COMMENTARY: Holiday message from 86 AW command team
by Brigadier General Josh M. Olson 86th Airlift Wing Commander To 86 AW Airmen, Team Ramstein, and families, As 2021 swiftly comes to an end, Chief Kelley and I remain incredibly grateful and humbled by what you accomplished during the last 12 months. You fought through COVID lockdowns, shortages of vaccines, remote learning, and so much more. You flexed and adapted to ever-changing COVID-19 regulations and preserved our airlift, airdrop, and aeromedical evacuation missions. You saved lives throughout Europe and Africa using our patient transport systems. Despite months of delay, we still became the most #VaccinatedAF community in USAFE, and provided thousands of vaccines to our local teammates. We achieved Agile Combat Employment Initial Operational Capability,
and we built multi-capable Airmen through our Cross-Functional Airlift Support Personnel (CASPER) program. On top of all of that, we remained ready. We answered our nations call when asked to become a safe haven. We were the U.S.’s primary evacuation hub for a historic humanitarian mission, providing temporary lodging, food, clothing, and medical care to thousands of Afghan evacuees. Watching Operation Allies Refuge/Welcome unfold between August and October left most of us with a renewed sense of service and pride. Our entire military, civilian, and family community exemplified incredible humanity and perseverance by transforming Ramstein into a welcoming transient location for strangers-turned-family. You delivered hope and helped more than 35,000 individuals on their path to a new life. Please take a moment to reflect on your role in history. 2021 was a year of challenges, growth,
exhaustion, and pride. This holiday season, connect with friends and family, decompress, and enjoy some much earned down time. Please be smart and make safe choices. If your holiday plans involve alcohol, please do not drive (or ride scooters!!). End the year on a high note… not with a DUI. If you’re in a bind or experiencing personal hardships…you’re not alone. Please, speak out if you need help or just someone to talk to. Seeking help is a sign of strength, so don’t hesitate to take advantage of the services from our helping agencies (numbers below). Chief Kelley and I are honored to serve alongside this remarkable team. Remember to take care of yourself, take care of your wingman, and have a Merry Christmas and a Happy New Year!

Construction 786 CES will be conducting work on the Mitchell Ave/Harmon Ave traffic circle on Ramstein AB. The traffic circle closure and detour plan is effective Dec. 27-29. LRMC information As you may have heard, due to rising numbers of COVID-19 infections, Germany has revised the Infection Protection Act, which is applicable to U.S. Forces in Germany in accordance with Article 54 of the NATOSOFA Supplementary Agreement. In the revision, workplace access is restricted to vaccinated, recovered or tested employees. In light of the revised act, we want to remind our communities that testing capabilities at military treatment facilities are reserved for official purposes and individuals who present medical symptoms. Unit Leaders who have questions regarding the revised act should reference Force Health Protection Guidance (Supplement 23) which established COVID-19 screening testing for unvaccinated individuals.

Afghanistan Evacuation Operations

Teamwork, elbow grease: 521 AMOW, 436 AMXS Airmen conduct first OCONUS C-5M engine change Story and photos by 1st Lt. Emma Quirk 86th Airlift Wing Public Affairs Maintainers completed the first overseas C-5M Super Galaxy engine change at Ramstein AB, Dec. 6. The 13-day task to change the largest U.S. Air Force aircraft engine was a team effort and took a diverse group of eight maintainers from various duty locations, specialties and backgrounds. The team included a Regional Training Center instructor from the 521st Air Mobility Operations Wing, headquartered at Ramstein AB, an engine technician from the 52nd Maintenance Squadron, Spangdahlem Air Base, and six members of the Maintenance Recovery Team from Dover Air Force Base, Delaware. The team consisted of three flying crew chiefs, two jet technicians and a quality assurance supervisor. Additional personnel from the 721st Aircraft Maintenance Squadron, located at Ramstein AB and hosts for the operation, also performed roles as spotters, observers and alternates, when needed. Tech. Sgt. Michael Cain, 521st AMOW, RTC, credits the group’s diversity in skills as a key contributor to the success of the engine change. Cain’s background on other airframes was his strength. “Each member brought their own experiences and thoughts on how to attack this engine replacement,” he said. “I worked the KC-10 [Extender] for years and the C-5M

engine is just an updated version, so I was able to compare a lot of similarities in the processes.” Along with Cain’s perspective on other airframes, the MRT members with more experience on the C-5M taught him about the engine and explained how all the systems tied together. “I personally love to turn wrenches and to do that on such a rare project was a great experience,” Cain added. Diversity wasn’t the team’s only strength. To lower and raise the original and new engines, the Airmen used an unpowered chains
and crank system, meaning the team relied on thorough communication and muscle power to successfully replace the approximately 13,000-pound engine. Cain augmented an MRT of fellow dedicated maintainers from
Dover AFB, the home station for the aircraft. Airman 1st Class Carsen Segna, the most junior airmen on the team, provided the preponderance prior experience on C-5M engine changes.

