Dr Sly Nedic MBChB(Bel) Dr Nedic is a founder of the 8th Sense Medi-Spa in Sandton, where she passionately combines aesthetic makeovers with anti-ageing medicine, bio-identical hormone replacement therapy and preventative genetic testing. Dr Riekie Smit MBChB (UFS) MSc Sports Med (Pret) Adv Dip Aesth Med (FPD) Dr Smit works in private practice in Pretoria with a focus on aesthetic and anti-ageing treatments. Her major focus is skin rejuvenation procedures with peelings, mesotherapy, lasers and facial injectables. She is also a lecturer and trainer for educational institutes and a faculty member of the American Academy of Aesthetic Medicine. Dr Smit is the current president of the Aesthetic and Anti-ageing Medicine of South Africa where she and the EXCO members work towards a better regulated and growing industry of aesthetic medicine. She loves to draw and write where she can integrate her art into scientific and public articles and literature.

Dr Willem Erasmus MBChB FCS plas surg Dr Willem Erasmus is a specialist plastic and reconstructive surgeon at the Renaissance Body Science Institute in Cape Town. When he’s not consulting at the practice, he can be found at Worcester – as well as at State hospitals where he performs pro bono surgeries. He is also deeply involved in Operation Smile, repairing cleft lips in children.

Dr Janine Olivier MBChB Dr Olivier obtained her MBChB in 2006, and is currently enrolled in a Masters of Medicine course through the University of Pretoria. She has participated in several Botox®, filler and anti-ageing workshops and has received formal training courses with Genop, the Bay Skin Institute and Allergan. Dr. Oliver’s initial training was obtained under the guidance of Dr K Semprini – and her field of practice in aesthetics is situated in Pretoria and Parys (Free State). She specialised in internal medicine until October 2013 at the University of Pretoria - whereafter her focus shifted to holistic and aesthetic medicine.

Dr Vernon Ching MBBCh (Wits) FCS (SA) Dr. Ching is a plastic and reconstructive surgeon based at the Netcare Waterfall City Hospital in Midrand. This hospital is home to the internationally accredited Centre of Excellence for Metabolic Medicine and Surgery, led by Professor Tess van der Merwe (Director of the Bariatric Centers of South Africa) and is considered to be the flagship model for Bariatric and Metabolic surgery in South Africa. He has a special interest in the management of patients after massive weight loss and is a member of the dedicated multidisciplinary bariatric team. Dr Loredana Nigro MBBch Dr Loredana Nigro graduated from WITS medical school. Having worked extensively in internal medicine, she left Charlotte Maxeke Academic Hospital in 2010 to pursue a career in Aesthetic Medicine. Particular areas of interest include: laser skin resurfacing, Botox, fillers and integrated anti-ageing medicine. Dr Nigro currently heads up Laserderm’s new Parkhurst branch.

Dr Claire Jacobsohn MBBch Dr Claire Jacobsohn is a full-time medical practitioner specializing in aesthetics for the Lightsculpt Clinic in Bedfordview. After completing her MBBch at Wits University, Dr Jacobsohn worked in the fields of forensics, trauma medicine and general private practice. She developed a keen interest in aesthetic medicine in 2005, and has since been dedicated to anti-ageing and beautification. Dr Jacobsohn has completed numerous advanced Botox and dermal filler accreditation with multiple institutions across a range of products – and also has a keen interest in mesotherapy, PRP, sclerotherapy and weight loss.

News Snippets From Around The Globe Apple Store Pulls ‘Barbie’ Plastic Surgery App Apple’s App Store pulled a Barbie-inspired plastic-surgery app in January, following intense backlash fueled in part by a Twitter campaign. The game was rated for children 9 years and older – and walked players through the graphic steps of liposuction that must be performed on an “unfortunate girl” to make her “slim and beautiful.” The dubious steps included injecting anesthetic, making incisions with a scalpel and suctioning out fat with a pump. This process was then repeated multiple times on Barbie’s different “problem areas.” The game’s description in the App Store was also highly questionable: “This unfortunate girl has so much extra weight that no diet can help her. In our clinic she can go through a surgery called liposuction that will make her slim and beautiful. We’ll need to make small cuts on problem areas and suck out the extra fat. Will you operate on her, doctor?” The app sparked outrage across the Internet, but it was the Everyday Sexism Project that started a Twitter campaign to have iTunes remove the app. In multiple tweets (which were quickly re-tweeted and shared hundreds of times), the group asked iTunes to “reconsider marketing this ‘game’ to players aged 9 and over.” The app was pulled just a few hours later. ‘Round-Head’ Plastic Surgery A lot of South Korean men and women are turning to “roundhead” plastic surgery, a cosmetic procedure geared toward creating a more attractive head. But why? According to Kotaku.com, surgeons said ridicule is to blame for the rise in those patients going under the knife. While some procedures involve lifting and plumping skin on your face, others are about helping people have a rounder-shaped head. South Korea is famous for its weird cosmetic surgeries, especially one clinic in Seoul. The establishment said they offer surgery for those “ridiculed” for having a flat or sunken back of the head.

Their “round-head” procedure takes under an hour and involves a small incision. Surgeons use a type of bone cement during the process, attaching it to the skull, and adding 20 to 80 grams onto the patient’s head. This technique isn’t new or mainstream, but has gained some steam in South Korea in recent years. But now, having work done on your head is gaining criticism. Recently, one website called the surgery fixing “abnormally flat heads”a bit much.” Some patients may seek out the option after being diagnosed with a condition called “plagiocephaly” or “flat head syndrome,” which is corrected by wearing a helmet for several months as an infant. Woman’s Nose and Lip Gnawed off by Mice is Offered Free Plastic Surgery A young woman whose nose and lip were gnawed off in a sickening attack as a baby is to finally receive free plastic surgery. Zhu Sanni, 23, from Qingdao, Shandong Province, sustained the life-changing injuries whilst left at home alone by her parents for three days. She had to fend for herself when the mice began nibbling at her face and was only discovered when her face was covered in blood. Her mother had left her daughter whilst she went out to find food, while Sanni’s father, who suffered from a mental illness, had disappeared from the family home. Unable to afford to pay for hospital treatment upon finding her, her parents simply washed her wounds. Yet now Qingdao Boshi Hospital has offered to provide free surgery to reconstruct her features. Director Wu Yuhua, from the medical affairs office, said that they will build Sanni an artificial nose and lip, however the entire procedure could take up to two years. Source: www.mirror.co.uk

Surgeons Cut 25kg Tumor from Woman’s Womb Surgeons in Spain successfully removed a giant benign tumour weighing 25 kilograms from the womb of a 47-year-old woman. The head of obstetrics and gynaecology at Torrevieja Hospital in eastern Spain, Eduardo Cazorla, said that in his 20-year career he had “never treated such a case”, the hospital said in a statement. It said that scientific publications recorded only about five cases of such giant tumours defined as bigger than 11.3 kilograms - in the world. The woman was referred to the hospital by her family doctor after complaining of discomfort and weight gain in her torso. This causes continuous expansion of the breast after surgery – often leading to extreme breast sizes. The implants have led to some of the largest breasts ever - but the procedure is rarely seen outside of the adult film entertainment industry due to the risks concerned. Avocados Aid in Weight-Loss

Fresh avocados have 3 grams of total carbohydrate, less than 1 gram of natural sugar per one ounce serving (the least amount of sugar per serving than any other fresh fruit) - and contribute 8% of the daily value (DV) for fiber. Each serving of nutrient dense fresh avocado is also a source of naturally good fats. Previously, a number of studies have suggested that avocado consumption may reduce a person’s risk factors for heart disease and diabetes, while supporting weight management goals. Joan Sabate, from Loma Linda University (California, USA), and colleagues asked 26 healthy but overweight adults to incorporate fresh avocado into a lunch – either by replacing other foods or by simply adding it to the meal. The team found that those subjects who added half of a fresh avocado to their lunch reported a significantly decreased desire to eat by 40% over a three-hour period – as well as by 20% over a five-hour period after the meal. This is compared to their desired CE after a standard lunch without avocado. Additionally, the subject reported increased feelings of satisfaction by 26% over the three hours following the meal. The study authors write that: “The addition of approximately one half of a Hass avocado at a lunch meal can influence post-ingestive satiety over a subsequent 3 and 5 hour period in overweight adults.”

New Botox Study

An American Academy of Cosmetic Surgery news brief has reported that a Houston oculoplastic surgeon (eye specialist), Dr. Charles Soparkar, has come up with a way to make Botox more effective and last longer. In a modified double blind, randomized study (and a subsequently completed formal study), the duration of effect was increased by 30% in over 90% of the patients studied. The study included both cosmetic and medical indications. The theory goes like this: Botox is very dependent upon zinc for effectiveness. Unfortunately, 50% of the US population is zinc deficient. Apparently, dietary zinc can be poorly absorbed due to binding elements found in many foods called phytates. Dr. Soparkar’s group suggested that adding zinc to the diet of patients having a weak response or short duration of effect to Botox may be beneficial. Additionally, a phytate munching phytase enzyme was added to increase zinc bioavailability.

The result is a prescription medication called Zytaze. Each capsule contains 25mg of highly bio-available zinc citrate and 1500 mg of Phytase. Patients take 2 caps daily for four days prior to AND on the day of injection. The drug is supplied in a 10 cap pack and costs around R600-R800. The result has been less frequent and decreased amounts of toxin needed and potential patient savings. (However, does the cost of the drug nullify these advantages?) This is great news for patients and a potential niche offering in the commoditized botulinum marketplace. Maybe it’s not so great for practices depending on a high turnover of renewable products and services though.

According to new research, people who exercised regularly were found to be more confident in handling the interaction of their work and home life - and less likely to be stressed at work.

Previous studies have shown that exercise helps to reduce stress. A previous study examined Tai Chi exercise programs over 12 weeks. Another study looked at high-intensity aerobic exercise. Both showed reductions of selfreported stress. What researchers didn’t know is if the reduction of stress actually helped empower individuals to feel they had better work-life balance.

