Our Expert Panel Lisa Raleigh Lisa Raleigh is a lifestyle and wellness expert who is well known for her television roles as the blue team trainer on SA’s The Biggest Loser, fitness guru on the Doctors’ Orders Show and health consultant on SABC 3’s morning show, Expresso. She is also the creator and director of Health Box SA – and is the events director for parkrun in Durban - a free initiative for the public that offers a collective 5km run once a week. Lisa enjoys writing for magazines and newspapers in her spare time, and presents regularly on radio talk slots and at wellness workshops and events.

Dr Simone Silver MBChB Dr Simone Silver obtained her medical degree from Wits University in 2007. After graduating, she completed two United States Licensing Examinations, a course in Good Clinical Practice and her internship and community service in Johannesburg. Since she was a medical student she has been fascinated by the intricate interplay between the physical, psychological, societal and environmental factors in establishing and perpetuating disease. She has developed a special interest in the pathophysiology of stress and disorders such as Chronic Fatigue Syndrome and Fibromyalgia. She is a faculty member of the South African Academy of Integrative Medicine, lectures frequently and currently practises in Bryanston, Johannesburg.

Dr. Noori Moti-Joosub MBBch (Wits), FC DERM After qualifying as a Medical Doctor and Dermatologist, Dr Noori Moti-Joosub completed her registrarship in Dermatology at the Johannesburg Hospital. It was here that Moti-Joosub undertook intensive dermatological training by being exposed to a wide variety of skin ailments. Although she does practice in general dermatology, Moti-Joosub’s focus lies in aesthetic practice and sun protection skin care. She brings a comprehensive Dermatological Consultancy to the Laserderm Dunkeld branch as well as the exclusive procedure of MoleMapping.

Dr Sly Nedic MBChB(Bel) Dr Nedic is a founder of the 8th Sense Medi-Spa in Sandton, where she passionately combines aesthetic makeovers with anti-ageing medicine, bioidentical hormone replacement therapy and preventative genetic testing. She is a board certified doctor of WOSAAM (World Organization Society of AntiAgeing Medicine), a member of IHS (International Hormone Society) and A4M (American Academy of Anti-Ageing Medicine). Dr Nerina Wilkinson, MBChB(Stell) FCS(SA)Plast Dr Nerina Wilkinson has a full time private practice at the holistic and anti-ageing Renaissance Body Science Institute in Cape Town. She is a member of the Association of Plastic and Reconstructive Surgery, an Exco member of the Aesthetic and Anti-Ageing Medicine association of SA – as well as a lecturer and trainer for Allergan Aesthetic Medical Academy. Dr Wilkinson also presents scientific papers at congresses, with her articles and reports being regularly published in health related magazines. Dr Wilkinson has a major interest in the aesthetic surgery of the face, breast and body - closely integrating surgical procedures with other non-invasive treatments.

Dr Anton Potgieter, MBBCh (Wits), FC (Plast Surg) SA Dr Potgieter splits his time between his Sandton private practice and Baragwaneth Hospital, where he forms part of the plastic surgery team that runs Smile Week. He performs other reconstructive surgeries as well, including breast reconstructions, craniofacial procedures and cancer surgery. He is an educational consultant for the WITS Med School Plastic Surgery programme, a member of APRSSA (Association of Plastic and Reconstructive Surgeons South Africa), the HPCSA, as well as the ISAPS (International Society of Aesthetic Plastic Surgery).

Dr Janine Olivier MBChB Dr Olivier obtained her MBChB in 2006, and is currently enrolled in a Masters of Medicine course through the University of Pretoria. She has participated in several Botox®, filler and anti-ageing workshops and has received formal training courses with Genop, the Bay Skin Institute and Allergan. Dr. Oliver’s initial training was obtained under the guidance of Dr K Semprini – and her field of practice in aesthetics is situated in Pretoria and Parys (Free State). She has also recently joined EDGE2000 with Trish Birkenstock – where her focus has shifted from Internal Medicine to holistic well being.

Dr Riekie Smit MBChB (UFS) MSc Sports Med (Pret) Adv Dip Aesth Med (FPD) Dr Smit works in private practice in Pretoria with a focus on aesthetic and anti-Ageing treatments. Her major focus is skin rejuvenation procedures with peelings, mesotherapy, lasers and facial injectables. She is also a lecturer and trainer for educational institutes and a faculty member of the American Academy of Aesthetic Medicine. News Snippets From Around The Globe HPCSA Taken to Court over Benoni Plastic Surgeon Four people have taken the Health Professions Council of SA (HPCSA) to the North Gauteng High Court over alleged malpractice by a Benoni plastic surgeon, The Star reported in September 2013. Four patients of Dr Luke Gordon want the HPCSA to take strong action against him after their allegedly botched operations. They claim the HPCSA allowed him to continue practicing despite a string of complaints. Gordon previously received fines and suspended sentences for medical malpractice from the council. One of the complainants lost a leg when Gordon performed a calf implant operation, and two others had elective breast reduction surgery which left them disfigured, it was reported. In court papers the four argue that if stronger action had been taken against Gordon, at least one of them could have been spared from disfigurement and pain. “The HPCSA is entrusted with the statutory duty to protect the public by conducting investigations into complaints lodged against practitioners under their jurisdiction - and they are empowered to act against them,” the court document reads. Gordon paid R4.5 million to clients in the last three years. The HPCSA found him guilty after complaints in 2000, 2007 and 2008. Gordon told the paper he was unaware that his patients had instituted legal proceedings. Source: SAPA

J.Lo Denies Rumours Of Plastic Surgery Jenny From The Block singer Jennifer Lopez has responded to a tweet claiming she has enhanced her looks with cosmetic

surgery. “Sorry Sir, but I have never had plastic surgery of any kind,’ the 44-year-old wrote after a post was sent from the account of London-based surgeon Dr. Ayham Al-Ayoubi. The doctor, who specialises in facial and laser surgery, posted a split of an older and more recent picture of the star, and captioned it: “These before and after pics of @JLo show many signs of #plasticsurgery - naturally pretty but now looks amazing.” However the entry was removed after the singer’s fans complained about his comments. He went on to apologise, saying the pictures had been posted in error. “Dear @JLo I wonder if you can accept my sincere apologies about the wrong tweet which was written by my assistant without my knowledge…” He also suggested that strategically applied makeup could be behind the perceived changes between the pictures. “U are naturally a very beautiful woman and look great, sometimes clever makeup can enhance features slightly changing their appearance @Jlo,” he commented. Man Has Nose Grown on Forehead We all know of celebrities who have had botch nose jobs - but this operation is certainly head and shoulders above the rest. A Chinese man has had a new nose grown on his FOREHEAD. The man, who has only been named as Xiaolian, had the treatment to create a replacement for his original nose which was infected and deformed – and was carried out at a hospital in Fazhou, Fujian province. The 22-year-old damaged his nose in a traffic accident in August 2012, but failed to seek treatment. As a result, an infection corroded the cartilage - leaving the surgeons unable to repair it. They were then left with no choice but to grow him a new nose in order to transplant it in place of his damaged one. The nose was created by placing a skin tissue expander onto Xiaolian’s forehead. This was cut into the shape of a nose and was supported by cartilage taken from the man’s ribs. Surgeons say that the nose has developed well and that the transplant surgery will be carried out soon. South African Man Too Fat for New Zealand An obese South African man has been told he is too fat to live in New Zealand - despite shedding 30 kilograms since he moved to the country six years ago, a report says. Albert Buitenhuis (who now weighs 130 kilograms), and his wife Marthie, said they face deportation after an application to renew their work visas was rejected because of his weight. Immigration New Zealand (INZ) cited the demands his obesity could place on New Zealand health services - and said their medical assessors deemed Albert no longer “had an acceptable standard of health”. The couple said they moved from South Africa to the main South Island city of Christchurch six years ago when Albert weighed 160 kilograms - and their annual work visas were renewed without any problem. “We applied year after year and there were no issues. They never mentioned Albert’s weight or his health once… and he was a lot heavier then,” Marthie told The Press newspaper. “The irony is that at the moment he weighs less than when we first arrived in New Zealand, and also less than in his first medical which was accepted by INZ.” An INZ spokesman said Buitenhuis was rejected because his obesity put him at “significant risk” of complications, which includes diabetes, hypertension, heart disease and obstructive sleep apnoea. “It’s important that all migrants have an acceptable standard of health to minimize costs and demands on New Zealand’s health services,” he said. Woman Sells House to Fund Surgery

You may have heard of a recent trend known as “couples’ surgery.” This refers to pairs of people (often husbands and wives - or parents and their adult children), opting to go under the knife together. However, a recent story out of Houston takes a dark twist on this concept, as a mother and her two daughters, Camile and Victoria Urgileshas were taken in along with her. This isn’t the first time that 50-year-old Maria Lorena Teran has been arrested for administering cosmetic medicine without a license. According to the Houston Chronicle, she was actually out on bail over a previous charge of the same nature. This time around however, her two daughters, Camile and Victoria Urgileshas were taken in along with her. As is so often the case with these sorts of stories, it seems that Teran was administering her illegal injections at a Medispa. The facility, La Femme’s, was located off Highway 6 (which runs through west Houston).

A British woman desperate to look good before her wedding sold her house to pay R330 000 for cosmetic surgery. Tsia Pullen, 34, was considered morbidly obese and struggled with obesity since childhood, the Daily Mail reported. Anxiety Linked to Suicide

The relationship between anxiety and suicide risk has long been debated, but now four papers in a special issue of Depression and Anxiety aim to settle the controversy by demonstrating that anxiety posed a greater suicide risk than depression. In the first study, Dr. Phillip Batterham examined 7,485 adults, and found that suicidal thoughts were more strongly predicted by incident symptoms of anxiety than depression. Holly Wilcox follows this by examining the role of Post Traumatic Stress Disorder (PTSD) in 1,433 adults with severe depression and a high lifetime rate of attempted suicide. PTSD increased suicide risk by 2.5 fold - and this increase was largely driven by individuals whose PTSD was due to assaultive violence. Zimri Yaseen examined 2,864 individuals with depression in a national U.S. sample, and found that panic attacks associated with a specific catastrophic fear of dying predicted subsequent suicide attempts. Amrit Kanwar co-authors the final study; the first metaanalysis of the role of anxiety disorders in suicide. Dr. Kanham reviewed 42 studies covering a total of 309,974 adults – and conclusively showed that patients with an anxiety disorder were more likely to have suicidal thoughts. We spent five years testing the ingredients on hundreds of women, and found this combination could reduce deep wrinkles within 14 weeks. There is nothing currently on the market which can do that”, Professor John Casey, vice-president for biological sciences at Unilever, said.

