BREAST RECONSTRUCTION

Breast reconstruction is preformed for several different reasons. It is done after the removal of one or both breasts for the treatment of cancer. It can also be done alone with mastectomy for preventative treatment in women with a strong family history of cancer. In some young women one breast never develops and reconstruction is done to give symmetry to the underdeveloped side. The overall goal in reconstruction is to help a woman feel more natural and more confident in their appearance following mastectomy. There are three main procedures used for breast reconstruction.

ack in the late 1970s and early 1980s, the building at 105 S. Estey Street in Luverne, Minnesota, was the home of Les’s Food Land, then Jubilee Foods. Mike Jarchow once worked at that grocery store, and he probably wouldn’t have believed you if you told him then that he would one day own the building and operate an events center there. Grand Prairie Events Center opened in September in the heart of downtown Luverne, and Mike and his wife, Wanda, couldn’t be happier. They are no strangers to event planning, having owned a tent rental business for 20 years. “Now instead of traveling around to other communities and worrying about things like putting up tents in the prairie winds,” says Mike, “we can focus on providing first-class events right here in Luverne.” The building had transitioned from grocery store to hardware store to a dollar store over the years, but it had been sitting empty since 2010. Now, the 11,500 square-foot building has been transformed to a flexible, functional event space that can accommodate all sizes of groups, from under 50 to more than 500. “We wanted to make it as user-friendly as possible,” says Wanda, noting that the facility can be sectioned off into different rooms to accommodate more than one group at a time. In addition, the building has multiple restrooms, a bride’s room and groom’s room in opposite corners of the facility, state-of-the-art audiovisual technology and Wi-Fi access. Eight different heating, ventilation and air conditioning units ensure that each area of the facility can be heated or cooled as needed. While the Jarchows included many modern updates during the remodeling,

they also incorporated a little bit of history by using bricks taken from the Jarchow’s home, which was built at the turn of the 20th century by A.D. LaDue. They used the bricks taken from the chimney, fireplace and walls while they were restoring the home to create rustic accent walls in the events center. The facility has its own kitchen, and the staff at Grand Prairie Events Center provides groups with a list of licensed caterers who can prepare food off-site and then use the center’s kitchen for final prep and serving. In addition, the Jarchows can help meeting and event planners with suggestions for area photographers and musicians. The Jarchows are committed to the success of each event that comes to their facility and want guests to have exceptional experiences. There is always an Event Facility Manager on site for guests or vendors who have any questions or requests related to the building, parking, equipment or services. “We are all about providing the best service possible, as well as being flexible to meet our guests’ needs,” says Wanda, “so we offer a variety of packages so that we can accommodate whatever people need for a floor plan, furniture, linens, dinnerware, sound system and even staging or a dance floor.” From their remodeled home to this remodeled events center, the Jarchows have completed two labors of love in a community that is at the heart of all they do. “We are so fortunate to live in a community like Luverne,” says Mike, “and we are so excited that our facility can now provide our friends and neighbors with a place to come together and enjoy life’s celebrations.”

M

arge Blaue has always loved sewing. In fact, in an interesting turn on tradition, she actually made a doll for her grandmother, and she was only in the third grade at the time. She has sewed all sorts of things over the years, but a 1999 Better Homes & Gardens magazine changed her life. “I saw this magazine, and it was all about making your own Santa dolls,” Blaue recalls. “I knew right then I wanted to do that.” She read in the article about Judith Klawitter, an artist recognized internationally for her highly-detailed “dressed sculptures.” Klawitter’s work has appeared everywhere from museum collections to the White House. Blaue noticed that Klawitter had a studio in Missoula, Montana, and was holding a class there. “I begged my husband to go on ‘vacation’ to Missoula,” she says laughing. “We went, and it ended up being ten ladies from across the country. It was like a slumber party for 50-year-olds!” The class really started her Joyful Friends journey, as Klawitter taught them to use special sculpting clay to make faces, give them dimension, wrinkles and even the tell-tale twinkle in Santa’s eye. She began creating at home, using the trusty Sears Kenmore sewing machine her parents had given her as a college graduation gift in 1971…a machine that lasted until just last year. “It was the best machine,” she says. “I didn’t know what I was going to do. I remember driving down Kiwanis Avenue saying a prayer… ‘God, help me find another Sears Kenmore.’” Blaue went into the Goodwill store, and there on a shelf was a Sears Kenmore sewing machine for twenty-five dollars. And, while she also has an industrial style sewing machine, the Sears Kenmore is still her favorite.

