lithuania of
the UN Convention on the Rights of Persons with Disabilities and, on its ratification, to planning and assessment of implementation instruments in accordance with international requirements. In the work of the organization special attention was given to the proper preparing to implement the requirements of Article 12 of the Convention relating to ensuring equality of persons with disabilities before the law in all spheres of life. Implementation of the relevant paragraph in Lithuania is related to reforming the legal incapacity institute. In this regard, suggestions to the respectful authorities were provided for the preparation of the amendments to the Civil and Civil Procedure Codes. Also, a lot of attention was paid to Article 19 of the Convention, which establishes the right for persons with disabilities to live independently and be included in the community. Ensuring effective implementation of this paragraph requires reforming of the social care system in Lithuania. Weighty contribution of organization's expertise was provided in the debate on proper implementation of Article 33 of the Convention in Lithuania, which establishes requirements for national implementation and monitoring mechanisms of the Convention.

I. IMPACT ON PUBLIC POLICY IN REFORMING THE MENTAL HEALTH CARE SYSTEM
MENTAL HEALTH STRATEGY ASSESSMENT Implementation period (years): 2008/05/01 – 20008/09/01 Lithuanian experts carried out external independent assessment of the mental health strategy of Lithuania and of the plan of measures for its implementation for 2008-2010. MHS guidelines (methodology) covered several different methods of analysis: 1. Mental health strategy (the document) and the logical analysis of the plan of measures of its implementation programme: an analysis matrix of the goals and tasks; 2. Analysis of legal documents; 3. Qualitative Delphi test applying a survey of experts method. Upon completion of the test, conclusions and recommendations were put forward. The assessment led to the conclusions and recommendations, that were presented to the authorities and other interested parties. INSTITUTIONAL CAPACITY BUILDING OF LITHUANIAN NGOS WORKING IN THE MENTAL HEALTH AREA AND THEIR COALITION-BASED ACTIVITIES

Implementation period (years): 2009/06/19 – 2010/06/18 The goal of the project was to assemble NGOs working in the mental health area into a coalition and to increase their institutional capacities. Achievements: During the project NGO staff members enhanced their capacity in lobbying and negotiating skills, communication with the media and representing themselves as well as their position. During the publicity activities society was introduced with mental health issues and legitimate expectations and interests of persons with mental health problems. Since the project was aiming at coordinated and constructive cooperation and communication with local and national authorities and the media, all major NGOs working in the mental health area, representing both persons with mental health problems and members of their families as well as 8
II. REDUCING SOCIAL EXCLUSION OF MENTAL HEALTH SERVICES USERS INTEGRATING PEOPLE WITH MENTAL AND INTELLECTUAL DISABILITIES INTO THE LABOUR MARKET Implementation period (years): 2004/12/01 – 2008/08/31 The key goal of the project was to promote integration of people with mental or intellectual disorders and of their carers into the labour market. The EQUAL project pursued three major goals: to change the image of people with mental or intellectual disabilities, to raise public awareness about mental health and mental health disorders. During the project various studies, public polls and employers' attitudes were assessed on the opinion towards people with mental disabilities, their employment opportunities as well as their skills and performance. Besides various publicity campaigns were carried out. The project aimed to legitimize the speciality of a “job coach” (a person helping people with mental and intellectual
disabilities to get employed and help them at work). Analysis of existing legal framework was carried out and recommendations for its development presented to authorities. The project also aimed to convince the government of the necessity to set up services in the community that would ensure full and equal enjoyment of all human rights for persons with mental and intellectual disabilities, the right to employment among them

institutions on how public information influences the image of users of mental health services by presenting the newest scientific data and justified methods elaborated in Norway and Sweden. Some of the objectives are: to conduct a survey on what image Lithuanian mass media is forming about users of mental health services; to compare Lithuanian and Scandinavian (Swedish and Norwegian) situations in this field; to summarize the findings and issue a specialized edition (intended to provide special knowledge to media workers) on the topic.