First Topical Gel to Enter Clinical Trials for “Double Chin” Topokine Therapeutics (a biotechnology company that develops topical medications for non-invasive contouring of the face and body) is currently in Phase 2 of a clinical trial for a topical gel that can reduce excess sumental fat (commonly known as ‘double chin’). “Patients are increasingly interested in non-invasive therapies to rejuvenate the face,” said Dr Jeffrey S. Dover, a nationally recognized dermatologist and member of the Topokine’s Scientific Board of Advisors. “If approved, XAF5 Gel would provide a convenient topical medication that reduces a double chin”. The Phase 2 study follows positive data from a 6-week Phase 1 clinical trial completed in April 2013. In that study, 32 healthy men and women applied XAF5 Gel or placebo once a night to a 100 cm2 area of abdominal skin. The study met its safety and tolerability endpoint at all doses. The only side effects were mild, temporary skin redness or itch, which occurred at similar rates with placebo. “We are excited to advance our XAF5 Gel program into Phase 2,” said Topokine’s Chief Scientific Officer, Michael Singer, MD, PhD. “Our research shows that patients would prefer a topical fat-reducing medication over therapies that cut or puncture the skin which are the only treatments currently available.

The Early Botox catches the wrinkle A look into Botox and filler use in patients aged 25-30 years… ®

I

have had quite a few lovely young ladies requiring Botox®. Mostly it’s wrinkle-free brides with rather small orbs that would like to have that ‘Eyes Wide Open’ fresh look on their wedding photos. Because not all of us are supermodels, a lovely (plain) girl has to take advantage of all the tools available to her in these modern times. There is nothing like an elegantly raised supra-orbital arch and a little plump pout to transform the lowliest Cinderella into the Belle of the Ball. But is it safe to use these products so early? And what are the long term effects thereof? After pondering these perplexing questions, I realized there are three possible problems that physicians specializing in aesthetics needs to consider when seeing a young(er) Botox®/filler

Creators The Creators are those clients who want volume where perhaps they never had volume before. They not only want to look young (not younger) but different.

Financial Issues Psychological Issues There are typically two types of clients: Restorers and Creators - and now with younger users, a third group I like to call: The Preventers. Restorers The Restorers like to cheat the old ageing by injecting just enough product to erase some of the tell-tale signs. They get rid of most wrinkles and restore lost volume in order to look naturally younger. This is mainly my target market, these ladies (or gents) often have realistic expectations regarding outcome and does not like drastic change.

Creators want a straight nose nonsurgically, different shaped lips, maybe a stronger jawbone or a longer, fuller chin. They also want no wrinkles at all – not even those tiny crinkles a two-year-old has when having a belly laugh or throwing a temper tantrum. This group experiments easily, but they have higher expectations regarding outcome - and often an unrealistic image of self and product. But don’t get me wrong, we LOVE these patients. Aesthetic doctors can be creative and sculpt a lovely malleable product to their heart’s desire in copious amounts. But it’s important to screen these patients

quality and make is excellent, then there should be no additional risk in a younger patient.

Creators need to be aware of the limitations of soft tissue augmentation. They need to be referred (if necessary) for counseling or plastic surgery if a realistic defect cannot be properly addressed with filler. Unfortunately most Preventors morph into Creators.

Because the patient has developed fully between the ages of 25-30, they are already in an ageing phase (although not physically visible now). They have exactly the same risk profile as another adult, and often less co-morbid conditions or medication use. This actually seems to decrease the physical risk in this age group.

Preventors Our lovely fresh faced youth. These creatures who want us to stick a needle into their unscathed faces to prevent that dreaded wrinkle to form. Mostly these ladies (girls, rarely boys) have seen an elderly relative (granny that used ‘boersoap’ for 90 years) that scared the living daylights out of them and think that they are genetically inclined to a boerseep face. Although skin quality is certainly genetic, to a large degree proper care and sun protection - as well as appropriate Botox® and filler when the time is right - will give you a good chance to beat the genetic odds. Some families have hyper-dynamic foreheads and intense facial expressions that will form unsightly lines early in life. My sister for instance, who is 27-years-old had horrible glabellar lines - even when she was not frowning (caused by all the prior frowning in her legal studies). In order to look less angry at her clients (or unintentionally animated), we had to resort to 4 monthly Botox® treatments from that early age. Now, the problem with young clients receiving regular augmentation is that they do not age naturally, and thus may have unrealistic expectations later. They tend to aim for ‘plastic perfection’… which we know is far from perfect... Physical Issues If the Botox® and filler is administered by a qualified medical doctor and the product

Financial Issues Unfortunately soft tissue augmentation can become costly and once started, is difficult to stop (akin to stopping waxing...doing roots etc.) Many young Preventors do not have financial stability so can cause a financial drain on a partner or family member. The danger to morph into Creators without the necessary funds available is a tangible one - and can lead to depression and anxiety later in life as one struggles to uphold an expectation. To Do or Not to Do? If you have terribly small lips or a nasty hole in your face caused by an overzealous GP who drained a ‘pitsweer’ when you were 13 – or you have acne scars because no one gave you Roacutane way back when - then there are solutions… no matter what your age. If you are a lovely (but plain) girl who wants to look extra special on your wedding day, soft tissue augmentation is a good option. If you have a hyper-dynamic face and often make inappropriate lines and expressions, then we must help you. However, if you are unhappy with yourself and cannot pinpoint the exact problem that makes you sad and anxious, then Botox® will not relax that feeling and fillers will not fill that hole. The results of the study showed that aesthetic intervention shaved a few years off perceived age - but did almost nothing to boost patients’ overall attractiveness. Unless it’s done intentionally, aesthetic makeovers don’t necessarily beautify the face - but rather deliver a younger look. The best motivation for aesthetic procedure is the knowledge that after a non-invasive and almost non-downtime procedure, the patient will look younger and fresher. Unfortunately though, some of the patients have unrealistic expectations as they expect to be molded into ‘movie star gorgeous’… which is not entirely possible.

Patient Type Generally there are two types of patients. One who says: “I am happy no-one really noticed anything major (except perhaps for their fresher appearance) - and the one who says: “No one noticed anything (meaning the patient was expecting to look more attractive so everyone will notice).

Divine Proportions Facial beauty is based on what is known as divine proportions. Golden or divine proportion is a mathematical constant (Phi1,618) that for some reason delivers proportions which pleases the human eye. This has been studied and used by artists and mathematicians since the Renaissance period. Even Leonardo Da Vinci employed it to measure and create beautifully proportioned faces in his paintings. Divine proportions show that beauty is timeless. In a recent website survey, the face that was voted the most beautiful by over a 10 000 voters had the same divine measurements as those of Nefertiti, dating back to 1350 BC. In modern society Angelina Jolie’s face fits into golden proportions. With this in mind, we try to follow this golden rule of proportion when we do aesthetic facial makeovers by using our artistic side. Symmetry Symmetry of the face is a visual sign which plays an important part in facial attractiveness. During an aesthetic consultation, symmetry concerns are always pointed out, and it’s

important to look carefully at this aspect for an aesthetic makeover. Extensive asymmetry should be corrected, as well as the one which is necessary to be corrected for beatification of the face. However, a small degree of facial asymmetry is actually desirable. When we use Photoshop to duplicate one side of a famously beautiful face – such as Marilyn Monroe – the uniqueness and beauty is somehow lost. So, we need this minimal asymmetry between the left and right side to create a unique and beautiful look. Beautiful Features? Many cross-cultural studies have shown that there is some agreement between nations and races over the features which constitute a beautiful female face. These features being: • Highly arched eyebrows • Almond-shaped eyes • Dilated pupils with dark circles around the irises • Enhanced cheek bones • Fuller lips • A wide smile • A long, thin, straight nose and small nostrils

Many of these facial features are easy to modify with aesthetic procedures (such as Botox and filler injections) - and this needs to be discussed and offered by an aesthetic physician. However, it’s also important to note that age plays a key factor. For example; fuller lips and highly arched eyebrows is not something which a women in her 50’s will typically ask for (as she worries more about sagging skin and unwanted jowls). Therefore it’s the duty of an aesthetic physician to explain how beautification can be achieved, while making the face appear fresher and more youthful. Prominent, broad cheekbones are fast becoming a new trend in aesthetic medicine (even in younger women), and are considered beautiful. This is why volume restoration and cheekbone augmentation are currently popular procedures in the 3D restoration of ageing facial features. They’re thought to add youthfulness to the look and beautification. As For the Men… When it comes to aesthetically pleasing facial features in the male species, the following traits are seen as desirable:

• Broad cheekbones • A relatively longer lower face • Well developed brows, and • Chiseled jaw lines. These features which differ between the sexes are known as sexual dimorphisms, and correlate with perceptions such as developmental stability, genetic health and a strong possibility to be successful. Men who have mid-cheek sagging with a prominent grove in that area are definitely seen as less attractive. So mid-cheek augmentation also plays a key role in the beautification of the male face. In closing, the duty of the aesthetic practitioner is to follow the recent trends in what is considered beautiful; to use divine proportions in making a face more attractive; to have a fine eye for beauty (as well as an artistic hand), and to use scientifically proven methods. ear on year, there is one thing that everyone in the aesthetics industry is sure of… that the market will advance faster than one can imagine. If we look back to 2012, we would’ve never dreamt that preventative medicine would become such a buzzword. Similarly in 2013, skin rejuvenation and facial volumising became possible with the advancement of both technology and technique. Through all the changes however, there have been stalwarts in the industry - and they continue to dominate the global popularity numbers. Botulinum Toxin is by far the most requested treatment globally, because with technique advancement, more and more treatment options became available using this toxin. Similarly, dermal fillers have advanced in leaps and bounds, as it offers all forms of anti-ageing treatments - from rejuvenation to non-surgical facelifts. These two treatments will dominate in 2014 in South Africa and also globally yet how they are used may differ. What are my trend predictions for 2014? First and foremost I believe there will be a continuation of a trend that began toward the end of 2013. And that is a move toward less invasive procedures. Non-surgical aesthetics offer the patients results that closely mimic surgery results, but without the downtime, recovery or price tag. Of course the results are not permanent, but for many patients this is seen as a positive as they can change their treatments as they age.