Every creation is different – from her dressed Santas to her “Diva ladies.” She sculpts the faces and hands, and then sews all of the outfits. She’ll use a variety of ribbons, bows, tassels and other accessories to customize each piece. Sometimes, all it takes is spotting a piece of jeweled ribbon, and she has an idea for a new Santa….or a dollar-store bow sparks a theme for a new Diva. I love finding fun, funky things,” Blaue says. “I am always scavenging and hunting. I find something and think, ‘I can use that somewhere.’” She creates sitting Santas, standing Santas and even Christmas stockings with Santa faces tucked in among fur, fabric and other “glitz.” Each one is beautiful and unique, with suits ranging from whimsical in non-traditional colors and materials, or luxurious with sparkle and fur. Her Diva ladies are definitely characters. While each one may start with the same basic pattern, that is where the similarities end. There are Divas for all seasons. One Christmas Diva holds a sign that reads, “Wanted: Jolly Men Bearing Gifts.” The

Pinot Noir lady is dressed in purples and holds a wine glass. All of them are dressed perfectly from hats to shoes (which are actually Christmas ornaments, but that’s the only trade secret we can reveal here!). Blaue will even create Divas that are sports fans, and she has dressed many a Diva for Nebraska, Iowa or Minnesota Vikings fans. In other words – note to holiday shoppers – she does a lot of custom work, both for her Divas and for her Santas. While she loved being able to do all of this creating at home, she realized she was becoming engulfed with all of her materials. She found a little bungalow just off Minnesota Avenue, and now she has room for shelves of ribbon, racks of material and tubs of accessories. Plus, the front room provides a showroom of sorts, where she has creations on display using some of her own family heirlooms like the china hutch from her aunt, the trunk her dad used after military service, and the round oak table that her family used growing up. “This is MY playhouse!” she happily

exclaims. Blaue travels to 7 or 8 shows a year with her creations, which are classified in the genre of “art dolls.” She will be at the Autumn Festival here in Sioux Falls at the end of October, and she will have items on display at Young & Richards floral shop downtown for the holiday season. She thinks back to when this all started, and it was just because she saw that Better Homes & Gardens and became an I’m-agunna. What’s that you ask? “You know,” she says, “when you see something you think you could make, so you say ‘I’m a-gunna do that!’” She started Joyful Friends at the age of 50 so, she adds, if you have an idea, just go do it. Blaue’s biggest challenge is that her brain is always running. “I have several unfinished projects at home because, as soon as I start one, I’m already thinking about the next one.” A Joyful Friend from Marge Blaue is certain to bring happiness to any home. ots of people move every day. Sometimes a move takes you to a new town or even a new state. For one Sioux Falls family, a move last March took them…ten blocks. While the distance wasn’t significant, any move is a big deal. You have to pack up at one place and move it all somewhere else. But, it’s also exciting to think about all of the things you’ll do and

see there’s a new movie out called Sausage Party, but I can’t bring myself to spend fifteen bucks to watch an hour and a half of anthropomorphized food, and from the reviews I’ve seen, there’s enough foul language to make even the heartiest of souls blush. Maybe I’ll get around to it when it’s on Netflix and I’m caught up on the rest of my queue, which probably means never. But the title got me to thinking about the great category of food that is encased meats. Sausage is one of those foods that just about every country and every culture has in common. The varieties of sausages from around the world – from spicy Moroccan lamb merguez to chunky Cajun andouille to a Chicago Style hot dog with bright green relish and sport peppers – there is a sausage to please just about every carnivore on the planet.