III. PROTECTION OF HUMAN RIGHTS DEVELOPMENT OF NGO ACTIVITIES IN KALININGRAD DISTRICT: PROTECTING HUMAN RIGHTS BY IMPLEMENTATION OF PATIENTS' ADVOCACY SYSTEM FOR MENTAL HEALTH SERVICES USERS – PERSON OF TRUST POSITION Implementation period (years): 2007/10/01 – 2008/12/31 The objective of the project is by supporting the NGO “Nit Ariadny” to prevent human rights violations of people with mental disorders and to protect and promote the human rights of persons in “closed institutions” – psychiatric hospitals in Kaliningrad city, Kaliningrad district of the Russian Federation Among the activities carried out are: establishment of 3 positions of persons of trust in Kaliningrad region psychiatric hospitals;
trainings for the selected staff – persons of trust and for psychiatric hospital staff, a launch of new positions of persons of trust; empowering NGO “Nit Ariadny” by providing trainings for the NGO members on NGO management, capacity building and human rights. A PERSON OF TRUST FOR MENTAL HEALTH INSTITUTIONS IN THE CITY OF VILNIUS Implementation period (years): 2008 – 2010 (tęstinis) The objectives of the programme: To contribute to the establishment of a
consistent chain of mental health services in Lithuania in accordance with the priority areas laid down in the Mental Health Strategy: 1) ensuring of human rights, 2) services responding to the needs of the patient, 3) promotion of autonomy and participation, 4) strengthening the role of patients, their carers and the non-governmental sector. In realizing the programme in the Vilnius city municipality, to develop and implement a representative model of ensuring mental patients' rights that could later be applied in other municipalities of the country.

to pursue equal opportunities for social integration of persons with mental disabilities. The key objective of the programme is to provide the person of trust services to mental health users by informing, counselling them and by providing to them concrete help and assistance on patients' rights issues. ELABORATION OF THE METHODOLOGY FOR IMPLEMENTATION OF PERSON OF TRUST POSITION IN LITHUANIAN PSYCHIATRIC HOSPITALS: APPLIANCE OF ADJUSTED DUTCH EXPERIENCE Implementation period (years): 2008/03/15 – 2009/10/31 The project aims to create an international network of experts for elaborating the methodology for implementation of the person of trust advocacy measure in Lithuanian psychiatric hospitals and to offer targeted support to people suffering from mental health problems in their sensitive life stages (hospitalization in psychiatric hospitals). The purpose of setting up an international 10
experts group is to seek for unified practice and application of Human Rights standards as well as to work out a methodology of patients' rights advocacy system adjusted for Lithuania's situation. The overall objective of the project is to prevent human rights violations and to protect the human rights of patients in “closed institutions”. PERSON OF TRUST IN THE MENTAL HEALTH SYSTEM OF LITHUANIA Implementation period (years): 2009/10/21-2009/12/31 The purpose of the programme is for the representatives of targeted groups to familiarize themselves with the Person of Trust programme
run in the city of Vilnius, to communicate best practice and to prepare methodological recommendations for the implementation of the programme on the national level. The project contributed to the development of the methodological publication 'The person of trust service in Lithuania. Methodological recommendations for institutions working in the mental health area' [„Paciento patikėtinio paslauga Lietuvoje. Metodinės rekomendacijos psichikos sveikatos srityje dirbančioms institucijoms“]. In it best practice and experience of foreign countries in representing and implementing mental patients' rights with the help of the person of trust are analysed and presented, the main principles of the person of trust activities, work ethics requirements and guidelines on communication with mental health care institutions are described, recommendations with regard to consolidation of the status of the person of trust in national laws and development of the programme in all mental health care institutions of Lithuania are

IMPLEMENTATION OF THE PERSON OF TRUST PROGRAMME IN TELŠIAI, KELMĖ, ROKIŠKIS, VISAGINAS AND MARIJAMPOLĖ MENTAL HEALTH CARE INSTITUTIONS AND PSYCHONEUROLOGICAL BOARDING HOUSES Implementation period (years): 2010/01/22 – 2010/09/30 The objective of the project is to familiarize, through provision of trainings, the staff of Telšiai, Kelmė, Rokiškis, Visaginas and Marijampolė municipalities' Health and Social Affairs Units, psychoneurological boarding houses, psychiatric hospitals and mental health centres as well as representatives of NGOs with the Person of Trust Programme methodological recommendations and to communicate the best practice of implementing the Person of Trust Programme in the city of Vilnius, promoting development of public services and independent human rights monitoring in the stationary and ambulatory mental health care institutions of the regions. The key activities under the project included: the study 'Analysis of documents regulating the procedure of handling complaints, disputes and conflicts in mental health institutions and
psychoneurological boarding houses' [“Dokumentų, reglamentuojančių skundų, ginčų ir konfliktų sprendimo ir nagrinėjimo tvarką psichikos sveikatos priežiūros įstaigose ir psichoneurologiniuose pensionatuose, analizė”]; 5 seminars on 'Person of Trust's activities: the city of Vilnius and Europe experience' [„Paciento patikėtinio veikla: Vilniaus miesto ir Europos patirtis“] during which a training was offered to 38 specialists from target regions; development of three models 'Implementation of the person of trust programme in mental health centres', 'Implementation of the person of trust programme in psychoneurological boarding houses' and 'Implementation of the person of trust programme in psychiatric hospitals'; publishing of informational materials on these models. UPGRADING FORENSIC PSYCHIATRY AND PRISON MENTAL HEALTH SERVICES IN KALININGRAD REGION Implementation period (years): 2007/05/10 – 2010/05/10