Pointing to the large, 14-foot engine, Segna reflected on his experience with engine changes stating the task is, “kind of daunting at first,” and added, “once you get into the motions of everything it just falls into place.” Segna went on to say that this particular engine change had, “most definitely been the easiest and smoothest,” and attributed this to the wealth of experience and knowledge within the team. This was the third operational engine change on the C-5M since the C-5 Reliability Enhancement and Re-engining Program replaced engines, landing gear and other structural elements. During RERP, the C-5 aircraft engines were upgraded from four General Electric TF-39 engines to General Electric CF6-80C2-L1F (F138) commercial engines. The update increased thrust and climb rate, decreased take-off roll, and ultimately allowed more cargo to be carried over longer distances. With new engines and other system upgrades, RERP modified C-5A/B/ Cs became C-5M Super Galaxies. The entire C-5 fleet completed the upgrade in fiscal year 2018. Considering the freezing temperatures of a European winter, the work took place in the largest aircraft hangar outside the continental U.S., which is owned and operated by the 521st AMOW’s 721st AMXS. The same hangar was recently transformed into an international terminal in support of the Afghanistan noncombatant evacuation operation in fall 2021, and was then used to host the Kaiserslautern Military Community’s Annual Ramstein Bazaar in mid-October. The 521st AMOW is an Air Mobility Command wing and has en route nodes strategically located across Europe, Africa and the Middle East to support strategic airlift and rapid global mobility with aircraft maintenance, aerial port operations, command and control, aeromedical evacuation and expeditionary aircrew support in the regions.

Lajes NCO oversees major contract fulfillments Ramstein’s geographically separated unit at Lajes Field, Azores, Portugal, has seen major contract fulfillments in the last year, thanks to U.S. Air Force Tech. Sgt. Dominick Cagle, 765th Air Base Squadron contracting flight section chief. They’re very helpful in everything, no matter what.” When Cagle, a Salt Lake City, Utah, native, isn’t working, he tries to take advantage of his unique location within the Azores, a group of islands off the mainland of Portugal. “I like to see what the island has to offer, like check out the different sights,” Cagle said. “I go on hikes with my dog, similar to the trails that they had in Germany.” Cagle also dedicates much of his time to education. His focus is not only on earning college degrees, but also on furthering his work skillset. He is currently within eight classes of graduating from an organizational management degree program. “Not only is he in school, but he recently got an increase to one of his certifications and one of his key work qualifications,” Ledford said. “We call it a warrant in contracting. It’s basically the purchase authority that we have.” His well-rounded leadership, hard work and dedication to positive growth and development led to Cagle’s recognition by 86 AW leadership as Airlifter of the Week, Nov. 18. “The biggest thing that he does outside of doing his job really well, is taking care of people really well,” Ledford said. “He does the small things that make life easy and life better. It’s one thing to do your job and do it well, but it’s another to do the things behind the scenes and care for your people. That’s rare, and he does both of those things very well.” Cagle has not been the only one to receive an award in his section. While he has been a section chief for the 765 ABS contracting flight, his team has received five individual quarterly awards and two wing-level quarter awards. “Them surprising me for this has obviously been humbling,” Cagle said. “I’m not one for recognition usually of myself. I take the time to always do it for other people because I think it shows them that I care as a leader, and obviously now I feel it coming back my way.” KAISERSLAUTERN

30 Med Brigade Victory Medics support COVID booster rodeo Story and photos by Eleanor Prohaska 21st Theater Sustainment Command Soldiers from the 30th Medical Brigade joined Service Members from Landstuhl Regional Medical Center and Ramstein Air Force Base to administer voluntary COVID19 booster shots to approximately 1,600 individuals at Kleber Gym, Kaiserslautern, Dec. 8. Victory Medics helped manage patient flow, checked paperwork and eligibility, pre-filled syringes and administered shots. According to Sgt. 1st Class Woodrow Wilson, 512th Field Hospital Emergency Management Team non-commissioned officer-incharge, the booster shot rodeo was a good opportunity for 30th MED Soldiers to interact with their LRMC counterparts and build camaraderie, as well as provide an important community service. The FDA has amended the emergency use authorization to allow for a voluntary single booster shot, for fully vaccinated individuals, of fully authorized or emergency use authorized vaccine. According to
the Centers for Disease Control and Prevention, people 65 years and older, 50–64 years with underlying medical conditions, or 18 years and older who live in long-term care settings should receive a booster shot at least 6 months after receipt of a primary mRNA (Pfizer or Moderna) series. People ages 18–64 years at increased risk for COVID-19 exposure and transmission because of occupational or institutional setting may get a booster shot based on their individual risks and benefits at least six months after receipt of a primary mRNA (Pfizer or Moderna) series. People 18 years and older should receive a booster shot at least 2 months after receiving their Johnson & Johnson/Janssen COVID-19 vaccine. According to Lt. Col. William Murray, LRMC, their team is administering about 1,260 vaccines each day. Eligible DOD ID card holders should make an appointment to receive their booster shot. Appointments can be made online at https://informaticsstage.health.mil/COVAX/.

