Critical Care Critical care involves the careful diagnosis and management of life threatening conditions by specially-trained healthcare professionals who use their expertise to provide care for the patient. They access highly sophisticated equipment and computer systems with real-time monitoring of vital data. Care is usually given in an intensive care unit (ICU), emergency department, surgical area or trauma center. The real-time monitoring systems play a critical role to closely tracking patient vital signs and translating patient physiological data into clinical information.

Perioperative Information Systems support clinical and administrative decision making from pre-operative through intra- and post-operative and procedures. The solution is a total anesthesia information system that records and documents each phase of the procedure to produce a comprehensive patient record which is linked to existing patient monitoring equipment, anesthesia machines, and other equipment.

Infection Control AMiS features a seamless design that allows hospital personnel to keep the station clean, meeting hospital standards and preventing the spread of infection. AMiS is designed to meet medical safety certifications. There are no sharp edges, and all components are built to minimize risk of pinched fingers or stubbed toes. AMiS is rugged, with the same trusted industrial design built into all Advantech products.

Mobile Nursing Care Currently most hospitals struggle with makeshift carts by strapping desktop computers or laptops on board and rolling them from place to place. Usually, these carts with their tie-wrapped wires wound about legs, have issues with cleanliness, power and working space. They just aren’t designed to do what healthcare workers need. Advantech mobile point-of-care systems are designed to move with busy healthcare professionals, and streamline workflow, providing accurate data recording, and reduced medication errors. They make use of wireless infrastructures, mobile devices and specialized applications to meet the needs of caregivers.

Intelligent Power Management Intelligence is built in to the AMiS via specially designed software. AMiS_Link is an application which provides management choices and settings for both hospital IT staff and its medical staff.

Integrated Design The front and rear handles have comfortable grips, making AMiS easy to move from room to room. The work area is smartly designed, with a keyboard/mouse tray and spacious work area, and its ergonomic design features adjustable height, either motordriven (AMiS-50), or supported by gas spring (AMiS-60), which makes for comfortable, stress-free use even on extended work shifts. AMiS also features an intuitive “dashboard” that shows power status and battery consumption, and includes the height control buttons (AMiS-50 only). AMiS is built with cost-effectiveness in mind. An intelligent upgrade path, flexibility and ability to be customized all make it an attractive long-term solution for hospitals and clinics. AMiS’ use of a DIN rail for accessory integration prevents damage to the frame. The work surface height adjusts all the way from seated to standing operation. The onboard PC is easily upgradeable, and features multiple I/O connections (USB 2.0 ports x 4, Gigabit Ethernet port x 1) to allow integration with barcode readers, HD cameras, and card readers.

Industrial Grade Embedded PC AMiS is a computerized medical cart built with a mini-ITX motherboard inside; this distinguishes AMiS from other carts which are stacked with desktop computers or laptops on board. This integration feature allows customers to flexibly and easily upgrade computing performance and minimize the total cost of ownership. The interface bracket at the rear of AMiS creates a convenient docking location to combine the mobility of a medical tablet with the productivity of a full workstation.

Enhanced Internet The system has been enhanced for internet use with two built-in 5-dB antennas for WLAN connectivity (802.2a/b/g/n), as well as standard Gigabit Ethernet connections for wired access.

Intelligent Power System AMiS products feature built-in long-lasting power supplies which utilize up-to-date LiFePO4 battery technology (Lithium-Iron Phosphate; LFP). This technology is a high performance, cost effective solution to upgrading power on mobile computer carts in healthcare environments. It is also manageable with high quality AMiS_Link software. Even more, battery replacement has never been as easy as with AMiS. With only a few simple steps (only 7 screws on the cart) the job is done! It’s simple, easy, and fast!

Patient infotainment refers to a variety of “bedside terminals” that allow patients to watch TV, make phone calls, play games or communicate via the internet. They can be used for email, web browsing, accessing hospital intranets, or even work (depending on the patient’s condition). They can control beds, lighting and curtains. Medically, they can be used to alert staff, look up electronic patient records, display lab results and tests, monitor vital signs, document treatment, and more. As a single integrated solution, patient infotainment terminals provide digital entertainment, communications, and clinical services, at the point-of-care.