Diabetes Drug May Have Anti-Ageing Effects

A diabetes drug given in low doses may have anti-ageing effects, US scientists have said following an experiment with mice. Scientists believe the drug, metformin, may mimic the effects of extreme calorie restriction – and eating a very low calorie diet is thought to promote healthy ageing, the BBC reported. Rafael de Cabo of the National Institute on Ageing in Baltimore, Maryland, US, said calorie restriction in laboratory animals has shown to increase their lifespan. However, the human implications of the study were unclear. Metformin is a widely prescribed treatment for type-2 diabetes, which occurs mainly in people over 40. It is used to treat metabolic syndrome, a combination of diabetes, high blood pressure and obesity. Work has shown that metformin can extend the lifespan of simple organisms like worms, yet studies in flies and mammals have given conflicting evidence. The scientists gave one of two different doses of metformin to middle-aged male mice, and found that lower doses increased lifespan by about five percent. It also delayed the onset of age-associated diseases. On the other hand, they also said that the higher dose of metformin was toxic and reduced the lifespan of mice. Herbert Chavez has transformed himself from a mild mannered dress maker into a real life Superman - thanks to 19 plastic surgeries to copy the comic book heroes look. The 35-year-old, from Calamba City, Philippines, has had nose jobs, skin whitening, lip liposuction, jaw realignment, pec implants and even abdominal implants to become a reallife Clarke Kent. Herbert first fell in love with the super hero - famed for his red cape and ‘S’ logo - when he watched him on T.V. as a child. Over the next 16-years, he has splashed around 300,000 Pesos (R66 000) on his obsession - a fortune compared to the average R15 hourly wage in the Philippines. He regularly takes to the streets around his home dressed in full Superman regalia, with his hair gelled in distinctive curly fashion. He aims to teach kids good morals while also brightening their days. Herbert said: “I feel like a Superhero whenever I pull on the costume, yet my mission is not to save the world - but to help in my own small way and bring a smile to the faces of local children. I don’t have any regrets at all. People come up to me in the street all the time and want their picture taken with me. They are all really excited to see a real-life Superman in the Philippines.” But his distinctive look has not come easily, with Herbert going under the knife repeatedly to copy every last feature of the super hero he worships - right down to his chiseled chin. And Herbert admits it is tasking trying to keep up with such a “perfect” person. He added: “If you talk about super heroes, the expectation of people is that they are flawless, they do not get hurt and they do not die. But as human beings, we can only copy their looks. Especially as a Filipino, I have very Asian features and did not look like the Caucasian Superman. So when I was able to save enough money to alter my features, I chose to copy those of Superman. “It is hard living up to people’s expectations as a living superhero. That is the reason I have had so many surgeries because I undergo maintenance procedures that need performing. “For example, my cleft chin is formed of hydrogel, but it does not always appear balanced. The more dehydrated you get, the smaller it gets - so you have to keep adding to it. “I am such a good customer of my surgeon, Dr Tito Garcia. He does all the work for free as part of an exchange deal. I help publicise his practice in return.” Herbert now plans to keep improving his look with more operations to make him taller – and surgery to beef up his abdomen and buttocks. But he acknowledges that there may be a day when he has to halt the extensive surgeries. Herbert said: “I totally don’t have any regrets. What is there for me to regret? r David Matlock met his 38-year-old bride, Veronica, in 2007 when she sought out his skills for a vaginoplasty procedure (more commonly known as designer vagina). At the time, Veronica had just given birth to her daughter Isabelle (now aged nine). While Dr Matlock agreed to the vaginoplasty surgery, he also suggested that the beautiful brunette (who was 36 kg’s heavier) undergo a ‘Wonder Woman Makeover’, involving liposuction of the chin, arms, and legs, and a Brazilian butt lift. ‘I couldn’t even look into his eyes because of the reason I was there,’ she said. ‘I was so shy and bashful, but he says it was love at first sight.’ Dr Matlock then took the now Mrs. Matlock out for a date, immediately proposed to her, and has been helping her to enhance her figure ever since. ‘I didn’t see her weight then, I just knew what I wanted to do,’ he said of their first meeting. ‘She opted for everything I suggested… even marriage.’ Prior to meeting her 49-year-old husband, full-time nurse Mrs. Matlock had undergone three breast augmentations. She has since had a Brazilian butt lift, vaginal rejuvenation, labiaplasty and G-spot shot, liposuction to her chin, arms and legs and, most recently, Botox injections. ‘I feel like I’m a walking advertisement for him,’ she said. Is there any way to rejuvenate and lift the neck area without surgery? I’m in my early 60’s and cannot stand my ‘turkey’ neck. I would love to have a more youthful décolletage, but I’m scared to go under the knife. Any suggestions? The neck is a difficult area because several components contribute to its appearance; muscles, fat, skin texture, age etc.. Facelifts, neck lifts and liposuction are the surgical mainstays and offer great rewards, but are costly and demanding options. There are, however, several nonsurgical possibilities: • Botulinum toxin (Botox) can relax any tight bands of the platysma muscle of the neck. • Dermal fillers can improve the fine wrinkles (crêpe-like skin) of the neck and décolletage very nicely. • Skin treatments and peels will improve skin texture and thickness. Others may argue for laser, dermabrasion, platelet rich plasma, mesotherapy etc., but the results of these treatments are less predictable and more variable in my experience. I am a plump 34-year old woman who is 15kg overweight. I finally decided to tackle my bulge, so I started a strict weight loss program where I’ve cut out all refined starch, sugar, dairy and oil. I’m also exercising four times a week and have cut down on my alcohol intake. So far, I’ve lost 4kgs in three weeks. While I’m thrilled about this, some of my friends say this is not healthy and unsustainable as one should only lose 1.5kg’s a month. Apparently it’s in order not to gain all the weight back. Now this has gotten me worried… is there any truth to this? I thought a loss of 1- 1.5kgs a week is acceptable? Firstly, well done for your success thus far. Losing weight ‘healthily’ by following a proper balanced diet with a healthy lifestyle is the best way the achieve to a sustained, long term weight loss result. The so-called ‘yo-yo’ phenomenon happens often, where people cut back too much on nutrient intake or exercise too much, leading to binge eating of refined foods and consequent weight gain. There is no absolute values that can be applied to your question. For example, on the television programme “The Biggest Loser”, participants are encouraged to lose as much weight as possible per week to remain on the show... (often in excess of 2kg/week!). The law of diminishing returns applies very well to weight loss. When someone makes favourable eating and lifestyles changes, small amounts of effort deliver significant rewards. Yet, as an individual approaches their goal weight, the reward curve begins to flatten and greater effort and much better diet and lifestyle choices are needed to continue seeing favourable physical changes. Long term maintenance can be as demanding as the initial ‘start’! In summary, there really can’t be ‘absolute values’ because everyone is different (1,5 kg of weight loss in a 130kg male in a week is very different from the same amount in a 50kg female). The best results come from a positive mindset, a healthy balanced diet, an active lifestyle and a good superviser (such as a fitness instructor, medical practitioner etc).

Sitting at my desk, my colleague pops her head over the copier and asks “What’s got your knickers in a knot?” “Nothing – I’m concentrating.” I replied. “Bitchy resting face is more like it,” she said walking off. Freeze, rewind… did she just call me a bitch because I was concentrating? Hell no! Being the politically correct person I am, I followed her. Briskly. “Please explain why I am a bitch,” I smiled, beaming with passive aggressiveness. “Oh no, I never said you were one - just that you have a Bitchy Resting Face. Google it,” she added, going back to her merry way. There’s a difference? By the time my fingers hit the keyboard, search engines came alive with Bitchy Resting Face hits. According to the wonderful world of the web, BRF was originally coined by comedian Taylor Orci, after she released a hit skit for her YouTube channel, Broken People (which subsequently received more

than two million hits). Although laid out as a hilarious parody, Bitchy Resting Face quickly touched a chord with females everywhere. And why shouldn’t it? BRF literally denotes a woman who looks thoughtful, sad or angry, for no apparent reason. Meanwhile, inside she is throwing streamers and doing the moon walk. In short, they are happy, if not content… yet their outward appearance is contradictory of their current state of mind. “Wow, what a bitch!” How can I have this affliction when I am a positive, go-getter?

But wait…how many other poor souls suffer from this hardship? Surely it can’t be that many? I decided then and there that I had no choice… I simply had to take this to the streets and see for myself. And where better than a shopping mall! I proceeded to seat myself in a corner booth and smile… watching people certainly has its ups and downs. I noticed women who were with a shopping partner smiled more – and this smile made them more approachable. Whilst the ladies on their own were lost in thought, their faces yelled “Not now! Not ever!”

Wait just a minute… have the clouds parted to let the light in? Here I thought people (guys more so) were less drawn to me due to my extroverted, over the top, bubbly, self-assuring persona. Yes, I am fully aware that these characteristics are somewhat scary to behold – but in actual fact, it’s been my face all along!

Now, I know that a bad attitude can be turned around by a positive outlook – or that a crummy day can go sunny side up. Yet if you got a bitchy face… well aside from the permanent-oh-my-cheeks-hurt-like-hell smile, nothing can rectify that. Or so I thought...

he quest for beauty has been an aspiration from ancient times. It has been speculated that many women spend up to one-third of their income on looking more beautiful.

We have seen the statistics on how beautiful people get better jobs, earn higher salaries and are treated as being more superior to their ‘ugly’ equals. This further amplifies the search for beauty, especially in women. So what is beauty? Is it really “in the eye of the beholder”? If it truly is in the eye of the beholder, then we probably would not have global celebrities and faces of beauty. The Face of Beauty An attempt to define facial beauty with facial proportions (as well as the golden or divine proportions), has been well accepted for a number of years. These proportions earned their fame with famous artist Leonardo Da Vinci – and hence, often refer to

his surname. These ideal proportions include aspects such as a perfect balance of the face with regards to eyes (size and distance), nose to chin, lips, and facial width to height balance. Having the ideal facial proportions when you are young will surely make you more popular - and you will be perceived as an attractive person. The Ageing Face Unfortunately, as we age your proportions alone are not the only thing that will maintain your beauty. Suddenly, other aspects on top of the perfect proportions can reduce your attractiveness. These changes that occur in the face over the years do not only make one look older or less beautiful. Small, gradual changes in the face lead to negative emotional expressions in the face as well. These negative expressions can make even the most perfectly proportioned person look less attractive. Facial Rejuvenation One of the aesthetic laboratories in France, Filorga Laboratories, developed a tool to assist physicians in treatment planning of ageing faces with negative expressions. The expertise of a French plastic surgeon and of a French artist was combined to show how negative emotions on the face could be corrected with the correct placement of fillers (the clay).

here are three chief factors that encompass a successful enhancement with fillers – these being: a good physician, a good patient – and of course, a good product. We know the importance of a doctor‘s continuous training and education in the developing medical field… but what about the patients and the products? The South African Patient Demystified I was privileged to have two of my friends (who are both dermatologists from Italy and Dubai) visit my practice for a week to view my procedures – and they observed some interesting behavourial patterns from South African patients. Firstly, they noted the SA patient has a very conservative approach when it comes to their aesthetic procedures – and secondly, tend to be more demanding in their approach when compared to patients in their respective countries. This is actually why I love the SA patients – they definitely know what they want!