The Humble Frankfurter The hot dog, it’s about as American as it gets. But the simple sausage that’s a staple for kids’ menus is also a favorite of mine. But like most things in life, there are good and bad examples; the hot dogs in the grocery store that are part chicken, part pork and part mystery meat are not the best examples. Stick to the all-beef franks; I like Nathan’s Famous Jumbo Restaurant Style. A little bigger, and the skinless dogs pack a lot of flavor. You may have seen the numerous products on the internet that claim they will change the way you eat hot dogs by spiral cutting or crisscross cutting using a special tool (that they are happy to sell you for just $9.95 plus shipping and handling!) But save your money, first of all, you can do the same thing with a knife. The

reason they say this makes the hot dog better is that it creates more surface area on the hot dog, which gives you more area to get caramelized when it’s grilled. But here’s a secret, you can get the same results by simply butterflying the hot dog. Slice it down the middle, leaving the two sides attached. This doubles the amount of area that comes in contact with the grill. (See Figure A) When the dog is cooked and placed in the bun, you can put your condiments of choice right in the crevice. (See Figure B).

Italian Sausage I love Italian sausage. There is something magical about the blend of sweet fennel and spicy of red pepper flakes. Links are great just grilled low and slow and served on a bun with roasted red peppers and sautéed onions, or browned on top of pizza. But my favorite thing to do with Italian sausage is a grinder. Browned sausage cooked with onion, tomato sauce and peppers, served on an Italian roll topped with melted mozzarella. That is heaven on a bun.

Bratwurst October means tailgating and that to me means bratwursts. While the sausage we know as a brat traces its roots to Germany, it’s really the good folks from Wisconsin who made the bratwurst popular here in the U.S. There’s a lot of debate about how to properly cook a brat. There are those who swear it must be simmered in beer, and those who just grill it. But even among

the grilling crowd there are some who want it cooked without breaking the skin, and the guys who pierce the skin to get let some of the fat out. I’m in the piercing camp, a few little holes to let out the fat as it renders is my preference, and I don’t want the grill too hot. When it’s done just right the fat will help to crisp up the skin as it cooks, giving a nice textural contrast to the juicy meat inside.

Cajun Boudin Boudin (pronounced boo-dan) is sold at just about every roadside stand in Cajun country. Drive just 75 miles from Lafayette, Louisiana to Lake Charles on Interstate 10, and you’ll see dozens of billboards proclaiming the best boudin and crawfish around every exit. But ask for boudin at the mega mart in South Dakota

and you’re likely to come up empty handed. It’s a spicy mixture of rice and pork in natural casings. I get my fill when I’m down there hunting ducks each winter. If you get a chance, give it a try. That’s only a few of my list of notable sausages. No matter where you’re from or what language you speak, sausages help to link us all together. Everyone has their favorites; everyone thinks their wurst is the best. Do yourself a favor, eat something good today. company’s machine breaks down. The company’s owner, an old school chum of Niels Bohr, calls in the physicist for help in fixing it. Bohr examines the machine. He draws an “X” on the side and says, “Hit it right here with a hammer.” The company’s mechanic hits the machine with a hammer. It springs into action. The company’s owner thanks Niels Bohr profusely and sends him on his way. A few days later, the owner receives an invoice from Bohr for $10,000. Shocked, the owner phones Bohr. “Niels! What’s this $10,000 invoice? You were only here for 10 minutes! Send me a detailed invoice.” Bohr agrees to send the invoice. A few days later, the company’s owner opens a new invoice reading: Drawing “X” on the side of your machine $1, Knowing where to put the “X” $ 9,999, Total $10,000. Knowing the value of your work is important. As a Level I Sommelier and a Certified Beer Server, I’ve gained some knowledge that allows me to know where to draw the “X” on a wine list (I know I’m comparing myself to a physicist, but there are some physics involved in opening a bottle of wine!) The following should help you out, hopefully in the near future, when you are wondering what to order from a wine list.