Volumising Techniques As a technique, volumising provides patients an ability to correct deformities or asymmetries - and in 2013 the cheek augmentation and nose augmentation became popular. In 2014 this trend will extend to include the chin and jawline. By the end of 2014 most trained physicians will be able to completely recreate youthful contours using dermal fillers alone. Skin Re-Surfacing Skin re-surfacing in 2012 meant single treatments of aggressive chemical peels. In 2013 this protocol included dermal fillers to add hydration and suppleness to the skin. In 2014 the advancements of the Dermapen and Vampire facial will mean that skin re-surfacing should become part of everyone’s treatment protocol. Treatments will offer significant results, and even more so when combined. Skin re-surfacing will become the starting point for all treatments in 2014, or at least they should be.

Scientists have been studying the longterm effect of caffeine in the skin, and what improvement can be expected through topical application. We have known that caffeine in moderation has fantastic anti-oxidant properties and can act as an antiinflammatory. As an added bonus, caffeine in the bloodstream causes vaso-constriction (making blood vessels smaller). What does this mean? These three properties make caffeine an obvious choice as an ingredient for topical eye creams. Eye rejuvenation is a popular but difficult treatment due the fragile nature of the skin in the area. The introduction of caffeine will reduce puffiness and redness. It must be made clear that no long-term studies have been completed yet.

Another trend I see continuing is one towards smaller treatments more regularly. What we have learned over the last few years is that we can achieve very natural results - with minimal social downtime by treating patients monthly with minor procedures - rather than yearly with aggressive treatments.

Brand New Trends for 2014 Of course these above-mentioned trends are simply tweaks and advancements on established treatments. So what is going to take 2014 by storm? Here’s my opinion, but remember, you heard it here first!

In addition, caffeine has been declassified as a diuretic and the anti-oxidant properties indicate that as an ingredient, caffeine may combat photo-ageing. Caffeine can cause damaged skin cells to die, therefore they cannot divide to form skin cancer, or other skin problems (including wrinkles). This means that you might find caffeine included in your sunscreens in the near future. Caffeine as a trend? Who would’ve thought?!

What will be the treatment trends for 2014? Botulinum Toxin One cannot call it a trend any longer due to global popularity, but Botulinum toxin treatments will continue to “trend” for very obvious reasons. There is no patient that would not benefit from a Botulinum toxin treatment. Advancements in technique will see

The Twists & Turns of

Varicose Veins

D

isfiguring means, “To mar the appearance or beauty of….” As people grow older they often notice unattractive veins appearing anywhere on their body - but most often on their legs and face. Slimmer people will also often complain of “ugly veins bulging or standing out on my arms, hands and feet”. The treatment of all these has brought about the medical speciality called “Phlebology”. Phlebology is the medical specialty devoted to the diagnosis and treatment of vein disorders. A medical specialist in Phlebology is termed a Phlebologist. The American Medical Association added Phlebology to their list of practice specialties in 2005. In 2007, the American Board of Phlebology (ABPh), now known as the American Board of Venous & Lymphatic Medicine (ABVLM), was established to improve the standards of phlebologists, as well as the quality of their patient care. For the patient or aesthetic client, it’s very important to realize that modern technology now requires that the physician treating these veins be an experienced and up to date “Phlebologist”, with the skills required thereof. On the legs, veins requiring treatment are commonly named as: • Spider veins (also known as Thread veins) • Reticular veins - these are blue green veins that have become unusually visible, and • Varicose veins - these bulge above the surface of the skin.

Cause & Effect The veins of the legs are intended to transport blood back from your legs to your heart. Since gravity works against this flow, nature has put one way valves inside the veins – which close if gravity starts sucking the blood backwards. Disfiguring veins appear due to this gravitational downward pressure, eventually producing leaking valves. A combination of health, habits and genetics will determine ones vulnerability, but eventually almost everyone will have some disfiguring veins. The veins below a leaking valve are likely to stretch and develop into tiny spider veins. A vein that has stretched enough to visibly bulge is termed a varicose vein. A varicose vein is most likely caused by some of the bigger vein valves leaking. In determining appropriate treatment, it is essential to understand the anatomy of the veins in the legs – the deep veins, surface veins and those connecting these two systems, the perforating veins. There are hundreds of valves involved and Phlebology requires that the doctor use duplex ultrasound technology to

determine the exact reasons for the appearance of disfiguring veins. This is termed Venous Mapping. In some early minor cases, the use of a hand held Doppler may suffice to exclude the leaking of the major valves. Treatment Options The current procedures for treating veins are: • Surgical Compression Stockings (not aesthetic) • Laser Therapy • Sclerotherapy • Surgical Avulsion (also called Phlebectomy) • Surgical Ligation and/or Stripping • Endovenous Laser Ablation • Endovenous Radio Frequency Ablation • Endovenous Steam Ablation (under development) • Cyanoacrylate Glue Ablation (under development) A Phlebology practise will provide, or have access to, experienced and competent laser therapy, sclerotherapy, surgery and one of the endovenous ablation procedures. Laser therapy, using the Nd:YAG 1064nm (although available on just about every street corner), requires careful case selection - and should still be restricted to only spider veins. Recent developments suggest that the bigger reticular veins may become agreeable to laser treatment in the future. Sclerotherapy; (which is the injection of a solution which stimulates the vein to grow closed), may be used on all forms of the disfiguring veins - but also requires ultrasound guidance for injecting relevant hidden veins that are spilling blood backwards into surface veins. Endovenous ablation technology has revolutionised the surgical treatment of varicose veins. Endovenous means from within the vein, and ablation means removing something by means of heat. Essentially, a laser or radio frequency catheter wire is threaded into the varicose vein through a tiny needle puncture under local anaesthetic (called tumescent anaesthesia), and when activated, it heats up, thus sealing or cauterizing the vein closed. The procedure does not require a general anaesthetic, unless the patient so chooses, and is therefore done out of hospital and in an appropriately fitted surgeon’s room. Surgical Stripping should now be reserved for the odd case where the varicose vein twists and turns too sharply for the catheter to be placed correctly to achieve a satisfactory closure. Spider veins of the face are very successfully treated by skilled laser therapy. Slightly larger blue veins around the eyes will do well with sclerotherapy, but it has the potential for very serious complications. Therefore, it needs a conservative approach from a very experienced and skilled Phlebologist. Sclerotherapy may also be used for the aesthetically ugly veins on the back of the hand, but each case must be assessed very carefully to avoid serious complications. In essence, the modern field of Phlebology now provides the assurance that almost every patient can have their disfiguring veins treated and thus feel confident again in a costume, shorts or skirt.

They say the truth is often stranger than fiction – this is true for anti-ageing treatments too. We’ve rounded up some of the world’s odder beauty treatments: which of these would you try? Compiled by Mandy Collins. Solid Gold / Bee Venom Facial Bling’s the thing these days, so why not add a little to your beauty routine? All you need is to move to Florida and come up with the cash for a 24-carat gold facial. But that’s not all – some salons thrown in a little bee venom for added benefits. Apparently the treatment offers a “complete skin rejuvenation treatment to smooth, ‘freeze’, lift, plump and brighten the complexion” using a special bee venom cream created by cult beauty brand, Rodial, which has been hailed as nature’s Botox®. That and solid gold, of course. Allegedly, gold has been used in skincare and healing for centuries. Cleopatra slept with gold on her face and ancient Ayurvedic treatments have always used gold. This treatment features several steps: first: a skin-smoothing micro-dermabrasion. Then, the bee venom and gold serum is slavered onto the face, and then it’s blasted with pure oxygen to help it sink deep into the epidermis. After that, the face is covered with a layer of 24-carat gold leaf and a mask made from gold, hyaluronic acid and collagen. The skin is then massaged and bathed in a special light to stimulate collagen production. Finally, a massage is also performed on the rest of the body using Rodial’s 24-carat gold body cream.

It’s Better with Butter

Betty bought a bit of butter, but it wasn’t for her butt… Unless she lived in Ethiopia, where a butter massage is the beauty treatment to have for women hoping to give their lady parts a ‘makeover’. The women are massaged in butter from scalp to toe, and butter is applied everywhere on the body (inside and out). The women then sit above a smoke hole in a gymnastic-like pose until the butter melts completely. This bizarre treatment apparently tightens vaginal muscles post-pregnancy.

In A Pig’s Eye, Erm, Foot

This is not likely to take off in any country where eating pork is verboten. The practice started in Japan but has since travelled west to New York. So all you need to do is eat pig’s feet. They’re rich in collagen, and therefore (they reason), will help get rid of wrinkles.

No, it’s not. You can now book yourself in for a facial – but for your other end...your derriere. Only in America. The treatment is designed to give your bum a little TLC, and claims to tone, deblemish and massage your poor, sat-upon rear end.

Bull Sperm Hair Care This treatment also requires you to not think too much about what you’re doing to yourself. This treatment has been dubbed the ‘Viagra for hair’, and no, it’s not designed to make your hair stand on end. It’s a hair conditioning treatment that’s derived from the sperm of a bull, and combined with protein-rich katera root. It promises to help repair broken hair and give it a silky, smooth look...

You can’t breastfeed in public without causing a stir, but how about washing your hands in breast milk? A move away from the chemicals in soaps, means that some people are now using excess breast milk to wash their hands, working on the principle that milk nourishes the epidermis.

Snake Venom Cream

Allegedly, celebrities such as Gwyneth Paltrow swears by snake venom facials and cream. Apparently, using a cream which has venom as an active ingredient helps plump up the skin in much the same way that Botox® does.

The Caviar Facial If gold is too rich for your taste, how about caviar? And no, you don’t eat it to reap its benefits – you smear it on your face. At a day spa in Chicago they think caviar facials are better than Botox® – that that it repairs sun damage and minimises wrinkles.

This one’s likely to pique the interest of more than a few beer-swilling louts – don’t just down an ice-cold beer at the end of the day, climb into a bath of the stuff! In West Bohemia, Czech Republic, they believe that beer has super healing powers, and spas around the country offer relaxing beer baths. Beer is rich in Vitamin B, which has skin boosting properties, but we’re not sure that the claim of helping with high blood pressure is more than skin deep. Not to be outdone, a spa in Japan called Yunessan boasts a giant bath full of sake, green tea, coffee and even ramen noodles.

Vagina Brightener

First there was anal bleaching, now you can lighten your vagina. In Thailand, Lactacyd White Intimate Wash promises to make your privates “safely fairer within four weeks”. According to them, sweat and excessive friction from tight clothing can darken the skin around the intimate area, causing self-consciousness, decreased confidence or inhibiting intimacy.