Trevor Derr, 721st Aircraft Maintenance Squadron flight chief, carrying an American flag while running at Ramstein Air Base, but they may not know the reason behind it. Derr’s motivation is his friend, Daniel, who he served with at Shaw Air Force Base, S.C. in the early 2000s. A permanent change of station moved Derr away, but the two Airmen remained close friends and in contact throughout the years. Daniel struggled with post-traumatic stress disorder, a condition that can develop after undergoing a traumatic event, and unfortunately took his own life at 30 years old. “In 2015, I found out that he had been battling things that he had to deal with from deployments and life events back home,” Derr said. “He was seeking help. From what I recall his mom telling me, it just got to a point where he just couldn’t deal with it all.” Originally, Derr began running U.S. Air Force Master Sgt. Trevor Derr, 721st Aircraft Maintenance Squadron flight chief, runs with an American flag at Ramstein Air Base, Dec. 7. Derr originally began running as part of a New Year’s resolution to run 1,000 miles a year. After losing a friend who struggled with post-traumatic stress disorder, Derr shifted his goal to running with the flag and raising awareness for mental health.

He later changed his focus and now runs as much as he can to spread awareness of PTSD and veteran suicide. “I’m just going to run as much as I can for Daniel and try to raise awareness,” he said. “I don’t have a huge platform. All I have is me. I feel that the only way I can do that is to just get out and run, carry that flag, and honor him and the fallen. I run to keep his and others’ memories alive.” Derr has had others from his unit join him on his runs and occasionally has cars honk or people shouting as they pass by, which he said motivates him, but wants others to know him running is more about the cause than it is for him. Right now, suicide and mental health across the military is an important topic. Derr said mental health is one of the most important things to take care of. According to the Defense Suicide Prevention Office, 580 military members ended their own lives in 2020. “[Suicide] is something that occurs way too many times,” he

LRMC incorporates high cure rate procedure for cancer Story and photo by Marcy Sanchez Landstuhl Regional Medical Center Over nearly two decades, Richard Reed has traveled from Germany to Walter Reed National Military Medical Center seven times. These weren’t holiday trips but rather medevacs for the treatment of his cancer. Reed’s treatments required a specialized dermatologist trained in Mohs surgery, which until now, wasn’t available in Germany for U.S. citizens living abroad. Recently, Landstuhl Regional Medical Center reintroduced the service for skin cancer patients, which had been missing for decades. “There hasn't been a Mohs surgeon here for many, many years,” explains U.S. Air Force Maj. Shannon Buck, a dermatologist specially trained in Mohs surgery. “The retiree population that hasn't had access to this to this service is huge. All the (Service Members) in Europe, Africa and the Middle East, would usually have to go back to the (Continental United States) causing a long time away from the fight. Having it here is not only streamlining the process, it gets them back to their job, back to the war fight a lot quicker.” Mohs surgery is considered the most effective technique for treating basal cell carcinomas (BCCs) and squamous cell carcinomas (SCCs), two of the most common

types of skin cancers. The procedure has a 99 percent cure rate for cancers that have not been treated previously. Additionally, the precise technique leaves the smallest scar possible while removing the entire tumor. “The technique has evolved over time, but involves the surgeon cutting out the skin cancer with a
very narrow margin, then processing (the tumor) in a lab which is usually located within the clinic,” said Buck. “The nice thing about the surgery is the way the tissues prepped you can actually see 100% of the margin, which is different than other surgical techniques to remove cancer, and the surgeon is the pathologist too.” As the patient waits for results, a team of health care professionals
work to present the surgeon with sectioned microscopic samples of the tumor. Following analysis of the tissue, the Mohs surgeon can determine if all the cancer cells were removed or if another removal is necessary. Following the full removal of the cancer cells, Mohs surgeons then perform wound repair by either allowing the skin to heal itself or through various reconstruction methods.