1. ASK. Sounds like simple advice, but one of easiest ways to discover something new is to ask your server or bartender. Asking a simple question like, “What would you drink?” will illicit a few choices for you to ponder over.

2. POINT. A technique that works really well when you’re looking for a suggestion is to hold out the wine list to the server and point to a wine that’s in your price point and then ask, “What would you recommend that’s similar?” Your server will know what price range to stay in and will recommend something you didn’t even know you were looking for.

3. SHARE. Share with your server wines you usually drink so that he or she can make an educated recommendation. “I usually drink KJ Chardonnay, what would you recommend?” or “I drink Louis M. Martini Cabernet all the time, I’m looking for something different.”

look around, lean in, and say, “I actually have a Grenache that we just brought in and hasn’t made the list yet. I’ll go get it for you.” A lot of restaurants will have wine in stock that does not appear on their wine list. Use this technique to discover a restaurant’s best kept secret. Another version of this, if you’re only looking for a glass, is to ask if the restaurant has any open bottles that are not on the By-The-Glass list. You might be in for treat if your timing is right!

6. SERVERS KNOW BEST. “What grape or region are you excited about right now?” is a great question to ask your server. Servers and bartenders will typically know what’s hot right now, or what new wine they just tasted at a pre-shift meeting, or what region is releasing wine from a stellar vintage. Servers and bartenders are your liaison to making a good wine decision at a restaurant. Use your tableside resources! In the ever-expanding world of wine, it is helpful to have some of these tips and tricks tucked away when you’re out to eat. Don’t

be afraid to use them. Remember, the more questions you ask, the more suggestions you’ll get. My most favorite question I was asked in the restaurant business was, “What do you recommend tonight?” One time, as a joke, I said, “The most expensive bottle on the list.” The gentleman, without missing a beat, said, “Great, we’ll have that.” One of my most memorable, and easiest, wine sales ever. 1 1/4 cups margarine, softened, divided 1 1/2 cups brown sugar, divided 3 1/2 cups old-fashioned oats, divided 1 cup flour 1 can (14 ounces) whole-berry cranberry sauce 1/2 cup finely chopped pecans or walnuts

Mix pumpkin, oil, sugar and eggs until light and fluffy. Sift the dry ingredients together and mix thoroughly with the pumpkin mixture. Bake at 350Ëš for 25-30 minutes in a 10x15 inch jellyroll pan. Frost when cool. Serves 24-36 depending upon how they are cut.

Cream 1 cup margarine and 1 cup brown sugar and stir in 2 1/2 cups oats and the flour. Press this mixture into a greased 9x13 inch pan and spread the cranberry sauce over the top. Next, melt the remaining margarine and stir in the nuts, remaining brown sugar and oats. Sprinkle over the top of the cranberry sauce and bake at 375Ëš for 25-30 minutes or until lightly browned. Dense breasts do not look or feel differently than those which are less dense.

M

illions of American women face a potential risk for breast cancer because of how their breasts are composed. They cannot feel the risk, but it is there, and it can make breast cancer harder to detect. Almost half of American women in the 40-74 year-old age group have what is called dense breast tissue. The breasts are made up of milk glands, fatty tissue and dense breast tissue. Dense breasts

do not look or feel differently than those which are less dense. That is part of the challenge for cancer-fighting physicians like Josie Alpers, MD, a mammography and radiology specialist with the Avera Medical Group Radiology. “Dense breast tissue is an independent risk factor for breast cancer. Research shows it increases risk for breast cancer 1.5-2.5 percent,” Alpers said. Alpers explained that physical exams

cannot determine the density of breast tissue and that women with less – or no – dense tissue can still get breast cancer. Mammography is the only way for physicians to tell whether or not the tissue is dense. “It is more common in younger, thinner women. However, studies show that gaining weight and going through menopause actually increases a woman’s risk for breast cancer,” she said. “Dense tissue is normal. It’s not a disease, but it can make it harder to detect