And when you’ve finished lightening it in Thailand, you can pop over to India to have it tightened. Not surgically – through a rejuvenating cream called 18 Again. It claims to have clinical trials evidence of its efficacy, and the cream has been advertised to women Bollywood style, through the medium of song and dance.

And finally, we once again go to Japan where the beautiful Geisha ladies, prefer facial treatments that use sterilised nightingale guano. And now the idea is making its way to the upmarket salons of the west. The treatment promises to brighten your complexion and contains enzymes that act as an effective skin cleanser. So, next time you’re sitting under a tree and you hear a bird – look up. You might be lucky and get yourself a free skin treatment.

double chins, the likelihood is that you will too. •A ge – as we age, our skin loses elasticity and we are inclined to store fat. If your body happens to store fat in the chin area, then only exercise and a healthy lifestyle could reduce the chances of developing a double chin. •E xcessive weight – this is obvious. It’s common amongst overweight individuals to present with a double chin. What are your treatment options? Your treatment options depend largely on you. For example: if you are young, fit and healthy and present with a double chin, then simple liposuction will suffice. At my practice, I prefer (a laser assisted liposuction). This

offers less downtime and can be performed within the rooms. With all cases where SlimLipo is used, the incision is hidden beneath the chin and the excess fat is removed via a cannula. Other treatment options include combination treatments. In older patients, a double chin SlimLipo and neck lift combines well to reduce visible age, while reshaping the jaw line. On rare occasions, a double chin that is the result of an undefined chin can be corrected with the addition of an implant. This will reshape the jawline and create a prominent chin. It also reduces the risk of a double chin returning. Unfortunately there is nothing that can be done non-surgically

to reduce your double chin. Aesthetic treatments cannot remove fat from the chin (or anywhere on the body), and this being the primary concern should always addressed as priority. Dr Deon Weyers – Plastic Surgeon, Medgate In my opinion a double chin needs to be addressed in terms of fat location. Age and skin condition do play a crucial role in where the fat is stored in the chin. The double chin fat is either superficial (near the surface of the skin) or deep below the delicate strap muscles of the neck. Depending on this, I can recommend the correct treatment approach. In most cases of “exercise and diet resistant fat” that causes a double chin, then Vaser Liposuction of the superficial submental fat would achieve fantastic results. This is a simple surgery performed within the rooms, whereby a metal cannula is inserted beneath the chin – removing the superficial fat and sculpting the jawline. The results are spectacular and recovery is quick and easy. Liposuction alone is not always the answer However, it’s important to understand that not all fat can be suctioned away, and other treatments may be necessary to achieve the desired result. For example, if the underlying cause of your double chin is age or obesity, then liposuction alone will not give satisfactory results. This is due to the deeper lying fat, so liposuction will not address the underlying issues causing the double chin. In these cases, neck rejuvenation treatments will need to be performed in conjunction with Vaser Liposuction for optimal results. The type of neck rejuvenation will depend on: • Skin quality • Age • Weight • Desired results A surgical neck lift will address the turkey wattle often associated with premature ageing of the neck, and will reshape the jowl and jawline. In other cases, a small incision beneath the jaw will allow for surgical removal of the deeper subplatysmal (deep fat compartment) fat – and offer more suitable results. The best for you In many cases a simple Vaser Liposuction will do. emale sexual health is rarely discussed in the doctor’s office. There seems little point to engage your female patients in a discussion for a problem which, as a doctor, you have no real solution. Likewise, there seems to be little reason for a woman to bring up these matters with her physician when previously, all that was on offer is hormone replacement - or perhaps psychosocial support by means of a self-help book or a therapist. The problem of sexual arousal and difficulty achieving orgasm is probably massively under-reported. The O-shot may be exactly what women need to give them renewed confidence in their sex lives. Platelet Rich Plasma, (also known as PRP therapy) involves drawing your own blood and harvesting the platelets for reinjection – in this case, for injection into key areas for sexual stimulation in the female anatomy. How is the procedure performed? At the doctor’s consulting rooms you will have 2 tubes of blood drawn, usually from your arm, like at a pathology laboratory. Approximately 16ml of blood will be taken in total from you. The blood is then placed into a centrifuge in the doctor’s office, which will spin and separate the platelet poor plasma, the platelet rich plasma (PRP) and the red blood cells from one another. This process will take approximately 20 minutes. The PRP is then activated to release growth factors, and will be re-injected into the target area (in this case, the clitoris and the notorious “G-spot”). With the PRP injected, growth factors stimulate the regeneration and rejuvenation of healthy vaginal tissue as the stem cells grow back younger tissue in the treated areas. With PRP, all of the growth factors released are from your very own platelets – and hence risks such as allergy to the product are unlikely to occur. Sounds painful? PRP can sting and create sensitivity on injection, regardless of the area to be treated. To reduce any discomfort that could be experienced during your O-shot - local anaesthetic cream will be applied 45 minutes before the time, to make sure the injections are minimally painful. Your treating doctor or the practice nurse will help you to apply this to the correct areas to ensure maximal numbing pre-procedure. The injections will be performed in a sterile environment similar to your gynaecologist’s rooms. The actual injections take less than 5 minutes to administer – so the procedure

itself is quick. What happens after? And when does the magic happen? Women can expect to feel mild to moderate tenderness and sensitivity initially after the procedure, gradually improving over 24 – 72 hours. No after care is required by you. Sexual activity can be resumed as soon as you are ready. In just three-six weeks after the O-shot is injected, most women experience more robust vaginal tissue, tightening and increased sensitivity and sexual responsiveness. Improvement of urinary incontinence is also often reported after women have received the O-shot, although this is not a primary indication to undergo the procedure. If more than one O-shot is required, these procedures are spaced by six-eight weeks. Am I candidate for the O-shot? Women frequently report in surveys that they have low to no sex drive. Difficulty achieving orgasm is also a massive frustration related in these studies across different age groups, cultures and races. The O-shot can be used in women of all ages. Postmenopausal women have, in fact, been known to respond extremely well to the treatment. Will it work? The current success rate is reported to be up to 87% with PRP. It is vital to book for a consultation with your practitioner to further discuss the O-shot before your procedure. One-three treatments will be required in the first year, then one to two per year thereafter to maintain your result. Remember – as revolutionary as the O-shot can be for your sexual health and confidence – it will not treat sexual dysfunction related to emotional, relationship, hormonal, psychosocial or metabolic health problems. The O-shot starts from R4000/session. There is currently no formal indication PRP and these injections are not covered by medical aid.

In an image conscious society, body perfect is no longer a fad but a musthave-or-else necessity. As aesthetic procedures gain popularity, consumers need to be more aware of what works and what is best to avoid at all costs. By Sally Harvey

D

esperately wanting to sculpt, define, smooth and lift (in much the same way as Photoshop miraculously erases all imperfection), we scout the market for the quickest fix and, in most cases, one that does not break the bank. Sadly, that is our first fatal mistake because going too cheap has shown time and again to have devastating consequences - often leading to serious scarring, burns and deformities. What’s even worse, is that once the damage has been done, it is sometime impossible to rectify - or can only be corrected by having surgery. The latter comes at a cost and even the most reputable of plastic surgeon’s cannot always guarantee that it can be completely reversed, leaving those aspiring for perfection with a long-term flaw. Let’s take Rebecca* for instance. She was not happy with the size and shape of her front upper legs. After consulting with a qualified doctor about performing a fat freezing procedure, she unfortunately could not afford the high price. Imagine her delight when she came across a special deal on the internet at a dramatically reduced rate. Although this is where warning bells should have been going off, Rebecca* was none the wiser, unaware that anyone performing this treatment would be unqualified (or, in some cases, not legally able to perform such a procedure). She was also unaware that the equipment being used in the facility was of an inferior quality – “they are meant to be professional and the therapist assured me of her qualifications,” she said. Two days after the treatment, Rebecca* was extremely despondent as large, unsightly dark red and black marks were visible on her legs, and there was a distinct oval indent in the skin, which has remained ever since. “The skin is still dead around that area,” she exclaims. “I have tried everything to fix the problem, but nothing has been able to take it away, and I don’t have the money for surgery.” Necrosis (or what is commonly referred to as frostbite) is being seen on patients having undergone treatment using sub-standard

Once Caught, Forever Shy equipment and, in most cases, the only way to treat the damage is to cut the dead, blackened skin away. For the equipment to successfully crystalise fat cells without harming the surrounding tissue, it needs to offer controlled cooling – a technology that Coolsculpting fat freezing by Zeltique offers. We are often unaware of the regulations in place regarding certain procedures. “When it comes to injectables, such as Botox® and fillers, these have to be performed by a doctor who is registered with the Health Professions Council of South Africa (HPSA),” says Dr Riekie Smit, President of the Aesthetic & Anti-Ageing Medicine Society of South Africa.

“This is due to the scheduled substances being used, she adds. “Non-doctors injecting patients with both registered and nonregistered substances is illegal, and these persons can be criminally prosecuted.” Regarding cosmetic surgery, this can only be performed by plastic surgeons or doctors that have attended similar courses, and these courses need to be accepted and accredited by HPCSA and the college of plastic surgery of South Africa. “The safest way to ensure that their plastic surgeon is registered is to visit: www.plasticsurgeons.co.za. The same applies to finding qualified and registered doctors for cosmetic procedures: www.aestheticdoctors.co.za or www.derma.co.za,” adds Dr Smit. According to industry expert, Sonette Donker, professional skincare therapists can legally work with hydroxy acid peels, no higher than a 30% active acid with a pH of 3. “If, however, the therapist is working with a doctor, she can use peels at a higher % with a

lower pH. The doctor she works with will be held accountable for any complications,” she adds. Needling rollers for salon and at-home use should not exceed 0.2mm, and the client should not bleed or be able to feel anything while the treatment is being performed. The main purpose is to improve penetration of actives deep into the skin. Medical needling (1mm and above), on the other hand, should only be performed by a doctor as it pierces blood vessels in the skin to cause bleeding (this is in order to promote skin healing and ensure prolonged regeneration). “In my opinion, a skin care therapist or a nurse can quite easily do the needling process, says plastic surgeon and pioneer of needling, Dr Des Fernandes. “However, when we talk about anaesthesia, then we enter a different field. Topical anaesthetics can give adverse reactions and they have to be used with caution. For this reason, I believe that skin care therapists or nurses doing needling should do so with the collaboration or