In retrospect, I have also observed the following traits in my practice: 1. The SA woman is very careful when she chooses a filler as it needs to deliver an exceptional and natural result. She wants people to notice that she’s looking more beautiful… however no one must know that she’s had some ‘work’ done. There must be no swelling in the treated areas - and it needs to be painless. 2. The SA woman will only have her lips plumped up if there is no risk of swelling, bruising – or ending up with the dreaded ‘trout pout’ look. 3. The SA woman does not like the mid-cheek sagging effect - nor a widely shaped nose. She wants a lunch time painless procedure with long lasting results. 4. O n the other hand, the SA male patient (unless metro-sexual or homosexual) does not believe in aesthetic procedures for him or his wife… that is until his best friend starts to look younger, well groomed and receives more attention during cycling classes! He is happy to include a face make over in his routine - but only if there is a good scientific explanation about the success rate – as well as the safety of the procedure.

What are Lasers? Laser stands for Light Amplification through Stimulated Emission of Radiation. The ability to generate photons from external energy was first proposed by Albert Einstein in 1917 - and Laser is a technology that’s been developing rapidly since the first visible Laser was demonstrated in 1962. Lasers are special lights because they have spatial coherence (light waves travel in the same direction, giving a precise, controlled beam), and temporal coherence (the beam is produced in a narrow wavelength band i.e. a single precise colour). The ability to produce blasts of controlled energy with precise properties are what makes Lasers so useful. Why are Lasers Useful in Aesthetics? Different types or wavelengths of light create specific tissue effects – e.g.Lasers can destroy pigmented/damaged areas of skin, hair follicles, or tattoo ink – all the while leaving surrounding normal tissue unscathed. The thermal energy delivered by a Laser also stimulates the body’s natural elastin and collagen production. Horses for Courses Skin type (including hair type and colour) is an important factor in treatment selection. Your aesthetic practitioner will evaluate your skin type (e.g. using the Fitzpatrick classification) against the objectives of the treatment – and use an appropriate Laser, configured precisely to maximise effectiveness and safety. Hair Removal Hair removal is the original use for Lasers in aesthetics, targeting dark pigments in the hair follicles, which are destroyed by the Laser energy. Surrounding tissue is generally undamaged. However, it must be noted that six – eight treatments may be required for full effect. Lighter hair colours are less susceptible to this treatment, and so may require

multiple treatments. Another option is for the hairs to be pre-dyed with a special pigment to help absorb the energy. Medical Uses – Skin Resurfacing Improvements in the last decade have seen a surge in Laser use for skin resurfacing, which can mitigate the normal features of ageing. This includes sagging skin, wrinkles and rough skin – and it can also reverse generalised damage caused by acne scarring or sun exposure. Lasers are divided into two major categories - ablative and non-ablative. It’s important for people to know the difference between the two types of lasers as the recovery time differs significantly between the two types. Additionally, there is a difference in not only the end result, but how the end result is achieved. Ablative The laser destroys target areas of existing skin, allowing the body’s natural repair mechanisms to produce new tissue within a controlled framework of ‘damage’. This maximises the efficiency of the repair process and minimises the chance of scarring. It’s an invasive treatment which requires several weeks’ recovery – and several days’ privacy is needed. Yet, the effects are profound, and normally only one to two treatments are required for full effect. Non-Ablative This technology directs energy below the surface of the skin, causing a thermal reaction which increases collagen production, skin smoothness and firmness, all the while stimulating the body’s natural regenerative processes. A single session over an area such as the face will regenerate 10-20% of the total area - so two to five treatments will probably be required for maximum effect. However, the recovery period is far shorter than an ablative treatment.

Tongue Patch Diet – Hard To Swallow If the idea of losing weight by having a patch sewn on your tongue seems hard to swallow, well… that’s the point. We take a look at the new extreme weight loss procedure known as the Tongue Patch Diet.

T

he tongue patch is a new cosmetic surgery technique that can allegedly help people lose up to 13 kilograms in a month via a postage-stamp-size patch on the tongue.

The rough patch is designed to make eating solid food so painful that patients only consume liquids, MSNNow.com reported. The material used in the patch is marlex, an abrasive material usually used to repair hernias - and is secured to the top of the tongue by six stitches. Side effects of the patch include difficulties in speaking as well as a tendency to wake suddenly from sleep. The Pioneer The tongue patch is the brainchild of Dr. Nikolas Chugay, a Beverly Hills-based cosmetic surgeon, who said all patients are given an easy-to-follow liquid diet that fulfils all nutritional needs while minimizing caloric intake. He introduced the procedure four years ago after first seeing it done in Latin America. So how does the tongue patch make eating solids a nightmare? “As you’re swallowing, say, a cheeseburger or something, that tongue is pushing things up against the roof of the mouth and back into the throat,” explained Chugay. “And so while it’s doing that, it’s going to basically be uncomfortable if you have that on top of the sutures, which are basically fishing line. So now you have two little sharp points on the tongue, rubbing up against the top of the mouth... and it’s going to be uncomfortable.” The whole procedure takes about 10 minutes and the price tag is a hefty R21 000. While the procedure is yet to be approved by the FDA, Chugay said he has not encountered any risks or

infections - and no patients have swallowed the patch. However, patients on his diet can only wear a tongue patch for a month at a time because after that, the patch fuses to the tongue surface, UPI.com reported. The Volunteers Now, due to the radical nature of this new weight loss method (not to mention the extreme pain factor), one would be forgiven in thinking that volunteers for this procedure would be hard to come by. Surprisingly, this is not so. One clinic in Caracas, Venezuela, said it had stitched patches on more than 800 tongues – while Dr Chugay has no shortages of patients lined up. In fact two of his patients, 20- year-old Marlene Beltran and 27-year-old Lysander Lanuza had their procedure filmed for ABC News. Weighing in at 76 kg, Beltran told ABC News’ Cecilia Vega, of her out-of-control appetite. “I’m willing to do anything at this point,” she said. “I’m so desperate to lose the weight.” Meanwhile, Lanuzua (who weighs 90kg) admits to her all-youcan-eat habit. “I love eating everything,” she said. “American food, Filipino food, Korean ... it’s heaven.” Their goal was to lose 9kg in one month as Beltran wanted to begin dating – and Lanuza had a bikini and a trip to Hawaii planned. Changay said that Lanuza was his 81st patient. With the tongue patch in place, her customary greasy fried food is now off limits. It’s a strict 800-calorie per day liquid diet of juice, broth and protein shakes until the patch is removed in one month. While Lanuza admitted to being terrified of eating solids and

experiencing intense pain, she succumbed one day and had a piece of popcorn when she was at the movies. “It hurt”, she said. “So, I’m like, forget it!” As for Beltran, she had an acute craving for In-N-Out Burger on day five of her diet. “I don’t care if it’s a bite,” she said. “I just want a burger.” Luckily she resisted. One month later both women are all smiles as Lanuza dropped an impressive 10kg – and Beltran shed 8kg. “My double chin isn’t as bad as it used to be,” says Beltran. “My arms are a lot thinner.” Lanuza also notices a huge difference: “I see myself better” she says. “And really, my husband is the happiest person. He’s over the moon about my weight loss, she adds. The Critics Needless to say, the tongue patch diet does not resonate well with several members of the medical community. “I think it’s ridiculous,” said Dr. Christine Petti, a plastic surgeon in nearby Torrance, Calif. “I could never advise a procedure that would cause a patient pain ... pain is not a good thing for anybody.” Dr. Rob Huizenga, who specializes in long-term weight loss and has spent 14 seasons as an expert on the television program The Biggest Loser, is not a fan either. “ This is so primitive an approach,” he said. “You could hire somebody to hold a gun to your head and threaten to shoot you every time you eat. The idea that you put this patch in - and every time you even take one morsel of solid food you get this stabbing pain, who the heck knows what the long-term consequences of that are,” he adds. Yet Chugay is not bothered. He feels the tongue patch can jump-start lasting weight loss. “It takes about 30 days to change a habit,” he said. “That’s what we are doing. The Tongue Patch Diet Fad or Flop? We ask Dr Riekie Smit to deliberate… This notorious diet is very radical and controversial. I agree that it can have severe side effects such as scar formation, infection, pain and ulceration. We all know that people can be so desperate that they could not care about risks - if only they can lose a few kilos. So even though numerous warnings may be given, there will still be multitudes trying it. One can be very narrowminded and harshly judge those having it done – as well as those doctors performing it. From another perspective, we can look at the devastating effects of obesity and realize that the complications from this are much more life threatening than those from such a radical treatment. It has – and always will be – a tremendous challenge to break the habit of individuals making incorrect food choices and overeating. This is why we see such drastic therapies appearing on the market. Mere education on healthy eating and lifestyle choices have been an approach for many years... with very little success rate. Recently, some states in the US have taken it a step further to bring in warnings on food labels and restricting advertising on certain classified unhealthy foods. We can only wait and see if this might bring down the obesity statistics… In medicine we work on a benefits: risk ratio. If the benefits outweigh the risks and will improve a patient’s quality of life and health, then that specific treatment can be considered. Primum non nocere is a Latin phrase that means “first, do no harm,” which originates from the Hippocratic Oath. This is always the most important aspect to keep in mind when managing a patient. Therefore, taking this oath and the benefits: risk ratio in mind, we have to decide what will harm the patient most? The complications from obesity - or the complications from the tongue patch. It then makes sense that this drastic treatment option may be considered in individuals with severe obesity, where the benefits outweigh the risks from this treatment. It would not make sense to consider this treatment option in a person wanting to lose just a few kilograms. But still, all other possible treatment options should also be considered - and then the safest and most effective method will be chosen wisely. The Skinny on the HCG Injection Diet

In their quest to beat the battle of the bulge, many dieters become frustrated at the slow progress of shedding their excess baggage. So who wouldn’t want to try a weight loss program that promises fast, dramatic results? Well, one such diet promises just that – the HCG injection diet. But how effective is it? And more importantly...is it safe? Dr Janine Olivier gives us the lowdown...