There is a growing trend in the market toward development of remote homecare for elderly people and patients. Remote medical services assist in the monitoring of health conditions of a care receiver by obtaining and analyzing their physiological parameters, and providing medical services and treatments at the most appropriate time. The infotainment terminal can serve as the computing platform to connect a care receiver and a family member, or a care receiver and a doctor at two different places. Information such as blood pressure, heart rate or blood sugar levels are transmitted to a family member or a doctor through the internet or via telenetworking, helping determine the care receiver’s current conditions and provide timely homecare or medical services.

The Hospital Alpes Léman in Contamine sur Arve, France, was originally built in 1841 and serves the Geneva area as a premier public hospital offering a complete range of medical and surgical specialties. The hospital has a 640 bed capacity, and its wide range of care options across various fields of competence is available to all. The hospital was completely rebuilt in 2012 which afforded planners the opportunity to computerize the facility in surgery, endoscopy, recovery, anesthesia, and other areas. Hospital staff identified the need to build systems that allowed full patient management, room scheduling, and regulatory checklists in surgery; intervention and recovery processes in anesthesiology; and patient plan care and prescription continuity in pharmaceutical dispensaries.

Solution

Advantech’s POC-S197F Slim Point-of-Care terminals were selected to handle the hospital’s needs. The all-in-one terminal is connected to the hospital’s information systems, and allows real-time access and recording of patient data. It was installed in the hospital using convenient VESA standard swing-arm mounts, making for an ergonomic workstation with mouse and keyboard suspended above worktables maximizing the work area. The POC-S197F’s 19 inch display gives medical personnel, such as anesthesiologists in surgical units, a broad visual interface. Having instant access to data optimizes workflow, reduces risks from errors and ensures premium access to information is always available.

System

Advantech’s POC-S197F is a 19’’ TFT LCD display powered by an Intel® Core™2 Duo processor. The 1.2 GHz processor is the basis for a powerful system which supports 2 GB of DDR3 memory, and has a full set of I/O connectors (3 USB ports, 2 serial ports, and 2 LAN ports) to meet the hospital’s requirements. WiFi and touch panel functions are also available to provide more flexibility, ease data entry, and allow the system to connect to other HIS platforms throughout the hospital.

Hamad Medical Corporation (HMC) is a premier, non-profit healthcare provider established in 1979 in Doha, Qatar. Their mission is to provide the best quality patient care—“Health for All” –irrespective of nationality. In addition, HMC is the only institute outside of the United States where all of its five facilities and twenty-three primary health centers are JCI-accredited. HMC has developed highly-specialized medical facilities and equipment with a continual upgrade plan to ensure the highest quality of care possible. The HMC facilities are equipped with more than 1,700 beds and annually provide care for more than 1.25 million patients on an outpatient basis. SIMSAFE Medical Instrument Management and Advantech’s POC-C177 at Shimane University Hospital

A Premier Teaching Hospital in Kaohsiung Adopts Advantech Point-of-Care Terminals in Large-scale Project to Meet EMR Mandate

Shimane University Hospital is a large, teaching hospital in Japan with a school of medicine, school of nursing, and a medical university. During surgical procedures, the hospital must keep track of its surgical instruments, counting everything from pins to scalpels. The hospital outlined a project to use advanced technology to build a system that would assist in instrument management. The goals were to prevent accidents like leaving an instrument in a patient, eliminate lost or misplaced instruments, increase safety and efficiency, prevent hospital infection and reduce work and medical instrument costs. These important safety issues have already led to legislation in Europe and the United States, and over time will likely become mandatory in medical care facilities worldwide.

Kaohsiung Medical University Chung-Ho Memorial Hospital (KMUH) was established in 1957. In its initial phase it had 350 beds which have since grown to over 1,600. KMUH was the first hospital in Taiwan to receive accreditation by the Ministry of Education and the Department of Health for its role as a “first class teaching hospital.” The hospital has also been certified by the Ministry of Finance for its “good accounting records.” Today, as one of the most important medical institutions in Taiwan, the hospital is chartered with teaching, research and administration of medical treatment.