guidance of a medical practitioner,” he adds. he study of 3,000 women, which was conducted by the well known supplier of hair and personal care appliance company, Remington, shows that women spend around R130 000 on hair removal in a lifetime. The findings also show that 88 percent of women will spend four months of their entire lifetime removing body hair with products like razors, epilators and waxing. Dental Care Query? Our resident dental specialist, Dr Sam Thandar, answer’s your toothy concerns... Question My gums are receding. I have no idea why or what to do! I’m in my late 30’s and aside from my gums, my teeth are fine. I’ve never had cavities, dental drilling or surgery... or anything else. What causes gums to recede – and what methods are available to rectify this? Answer Before we address the procedures available to augment gum recession, let us first address the various causes of gum recession, as the treatment differs depending on the cause and nature. The most common cause of gum recession is gum or periodontal disease. This occurs with patients that are primarily genetically predisposed. Now, add one of the causative agents, like inadequate oral hygiene, irregular visits to the oral hygienist and smoking in the mix – and the damage ensues. This type of gum disease affects the underlying bone structure supporting the gum, and also causes an inflammatory reaction within the gum. Treatment for periodontal disease is case and progress dependant, but in a nut shell, requires the removal of the causative agents. This means regular 3 monthly visits to the hygenist, alterations in brushing and cleaning techniques and addressing dietary issues. Furthermore, we assess the progression of gum disease clinically by measuring the amount of bone loss around each tooth (as well as gum loss). We also assess the mobilty of the affected teeth. With this information we then decide on the best approach. Unfortunately in

some cases, once we have advanced bone loss (as well as movement and mobility of the affected teeth) we may have no option but to remove these teeth and replace them with dental implants. This is provided that there is adequate bone present as in some instances, periodontal disease can damage the underlying bone to such an extent that there is no bone present once the affected teeth are removed. An important point to mention is the progress of periodontal disease. It is however a slow progressing disease, and if caught early can be treated adequately and non-invasively. Other Treatment Methods Incorrect or aggressive brushing can cause recession in specific areas of the mouth, rather than a generalised presentation that we see in gum disease. Most commonly (as there is a higher percentage of people that are right handed), we see the most significant impact of recession on the left hand side of the mouth, as most pressure is directed in this angle. With this type of recession, specific teeth are generally affected - and the surrounding gum tissue generally lacks an inflammatory response (as there is often no bleeding during brushing). However, the affected teeth are generally sensitive to temperature. These cases can often be repaired by a gum graft procedure (grafting gum tissue from the palate of the mouth), or a flap procedure (where the surrounding gums are “pulled’ over the affected area), or by means of a soft tissue pocket graft. In these instances, the underlying bone is fairly intact, so the teeth have no mobility and the gum is not inflamed... generally speaking. I hope this has answered and addressed your question and needs. However, I would advise that you consult a Periodontist (gum specialist) to make a fully informed decision.

So while tattoos are hugely popular, there are many people for whom remorse is a significant reality. Ink is easy to access, but as a person’s life changes with age, they may no longer want their inkings to be visible. Below we discuss three options for management and removal.

T

attoos are part of human history - Ötzi the Iceman, whose frozen body was dated to 3,500 B.C, had soot tattoos on his back and legs. Western popularity increased following the expeditions of the European Explorers to Polynesia, where tattoos were common, from the 16th century onwards. Inked skin has an aspect of possession – either voluntarily to a group; or involuntarily in the case of prisoners and slaves. It’s a permanent mark – a sign of belonging that can’t be removed. Popularity exploded during the 20th century, and while statistics for South Africa are not available, a 2013 Pew Research study looking at US data makes interesting reading: • Annual U.S. spending on tattoos – $1.65 billion (8 times the entire e-toll revenue!) • Americans with at least one tattoo - 14 % •U .S. adults 26 – 40 years old with a tattoo - 40 % •P eople who have regret after getting their tattoo - 17 % •P eople who are removing or have removed a tattoo - 11 % •U p to 70% of people with tattoos keep them regularly concealed

1. Cover-ups –modifying a mark with more ink. This is popular for erasing names of lovers or groups with whom the individual is no longer associated, or have amateurish or “humorous” inkings that are no longer funny! The cost of this treatment is as for an additional tattoo; dependent on the size, and the artist. 2. Q-switched lasers – the best non-surgical option for removal. These lasers send out short pulses of very intense energy which break up the ink pigments, allowing the body to reabsorb them. Different wavelengths are used depending on ink colour. Removal by a doctor of a coinsized tattoo will cost approximately R 350 per session; increasing to R1, 200 for a palm-sized tattoo. However, up to 15 sessions over two years may be required. 3. Surgery – suitable for many locations, but not for “sleeves” or “bands”, or for keloid-susceptible patients. Small tattoos can be removed by single excision, but larger areas may require serial elliptical excisions for optimal result. Looking to the future – technology is evolving, and photo-active ink additives are starting to become available. These are chemicals that are broken down by laser treatment in a way that stimulates the body’s natural defences to reabsorb the associated pigments. There is some understandable resistance from tattoo artists to use additives that will make their art less “permanent”, but it may be possible in future to introduce these additives to a pre-existing tattoo. This is in order to make the entire area respond more favourably and quickly to laser treatment.

NEW Hope In South Africa 3,2 % of the population (more than 1,6 million) sustain burn wounds every year, leaving them disfigured and in many cases, loss of mobility. However, with a new laser treatment available, there is now new hope for them. By Kéki Pienaar

n 1994, two-year-old Corné van der Merwe played happily around the braai with her baby sister, mother and grandmother - when her mom poured spirits on the dying coals. The spirits exploded and the flaming liquid sprayed all over Corné. That moment changed her life forever. With third degree burns on her face, neck, upper body and arms, a life of endless operations and therapy started. She was 13 when she finally said, “no more”. She made peace with her disfigurement and carried on with her life. She even went on to qualify as a beauty therapist last year, and landed her first job in Secunda. “I was too small when it happened to remember much of the pain and trauma. But the teasing in primary school... that I do remember. When I was 13, I couldn’t take the endless operations anymore and decided I will make peace with my looks and carry on with my life. The scars on my face, body and arms didn’t bother me as much as the

Just as she decided to go back to her plastic surgeon to enquire about further treatment to improve the mobility of her neck, Corne met Naomi Olivier of Hitech Lasers. Naomi was doing a presentation of their laser equipment at the Potchefstroom Academy where Corne was studying towards a Diploma in Somatology. Naomi saw her scars and promptly invited her for a pro bono treatment with Dr Tretti Clementoni – a plastic surgeon from Italy who was in South Africa to present a workshop on the treatment of scars. Dr Tretti Clementoni, who teaches advanced laser courses at various international aesthetic training academies, was demonstrating the Lumenis UltraPulse Fractional CO2 laser with SCAARFX laser technology to a few local plastic surgeons on four burn victims (skin types I to 6). Corné was one of them. This full day training was part of the “No Scar Untreated” international initiative from the Israeli company Lumenis. This was in order to train physicians worldwide to bring new hope to patients with visible scar wounds. “Burns are considered amongst the most devastating of injuries”, says Dr Tretti Clementoni. “But survival and mortality rates have improved substantially as a direct result of medical advancements. “Clinical results in a study with 24 patients with mature burn scars show that fractional CO2 laser treatment induced mature hypertrophic scar regression, with the result of enhanced skin condition (appearance) and improved quality of life for patients,” Dr Clementoni added. “Every treated patient reported an improvement of their physical and psychological situation.” Although Corné van der Merwe only had one laser treatment due to her busy study schedule, she is so pleased with the improvement that she wants to carry on with the treatment. “I could feel the difference in the mobility of my neck the next day. I can tilt my neck farther back than before, and the appearance of the skin on my face is much smoother. I expected the treatment to be painful, but with the topical anaesthesia,

I didn’t feel a thing. My skin was a bit red afterwards and a few scabs formed, but that was all.”

of deep contracted skin lesions (such as surgical scars). This leads to greater mobility and enhanced skin appearance.

Corné saw Dr Chris Ladas (the plastic surgeon who treated her since she was six years old), early in February this year for the first time since she had the laser treatment six months ago.

The laser beam is applied in a fractional pattern, leaving “bridges” of untouched skin. This technique makes the healing process much faster. Each laser pulse removes a narrow column of scar tissue, while simultaneously creating a controlled narrow band of surrounding injury.

“The treatment resulted in a softer, more pliable scar which does not ‘pull’ as much,” Dr Ladas said after examining her. He envisaged three more treatments at intervals of two to four months, depending on the response. According to Wicus Olivier, managing director of Hitech Lasers, treatments with SCAAR FX have shown remarkable results and minimal side effects. Using precision and high energy impact (up to 150 mJ per pulse per spot), SCAAR FX allows direct impact into the skin tissue – up to four millimetres.