T

he HCG Diet - an extreme diet that involves injections of HCG (human chorionic gonadotropin) has been around for more than 40 years. HCG is the hormone women make when they are pregnant. Previously, the HCG Diet was defined by a 500-calorie diet paired with daily injections of HCG. This diet is associated with dramatic weight loss. But the problem, up until now, was that no doctor could explain how it worked - and critics claimed the caloric restrictions are responsible for the weight loss and not the HCG hormone. Critics of the diet have also maintained that the weight lost cannot be kept off. HCG Origins The original HCG Diet that Dr Simeons (a UK endocrinologist) created had some strange rules. For example, it required the consumption of only one vegetable per meal, prohibited the use of oil, body lotions and hygiene products, and limited use of the HCG hormone to odd cycles of 23 and 46 days. The original diet also did not allow exercise. In addition, there had been no published guidelines on how to dose the HCG hormone - and as a result, most providers of this diet were generally using random dosing of the prescription hormone. This was called the old Simeons Protocol (Simeon A), and according to the new wave of medical professionals that offer this type of diet, it was unhealthy and no supplemental support was offered. Present Day A recent guest on the Dr Oz Show – Dr Sheri L Emma (MD) has done extensive research on the diet and explained why HCG does help you to lose fat and still

retain valuable muscle mass. After years of research and treating thousands of patients, Dr Emma set out to put the record straight. The original Simeons Protocol (Simeon A) should no longer be in practice by any physician. Her protocol is supported by numerous studies and clinical trials, the latest of which was presented to weight loss physicians at the October 2012 meeting of the American Society of Bariatric Physicians. It’s been well documented that our fatty tissue contains a rich supply of stem cells – and is widely used in the aesthetics field because of its antiageing effects. However, did you know that our fat derived stem cells may not only be the answer to curing many diseases - but for improving scars as well? Dr Nerina Wilkinson reports…

W

ith the introduction of liposuction by Dr Illouz in the 1980s, there have been many newer developments over the past 10 years in using the fat that would have been discarded. Liposuction has developed into liposculpting - and the emphasis is on a process of re-modeling (and not just removing unwanted fat deposits). Many lipofilling techniques have been described where the fat cells are reinjected into areas of volume loss to improve contour and shape. Recent advances in stem cell research have found that adipose (fat) tissue is the adult organ which is the richest supply in stem cells. The clinical potential of fat-derived stem cells has created enthusiasm in medical circles. Besides the anti-ageing effects of stem cells, doctors have high expectations that adipose/ fat derived stem cells may not only be the answer to curing many diseases - but for reconstructing anatomical defects and improving scars as well. These stem cells have immense restorative properties, and can help replace missing or damaged tissues once they have been injected. How Do These Stem Cells Work? Adipose / fat derived stem cells have multipotent cells that can differentiate/ change into many different kinds of cells – and thus, have uses in regenerative medicine.

growth factors they contain, they can stimulate the cells of the blood vessels to grow and vastly improve blood supply to the affected area. The improved blood supply and growth factors stimulate the fibroblasts and platelets to heal and regenerate stronger tissues. 2) S tem cells are antioxidative, which is important in the body as it naturally fights damaging toxins. 3) S tem cells have also shown to suppress inflammatory cells, while stimulating the production of anti-inflammatory cells. A report in 2010 by Ichim et al, demonstrated the treatment of a 67-year-old lady with rheumatoid arthritis using stem cells isolated from liposuction. They reported considerable resolution of her pain and stiffness, probably due to the suppression of the pain causing inflammatory cells. Toxins: How Are the Stem Cells Harvested? Conventional liposuction is performed under general or local anesthetic, depending on the amount of fat cells required and the donor areas. The liposuction incisions are very small, and are hidden in natural skin folds to minimize the visibility of any scars. The fat is aspirated with a narrow cannula, and then purified through centrifugation to increase the concentration of stem cells in the aspirate. This fat, which is rich in stem cells, is then reinjected into the planned area - and has a better capacity to survive and regenerate. It is important to remember that this is a surgical procedure, which must be performed by a competent plastic surgeon that will operate in a licensed surgical clinic. What Are the Regenerative Uses of Stem Cell Enriched Fat Transfer? Besides the well-known anti-ageing benefits of lipofilling of the face, stem cell enriched lipofilling has recently been widely used in reconstructive procedures. This is due to the well-documented added benefits of tissue regeneration.

2) Scars: Lipofilling is frequently used to improve traumatic, surgical and acne scars. Most scars do however require more than one modality, such as lasers and lipofilling for the best results. Patients need to be realistic in expectations, as no treatment will totally remove scars. Yet, the use of stem cells in the treatment of scars definitely improves the blood supply to the scar - and helps to regenerate new healthy collagen and elastin in the area. This results in a much-improved appearance of a scar, as well as a softening of the surrounding tissues.

1) Wound healing: Many recent studies have demonstrated the promotion of wound healing using adipose derived stem cells in the treatment of chronic wounds. We are seeing dramatic results in post radiation wounds following cancer surgery. The radiation therapy is designed to kill any residual cancer cells; however this can also have devastating effects in patients as it causes irreversible damage – as well as radiation ulcers of the healthy surrounding tissue. These wounds would have been extremely difficult to treat in the past, and could take years to resolve. Stem cell enriched treatment of the ulcers shows an accelerated reduction of the wound area and volume, due to new blood supply and differentiation into epithelial cells.

3) Soft tissue augmentation: Besides the conventional aesthetic anti-ageing effect of volumizing faces, lipofilling can be used to augment soft tissue volume in reconstructive surgery. Post traumatic depressed scars are injected subcutaneously to improve the contour and the final aesthetic result. Breast deformities after lumpectomy can also be distressing to patients - and in those who prefer not to have a silicone breast implant, fat cells can be injected into the defect to fill out the depressed area. Patients who’ve had removal of breast implants due to problems, are advised not to reinsert new implants. Improved breast fullness can be created by injecting a patients own fat cells into the area for a more natural breast shape.

In my practice, I have seen the “unfriendly” radiated breast tissue change into softer, warmer breast tissue after lipofilling of the area. Patients have also described an improvement in the sensation of the breast.

As medical specialists, we are still in the infant phase of understanding the vast potential of adipose derived stem cells and in the future we will be using the regenerative capacity of these cells to treat many more medical conditions.

We spend a great deal of time and energy thinking about the ways we should change our teeth in order to perfect our smiles. But what if our teeth aren’t the culprit to a flawed smile – but our gums? Dr Sam Thandar discusses gummy smiles and its treatment options.

The Gummy Smile Gums & Smiles The lips, teeth and gums all play an important role in the appearance of a beautiful smile. They should all be in perfect harmony, with the ideal smile line revealing very little (or if any) gum tissue.

Now, if there are any alterations or delays occurring in the passive eruption phases, it causes a gummy smile.

Therefore, people who show more pink and red than dazzling white when they smile are said to have a ‘gummy’ smile. Often, these individuals are under the impression that they simply have excessively small teeth. However this is not the case.

From Gummy Smile to Toothy Grin How do we fix a gummy smile? Well firstly, it’s of paramount importance that whatever surgical or non-surgical intervention we elect, we make provision for the biological width zone and gum pocket. Both surgical and nonsurgical interventions can consist of just soft tissue gum removal, or a combination of gum tissue and the underlying supporting bone.

Gummy Causes During the eruption process of our permanent teeth, there are 2 processes: a. Active eruption – the tooth moves downwards out of the gum and towards the opposing tooth and, b. Passive eruption – the gum migrates upwards towards the neck area of the tooth

With a gummy smile, the gum pocket may be anything from 2-10 mm - bearing in mind that just below the gum pocket we have solid bone. Moreover, the average length of our front teeth in an aesthetically pleasing smile is about 10mm, so we see about 10mm of tooth structure which makes up the crown of the tooth.

From the above, we can see that there are various parameters to consider clinically when making a decision on aesthetically restoring your gums and your smile. In a nut shell, we look at your smile line and the tips of your upper front teeth. They should ideally make very light contact with the inner border of your lower lip, so as you smile, as do your teeth when it follows this curvature. Then we look at your lip line and how far your lips go - as well as how much gum tissue you expose when you smile. With female patients that generally have a high lip line, they would show 2-4mm of gum when smiling - however in a patient with a medium lip line, they would show 1-2mm of gum tissue on certain teeth (or no gum tissue at all - just tooth structure). When planning on any elective plastic gum re-contouring, we need to consider all these parameters and more. Gummy Fix: Treatment Options For removal of gum tissue alone (without any removal of the underlying bone), we have various soft tissue lasers and cautery devices. Some use radiofrequency, while others use electric frequency (such as the radiosurge and electrosurge). The lasers result in abblation of soft tissue, while the other cautery devices literally burn the excess gum tissue away. When it comes to cases where we are required to remove some of the underlying bone supporting the gum, the conventional approach was surgery – either periodontal surgery or crown lengthening. These would require anything from two-six weeks healing time, during which the gum matures to a new position. However, some of the more recent lasers available are capable of both soft and hard tissue removal – and these lasers would remove gum tissue as well as bone with minimum down time. In closing, proper assessment and diagnosis of each individual case is required in order to ascertain what the best or most appropriate protocol is to correct a gummy smile.

Our adventurous columnist, Rochelle Friedman, has been exercising and eating right for years. However, her struggle to lose those final centimetres and smooth out the stubborn fatty parts of her body called for expert help. How invasive is ‘minimally invasive’? In simple terms, a small incision is made (half the length of your fingernail) in areas to be treated. A cannula device is inserted, which performs a dual job: melting away fat and gently sucking it out. There is no abrasive back and forth movements like you see on TV, and this is why there’s little to no bruising and very minimal downtime (most people return to work the next day.) Was it painful? During the actual procedure? No. Dr Claire gave me a tranquiliser tablet before we began, which helped me to relax. The entire procedure is done under local anaesthetic – and I couldn’t feel a thing. I could feel that Dr Claire was “working” on me; however, I just plugged in my earphones and listened to my audiobook throughout the procedure. Before I knew it, we were done. How was recovery? I drove myself home immediately afterward – I did feel tender, but it was completely bearable. During weeks one to three, my muscles in the areas worked on felt tight and stiff, similar to the feeling of having done too many stomach crunches and squats.

How long until you began to see results? The swelling decreased after two weeks and by week four, I really started to see the difference. I’ve been told that between two to six months I will see incredible improvement. Are you happy with the results? Ecstatic. It’s been six weeks and I am beginning to see stunning results. Despite my abdomen still being slightly swollen, it is flatter than it has ever been before. It’s very exciting. What side effects were there? Bear in mind I followed Dr Claire’s advice stringently: I don’t smoke, stayed away from taking any blood thinning medication like Aspirin and I don’t drink heavily etc. • I experienced no bleeding or leaking afterward. • Minimal bruising on my thighs (saddle bags) that looked like I walked into a table. This

went away within 10 days, just like any normal bruise. What does BodyTite cost? Pricing starts at R15 000 (depending how many areas you choose). You will need a consultation for a confirmed price. Do: • I recommend planning ahead and having this done in winter. Having to wear the compression garment 22hrs of the day for three weeks (aids recovery) can be uncomfortable on a hot summer’s day. Additionally, the garments are difficult to disguise under strappy tops and dresses. • Go back to gym as soon as possible and do very light exercise. I went three times a week and enjoyed strolling on the treadmill for just 20mins. It helped my healing significantly, kept me moving and kept things flowing.