The project aims at creating an up-to-date forensic psychiatry and prison mental health system in Kaliningrad region. The system will ensure provision of adequate help to target groups and more efficient crime prevention. During the execution of the project, cooperation has been maintained with Kaliningrad Psychiatry Hospital No 2, Cherniakovski Forensic Psychiatry Hospital and Kaliningrad region correctional institutions. While implementing this activity a special training program was prepared and trainings seminars for KNTI PN-staff conducted. During the seminars following topics were covered: Team work. Creative tasks in the work with juveniles. Social skills development programmes. Reduction of aggression: complex intervention under the ART (Aggression Replacement Training) methodology. In 2008, the Global Initiative in Psychiatry was granted permission to translate the 'EQUIP' programme from English into Lithuanian and to use it in the correctional system of Lithuania. Following the recommendations for the assessment of the programme efficiency, a study was carried out on the implementation of the programme in the Kaunas Juvenile Interrogation Isolator-Correctional Facility, in Panevežys Women's Correctional Facility and in 7 correctional inspectorates. Besides, counselling and supervision were provided to ensure continuous support and supervising to those executing the programme.

„Global Initiative on Psychiatry” was granted permission of the authors of the sexual violence risk assessment methodology SVR-20 to adapt and use it in the correctional institutions and correctional inspectorates of Lithuania. The work done included: translation into Lithuanian of SVR -20 manual and coding sheets and their scientific and textual editing, collection of primary data for the validation study (with the representative sample of 100 individuals), a statistical descriptive analysis of the data, a review and analysis of research carried out abroad on sexual violence risk assessment.

In executing the project 'Upgrading the quality of services provided by correctional inspectorates in organizing reintegration into the labour market of offenders whose sentence was suspended and those who have served their sentence' the Prison Department under the Ministry of Justice initiated a study, which was carried out by the “Global Initiative on Psychiatry”. The study was aimed at establishing major obstacles preventing integration of convicts into the labour market. A focus group was set up, with 11 people under the supervision of Vilnius city and region correctional inspectorate. Among the key obstacles 13

established were: the negative public opinion in respect of persons with a record of conviction and, specifically, toward those who have served a sentence in prison, lack of education and qualification (or absence of a document confirming acquisition of education or qualification), the sub-culture of convicted persons, which not only persists in custodial facilities, but is 'transferred' to the public. The findings of the study were systematized in a report and presented during the training given for a group of 15 participants of the project on December 16, 2009 at the Kaunas region correctional inspectorate.