cancerous tumors.” Shades of gray make up the appearance of a mammogram, and the shades indicate the types of tissue that make up the breast, Alpers said. Fatty tissue appears as dark areas, while dense tissue will appear as lighter area. Tumors also show up as light – or nearly white – areas, which can lead to difficulties in pinpointing their locations. “Since tumors display as white on mammograms, much like dense tissue, the tumors in that tissue can be hidden,” she said. “But with the advent of digital, and now 3-D breast tomosynthesis mammography, cancer can be better

differentiated from dense tissue.” Another tool Alpers can use is contrastenhanced spectral mammography, or CESM. Using this method allows her and other Avera cancer physicians to detect the tumor, even in situations where the dense breast tissue makes it hard to see. “Contrast-enhanced spectral mammography allows us to avoid false positives, and it works based on the physiology of the breast,” Alpers said. “It’s important for all women to realize that we don’t do additional testing in the case of dense breasts. The American College of Radiology doesn’t recommend additional

imaging, especially for women who have average risk. But for women who have an abnormal screening mammogram, a gene mutation, or have a distinct family history, contrast-enhanced spectral mammography and MRI are options. MRIs are more costly and time-consuming than contrastenhanced spectral mammography.” Alpers recommends any woman with questions about their breast density discuss it with their doctor during their next mammogram appointment. “Breast density is – like many things – a single factor, one to be considered closely, but not a dire warning,” she said.

alloween is fast approaching – have you mentally prepared for navigating this holiday in which your kids will be amped for a sugar rush and you’ve got the added pressure of delivering great decorations, costumes and candy at your doorstep? Well, there are several ways to have a successful Halloween without sacrificing health and safety. Try these on for size.

Candy in Moderation Here’s the crux of staying healthy and fit amidst a holiday built around gathering as much candy as possible – enjoy in moderation. A good rule of thumb is to have your kids select their three most-wanted pieces of candy once they return home and save the rest for later. You, as the parent, can set the tone by following the same rules.


Trick-or-Treating Ground Rules You’re the best judge of how old your kids should be before they can head out on the trick-or-treating trek alone, but it’s always a good idea to keep an adult close by when wandering around the neighborhood at night. Make sure you walk through the rules with your kids before they head out so they know how to navigate traffic, unfamiliar houses and sticking with the sidewalk. It’s a good idea to pack a flashlight, a phone or some other form of contacting you for help and information with your name and address in case your child gets lost and asks someone for help.

Pumpkin Precautions Carving jack-o-lanterns is a pretty important part of many people’s Halloween celebrations, but when it comes to involving your kids, make sure you’re keeping an eye on those cutting utensils. Most pumpkins have enough give that a fairly dull or kid-friendly carver is totally appropriate (maybe skip the highend cutlery). And when it comes to fire safety, make sure you adequately clean out the innards of your pumpkins and try swapping out your candles for flameless tea lights or glow sticks.

De-Complicate the Costume Remember that old adage, “take one thing off before leaving the house?” Maybe implement that when it comes to cumbersome Halloween costumes. Skip the clunky costumes that could cause your children to trip and injure themselves like long robes or capes, and avoid heavy masks that could hinder breathing. (And some reflective tape never hurt when it comes to being noticed by drivers.)

Post-Halloween Fitness

Another good route is to feed them a filling, nutritious meal prior to trick-or-treating to curb their appetites for later. (Plus, there are ample opportunities in Sioux Falls for your kids to donate their candy to charities or food banks – have them take a handful of their favorites and then do some good for their community with the rest.)

It’s inevitable sometimes – the trick-or-treating fest could end with a pretty substantial bellyache among your kids (and possibly even yourself). Here’s a handy trick to get back into the swing of things post-Halloween – make Nov. 1 a day of family physical activity. This year it falls on a Tuesday, so plan on taking a family outing to the park after school or the tennis courts after dinner. (Or maybe even bring a picnic with healthy fruits and vegetables and lean proteins to help your bodies recover from that sugar rush.)