Don’t: • Do not have the procedure within one month of a special occasion. You will just feel uncomfortable. Give yourself at least three months leeway! • Be aware of your mindset. Just because fat has been removed, it doesn’t mean you can now eat whatever you want. • Manage realistic expectations emotionally and physically.Like any cosmetic or aesthetic procedure, you are not suddenly going to turn into a flawless being. It’s that glorious time of year again where shed our winter woollies; throw on our bikini’s and head for the beach. However, before we reach for our trusty sun-block – take heed. According to a new study, the chemicals found in our sunscreens can be hazardous to our health. By Dr Noori Moti-Joosub

T

hey are designed to protect our skin from the sun’s harmful rays - but some experts are now questioning whether some sunscreens could be harmful as well as beneficial. New research suggests that the complex cocktail of chemicals which helps prevent sun damage, lessens the risk of skin cancer and prevents premature ageing – may ironically contain carcinogenic substances – while depleting the sun protecting actions as well. Therefore, it’s vital that special attention is taken when choosing your sunblock. Question is: can you identify what is in your favourite sunscreen?

UV Blockers The first thing you should know is that there are two types of UV-blockers in sunscreens - namely: a) C hemical blockers: Organic chemicals that absorb UV light b) P hysical: Inorganic particles that reflect, absorb or scatter UV light. It’s been thought that physical blockers are better for sensitive skin - and also don’t contain harsh chemicals that may damage the skin. It’s also thought that the higher the SPF, the more chemicals are taken to get to these heights. However certain sunscreens only use three chemical blockers and achieve an SPF of 50. A general rule can sometimes be applied, but there are always exceptions to the rule. Sunscreen and Children Sunscreens are only safe in children over the age of two. Yet there are some who advocate

the use of sunscreens in babies aged six months and over. In this case, it’s best to use a children’s brand which is hypo-allergenic, has an SPF of 15-30 – and is waterresistant when swimming. These generally have a lower chemical content than other sunscreens. Or rather choose physical blockers for children’s sunscreens. All sunscreens should be reapplied despite their SPF, as most wear off in three hours. Unfortunately, many people have problems reapplying as this is an added chemicalburden on the skin. So the question is: are chemicals all that bad? Here is a synopsis of chemical and physical blockers found in sunscreens: 1. Zinc oxide: A physical blocker found in sunscreens Excellent scientific studies show that nanozinc oxide in sunscreen doesn’t get into human skin. Zinc Oxide is a powdered mineral that works as a sunscreen by sitting on top of the skin, scattering and absorbing ultraviolet (UV) rays. Zinc Oxide is the safest & most effective single active sunscreen ingredient. It protects from UVA and UVB rays and does not enter the body through the skin. 2. Octinoxate: It’s most commonly known as an ultraviolet light absorber and UV filter. Currently, octinoxate is the most popular product for blocking UVB. But there are several side effects to octinoxate – namely: • Skin Irritation: One of them includes irritation of the skin, which can linger in

ones system for several years. • Toxicity: Through skin absorption, octinoxate can lead to reproductive as well as developmental toxicity. It is also known to damage the immune system in rare cases, impairing a person’s ability to fight disease and repair tissue damage. • Hormone Disruptions: Women who are pregnant should not use octinoxate as it produces estrogen-like effects. Use of octinoxate can cause endocrine disruptions. It can also interfere with signaling between cells - resulting in mutations or the death of cells. Lastly, it’s vitally important to avoid the use of octinoxate in children. I must stress though, that some have disputed the claims and finds it less toxic than Padimate O. Personally, I would opt not to use it in children and young woman. 3. Octisalate or octyl salicylate It’s used to augment the UV-B protection in a sunscreen. Salicylates are weak UV-B absorbers, and they are generally used in combination with other UV filters. Other

Avobenzone Avobenzone provides superior protection through a large portion of the UVA range (including UVA I), and is a significant addition to sunscreen products for true broadspectrum UV protection. Nonetheless, concerns have been raised regarding its photostability – as well as its potential to degrade other sunscreen ingredients in products in which it’s used. Luckily, this photoinstability can be offset by combining avobenzone with octocrylene - or with other non-sunscreen ingredients.

5. Oxybenzone: The Environmental Working Group says 56% of beach and sport sunscreens contain the chemical oxybenzone. The primary function of oxybenzone is to absorb ultraviolet light, but some research shows oxybenzone can be absorbed through the skin. The Environmental Working Group and other toxicology experts believe that oxybenzone is linked to hormone disruption - and potentially to cell damage that may lead to skin cancer.

This has been disputed by many doctors as the study showed absorption of UV rays with the sunscreen - and the use of sunscreens should counteract absorption of the rays. This is a difficult one. I do think there is not enough evidence to show toxicity, but if an oxybenzone-free sunscreen is within your budget, then use it. As you can see from above, there is much dispute over chemicals in sunscreens. No concrete evidence exists over many of the controversies arising. The Cause & Effect of Photoageing Skin Ageing is divided into two different processes – Photoageing and Chronological Ageing. Photoageing is the premature breakdown of skin cells due to excessive exposure to the sun’s rays or tanning beds – while chronological ageing is the natural genetic changes of the skin we experience as we get older. Unlike the chronological process of skin ageing, photoageing causes the skin to become coarse, deeply wrinkled, and irregularly pigmented... and is the single greatest factor causing prematurely aged skin.

taying young and healthy is a prized wish of most individuals. Though scientists have not been able to slow down our chronological ageing, other causes of rapid ageing have been clarified - and thus measures can be put forward to prevent them. Poor habits such as smoking; lack of exercise; bad food; excessive make up – and the sun can all contribute to us looking aged. The latter, also called photoageing, has been in the spotlight since the 1980’s. Yet, people still don’t seem to grasp the concept of sun protection. People trying to reverse the signs of ageing caused by the sun help to sustain the multi-billion rand beauty and cosmeceutical industry. Chronological Ageing VS Photoageing Know the difference Photoaging is caused by the action of ultraviolet rays on the skin. UVB as well as UVA are responsible for the accelerated ageing of one’s skin. The fairer the person, the more prone the individual is to be affected by UV-rays - and it’s more apparent on areas that are usually exposed to the sun. It’s very easy to identify a person suffering from premature ageing as a result

of UV rays - and the table below helps to distinguish it from chronological ageing.

The Dark Side of The Sun When fair skin is exposed to sunlight, UV radiation is absorbed by the skin cells, leading to the generation of free radicals. When the rate of formation of the free radicals becomes higher than the rate that they can be cleared from the skin cells - they start causing damage to the collagen fibres; elastic fibres; and other cellular components of the dermis. Repeated sun exposure leads to imperfect repair, leaving a photodamaged skin. Lately, a lot of emphasis has been laid on UVA rays as they penetrate the skin deeper and thus, can cause more damage. More studies are underway for the role of visible light and IR rays, however no conclusive results have been obtained yet.

Other methods of sun protection are the use of broad hats; avoiding the midday sun; avoiding sun-tanning beds – and the use of sun protective clothing can be very helpful. One should always remember to reapply sun protection every few hours - and also on a regular basis when swimming.

Sun Safety It goes without saying that sun-protection is the most effective way to stay younger. One has to use strong sunscreens that have broad spectrum protection. The use of one with a high SPF as well as high UVA protection is now recommended by dermatologists. Most sunscreens that contain titanium dioxide; zinc oxide; oxybenzone; or avobenzone,

Can Sun Damage Be Reversed? Once damage has taken place, there are very few measures that can be taken to partially reverse some of it. Very few creams and substances have proper studies proving their efficacy. Retiniods are derivatives of vitamin A - and are clinically proven compounds that help reverse some photoageing. These compounds can be quite irritating to the skin for the first few weeks, but thereafter the effects can slowly be noticed. Maximal effect is noted in about six months from commencement of treatment, and the retinoid cream can be continued lifelong. Most good cosmetics contain some retinol, but, one can obtain higher concentrations of this compound in creams like Retin A or Retacnyl creams.

Lotions and Potions There have been numerous claims from the cosmetic industry of products that help with photoageing. But one should be very careful as it doesn’t mean that the most expensive products are the best. Cosmeceuticals are not required to undergo rigorous testing, and therefore are not always endorsed by qualified dermatologists. Some creams containing idebenone or vitamin C have shown to have a positive effect on the aged skin, however many other claims are not supported by proper clinical trials. Aesthetic Procedures Nowadays, there are several other procedures that can improve or hide the appearance of photoageing. The use of skin peels can help in the rejuvenation of skin, as well as the stimulation of collagen fibres.

If performed by qualified personnel, it can yield excellent results at a very affordable price. The use of Lasers and intense pulsed light also have beneficial effects on selected people. However, one should be very careful of unqualified personnel offering these treatments - as there has been a huge surge in law suits against them in the USA. Other methods include the use of LED lights and photodynamic therapy, where with the latter, a substance is applied to the skin and activated by the light. This results in damage to abnormal skin cells, allowing new cell to grow – and thus rejuvenating the skin. In cases where there are the deep wrinkles and volume loss on the face, one may need to resort to the use of botox or fillers. The abuse of fillers and botox by celebrities (who

end up having over-filled cheeks and gold fish pouts) - has naturally installed fear in many people who could have benefitted from them. If performed by qualified health professionals, these are very safe while giving a natural look.If everything else fails, one can make use of foundation and concealers to try to hide the aged look. Prevention Better Than Cure Photoageing is an indication of the health of one’s skin - and makes the person more prone to various skin diseases, including cancers. Therefore, one must start taking care of their skin from an early age, as no amount of procedures can reverse the damages fully. Sunscreens do not need to be from expensive companies - but they do need to have high SPFs, high UVA protection, and of course - applied liberally.

Obsession for a Soft Skin Causes a Korean Woman to Inject Cooking Oil into Her Face

H

ang Mioku, a former model from Korea always wished to have soft skin - and this turned out to be a lethal obsession when she continued to approach doctors for silicone injections.