Aside from being an outpatient procedure, Buck notes that other benefits for patients include the ability to walk and eat while the tissue is processed, fewer medications required during the process leading to speedier recoveries, and minimal scarring. “Ideally, we're getting cures by doing this procedure of skin cancer, and we're also keeping the scars as small as possible which are usually in very cosmeticallysensitive areas, as most of these cases are on the head and neck,” said Buck. Reed, a retired U.S. Army warrant officer, has had a long history with skin cancer, dating back to the 70s. Although his health insurance covered the costs of lodging and transportation during his first two medevacs, changes in policy required him to bear the costs for following treatments. Additionally, his treatment, as well as many others who undergo the surgery while living overseas, require a three-day stay near the treating facility. Reed notes the procedure in German health care systems isn’t any quicker. “(Going to the U.S. from Germany), we spend three or four days going through that operation,” said Reed, 81, a native of Niagara Falls, New York. “(At LRMC) it's one day, less than one day. It's basically an outpatient visit. I don't have to worry about getting on the plane, I don't have to worry about traveling back and forth. I just drive here in the morning and drive home in the afternoon.” “I think this will be a great thing for LRMC and a great thing for our patients. That's why we're here: to take care of our active duty Service Members, their families and our retirees,” said Buck. “We want to make sure they have high quality care that's both really good, and convenient for them.” Buck and the LRMC Dermatology Clinic are working toward increasing access to care for those in need of Mohs surgery and anticipate opportunities for more beneficiaries soon.

WILDPARK from Page 1 There is a German saying: “Es gibt kein schlechtes Wetter, nur schlechte Kleidung.” It means: There is no bad weather, just bad (or wrong) clothes. While here, take this idiom to heart. Get a good coat, hat, gloves, and walking shoes. Whether misting or snowing, a bit of time outdoors will do you good. German infrastructure makes it easy, too, with trails and forests in abundance. My family’s favorite walk is near our home: the Wildpark in the Betzenberg neighborhood of Kaiserslautern. This free attraction is nestled in the Pfalzerwald (Palatinate Forest), on the east side of Kaiserslautern quite near Kleber Barracks. It’s an easy walk, with some hills, but could also be expanded as the Pfalzerwald has 7000km worth of trails – it even extends down into France! But, that’s another article. Wildpark Kaiserslautern is a small wild animal zoo set up by the city of Kaiserslautern’s forestry office back in the 1970s. It has a beautiful setting surrounded by some of the oldest trees in the area – many over 120 years old. It doesn’t feel like a zoo; the enclosures are massive, giving the animals lots of wild space to wander. Take time to visit the animals Many of the animals at the Wildpark are those you may glimpse while hiking in the area. But, here you can get up close and personal. There are many different species of wild sheep and deer. Over the course of the year, we have watched one buck, in particular, grow his majestic antlers. We’ve named him Magnus, and often bring a few carrots to feed through the fence. Feeding deer and sheep appears to be OK; the other animal enclosures are clearly marked with an “Absolute Feeding Ban.” However, any older food can be donated for the rangers to appropriately use. There is a marked bin for these drop-offs in the middle of the park near the offices – a quick glance inside shows lots of bread and veggies. Watching the wild boar fight over food is hysterical, but also eye-opening. These are the animals that wreck lawns and farmers’

fields with their digging; they are strong! Feeding happens daily from 9-10 am. The most elusive animal at the park is the beautiful lynx. We haven’t glimpsed her at every visit, but when we do we find that she has been watching us for a long time. Her camouflage is so good, it can sometimes take a while to find her lying in the middle of her enclosure beside a log. Preserving extinct wild animals One of the most interesting things about the Wildpark is its mission to help preserve animals that are extinct in the wild. The auroch, for example, is a wild cattle that once roamed all of Europe. There are four wisent, or European bison, that also live at the Wildpark. You may notice their smaller heads when compared to an American bison. Both these types of animals went extinct in the wild during the middle ages, so it’s incredible to be able to see them locally. One of the coolest projects at the Wildpark is its participation in Tarpan backbreeding. The tarpan is an ancient breed of wild horse, which ranged all over Eurasia prehistorically and may be the ancestor to today’s horses. There was still tarpan roaming wild as recently as the 1800s. Scientists have been trying to breed them back in a program started in the 1930’s. They use some of the oldest and most isolated horse herds, like Icelandic or Mongolian horses, to breed offspring that closely resembles the ancient tarpan. The two tarpans at the Wildpark are part of this program. The Wildpark makes for a great walk in the woods, and you can spend as much or little time as you’d like. There are ample benches and picnic tables around if it’s a warm enough day to eat outside. Dogs on leashes are also welcome.