Q-Tip Skeleton Materials: Q-tips, black construction paper, white paper, black marker, scissors, and glue. Instructions: On white paper, use a black marker to draw a skeleton face. Cut out the face and glue it to some black construction paper, then glue Q-tips to the black paper to create the bones of the skeleton. Cut the Q-tips to make shorter bone parts, such as fingers and toes.

Pillowcase Trick or Treat Bag Materials: White pillowcase and fabric paint. Directions: Using fabric paint, paint on various designs to your white pillow case and let it completely dry. You can make the bag look like a ghost, a pumpkin, or decorate it with a lot of different Halloween themed rubber stamps your kids helped you pick out. Write their name in stencil on their bag and make it unique for them. They can drag it around as they collect their candy and look adorable while they are at it!

Paper Bag Frankenstein Materials: Paper lunch bag, black and gray cardstock paper or a black marker, scissors, green paint, tacky glue, googly eyes. Directions: Paint the front of your paper bag with green paint. Let it completely dry while you cut out pieces of cardstock paper that will make your Frankenstein’s hair, mouth, scar pieces, eyebrows, and bolts. Use tacky glue to attach your pieces onto the paper bag. Make sure to keep the bag open while you do this. You can also choose to draw on the facial features with a black marker.

Balloon Printed Spider Materials: Small balloons, paint, small paper plates, white paper for your canvas, fine tip black marker, small googly eyes, and glue. Directions: Blow up the same number of balloons as colors of paint you want to use. Pour a small amount of paint onto separate paper plates, then dip the bottom of each balloon into paint and make sure to completely cover it. Press the balloon down onto your paper, making a round shape. Repeat this with different colors on the same piece or different pieces of paper. Let your paint completely dry, then glue several various sizes of googly eyes onto the bottom of your circles. Use a fine point black marker to draw spider legs and a string coming out from the top of the circles.

Paper Plate Spider Web Materials: Paper plate, white yarn, bobby pins, hole puncher, and a fake spider ring. Instructions: Punch holes evenly around a sturdy paper plate. Attach and tie off one end of the white yarn through a hole in the plate. Attach a bobby pin to the other end of the yarn, then weave it through the hole moving across the plate and back, tying off the end of the yarn when it runs out. Cut the ring portion of the spider ring in the middle and attach it to the white yarn web, so that it looks like a spider is hanging out in the middle of the web.

yker Kolbrek and his family are no strangers to hospitals. In his first two years of life, doctor offices and visits were all too regular. “He was sick quite often,” explains his mom Misty. “He had had a severe case of RSV the year before and had a few other health problems, but never anything like this.”

Ryker contracted a viral illness, which made it difficult for him to breathe. He was taken to Sioux Falls and placed under the care of Lesta Whalen, MD, a pediatric critical care specialist at Sanford Children’s Hospital. “Dr. Whalen said they would do some tests and that we could go get some rest,” says Misty. “Worst-case scenario he would have

Have a wickedly fun fashion show to show off. Decorations are a must if you want to Halloween in style. Steadfast spooky sounds and lights are always crowd pleasers. kind of scare. Make a new scarecrow or if you are one that likes the horror side of Halloween, visit the Halloween shops for frightening decorating ideas. Lastly, you need food. Lots and lots of tasty, tantalizing, sweet and even kinda creepy things to eat. A fruit kabob as a center piece set into a freshly-picked pumpkin adds a nice touch. Fruit and vegetable trays that take shape as skeletons. And make your own pizzas. Make sure to have all kinds of wacky topping ideas, so your guests can make a monstrous design. Don’t forget the dry ice for your bowls of punch and the ice hands. Try hot dogs dressed up as mummies, deviled eggs

complete with olive spiders, and of course, decorated cookies. Your pumpkin party is almost complete. You need to send out festive invitations complete with a list of what people need to bring. Plan a whole day to decorate and invite your guests to stay. Pick out plenty of music that sets the mood and maybe even a movie or two while you wait for paint and glue to dry.