She was advised to see a psychiatrist as they found her obsession with soft skin stemmed from low self esteem. Having undergone many cosmetic injections over the years, doctors refused any further cosmetic treatment on her face. In a desperate attempt, she took to injecting cooking oil after doctors refused to give her any more silicone injections – and this left her face so severely swollen and disfigured that even her own family members could not recognize her. She was also called ‘standing fan’ by the local children as her head was too large for her tiny frame. Reconstructive Surgery After the DIY cooking oil injections, her face was beyond recognition. Her plight was featured on Korean television, prompting thousands of sympathetic viewers to donate money in order to fund her corrective surgery. Now, thanks to the considerable donations Hang has received, she has undergone many corrective surgeries to reduce the swelling on her face and neck. On her first operation, surgeons removed 60g of silicone, oil and other foreign substances from her face – as well as 200g from her neck. The Ideal Neck What makes a neck appealing? Well, consider actresses like Keira Knightley or Natalie Portman. These ladies possess long, thin necks with a soft, yet defined jawline – which is seen as beautiful and extremely desirable. The Aesthetic Concepts Of A Youthful Neck Include: • A smooth, straight jawline • A high cervicomental angle (the transition from horizontal jawline to vertical neck) • Good skin tone, colour and elasticity • All transitions are soft and smooth • The lines are curvilinear • No prominent cartilages (a prominent thyroid cartilage gives a masculine appearance) • Definition of the bones of the jaw and chin • A long, sinuous shape (necks shorten with age because the cervical spine “collapses” over time) How the Neck Ages… Now that we can appreciate a neck, we need to understand the age-related changes that occur over time: • There is loss of dermal elastic fibres (collagen and elastin), which produces sagging and wrinkling • The underlying platysma muscle, which is smooth and well contoured during youth, develops bands and irregularities as we age • All structures of the neck sag over time (the muscles, fat pads and salivary glands) • The aesthetic lines and borders of the sternocleidomastoid muscles, the jaw and the chin become indistinct (the bones re-absorb over time and the tone of the muscles decreases) • There is increased fat deposition in both the superficial and deep layers What To Do About It? A complete evaluation of a patient is essential. Many people think that telephonic and email enquiries about specific procedures are sufficient to plan their surgery. There are so many variables and options worth considering that the value of a full consultation and evaluation with a plastic surgeon should never be underestimated. By investing time and thought into pre-operative assessment and planning, you will definitely achieve a better outcome. Many patients don’t actually know what they need… (or want). There is a spectrum of options to address the midface, lower face, jawline and neck, which include facelifts, mini facelifts, necklifts and ancillary procedures like liposuction. Evaluation: • Quality of the neck skin • Laxity and fullness • The cervicomental angle • Definition of the jaw and chin • Volume of fat and condition of the platysma muscle • Position of the salivary glands

Surgical Options There is no single ‘ideal procedure’ for anyone. The most appropriate approach is chosen and thoroughly discussed with the patient in order to get an idea of a realistic outcome. • Liposuction Standing alone, this is a good procedure as it has minimal incisions. It’s ideally suited for patients with good skin and variable to small amounts of fat – which is mostly accessible in the superficial layers of the neck (the deeper the surgery, the greater the risk of damage to nerves and other structures). • Suture Suspension An interesting and useful option for patients with good skin tone. Through minimal access incisions, the platysma muscle is plicated (secured with sutures) in the midline - and strong suspensory sutures are looped through the muscle. This is then anchored to the strong fascia behind the ears in order to suspend the neck. • Platysmaplasty This is performed through an incision of three-four cm under the chin to visualize the platysma, remove excess superficial and deep fat under vision - and then reshape the appearance of the muscle. The platysma is cut (if necessary) to remove bands and improve its contour - and is then plicated in the midline again. This is a good procedure for patients with platysmal bands and divergence of the muscle in the midline with herniation of fat (which tends to sag). • Retroauricular skin excision This is an option for patients with excess skin, a supple platysma and mobile structures. Through incisions along the hairline behind the ears, the neck skin is mobilized, tightened and sutured in place.

as a facelift of the lower third (which can be done through short, minimal access facelift scars). The facelift accentuates the jawline and removes the jowls - while the cervicoplasty improves the neck skin, platysma and neck structures.

• Ancillary Procedures These are additional components which can added: - Chin implants to improve the underlying bony landmarks - Liposuction of specific, troublesome areas - Salivary gland debulking (a controversial option due to the risk of nerve and vessel damage) - Digastric muscle ‘shaving’ (mentioned earlier) to improve neck and jaw contour - Low density dermal fillers and skin treatments can greatly improve the texture and appearance of neck skin (those creases and lines can be improved in the office… hence, no surgery!) In summary, all carefully planned and executed procedures of the face and neck should achieve good, longstanding results that last for years. In general it takes about 10-14 days to recover from most forms of face and neck surgery. It’s worthwhile mentioning that procedures of this nature doesn’t ‘stop the clock’, so to speak – it simply ‘dials it back’. After all, time and gravity exert inevitable consequences upon our appearance. Pricing It’s pointless elaborating too much about the costs since the procedural options range from the simple to the very elaborate and complicated. The old adage applies: “You need what you need… And you get what you pay for…”

• Cervicoplasty (Full Neck Lift Procedure) This procedure incorporates all the previous options. Through chin and hairline incisions, the neck skin is undermined, the platysma shaped and plicated – and the superficial and deep fat is excised. The digastric muscles can then be ‘shaved’ or debulked, and the entire neck is tightened and held with suspensory sutures and/or skin excisions at the hairline.

Take Home Advice Patients are encouraged to investigate and research their options thoroughly and talk to their plastic surgeons. I can assure everyone that we’d much rather be talking to our patients and evaluating them (it’s what we doctors are trained to do) than answering emails and sms’s.

• Cervicofacial Face and Neck Lift This is the most extensive possible option. It includes all the components of a cervicoplasty (above), and is performed at the same time

The good news is that all the procedures described are completely reproducible and patients often consider further surgery years later to maintain their results.

Bell’s palsy is a form of facial paralysis that results from damage to the facial nerve that controls muscles on one side of the face. Ear-nose-throat surgeon, Dr. Matthew Kircher of Loyola University Medical Center is giving patients Botox® injections to treat facial nerve disorders that sometimes occur after Bell’s palsy. This also includes unwanted facial movements known as synkinesis. Facial synkinesis is the involuntary movement of one set of muscles when the patient tries to move another set of muscles. For example: when the patient blinks, the mouth smiles or grimaces.

Botox® injections work by weakening or paralyzing certain muscles - or by temporarily blocking the nerve input into the muscles. It can also improve the symmetry of the face and reduce muscle contractures and spasms – as well as being an effective treatment for platysmal banding (verticle lines that develop in the neck as a result of muscle contractions). Kircher said that he starts out conservatively, treating patients with dilute doses. After seeing how well the patient does, Kircher then adjusts the dose if necessary. Plastic Surgery: Bizarre & Surprising Procedures of 2013 As with anything in life, there are always people who take things just that little bit too far. We looked back over the past year and found some of the more outlandish cosmetic procedures people are undergoing. Compiled by Mandy Collins. The Facebook Facelift

Palm Surgery

Apparently Facebook profile pics are important enough to warrant a face lift – have you checked yours lately? An increasing number of young people are going for cosmetic surgery so that their profile pictures look perfect, a procedure cosmetic surgeons are calling the Facebook Facelift. One surgeon in India says he sees at least 20 Facebook facelifters a month. Apparently a good image online is linked to successful dating – as well as marriage proposals and professional networking. Facial procedures popular among women are facial skin tightening, brow lift, laser hair removal, de-pigmentation, wrinkle removal, chin fillers, jawline correction, rhinoplasty and lip augmentation. Men prefer laser procedures for scar removal. Psychologists however, have cautioned against what they see as a disturbing trend, because of the focus on the superficial. The hunger for online appreciation also threatens to descend into a negative spiral - with people getting depressed if they don’t get a high enough number of ‘likes’.

The ancient art of palmistry is hugely popular in Japan, where palm readers charge customers over R600 a session to look into the future. Now, for those who don’t like what they hear, there’s a solution – simply have the lines on your hand changed in the hope that your destiny will change with it. The surgery (which has also been known to be carried out in Korea), is performed with an electric scalpel that burns the flesh - leaving a semi-permanent scar. And it doesn’t come cheap – one clinic offered it at approximately R9000 a pop, and now no longer advertises the treatment, because they couldn’t keep up with the demand. Japanese surgeon Dr Matsuoka, who has performed 20 of the operations, told The Daily Beast that a laser wasn’t a suitable tool for the procedure, as it doesn’t leave a clear enough mark. Apparently you need to use an electric scalpel and make a shaky incision on purpose, because palm lines are never completely straight. The procedure usually takes 10 to 15 minutes and can include as many as five and 10 lines on the palm being altered. Generally speaking, men want their financial lines lengthened and women opt for enhancement to their marriage line. Some patients even use a marker to show the surgeon which lines they want extending prior to surgery. It takes around a month for the wounds to heal and for the new palm lines to form.

Dimple Surgery/Dimpleplasty Born without dimples? Never fear! You can simply pay your friendly plastic surgeon to create them. All it involves is a small incision on the inside of the cheek, and a small absorbable stitch that puckers the skin and creates a dimple. But, be warned – when you age, your skin starts to sag and droop, so you might well have a rather strange looking dent in your cheek…

The Joker Smile You’ve always wanted to look like a cartoon character, right? And not just any cartoon character – the Joker! It’s no joke – a photograph has appeared on a social networking site that seems to show a woman who has undergone surgery to lift the corners of her mouth into a permanent smile. The photograph first appeared on Reddit, and hasn’t been authenticated, but the Huffington Post reports that there is a clinic in South Korea offering this procedure. The AONE clinic performs the ‘Smile Lift’ procedure to both lift, and curl drooping mouth corners by manipulating the facial muscles supporting the mouth. The clinic has produced a YouTube video, which explains that some people have sagging mouth corners due to low strength of the muscles that lift the mouth edges - and overly strong jaw muscles that pull the mouth down. In general, it says Western people have longer mouths and higher mouth corners than Korean people. Dr Taek Keun Kwon from AONE told Business Insider that in developing the procedure, some of the problems included a potential for scarring. He said it was very difficult to elevate the mouth corners by just excision and closure of the skin around the mouth corner area. Instead, their technique included weakening the lip depressors to gain unopposed mouth corner lift. Limb Lengthening

Earlobe Surgery

A man in New York has “grown” over 15cm through painful limb-lengthening procedures intended to artificially increase his height. This rare procedure is performed by only a few doctors in the United States and costs roughly R870 000. Painful, timeconsuming, and expensive are just a few words to describe this incredible surgery.

Pinning back “bat ears” has been around for some time, but now there’s additional ear surgery for those who are self-conscious about their drooping earlobes – just have them trimmed away! Earlobes are one of the first body parts to give away our age, and will often lose volume while the rest of the face remains taut. All the surgeons do now is lop off a small wedge of tissue and you’re good to go. But ask yourself – when last did you notice someone’s earlobes. And now that we’ve mentioned it, will you be able to tear your gaze away? Chubby Cheek Surgery/Buccal Fat Removal You know how your grandparents love to grab your chubby cheeks and give them a good pinch? Good news! You can thwart that annoying habit by having some of your cheek fat removed. Surgeons make a small incision inside your mouth, and remove the fat that way. Ironically, the biggest risk is excessive post-op swelling.