This in turn stimulates new collagen production deep in the dermis. The new collagen is then produced in an organised manner, rather than the disorganized collagen matrix of a scar - resulting in the softening and smoothing of scars and improved texture and mobility. H

yperpigmentation is the medical word used to describe dark areas on the skin, specifically where the area becomes darker than the surrounding skin. The hyperpigmented areas form due to excess melanin deposits within the skin in that area - or due to increased amount of melanocytes in the area. Melanin is the brown pigment in the skin formed by the melanocyte cells in the skin layers. It’s a very common and harmless skin disorder, yet it often causes a lot of social embarrassment, especially in woman. Hyperpigmentation occurs more commonly in darker skin types, but can occur in any skin colour individual. There are various types of hyperpigmentation with different causes, and it may develop the following excess sun exposure, inflammation of the skin or due to hormone fluctuations. We know there are numerous excellent medical treatments (peelings, lasers, light based treatments) developing each year for the treatment of the various types of hyperpigmentation. But let’s focus a bit on the topical creams available to lighten pigmentation. Sunscreen The first important topical agent that will benefit any type of hyperpigmentation is a good sunscreen as this reduces the absorption of the UV light on the melanocyte cells. Lightening Creams The depigmenting or lightening creams are usually a single or combination of ingredients that reduce pigmentation in various ways. This is what we call the melanogenesis pathway. I

think it’s important that you understand the ingredients to ensure you buy a good quality product. The commonly used agents in lightening creams are: 1. Hydroquinone Hydroquinone is prescribed by doctors in a specifically adapted mixture. It’s most often combined with a low strength steroid and a tretinoin. Hydroquinone is banned in many countries due to the possibility of side effects and due to people overusing it. It can only be used for short periods of time in order to limit the danger of thinning of the skin. There’s also the danger of reactions and broken vessels which may develop after longterm use or overuse. It’s not a solution to all hyperpigmentation conditions and certainly not a long-term solution. While using this mixture, the use of a sunscreen daily is of utmost importance - and one should be extra careful with peelings and lasers as the skin is very irritated while using it. 2. Arbutin and its derivatives. It’s often referred to its origin such as bearberry extract, wheat extract or pear skin extract. These agents mimics hydroquinone, but without the side effects of skin irritation. It’s also considered to be an effective and safe ingredient for reducing skin pigmentation. 3. Azelaic acid This is most commonly used for treating acne and rosacea, but has good lightening action. Studies have shown it to be effective for post-inflammatory hyperpigmentation (spots after acne or injuries to the skin) and also for melisma. The only side effect that can occur is

mild skin irritation or flaking. 4. Kojic acid Kojic acid has lightening, anti-oxidant and antibacterial actions. While it works well, it may cause some redness if overused. It received some attention a few years ago when it was thought to have carcinogenic properties (cancer forming). However, this has shown that it may rarely occur, and if so, only when used in very high concentrations. 5. Licorice extract Derivatives such as glabridin and liquiritin are often used for its effective lightening action in topical creams. Studies have shown it to be effective and with good tolerance. 6. Flavonoids Various types of flavonoids from plant extracts are used for its safe and effective methods of reducing pigmentation. These include Oxyresveratrol (Morusalbla or mulberry extract) or Catechin and Gallic acid from green tea leaves, Ellagic acid or Aloesin (from aloe plant). Many of them are effective, but it’s important they’re used in the correct concentrations and the correct carrier. This is in order to ensure that it can actually penetrate the skin to have these lightening effects. 7. Alpha Hydroxy acids Glycolic acid or lactic acid is most commonly used, and it enhances epidermal turnover. It’s also very effective for overall sun damage of the skin. 8. Alpha-tocopherol This is a very potent anti-oxidant with very effective and promising studies on skin lightening – it also has a very good tolerance.

9. Ascorbic acid An anti-oxidant which has tremendous amounts of studies showing its lightening effect. However, it’s only effective when used in a good quality product at high concentrations and when stabilized. 10. Retinoids Retinoids are effective anti-ageing and lightning agents. It’s most effective when the prescription strength product is used, but the derivatives are often used in overthe-counter creams. 11. Topical steroids These are sometimes prescribed to treat the inflammatory aspect causing pigmentation. It should not be used in high concentrations or for long periods, as it causes skin thinning and broken blood vessels on the skin. 12. Niacinamide This is B-vitamin active with many beneficial effects and good safety. 13. B-Resorcinol It reduces tyrosinase activity – which therefore decreases melanin production and hyperpigmentation. The problem of the depigmenting creams that do not work is mostly due to too low concentrations of good ingredients. The manufacturer may refer to studies of, for instance, Vitamin C and its lightening action, but their concentration is way too low - or the ingredient is not stabilized correctly. Then you will not see results. The secret remains in ensuring that you purchase a product from a reputable company that is actively involved in research and development of products (such as the medical or skin care laboratories). The most reputable laboratories are those from Europe, especially France, Switzerland and Italy. Their reputations are unmatched due to their many years of expertise.

The Outer Thighs These can be lifted at the same time as a tummy tuck, with one continuous scar along the anterior belt line. The same scar can be taken all the way around the back to lift the buttocks as well. This combination of thigh and buttock lift is commonly referred to as a lower-body lift. Lifting the

thighs and buttocks is a bigger procedure than a tummy tuck, can add hours to the operation, and will require a longer recovery. It does, however, give very powerful results that reshape the entire lower body. Careful planning usually allows incisions to be placed in locations where they can be hidden by most types of clothing and swimsuits. Incisions may be extensive. The Inner Thighs In milder cases, inner thighs can be treated by placing the incisionin the groin crease. One technique used for a thigh lift places incisions in the groin, extending downward and wrapping around the back of the thigh. The underlying tissue is reshaped and tightened, skin is then excised and re-draped, resulting in a more balanced and smoother body contour. In cases with severe excess of skin with poor elasticity on the inner thighs, liposuction and a long vertical incision are necessary to tighten the skin. This scar may be quite visible. One or more small, thin tubes may be temporarily placed under the skin during surgery to drain excess fluid or blood that may collect. The results Smoother, tighter contouring from a thigh lift is noticed almost immediately. Patients need to wear a compression garment for several weeks to reduce swelling and support the tissues as they heal. Following a thigh lift, skin quality is dramatically improved - both in appearance and texture. On Discharge If you experience shortness of breath, chest pains, or unusual heart beats once you go home, then seek medical attention immediately as you may require hospitalization for additional treatment. Patients that have had Bariatric surgery do not always heal in the same way that ‘normal’ patients do. The practice of medicine and surgery is not an exact science and although good results are expected, there is no guarantee. In some situations, it may not be possible to achieve optimal results with a single surgical procedure, and therefore another surgery may be necessary. So in order to optimize your results and safety, it’s to your advantage to choose a plastic surgeon who works in a multidisciplinary team, and has experience in performing skin loss surgery on patients who have had massive weight loss.

Elective reconstructive surgery for the ears is one of the most rewarding procedures in plastic surgery, writes Dr Anton Potgieter

T

he ears are very intricate and elaborate. Apart from their role in collecting and focusing sound waves for hearing, they contribute towards a person’s appearance - and are highlighted and adorned with jewelry (mostly by women), all over the world. It follows that an aesthetically pleasing ear will add to the attractiveness on an individual. Anatomy: Understanding how an ear forms embryologically is crucial to appreciating how its convoluted shape comes into existence. It also gives an even greater insight into how the “hills and valleys” can be manipulated to achieve a desired outcome. The ear begins to form during the first few weeks of life and develops from three anterior and three posterior mesenchymal “hillocks” of tissue, which form on either side of the first branchial arch. Seems complicated, but the beautiful shapes that result are given names like: concha, cymba, scapha, tragus, helix, antihelix and more. No two ears are identical. They can look quite different on the same individual (many celebrities have asymmetrical ears). The goal of an ear reconstruction is an aesthetic and proportional ear that is in harmony with the face. Ear deformities have many unusual names, such as: • Anotia (absent ear) • Microtia (small ear) • Macrotia (large ear) • Constricted ear (tight and folded over in the superior third) • Cryptotia (where parts of the cartilage framework are obscured under skin or scalp) • Shell ear (underdeveloped helical rims) • Stahl’s ear (Vulcan or Spock ear) • Cauliflower ear (after traumatic blood clots of the ear cartilage) and lastly • Prominent ears (bat ears). Timing of Surgery: Congenital ear abnormalities are generally obvious at birth, but very few children undergo surgery until much later on. The reason is that surgery achieves better results when it’s delayed until the moment the ear more closely resembles an adult ear in size and proportion. Most children undergo ear surgery between the ages of four and 14 years (ideally when

the ear has reached 85-90% of its adult size). There is a peak at school going age in order to prevent teasing. Demographics: Setback otoplasty for prominent ears is the most commonly performed procedure for ear reconstruction. Exact statistics are difficult to standardize around the world, but about 8% of patients treated for this deformity have a family history. Protruding ears appear to be equally common in males and females; however it’s easier for girls to conceal their ears with hair. This is probably the reason why the procedure is slightly more common in boys (53%). Most children undergo their surgery while at school (peak at ages of five to seven years) but interestingly, the second largest group of patients are women in their 20’s and 30’s. Benefits of Otoplasty: Anyone that has had to endure teasing will remember that people, and especially children, can be very cruel. Abnormal ears invite ridicule, and this can lead to loss of self-esteem and poor social interaction. Most parents notice that their children are confident and more outgoing after successful surgery. The Surgery: Otoplasty is done under general anaesthesia for children, but can be performed under local anaesthesia with sedation in adults. It takes about 90-120 minutes to complete the surgery. A combination of permanent and dissolving sutures are used. The Method There is no universally accepted technique. The surgery is generally performed through an incision behind the ear. Several distinct maneuvers can be undertaken to alter the shape of an ear: • The angle between the ear and the skull can

be narrowed by using permanent sutures to hold it closer to the underlying bone • The height of the ear can be decreased by removing cartilage • The ‘folding’ of the ear can be improved by using scoring techniques to ‘warp’ the cartilage, and permanent sutures to shape and define the natural convolutions of the ear • The posterior skin can be excised in a manner to ‘pinch and shape’ the final result. The skin is closed with absorbable sutures so that there is no need for post-operative ‘removal of stitches’, which can be traumatic for a child. Recovery: One of the major benefits of the surgery is the relatively short wait for a result. The bandages are kept on for seven to 10 days, after which the structural changes will be clearly visible and easy to appreciate. The recovery period is generally uncomplicated, however there are a few precautions. The ears will still be painful and bruised. Care must be taken not to injure them in the subsequent weeks, since it takes at least three months to achieve the ‘final result’. Alternatives: Ear moulding, if instituted within the first six weeks of life, may improve the shape of an abnormal ear enough to avoid surgery. There are however no effective alternatives to otoplasty for treating ear deformities or injuries in adults. The Future: There continues to be regular advances in cell biology and tissue engineering which stagger our minds, but at present the best results are obtained by using the natural (autologous) constituents of the ear and the body. At present, there are no artificial (alloplastic) materials or substitutes that are regularly used in ear surgery.

ver since boyhood, as it’s distinctly different to childhood, my heroes have always been raw, rugged and rebellious men.

With the exception of one James Dean, the likes and looks of Harrison Ford, Steve McQueen, Liam Neeson and of course, my dad, comes to mind.

None are pretty but all are undoubtably men of character. Their wrinkles are as much a part of them as their tempers. Do they prefer bar fights to manicures? Well, undoubtedly - but on occasion they show their depth with an unfathomable act of kindness. Understanding that gives us a clue as to what men want in plastic surgery. Most men want the change in appearance that will remove the edge off of their age, and the difference between a has-been and a middle-aged man in his prime is a subtle one. The truth is however that they are not the rule, but the exception - and the exceptions are far more likely to have a procedure than the rule. Logical if you take the time to think it over. What Men Want What men are likely to have done does not differ so much from women, if you exclude the obvious.