The hospital implemented a solution using RFID technology and medical-grade terminals to trace and manage medical instruments. KRD Corporation Ltd, a technical solution provider founded in Yamato, Japan in 1991, created SIMSAFE, a surgical instruments safety system which uses RFID tags to individually manage surgical instruments. Advantech’s POC-C177 Point-of-Care terminal was chosen as the platform for the system. Using SIMSAFE provides the hospital with the ability to trace instruments from start to finish of a procedure. Advantech’s POC-C177 is an easy-to-use multimedia computer which can recognize and record individual RFID tags using its RFID reader and antenna. It is installed on mobile medical carts and used for registration of instruments prior to commencement of surgery. The cart can then be moved out of the way during an operation. Shimane’s use of the system saves in labor, and more importantly, prevents accidents and increases safety with its sure-fire management control, done right at the “front-line” of medical treatment. Shimane University Hospital installed Advantech’s POC-C177 Point-of-Care terminals. The terminals have 17” TFT-LCD displays, and an Intel® Atom™ processor with 1GB of SDRAM. They support a wide range of peripherals, 100 Mbps Ethernet, and touchscreen control, making them extremely flexible. The RFID controller and antenna are attached to a COM port, and the system uses Bluetooth to communicate with a PDA barcode/RFID reader. The system is attached to the hospital’s information systems, and the SIMSAFE application software handles the instrument management, recording and storing of RFID tag information, and keeping tabs on where the instruments are located. Pre-op registration and post-op verification ensures nothing is misplaced.

Solution

Advantech’s POC-S176 Slim Point-of-Care terminal was chosen by the hospital. Internal IT staff purchased the terminals directly from Advantech and then set out to replace the paper-based system with a modernized EMR solution. They broke the project into two phases, the first to implement 100 units and the second to implement an additional 86 units. IT Manager, Zhang Hao Zi was charged with designing the solution. He understands of the needs of the hospital and is the designer of the hospital’s existing intranet. “After strict research for a product that met our specifications for a computerized medical cart, we decided to implement Advantech’s POC-S176 throughout the hospital. Advantech has a good reputation for stability, and they were very cooperative with us in this opportunity.” The POC-S176 terminals were placed on existing medical carts for use in hospital rounds, wards, and patient areas, and used as free-standing units in the ICU. There were some challenges faced along the way, the most critical of which was interoperability with existing wireless infrastructure. With Advantech’s experience, a customized POC-S176 terminal was designed to meet the unique requirements of the hospital’s wireless LAN topology.

System

Advantech’s POC-S176 terminal has a slim design and a 17” TFT-LCD display. The units are powered by Intel® Core™ Duo 1.66/1.2 GHz or Intel® Core™2 Duo 1.5 GHz processors. The fanless design, IP54 rating and IP65 dust-tight front panel meet the hospital’s requirement for a medically-certified device that doesn’t take up a lot of space on the carts. The POC-S176 terminals fit perfectly into the hospital’s environment: they are quiet in operation, don’t accumulate dust, and are easy to keep clean. 802.11 wireless antennas connect the terminals to KMUH’s information systems allowing real-time data entry and doing away with the previous, paper-based system.

Achieving the goal of an EMR is important to the hospital. As a training hospital, KHUH is charged with the education of future medical personnel. The University College London Hospitals (UCLH), established by Her Majesty the Queen, opened in the U.K. in 2005, and provides 665 in-patient beds over a campus network of 75,822 square meters. They defined a project to implement a digital healthcare system that allowed medical staff the ability to record patient information and data from robust, medically-certified terminals, with connectivity to the hospital’s existing information systems. They turned to ECA Services, an integrator that had successfully worked with them on past projects for a solution.

ECA Services is a long-term partner of Advantech. They delivered a timely solution that included Advantech’s POC-S197 Point-ofCare terminal with an Intel® processor, Windows® XP operating system, and GE Centricity Healthcare software.

System

The POC-S197 terminals were mounted using wall arms to allow easy access for clinicians and hospital staff. Using the hospital’s existing medically-certified keyboards and mice, the new system delivered a complete ergonomic solution that was flexible, reliable and ideal for infection control. POC-S197 powered by an Intel® Core™2 Duo 1.2GHz processor easily met the hospital’s requirements with touchscreen and medical certifications, including IPX1-rated water resistant enclosure, IP65-rated dust-resistant front panel, and UL60601-1/EN60101-1, CE & FCC Class B certifications. POCS197 has a large 19” LCD display and has the additional safety of electrically-isolated COM and LAN ports. It is fanless, operating noiselessly in the hospital, and easy to clean and sanitize.

The average 500-bed hospital in France generates a turnover of about 548 euros per day per bed, with an operating staff of about 500 to 600 members. This equates to one medical professional per bed, but with about 30% of their time taken up by administrative paperwork, which includes regulatory forms, prescriptions, recording lab results, measurements and wait times to transfer an average of 25 pages of information per patient, staff members really are left with about eight minutes per day per patient for actual care.