IMPLEMENTATION AND DEVELOPMENT OF EFFICIENT METHODS FOR REPETITIVE OFFENCE PREVENTION AND THIN-

The goal of the project was by invoking the best practice of Norway, to contribute to the implementation and development in Lithuanian correctional inspectorates of efficient methods that would facilitate prevention of repetitive offenses and correction of thinking and behaviour patterns. During the execution of the project, a study visit to Norway was organized for correctional inspectorate officers, Prison Department officers and NGO representatives; also, 8 specialists were selected for the individual cognitive-behavioural correction programme “One to one' manager development trainings, which were conducted by experts from Sweden. A description of the implementation and development of correction programmes in Lithuanian corrections system was prepared; it was approved on December 13, 2010 by order No V-315 of the director of Prison Department under the Ministry of Justice of the Republic of Lithuania (Official Gazette 21/12/2010, No 149-7658). On September 13, 2010, a conference 'Re-socialization of convicts: implementation and development of correction programmes in Lithuania' was held with the aim to present thinking and behaviour correction
programmes, to discuss their importance and their place in the corrections system of Lithuania. It was also the final event of the project and it received a lot of attention from both nongovernmental organizations and representatives of education and research institutions. SeNAT (Sex offender therapy) programme commissioned by the Prison Department was developed in 2009. In 2010 it was tested in two facilities: the Alytus Correction House and Pravieniškės Correction House No 2 – Open Prison Colony. The pilot implementation of SeNAT included 11 inmates participating (4 from Alytus and 7 from Pravieniškės facilities). “Global Initiative on Psychiatr y ” developed an implementation quality assessment methodology and carried out SeNAT implementation quality assessment study, as well as organized scientific and textual editing of the programme manual and its scientific review. During 2008-2009 GIP-Vilnius continued its activities in the project, which aimed at working out recommendations and methodologies for developing efficient child and adolescent mental health practices and policy in the European Union, at putting special emphasis on the current situation and the necessary changes to be introduced in the new member states while preparing modern prevention and incentive measures and practices. The project pursues four major objectives: To establish a partner network of EU member states, which would facilitate effective exchange of experience in implementing modern public health measures; To work out evidence-based recommendations on the implementation of advanced child and adolescent mental health measures; To promote innovative child and adolescent mental health initiatives in the new member states and applicant countries encouraging involvement and participation of children, families and communities in the measures delivered; To prepare for the responsible EU and EU
member state institutions a set of recommendations, based on the information and experience acquired during execution of the project, highlighting implementation of modern approaches in the field of child and adolescent mental health field and the necessity to change obsolete models. CAMHEE project involved 35 associate partners from 16 countries. “Global Initiative on Psychiatry” was one of Lithuanian partner organizations participating in the working group ‚Development of modern parenthood promoting approaches with special emphasis on parents who have mental disorders or represent other risk groups', which was devoted to the development of parenthood programmes. During execution of the project, “Global Initiative on Psychiatry” together with the Children's Rights Ombudsman's office, carried out a study 'The situation of children whose parents are instiėįętutionalized – safeguarding their legitimate rights and best interests, the need for support to the child and his/her family and opportunities for provision of support'. In the report of the study the CAMH - related legal basis applied in Lithuania was reviewed, the relevant information provided by other researchers was summarized, the data received from the study were described and recommendations on improving the situation were put forward. The project aims to contribute to the economic development in Sughd region and to promote the participation and integration of people with mental problems into local communities by launching activities in institutions delivering mental health services that would generate additional income and ensure occupation of patients. Objectives of the project and related activities: 1. To provide trainings for the administration and the personnel of establishments delivering mental health services on development of 17
income generating activities. At a later stage of the project 6 best project proposals were awarded grants to develop income generating activities of therapeutic nature (five psychiatric hospitals/mental health centres and one organisation for people with disabilities). All funded projects undergo regular supervision; if/when necessary, consultations of craftsmen, agronomists or
veterinary specialists are provided. 2. To implement occupational therapy elements in psychiatric hospitals and psychoneurological boarding houses. The personnel delivering services were familiarized with the basic principles of organizing occupational therapy. The project also aims at enhancing the patients' working, independent living and self-care skills that are necessary for successful integration into society. 3. To familiarize representatives of local authorities ( Khukumat and Jamoat corresponding to Lithuanian seniūnija) with the principles of and demand for community mental health. 4. To inform the public of Tajikistan about the project underway, thus increasing tolerance for people with mental disabilities. Based on the assumption that most myths and fear stem from ignorance, the project seeks to reveal the other side of disability, i.e. to show that the disabled can also work and engage in creative activity. The achievements of the project are reflected in a documentary produced during the implementation period, which will be shown at the end of the project on local TVs as well as in Lithuania and will then be forwarded to the Commission for further publicizing. The materials of the trainings given during the project have been collected and are presented in a separate publication to be distributed in Tajikistan free of charge to all those interested in the improvement of services

The major activities carried out under the project include setting up of two mobile teams of mental health professionals, who provided mental health services to war affected children and their families in refugee camps and buffer zones of Georgia. Each year “Global Initiative on Psychiatry” organizes public events for the WORLD MENTAL HEALTH DAY. On the 10th of October we are trying to draw attention to the importance of mental health in human's life, main mental health problems and those who suffer from various mental disorders.
ON THE 10TH OF OCTOBER, 2008 Open house event was organized by “Global Initiative on Psychiatry”. Mental health service users, professionals, representatives of NGOs working in mental health sphere, representatives of appropriate authorities and other people concerned were invited and had possibility to visit organization and to get acquainted with its activities.

HCR – 20 ОЦЕНКА РИСКА НАСИЛИЯ This is the second "HCR – 20. Violence risk assessment” edition in Russian. The world's widely used risk assessment measure is truly universal: it can be applied in prisons, psychiatric hospitals, programs providing community services.
This publication is adapted Russian translation of 2007 publication "Incapacity issues in the context of the declared European Union values". The publication indicates international principles for regulation of incapacity issues, as well as rights of persons being declared as incapable. It also presents legal framework and practice of determination of incapacity and guardianship in the Russian Federation. Besides, the publication presents the practice of Lithuanian courts and decisions of the European Court of Human Rights on the matters of incapacity. 23
MODELS OF IMPLEMENTATION OF PATIENT'S PERSON OF TRUST PROGRAMME IN MENTAL HEALTH CENTERS, PSYCHIATRIC HOSPITALS AND SOCIAL CARE HOMES Publication aims to introduce models of patient's person of trust programme adapted to certain mental health institutions taking into consideration all necessary elements needed for the smooth implementation of the programme: the legal framework, financing mechanisms, the specificity of work of patient's person of trust patient's, etc. The publication includes three models that cover all mental health care levels.