Eyelid surgery for extra upper lid skin, or bags beneath the eyes remains at the top of the list, followed by liposuction of the flanks and other surgical procedures to the face (rhinoplasty, bat ears and face and neck lifts). But again, the changes requested are subtle. Procedures more unique to men are jaw augmentation and chest reductions for gynaecomastia. Gynaecomastia procedures in particular have seen an increase in recent years, and the main treatment modality in my practice is liposuction. Peculiarities in men coming for procedures are fortunately few, however as hazardous occupations go, the surgeon who specializes in male-rhinoplasties should check their life insurance

policies are updated.

Apparently, more surgeons have been offed by this particular group of patients than any other. This does not highlight the degree of difficulty in performing this procedure, as most men require less tip work and dorsal reduction than women, but rather the type of patient we as surgeons need to be on the lookout for. The acronym we use to describe these men is SIMON. Single, Immature, Male, Obsessive, Narcissistic. If you know such a man and he is thinking of having a procedure, please send him to my colleague? Queer Eye & the Straight Guy Misconceptions about gay men and plastic surgery, are a dime a dozen. Now here is a topic everyone seems to have a minor opinion on, as gay men with their “pink money” seem to be bundled into one pigeon hole, all wanting abundant amounts of product and procedures. Yes, there are more gay men patients visiting my practice than straight. My practice is situated in Cape Town so one would almost expect there to be a majority of gay men if you listen to the proverbial “they”. Although the fact remains that women are still the largest group of patients - and more to the point, the gap between gay and straight men is closing fast. The explanation is that the general public feels more comfortable with cosmetic surgery. Like air travel in the 80’s, when only the well off could afford to fly off - and now everyone is jetting about. The future in cosmetic surgery for men is not likely to exceed the demand from the fairer sex, but then again… we thought the earth was flat not too long ago…

PIP Founder P Jailed

oly Implant Prothese (PIP) boss Jean-Claude Mas, 74, was convicted of aggravated fraud after using industrialgrade silicone in implants that were sold worldwide.

Mas, dubbed “the sorcerer’s apprentice of implants” by prosecutors, plans to appeal his sentence, his lawyer said. Four other ex-PIP staff were also convicted in Marseille, France and given lesser sentences. The scandal first broke in 2010 when doctors noticed abnormally high rupture rates in PIP implants.

The founder of a French firm that made dodgy breast implants used by hundreds of thousands of women was jailed earlier this year.

A global health scare then erupted, with some 300,000 women in 65 countries believed to have received the faulty implants. Mas was also ordered to pay a £63,000 (roughly R1.1 million) fine and has been permanently banned from working in medical services or running a company. With over 7,000 women registered as plaintiffs, the trial was one of France’s biggest ever legal cases. The defendants admitted using the industrial-grade silicone but denied the company’s implants posed any health risks. More than 7,500 women worldwide have reported ruptures in the implants – and in France alone, 15,000 have had the PIP implants replaced. But health officials in various countries have said they are not toxic and do not increase the risk of breast cancer. Mas, a one-time travelling salesman who got his start in the medical business by selling pharmaceuticals, founded PIP in 1991 to take advantage of the booming market for cosmetic implants. He built the company into the third biggest global supplier of implants, but came under the spotlight when plastic surgeons began reporting an unusual number of ruptures in his products. Investigators subsequently discovered he was saving millions by using industrial-grade gel in 75% of the implants. PIP’s implants were then banned and the company eventually liquidated. Labiaplasty on the Rise P

op culture and the media has had a long-standing history of influencing the way people perceive their own bodies — especially women. According to a recent study, researchers from Australia’s University of Queensland School of Psychology speculate that the perceptions of what women considered normal and beautiful (in regard to female genitalia) may have to do with the modified images of genitalia to which they are exposed. The results also helped researchers better understand why there has been a rise in labiaplasty in the past few years. Labiaplasty is a type of cosmetic plastic surgery that alters the labia minora and labia majora. Pornography might have influenced the rise in this type of surgery, but researchers are trying to figure out if it’s just more than images - or something else that have women ready to go under the knife. “Our results showed that exposure to images of modified vulvas can significantly influence women’s perceptions of what is considered a normal and desirable vulvar appearance,” said Claire Moran, postgraduate student at the University of Queensland School of Psychology, and lead researcher of the paper.

The study, published in BJOG: An International Journal of Obstetrics and Gynecology, used a total of 97 women, ages 18 to 30 years old. The women used a fourpoint Likert scale with choices ranging from “strongly agree” to “strongly disagree.” The tests were broken up into two different screenings. The first screening exposed one group to a series of images of surgically modified vulvas, one group to a series of non-modified vulvas, and the third group viewed no images. During the second screening, all of the groups were given the same images and the women rated each image on “normality” and the “society’s ideal.” The modified images were rated as more like society’s standards than the non-modified images in all three groups. And the women who had first viewed the modified images as ideal were 13 percent more likely to rate the modified vulvas as more society’s ideal than the control group. hinoplasty, no matter which way you define it, is an art and science. An art because it requires artistic and aesthetic skills, not usually associated with other forms of cosmetic surgery. A science because the nose is a complex 3-dimensional organ constructed from 240 odd anatomical variants. Some patients require only superficial aesthetic improvements; others structural modifications. Good surgeons work on margins of error of a single millimetre. To complicate it further, cosmetic surgeons term cosmetic rhinoplasty an ‘elective’ or ‘luxury’ procedure: it is strictly not necessary; you don’t operate to save a life. This is surgery by choice. The patient expects, and feels entitled to a good result. Given the not often appreciated realisation that the results of rhinoplasty – including revision surgery - display permanently on the most prominent part of the face, it then makes sense to identify a surgeon capable of delivering a good result. Social developments over a last decade reveal why it’s important to identify a competent surgeon. Rhinoplasty is a popular and common operation. The procedure (as we know it today) has evolved from a basic operation to correct obvious deformities of the nose - to the introduction of subtle aesthetic enhancements of the face. It’s treasured not just by models and actresses - but also ordinary people simply wanting to look and feel better about themselves. This could entail a routine ‘hump removal’ procedure, the lifting of a ‘saddle depression,’ or a refinement of the tip of the nose. Surgeons promote procedures like this on television, they’re downloaded, they’re the subjects of online forums, and if you search the Internet for a surgeon, the response suggests that all surgeons are equally qualified (and all equally capable of the delivery of good results). Yet, few patients appreciate the complexity of the procedure, or the training and skills needed to execute a successful result. They may not realise it, but cosmetic surgeons do not undergo identical training or share equal skills. Experience confirms this. If you want a good result, you can depend only on the best trained and most experienced surgeons to perform this kind of complicated surgery. The challenge lies in identifying them before surgery commences. The most objective indicator of competence, says nose surgeon Dr Pieter Swanepoel, is a before and after image. “Is a surgeon – with the necessary patient consent and respect for privacy – prepared to show prospective patients an earlier record of ‘before and after’ images; or even to display these images on a website.” “Patients also need to ask pertinent and relevant questions about the surgeon’s capabilities, post-graduate training and whether the surgeon has been following recent developments in the field. What are your credentials? Have you undergone formal and specific training in cosmetic and functional surgery,” explains Swanepoel. This is pertinent, because few medical schools, even internationally, offer specialised post-graduate training in nasal cosmetic surgery. Most reputable medical schools concentrate on fields where there is a high demand for specialised life-saving skills. Cosmetic surgery, in this context, is not considered essential. Surgeons embarking on a rhinoplasty specialisation submit themselves to a mentoring process under recognised ‘master’ surgeons (often in the United States), or sign up for advanced courses offered by associations linked to the specialisation of this surgery. Qualifications arrive hard earned usually

after several costly years. Yet, degrees and diplomas only reflect historic qualifications. They’re not a measure of current competency. “Continuing Professional Development(CPD) is vital for any surgeon,” explains Swanepoel. “The half-life of most medical disciplines today is three-and-a-half years. In other words, half of your doctor’s knowledge today outdates itself within three to four years.” This is a consideration for South Africans. Although the Health Professions Council of South Africa (HPCSA), the body that regulates and monitors medical standards, requires an annual updating of medical skills, rhinoplasty is not one of them. A further concern emerges when functional surgery to correct a breathing related problem overlaps into cosmetic surgery. Cosmetic surgeons do not specialise in functional surgery to correct breathing problems. Ear, Nose, and Throat specialists (ENT) do not undergo specific training in cosmetic rhinoplasty. When overlaps between the two disciplines occur, two surgeons usually attend the operation. One, the ENT surgeon for the breathing problem, the other, the cosmetic surgeon to refine the shape of the nose. If something goes wrong – and they occasionally do – who does the patient approach for resolution? “It is a question of accountability,” explains Swanepoel. Specialised experience is another important indicator. How long have you been performing rhinoplasty operations, and how often do you perform them? “I would say that 100 cases per year is an absolute minimum to maintain expertise in this field,” says Swanepoel. “If you practice your profession for an hour a day, your skills will improve. If you practice once a week or twice a month, your skills, by comparison, will lag. You may never attain the delicate skills needed in this profession.” “What patients need to realise is that a good, well trained and experienced surgeon can generally be depended on to achieve a pleasing result, but that makes it imperative that you first identify a competent surgeon,” says Swanepoel, “they’re the ones with advanced training, experience and a dedication to the field.” Source: www.thenoseclinic.co.za

Getting to the Root of Hair Loss in Women… While it’s usually seen as a male affliction, hair loss in women is a frequent condition – and affects more than 40% of hair loss sufferers.

H

air grows in three different cycles: anagen, catagen, and telogen. About 90% of the hair is in the anagen (or growth phase), which lasts between two-eight years. The catagen (or transition phase) typically lasts two-three weeks, during which the hair follicle shrinks. In the telogen cycle (which lasts around two-four months), the hair rests. Hair grows around 6 inches a year and a person loses an average of between 50 to 100 strands a day. Men’s hair tends to recede from the forehead (or the crown of the head), while women tend to notice thinning on the top third to one half of the scalp. Sometimes their frontal line stays intact and in other conditions can get affected. Women may see a part in the middle that is gradually becoming wider, or they see more of their scalp than normal when their hair is pulled back.