Solution

A medical computing system was needed at patient bedside. It had to be medically-certified, easy to clean, have access to real-time patient information and connectivity to hospital information systems for diagnosis, test/lab results, medication information, billing, and insurance records. The need for the system at bedside was to insure that medical personnel could have access to the data they needed while actually providing patient care. The system would be installed at a service table near the bed so it also had to operate quietly with an intuitive touchscreen interface and comprehensive application suite. The system would also serve a dual-purpose; in addition to being used for decision-support, allowing staff more time to care for patients and improve the quality of care by having relevant data at hand, it would also be used by patients for information and entertainment options. Therefore the system should have infotainment options such as camera, movies, and voice and web communications. MaSanté, a medical-solution provider, developed a medical cart called MonAmi partnering with Advantech to deliver the hardware required to fill the hospital’s needs. Alexandre de Jonge, founder of MaSanté had this to say about Advantech. “They really listened and implemented the ideas of our users in their products. System

MonAmi is a comprehensive service table, computer combo with a sleek look, the result of interviews of personnel in the hospital, and the technical partnering between MaSanté and Advantech. It is based on Advantech’s HIT-W181 terminal, which is high-performance system with an Intel® Atom™ dual-core processor inside. It has an 18.5” multi-function touchscreen, WiFi, RFID, a handset for voice communications, a smart card reader and a 2 megapixel camera and runs Microsoft® Windows XPE Embedded. It has ITE & Medical Dual Certifications (EN-60950 & EN-60601-1) and operates fanlessly and quietly. Patients can access movies, communicate with family, and browse the internet. Doctors and other staff members use identification recognition to gain access to hospital systems, real-time access to lab & test results, and other clinical data that assists them with decision-making, reducing the time required for administrative paperwork, and improving overall patient care.

Blood Lab Information System Increases Hospital Efficiency

Imatis Integrated Healthcare Solutions on Advantech Infotainment Terminals

The Mennonite Christian Hospital in Hualien, Taiwan was founded by missionaries in 1954. It has 539 beds, 990 employees, and offers a full-range of services to the community. The hospital is bordered by the Pacific on one side and mountains on the other, and its reputation spans the entire east coast. It is a premier healthcare provider for Taiwan’s eastern communities with patients coming from the city, from far away villages, and from other more distant cities in the region. The reason for this is due in part to the hospital’s core ethics: it values community and providing caring service more than profit and turnover rates. People often travel a great distance to get to the hospital and cannot return for a second visit. So, the hospital goes out of its way to serve those patients on the spot in a single visit. For the hospital, efficiency and quality are key concerns, and medical personnel were looking for a way to improve response while enhancing accuracy for patients in the hospital’s blood laboratories.

Solution

A software integrator has developed an intuitive, touch-based laboratory information system for the blood lab that works flawlessly with Advantech’s HIT-W151 healthcare terminal. Five of the touch-based systems have been installed in the lab. They are equipped with barcode scanner, IC card reader and camera. A staff member can access the system after positive identification is confirmed with a swipe of an ID card. The patient uses a check-in slip issued by the doctor which is scanned with the barcode reader. This gives lab personnel access to the patient’s record, blood work ordered, doctor’s instructions, and other vital data. HIT-W151 was chosen because it offers an all-in-one solution; it has a slim profile with cable management system that minimizes its space on the desk; and the touchscreen and software application are easy for lab technicians to use. What’s more, HIT-W151 is UL60601-1/EN60601-1 and IP65certified; it is easy to clean, dust and water-resistant, and conforms to medical standards.

System

HIT-W151 is a fanless all-in-one system powered by an Intel® Atom™ D510 processor that uses low-power while delivering high performance. The full-flat 15.6” touchscreen offers a 16:9 natural viewing area. HIT-W151 was attached to a desktop stand at the Mennonite Christian Hospital using standard VESA mounts; both 75x75mm and 100x100mm are available. The cables, conveniently tucked away, kept the desktops clean, and connected the systems via Ethernet to the hospital’s LAN network & HIS system. Options available for HIT-W151 include an RFID reader, a Smart card reader, a barcode scanner, camera, and magnetic stripe reader. The system is used to access patient information, blood testing data, and also has access to the queuing system at the lab’s entrance to keep track of patients in the hospital waiting area.

Benefits

The result of Mennonite Christian Hospital’s project has been good.