Iris Colour Surgery The final frontier of cosmetic surgery was the colour of your eyes. Until now, that is. You really can turn your brown eyes blue with a procedure pioneered by Dr Kenneth Rosenthal – the artificial iris implant. Originally developed to correct various eye defects like heterochromia and ocular albinism, it’s now being used to assist those who want to permanently change the colour of their eyes. The artificial iris is a thin, non-toxic prosthesis made of the same ophthalmicgrade silicone used in intra-ocular lenses. It’s very flexible, so it can be folded and inserted into the eye through a peripheral corneal surgical incision about 2.8 mm long - and then unfolded over the natural iris. According to the website of BrightOcular (the company behind the increasingly popular cosmetic eye surgery), the procedure is “short, safe, and painless,” taking about 15 minutes for each eye. Unlike other laserbased procedures that remove a layer of melanin from the iris in order to permanently change its colour, BrightOcular claims that their iris implant can easily be removed in case of complications... or if the patient so desires.

Turkey’s cosmetic surgeons have branched out into doing hair transplants not only for the top of the head, but also for the upper lip. Men from the Middle East, Europe and Asia are flocking to Istanbul to have their taches tuned. The procedure uses a technique called follicle-hair extraction, in which doctors remove clusters of hair from the more hirsute areas of the body – and implant them along the lip or cheeks to magnify a moustache or beef-up a beard. Performed under local anaesthetic, the surgery takes around five hours and can cost up to R50 000. Tourism agencies have begun offering “transplant packages”, combining facial-hair operations with a shopping vacation in Istanbul - or a beachside retreat on the Mediterranean coast.

Rhinoplasty surgeon, Dr Pieter Swanepoel, recalls examining Ansie during the winter of 2013. He’d formed a valuable first impression of the girl as she walked into his consulting room. She had the constrained bearing of a child attempting to mask the awkward embarrassment of a pronounced nasal hump and beneath it, a receding chin. The ensuing ‘hump removal’ surgery, one of the most common procedures in Rhinoplasty, included ‘tip work’ and a chin augmentation. The result had an instantaneous affect. From the moment Ansie saw the result, says Swanepoel, it transformed her from an awkward teenager into a happy young woman. “I can say that within the space of a few seconds after I showed her the result, something spontaneous occurred,” Swanepoel recalls. “All the earlier, negative feelings vanished. I cannot describe the relief and the joy she and I shared in that moment.” Transformations on this scale are pivotal, so it’s worth stepping back, figuratively, to the actual moment of this revelation... as it occurred in Swanepoel’s rooms.   A Glimpse Behind the Scenes Rhinoplasty surgeon, Pieter Swanepoel, leans forward and gently removes the skin coloured aluminium brace from 17-year-old Ansie Maree’s nose. He leans back, hands her a mirror and waits. She grasps the mirror with both hands and appraises herself, rotating her head side to side. Relief, a great emotional release, washes through the young child. “Wow, oh, wow,” she cries. Tears trickle down her cheeks. Andre Maree, her father, standing off to the left edges closer. Tears glisten in the corners of his eyes. The surgeon too, feels a familiar surge. This, he knows, observing Ansie is cosmetic surgery’s ‘magic moment,’ - a moment he’s often shared as patients first glimpse a nose transformed by cosmetic surgery. It’s the moment when the pendulum magically pauses between heartbeats, between what is old and not yet gone, and what is yet to become. She was 11-years-old. Why do memories of trauma linger? To answer this, we need to understand the affect trauma has on the neural system, and how the system encodes memories of trauma into the ‘wiring’ of the human mind. Conclusion “There are always exceptions,” says Swanepoel. “There is a small category of patients that a surgeon can never please. But, I can tell you this, that the almost instantaneous way a patient’s brain accepts a good result is amazing. In the past, doctors mostly performed voice lift surgery on patients with voice robbing diseases or injury. Now it has become increasingly popular in the cosmetic surgery field - particularly with ageing businessman who want a more booming, authoritive voice.

ccording to the Daily Mail, a number of aging businessmen are opting for “voice-lift” surgeries in order to improve the strength of their voice. Ear, nose and throat surgeons say the operation to restore youthful tones can boost the confidence of business people who believe their authority is eroded as their voice ages. The voice lift, which costs up to R80 000, is expected to become increasingly popular as more people now work well past retirement. The doctors treating them, known as phonosurgeons, claim it reduces the reediness in the voice. John Rubin, a consultant at the Royal National Throat, Nose and Ear Hospital in London, advocates injecting stomach fat into the vocal cords to bulk them up and boost the voice. Some surgeons use collagen or other fillers more commonly used to plump up lips, but these can make the vocal cords too stiff or work only in the short term, Rubin said. “The biomechanical properties of fat aren’t that different from the vocal cords. Fat has high water content, so half of what is injected disappears within 36 hours. The rest stays put and will be effective for up to18 months,” he explained to a Sunday newspaper. Yakubu Karagama, an ear, nose and throat specialist at the Manchester Royal Infirmary, added: “A lot of doctors and patients still don’t know we can do something about ageing voices, but this field is growing rapidly because we now have a variety of innovative techniques to help them.” Meanwhile in the U.S., scientist Robert Langer and Steven Zeitels, the throat surgeon who saved singer Adele’s voice earlier this year, are perfecting a rubbery gel that restores elasticity to aged and damaged vocal cords. Treatments with the gel could be available next year and Dr Zeitels believes it will eventually become commonplace for the elderly to have voice lifts. lmost every female patient who enters my practice for cellulite treatment complains about those dreaded saddlebags – the body fat that sits under their buttocks and hamstrings. What’s worse; no amount of exercise or dieting can eradiate this.

Understanding Saddle Bags Saddlebags are simply a hormone mediated condition. A woman’s body is designed to store Gynoid body fat (which is estrogen dependant) for various reasons. These Are The Characteristics Of Gyneoid Body Fat: • Fully developed by 15 to 16 years of age (Ferin- et al. 1993) • Stored in the hips, thighs and buttocks, and has distinctive visual and tactile qualities • Optimal storage affected by environmental and heritable factors, nutritional status & ability to utilize energetic resources • Rich in long chains of poly-saturated fatty acids (this is important for the development of the fetuses brain – as well as mobilization during lactation (Agostoni- et al . 1999) • Women with a higher ratio of Gyneoid to Android fat (curvaceous women) are considered more fertile, have less risk of CVD and less cortisol reactivity. They’re also in overall better health. Unfortunately in our modern society, Gynoid fat metabolism is disrupted by very dangerous molecules called Xenoestrogens – and these act similarly to our own natural hormone, estrogen. Where Do We Find Xenoestrogens? These foreign estrogens are passed into our environment through pesticides, herbicides, fungicides, plastics, fuels, car exhausts, dry cleaning chemicals, industrial waste, meat from animals (which have been fattened with estrogenic drugs), and countless other household and personal products. The problem is, we’re in contact with most

Cause & Effect There are several mechanisms involved in Xenoestrogen activity in our body. Chronic exposure to these estrogens contribute to estrogen dominance - and whenever we have estrogen dominance, there is a higher activity of beta receptors in Gyneoid fat tissue (which is responsible for vasoconstriction and increased fat deposition). This is what we often see in patients taking contraception pills or HRT. Xenoestrogens have a direct epigenetic effect on an estrogen receptor, and they disrupt endogenous estrogen metabolisms. But one of the main problems is the overburdening of phase 1 and phase 2 liver detoxifications with Xenoestrogens. This has a profound effect on the creation of dangerous estrogens. It’s this overburdening that may also decrease the clearance of Xenoestrogens itself. They are ready to ‘wander around’ and bind to an estrogen receptor. Multiple vitamin deficiencies often found in a typical western diet, vegetarian diet (low protein intake), the use of chronic medication, low or increased stress levels, and affected gut detoxification all add to an altered liver detoxification. One can just imagine that in this situation, even our own estrogen and cortisol cannot be detoxified sufficiently - and thus, an overwhelming amount of Xenoestrogens just worsens the situation.

It’s very important to explain to the patient that there is actually no diet plan or exercise routine (no matter how rigorous) that can reduce the saddlebag condition. Liposuction can temporarily reduce this condition, but it will return as the underlining cause was not treated. What Can Reduce Saddebags? It’s best to consider it as a medical condition rather than an aesthetic problem, as it often correlates with other estrogen dominance medical problems. The patient needs to fill out the toxins questionnaire in order to identify as many xenoestrogens in their environment as possible. Blood tests confirming estrogen dominance and altered liver detoxification should be added. Genetic tests to identify liver detoxification weaknesses can be done for more personalized treatments, especially in patients who are already eating organic food - but have high levels of estrogen dominance (history of sporadic breast cancer, etc). General Tips for Reducing Saddle Bags: • Filter your drinking water and drink from a glass bottle • Eliminate plastics by storing your food in glass jars, cook in stainless steel pots and pans, and unpack your vegetables from the plastic packaging’s • Eat organic food • Reduce medications (contraception pills and conventional HRT) • No spray / fake tans (it’s packed with xenoestrogens) • Drink water every hour (approximately 1.5 litres) • Eat free range steamed protein for every meal (including breakfast) - then fill up on cruciferous and other vegetables. Snack on fruits, nuts and seeds (GMO free, organic) • Use supplements to increase phase 1 and 2 liver detoxification (prescribed by a physician. Mature Women Weigh in on Body Image Satisfaction According to a new study, only 12% of middle-aged women are happy with their bodies. By Belinda Weber

he writer of the 1930s show tune lyrics, “Keep young and beautiful, if you want to be loved” was probably unaware that these sentiments would resonate with so many women. And the line, “It’s your duty to be beautiful,” must seem like manna from heaven to the billion rand advertising and cosmetic industries. So it’s hardly surprising that a new study published in The Journal of Women & Ageing, found the majority of women aged 50 years and older are still not happy with the way they look. But instead of focusing on the negative, this study focuses on the positive: those who are happy with their body size - in an attempt to unlock the secrets of satisfaction. Using a sample of 1,789 American women from the Gender and Body Image study (GABI), researchers discovered that just over 12% of participants reported they were satisfied with their body size. Satisfaction was defined as having a current body size equal to their preferred size. Concerted effort Research shows that women who are happy with their body image are more likely to take regular exercise and less likely to have eating disorders.