There are many types of women hair loss and identifying the exact cause will lead to an accurate treatment: Involutional alopecia (or ageing) is a natural condition which generally happens over the age of 50, in which the hair gradually thins. More hair follicles enter the resting phase, and the remaining hairs become shorter and fewer. This condition can be postponed with antiageing treatments. Androgenic alopecia (or female patterned baldness) is a very common condition which needs much attention in order to be diagnosed and treated properly. Androgenic alopecia in women is related to the Androgenic disorder, which is a genetically determined one and involves the AR gene (located in the Xq12 chromosome). It’s often associated with Polycystic Ovarian Syndrome and Hirsutisam. Increased levels of androgens in hair follicles

can lead to a shorter cycle of hair growth, with the growth of shorter and thinner strands of hair. Additionally, there is a delay in the growth of new hair to replace the strands that are shed. In the case of androgenic alopecia in women (depending on the polymorphism of this gene), we either have increased DHT (dehidro-testosteronean androgen affecting follicles) or an increased sensitivity to DHT. Women normally notice their hair becoming shorter and thinner from 30-40 years, and it’s diffusely on the entire scalp. Diagnosing this condition needs genetic testing and early treatment before hair follicles become atrophic.The best treatment would be to block the enzyme II 5-areductase, which is responsible for the production of DHT (the way it’s used in males). However, this treatment is not approved in females. Using Minoxidil at a lower concentration with beta-sitosterol would be a better solution in diagnosed cases.

Telogen effluvium is the temporary hair thinning over the scalp, which is due to the changes in the growth cycle of hair. A large number of hairs enter the resting phase at the same time, causing hair shedding and subsequent thinning which is perceived as hair loss. Possible causes include: • Surgery • General anesthesia • Change in medication • Childbirth • High fever and flu • Extreme psychological stress and • Extreme weight loss Women with telogen effluvium typically notice hair loss between six weeks to three months after the stressful event. Sometimes they notice a handful of hair. Majority of patients have normal blood results, and only a look into their history can help find a possible cause. This temporary hair thinning usually disappears if the stressful event does not continue. In some cases, hair shedding and thinning continues, so some ongoing causes must be identified to combat the hair loss. There are a variety of causes, namely: Hormonal Hypo and hyperthyroidisam, high cortisol, low progesterone, testosterone imbalance with high DHT production, altered hormone ratios, conventional HRT, contraception pill, prednison and steroids Nutrition Low protein intake, low vitamin B (especially B12), high vitamin A, low iron, low biotin and folic acid, low Zn Medication Cholesterol lowering drugs, BP medication, anticoagulants, antidepressants, epileptic medication, isotretinoin, etc Toxicity Environmental toxicity and physical damage of the follicles by harsh hair products and procedures Diagnosis & Treatment To successfully diagnose hair loss in women; a detailed history, blood tests and genetic testing must first be undertaken. Treatment then includes addressing an underlying condition medically (for example: if there is frontal thinning in peri-menopausal women with low progesterone, then adding a bioidentical progesterone will cure the condition.) In some cases where the underlying condition remains uncertain, we use general hair loss vitamin enhancement with combinations of Biotin, Niacin, Vitamin B5, B6, B12, PABA, Inositol, Vitamin C and essential fatty acids. Could Seaweed Help Cut Fat in Your Chocolate? Are you a chocolate lover at heart but struggling to avoid this sinful treat? A new study is all set to come to our rescue: this new discovery could help you relish the yummy temptation without piling on the kilos. A team of researchers from the Warwick University in the United Kingdom have now developed a new technique to cut down the fat content in chocolate by using seaweed. This new low-fat chocolate known as choco-lite supposedly contains very tiny microscopic bubbles of a special type of jelly prepared from seaweed extracts. It is this jelly that bulks out the chocolate and lessens its fat content, which is what makes it unhealthy. What’s more, the agar bubbles are a good source of fiber, amounting to around 80 percent. The fiber suppresses the appetite and prevents you from overindulging. This new low-fat chocolate seems much more promising than other previous attempts to create a chocolate that has reduced fat content – without compromising on its taste, flavor, and texture. Earlier, air bubbles were used to make chocolate healthy, but it left the chocolate feeling too light. Water bubbles were then used for

the same reason, but they reduced the crispiness of the chocolate and posed a greater risk of a white bloom forming on the bar. Thankfully, seaweed jelly doesn’t have any such effects, and is already used in many areas in the cosmetic industry. One of the best features that makes this low-fat chocolate a sure winner is the fact that this agar (when heated in the correct way) has the ability to solidify at the same temperature as the fat in chocolate - and due to the small bubbles, the tongue cannot distinguish it from fat. This means it will probably taste the same as any other chocolate. “Fat replacer is something the food industry is very keen to have,” Jianshe Chen from Leeds University, explained. The researchers also claim that this low-fat chocolate tastes ‘pretty darned good,’ and now they are working on developing other flavors to this chocolate too. any of the popular foods we deem ‘healthy’ are in fact laden with hidden sugar and fat. Here are a few foods that need to be ‘undressed’ in order for us to know precisely how they affect our diet. Hidden fats in popular foods: Mayonnaise Regular varieties of mayonnaise contain a whopping 79g fat per 100g. Most of your favourite mayonnaises are available in low fat varieties, so give those a try. Also consider swapping mayo for other options, like fat-free cottage cheese, which has an impressive amount of protein and no fat to speak of. Even cutting out half the mayo you usually use and substituting for fat-free yoghurt makes a healthier spread that still has plenty of flavour. Nuts We’re definitely not knocking nuts, as they contain some seriously healthy monounsaturated fats and are cholesterol free. But that’s a high fat content nonetheless – with almonds racking up roughly 47g of fat per 100g – so you need to monitor your intake. Never have more than a small handful per day, and stick to the leaner options like almonds and cashews. Other options – like peanuts and macadamias – have almost double the fat content. To make the most of the health benefits, stick to the raw and unsalted varieties. Peanut Butter Much like nuts, peanut butter is another source of mono-unsaturated fats. Along with the high fat content of roughly 50g per 100g, many nut butters are also packed with unhealthy sugars. Finding the sugarless varieties could lower the total calories of the product, and improve the health benefits. Stick to peanut butter in small quantities only, or remember to factor it into your recommended fat allowance. Avocados We’re no strangers to the myriad of health benefits that avos offer, but they do still pack a punch when it comes to fat with approximately 17g fat per 100g. Again, these are the healthy mono-unsaturated kind, but it’s still far too easy to devour a whole avo in one go. Keep portions to a quarter-avo at a time to keep your intake on track.

Hummus This tasty blend of chickpeas, oil and sesame seeds is one of the healthiest spreads on the market, packed with protein, fibre, good fats and nutrients. What it isn’t, though, is easy on the waistline, with 15-30g fat per 100g. There are lower fat varieties out there, which usually contain less oil and still taste great. Grab those if you can, or just moderate your intake, as it’s all too easy to overindulge.

Cereal Bars Cereal bars might seem diet friendly, but while wholegrain varieties are high in slow release energy, vitamins and minerals, the sugars and fats used to flavour and bind them send the calorie count whizzing upwards (the average bar contains 250-300 calories). Opt for naturally flavoured bars free from trans-fats and preservatives and factor the calorie count into your daily intake.

Dark Chocolate Dark chocolate has long been celebrated for its whopping offerings of antioxidants, but while it may be wiping wrinkles off our faces, it’s racking up our fat intake with every bite – approximately 34g of fat per 100g. Remember, chocolate is meant to be enjoyed in small quantities – the average slab is 90 or 100g – so polishing off the whole lot means you’ve well exceeded your daily quota.

All-Milk Coffees Cafe Latte with full cream milk and 2 sugars is already 212 calories without even eating your biscuit or ordering your meal! Filter coffee with fat free milk and 2 sugars is around 46 calories. This is a much better option and you now know you can order a meal without overdoing your calorie intake in a few sips.

Olive Oil Olive oil is a full-fat food – every gram of oil is a gram of fat. Whilst we obviously don’t use it in enormous quantities, portion control is still important. To get an idea, one tablespoon alone contains a whopping 120 calories. Remember too, that if you’re not enjoying olive oil raw, you’re not getting any of the healthy fat properties either. Hidden calories in ‘healthy’ foods Dried Fruit and Fruit Juices The sugar level in a piece of fresh fruit won’t be higher than its dried counterpart, but the dehydration process means you eat far more in one sitting than you would when fresh. Certain fruits, like cranberries and blueberries, are particularly tart when dried, and will most likely have had extra sweetener added to them. On average, expect to take in 67 calories per tablespoon of mixed dried fruit. Fruit juices, too, usually have a whopping surplus of sugar in them to make them more marketable. Even if you are making your own fruit juice, remember that you are removing the fibre, skin and flesh of the fruit, but still keeping the high sugar content. This means that a small portion of juice will contain the sugar from multiple fruits, an estimated 115 calories per 250mls, which is far more than if you simply ate the fruit whole.

Vitamin Water They draw us in with their promises to energise and refresh us, but the majority of specially fortified vitamin waters are also high in sugar and therefore pretty high in calories (some as much as 350 calories per bottle, which is the same amount as a small meal). Rather stick to good old-fashioned water instead, it will save your wallet and your waistline. Low Fat Yoghurts Remember – low fat doesn’t automatically equal ‘low calories’ or ‘healthy’. Many ‘low fat’ yoghurts – while high in calcium and protein – are packed full of sugar, making some even higher in calories than regular yoghurts. Opt for natural, flavour-free Greek yoghurt instead, keep your portion sizes small, and if you need sweetness, add it yourself with a small drizzle of honey and fresh fruits. Another give-away that something contains sugar even though it’s fat-free or low-fat is when the packaging states ‘sweetened’. Jars of Tomato-Based Pasta Sauce So often, we’re advised to opt for tomatobased pasta sauces over creamier version, but these are far from calorie-free. Readymade pasta sauces can contain as much as 12 teaspoons of sugar per 500mls. Check the label for lower sugar if you’re buying ready-made pasta sauces, else make your own – olive oil, onions, tomato, garlic and fresh herbs are all you need! Adding in some balsamic vinegar will sweeten the dish.