However, this may not be something that comes automatically. The researchers say that those midlife women appear to have to make a

concerted effort to both achieve and maintain this satisfaction. But the women who felt good about themselves had lower body mass indices and reported fewer

eating disorder symptoms. They were also more likely to take regular exercise than dissatisfied women. Cynthia Bulik, director of the University of North Carolina’s Center of Excellence for Eating Disorders (and co-author of the study), says this does not mean that they are delighted with every aspect of their appearance. “They are not impervious to dissatisfaction with other aspects of their physical appearance; especially those aspects affected by ageing.” The appearance of their skin caused the most upset, with almost 80% of the women citing this as a cause for concern - followed by their stomachs (56%) and faces (54%). In addition, weight monitoring and appearance-altering behaviors (including cosmetic surgery) were the same between satisfied and dissatisfied groups - showing that weight and shape still play a considerable part in women’s self-evaluation. In actual fact, CFS is an incapacitating disease – significantly impacting on the quality of life of the sufferer. By Dr Simone Silver

What is it? Chronic Fatigue Syndrome (also known as Myalgic Encephalomyelitis) is a complex and debilitating illness. It’s mainly characterised by profound mental or physical fatigue that persists or relapses for at least six months, while being a host to a wide variety of other symptoms. This is a disease that not only significantly impacts on the quality of life and wellbeing of those who suffer from it, but their close friends and families as well. It has emerged as one of the most misunderstood diseases of our times due, in part, to the lack of a clear aetiology (cause) and continued misperceptions. This is based on ignorance by the general public and health care sector. The plight of the patients who suffer from it is only increased by a sense of medical uncertainty and social isolation they endure. While many questions around ME/CFS remain to be answered, much research has been conducted to date– and this can certainly help us better understand this complex illness. It also provides a sense of hope in the way forward for patients. Although classified in 1969 as a neurological illness by the WHO (World Health Organisation), ME/CFS is a multisystem disease- meaning that it affects all organ systems of the body. This includes a number of symptoms such as joint or muscle pain; concentration difficulties; changes in sleep patterns; bowel or bladder problems; sore throat; swollen glands; dizziness; heart palpitations and reduced exercise capacity. Surprisingly, it is currently estimated that up to 17 million people worldwide are affected. Cause: While infectious organisms like viruses (Epstein Barr Virus, Human Herpes 6, Cytomegalovirus), bacteria (Mycoplasma and Chlamydia Pneumonia) and Borrelia Burgdorferi (causative agent of Lyme’s Disease) have been linked to CFS/ME, no studies have proven that one infective agent is present in all patients. Furthermore, it’s still not clear to scientists whether the infection itself or rather, the immune response, is ultimately responsible. Other potential causes may include severe physical or psychological stress, environmental triggers (heavy metals or toxic chemicals) and genetic vulnerabilities. Is It A Real Disease? Before the emergence of significant research, ME/CFS was an illness shrouded in controversy and stigmatization. Many in the medical fraternity doubted its existence - and assumed that patients were either presenting with a form of chronic stress, mood disorder or even feigned illness. We now know that ME/CFS is a real disease characterised by a variety of physical abnormalities in the nervous, immune, hormone and energy transport systems of the body. A number of studies also clearly distinguish ME/CFS from major depression, based on neuroimaging and biochemical studies. While it is not uncommon for patients to experience psychological phenomena such as anxiety and depression, this is more commonly related to the loss of quality of life these patients experience - rather than being a primary psychiatric disease. The disease can affect any individual of any age, race, gender or socio-economic group.

What Actually Goes Wrong? Whilst the exact cause and treatment of CFS/ME seems to evade the scientific community, research is now establishing (with more clarity) the extent and complexity of what actually goes wrong physically in these patients. A wide variety of neurological and immune system parameters are altered. Some research has described disturbances in the body’s delicate hormone system balance (the HypothalamicPituitary-Adrenal Axis). Changes in ion transport and cellular energy production has been documented, which may account for the physical and mental fatigue present in these patients. The ever-expanding field of Psychoneuroimmunology, which focuses on the complex interactions of different organ systems in the body, may well continue to unravel the intricate web of dysfunction seen in many different organ systems in ME/CFS patients. The Medical Puzzle: CFS/ME presents a unique challenge to the doctors who are treating it, because of the diversity of symptoms. Different patients present with different symptoms, in different combinations and with different levels of severity. Since there is currently no single diagnostic test, ME/CFS is a diagnosis based on a patient’s thorough history. This is taken by a medical professional through a series of tests to rule out the presence of other major diseases such as multiple sclerosis, autoimmune diseases, thyroid disorders, muscular dystrophies, iron and other nutrient deficiencies and certain cancers. Treatment: There is currently no FDA-approved drug for the treatment of CFS/ME, although a drug called Ampligen has been under research for some time. The current treatment recommendations (based on the International Consensus Criteria) are targeted toward control of the most debilitating symptoms for each patient (such as pain control and sleep regulation). An individualised and multi-disciplinary treatment approach is essential. Studies have indicated therapeutic value in techniques such as: • Cognitive Behavioural therapy (CBT), which facilitates the coping process for patients and their families living with the limitations of this illness, • Graded Exercise Therapy (GET) which can be supervised by a trained physiotherapist and, • A technique called “Pacing”, which teaches patients how to effectively ‘bank’ their energy stores appropriately to diminish the frequency of illness relapses. A Touch Of Humanity… CFS/ME is a real disease with devastating effects on the lives of the patients who suffer from it. Patients often enjoyed active, successful and enriched lives before becoming afflicted - and the transition into a life of pain and profound fatigue is a hardship few of us can conceive of. The journey ahead should be a partnership between patients and a health care professional team, who can provide answers, support, patience and an openminded approach.

Since your body cannot store alcohol, it must break it down and dispose of it as a waste product. Your liver is in charge of this, and it takes around one hour to break down just one unit of alcohol. While your body is trying to get rid of the alcohol, sugars and fatty acids in your body are not burnt off as quickly, often causing unwanted weight gain. Before You Drink If you are on a healthy eating plan, remember that just ONE unit of alcohol takes the allocations of one of your daily carbohydrates. Treat each alcohol unit as a portion of carbs if you want to enjoy a few drinks and still keep your calories balanced. If you predict a night of heavier drinking, try deficit out carbs on the day of the event, as well as the day after. Take in plenty of water as well, as alcohol is dehydrating and going to zap your body’s water supplies. Healthier Drinking - Sweet and fizzy coolers are the worst offenders, as they are packed with calories and have artificial ingredients and sugar. This will accelerate the effects of alcohol on your system. - White wine and whiskey are the lowest in calories – but calories aside, red wine is probably the healthiest choice to make as it’s packed with immune-boosting antioxidants. Once you’ve taken in more than two glasses however, the health-boosting benefits are zeroed. - Try squeeze in at least one glass of water for every drink you have. Killing two birds with one stone would be drinking a whiskey and water - or vodka, lime and water. Alternatively, try mixing soda into your wine for a wine spritzer. Hangover Cures - Milk thistle is a supplement you should NOT be without when heading out. This handy herb promotes liver function and will help take the edge off a rough night. Take two before you drink, two during (if you are drinking long enough to have a half-way mark!) and two before bed. Milk thistle will help in filtering harmful toxins, leaving you less prone to a hangover the following day. - Honey is great for soothing a hangover! Pop some into rooibos tea, along with fresh lemon juice, and enjoy the benefits it brings. - Protein, caffeine and aspirin are the best-known combination of ingredients to bust a hangover when you’re suffering. Cooked eggs for breakfast are an easy choice, and swap the coffee for green tea - which is a powerhouse of goodness, packed with natural caffeine. - Salicylic acid, the active part of aspirin, occurs naturally in many foods. So skip the headache tablets and rather stock up on apricots, dates, grapes, oranges, pineapple, raisins and raspberries instead.

A woman’s guide to orgasms The female orgasm: we want it, crave it, got to have it. Why? Well… besides the obvious toe-curling pleasure it bestows on us, it’s also a great anti-ageing tool – boosting our immune system, overall health and general well being. Sadly though, many women cannot experience all the joys of the Big ‘O’ – as according to research, a whopping 70% of females find it difficult to reach climax... With this in mind, we ask Clinical Sexologist, Catriona Boffard to discuss the factors that can hinder the elusive orgasm.

A

woman’s orgasm is not quite the same as a man’s – generally we need to ‘learn’ how to climax through experience and touch. We also need sufficient sexual stimulation and foreplay to get us started... and it’s not all about the physical, but the intimate and emotional as well. The Science Behind the ‘O’ We are all after the exciting and pleasurable sensation that we get from climaxing – not only for the blissful state that is leaves us in, but also for our overall health and well-being. Having an orgasm boosts our immune system, makes us happier, improves overall body tone, and promotes good sexual health… so what’s not to love? We orgasm to relax, de-stress, bond with our partners and to fall asleep. A lot of women find that they have no problem reaching climax when they touch themselves - but when it comes to sex, it’s just not as easy. This is most likely due to inadequate foreplay, losing the rhythm - or her partner getting theirs before she’s come close to hers. When a woman touches herself, she knows exactly what and

where to touch – as well as the exact pace that will get her where she wants to be. So when it comes to sex, it’s about utilising her knowledge of her body and how it responds to get her to the ultimate sexual goal. However every woman is different, so it’s important to ask, observe, and even listen to how she responds to touch. Furthermore, other factors which may inhibit orgasm include stress (either at work, with family and especially in your relationship), illness, hormonal imbalance, negative sexual upbringing, education, and even having cold feet during sex! Getting There... As a woman, you can try these seven healthy orgasms tips: 1. E xercise your body: Getting your heart pumping regularly helps with blood flow, thereby boosting your sexual arousal. 2. Exercise your Kegels: Learning to squeeze and release your Kegel muscles makes them stronger and improves blood flow to the vagina. This means better and more intense orgasms!

3. Practice makes perfect: Having sex frequently often means that your brain starts learning how to make things happen faster and more often, and therefore orgasms become easier to reach and with less effort. 4. Pressure is your worst enemy: Feeling pressure to orgasm leads to anxiety and fear and therefore decreases performance. Don’t make orgasm the absolute goal, make it part of the experience. You need to focus on enjoying the moment... rather than crossing the finish line. 5. “ Wetter is better”: My top tip to women is to use lubricant - and generously. As women, when we get turned on we become naturally lubricated, but sometimes a little help goes a long way – which makes climaxing much easier too. If sex if more pleasurable, then orgasm should be more attainable. 6. Quit smoking: Smoking is not only bad for your health, it’s bad for your sex life too! The nicotine in cigarettes affects your capillaries (part of our blood vessels) and therefore means less blood reaches the vagina. This can make both arousal and orgasm more challenging. 7. No Faking: Finally, don’t make a habit of faking: The more you fake it, the more your partner thinks they’re doing the right thing. Talk to them, tell them and guide them on what pushes you over the edge. Knowing your own body and what pleasures you, makes it much easier to tell your partner how to get you to orgasm.
