women are not aware of their breast reconstruction options following mastectomy

BREAST RECONSTRUCTION Breast reconstruction is performed for several different reasons. It is done after the removal of one or both breasts for the treatment of cancer. It can also be done alone with mastectomy for preventative treatment in women with a strong family history of cancer. In some young women one breast never develops and reconstruction is done to give symmetry to the underdeveloped side. The overall goal in reconstruction is to help a woman feel more natural and more confident in their appearance following mastectomy. WE CAN HELP!

Bread and Circus Sandwich Kitchen:

No Clowning Around, Just Good Food BY MARY MICHAELS | PHOTOS BY JULIE PRAIRIE PHOTOGRAPHY

his quote from ancient Rome reflects a time when government kept the populace happy by distributing food and staging huge spectacles…all while government essentially did whatever they wanted. Long-time friends Barry Putzke and Jordan Taylor named their restaurant Bread and Circus Sandwich Kitchen at the suggestion of a friend, who related the quote from Juvenal to taking bread and then “doing whatever they wanted” to create unique sandwiches. The opening of this restaurant was about 20 years in the making. Putzke and Taylor became friends while attending Roosevelt High School in Sioux Falls. After graduation, Putzke was on a finance career track, and Taylor attended culinary school. While each was on his own path, they both ended up in Portland, Oregon. Taylor’s passion became the farm-to-table food experience he

had on the west coast, and it wasn’t long before he and Putzke were crafting a plan to come back home to South Dakota and share what they loved about Portland. “We really wanted to create a comfortable, casual environment where we could focus on handcrafted food using as many local ingredients as possible,” says Putzke. The restaurant is located on the street level of the historic Dakota Moline Plow Company Building on North Main Avenue. The space previously housed a restaurant, so they didn’t have to completely start from scratch. A local woodworker helped craft pieces like the front counter, bar top, wall art made from twisted Juniper, and the sturdy handle of the over-sized, South Dakota-shaped cleaver that hangs on the white subway tile wall near the front door. Artist Mitch Torbert

created a 40-foot, wall-length mural depicting a lush green prairie and a woman at a fishing hole in the middle of the scene, which certainly illustrates the farm-to-table feel of the menu. While there are many regular items on the menu, the restaurant also features daily specials and seasonal selections as various produce becomes available. They put their own twist on more “standard” sandwiches like the cast iron steak, the Italian, and their two-patty burger (with bacon and tomato jam!). But then they have also crafted some of their own flavor-filled combinations, such as: pork carnitas with lime and black bean puree, avocado, Oaxacan cheese and pickled radish; the Porkstrami with pork belly, beer-braised sauerkraut and fontina cheese; Ahi Tuna Tartine, with seared #1 Ahi, Ratatouille, basil chimichurri and baby greens served open-faced on a ciabatta

roll; falafel with chaat spiced raita, hot sauce and romaine on a pita; and the very popular Vietnamese Fried Chicken, with nuoc cham, bahn mi pickles and cilantro aioli on a potato bun. Sandwiches are served with kettle chips, but you might want to try their fennel slaw, or the one item that seems to have garnered the most attention for the restaurant so far - the Naughty Fries topped with chili pork verde, taqueria pickles, queso fresco, cilantro and radish. Bread and Circus Sandwich Kitchen has a house salad and entree salads on the menu, such as the Moroccan Chicken Salad, but Putzke says any of their sandwich offerings can also be served as a salad. And, they always have menu items just for kids who might not be keen on tuna or pork belly. So after a successful first summer, what’s next? Winter will likely bring some heartier selections and additional soups, says Putzke. They also hope to expand what is in the deli case near the front counter, such as house made sausages or other items that downtown residents, employees or visitors could “grab and go.” Putzke says they have already had some special events and will continue having random themes or events, perhaps transforming into a Taqueria for the day with a custom menu or offering a familystyle dinner. So, he says, the best way to stay up to date is to connect with Bread and Circus Sandwich Kitchen on Facebook and Instagram (mouthful_of_sandwiches). Even as the days get cooler and customers move from the patio seating indoors, they can enjoy a great sandwich with a beer (bottle and draft) or a glass of wine. A row of clipboards on the wall keeps customers informed of what’s on tap, featuring several local and regional beers. And, happy hour is Sunday through Thursday from 3-6 p.m. with specials on wine and draft beers. Come enjoy this culinary circus in Uptown Sioux Falls!

She Said Yes to the Dress Ten Decades of Bridal Fashion on Parade

Saturday, November 4th • Trinity Lutheran Church in Vermillion

P

icture this: 25 brides…and 1 groom. Admittedly, the ratio is unusual; but in this instance – She Said Yes to the Dress, a historical wedding dress fashion show – it works perfectly! Organized by the Theta Omega Chapter of ESA (an international service organization), this third annual event is a fundraiser for Relay for Life. Proceeds from the show will be used to fund vitally important cancer research as well as patient education/support. A narrated parade of wedding gowns that represent ten decades of bridal fashion will captivate assembled guests. This year, the oldest gown dates back to 1927. A family treasure, this silk dress adorned with ribbon embroidery exemplifies the Flapper fashion relished by young and flirty women of the Jazz Age. It definitely evokes images of F. Scott Fitzgerald’s The Great Gatsby! The final gown in the show will introduce international flair and vibrance. An Ao Dai bridal ensemble which honors Vietnamese tradition, this stunning gown with matching hat features a rich red and gold color combination; in the Asian culture, red represents

good luck, while gold symbolizes royalty. Two local celebrities have generously agreed to support this event. Morgan Peck Kronaizl, Miss South Dakota 2009, will showcase her wedding gown from 2015. Morgan’s personal bridal journey is extraordinary; it is guaranteed to impress and amaze. Vanessa Gomez, TV reporter for KSFY, is lined up to model in the show; she will breathe new life into a lovely rose-colored attendant gown from 1945 - a charming example of World War II bridal fashion that definitely exudes nostalgia. To keep their event fresh and interesting, the dedicated women of ESA continually brainstorm and inject innovative ideas. This year, several new elements will be introduced, such as the memorabilia display. Participating brides will share wedding mementos; the table exhibit will encompass a diverse array of items, from invitations to cake toppers and beyond. Reflecting the era in which they are situated, these bridal artifacts will surely inform, entertain, and delight.

In addition, the venue entryway will feature a ‘Salute to Street Dresses’ – those church/street dresses that did double duty as wedding dresses. Needless to say, these wonderful dresses deserve their moment in the spotlight, too! One of these dresses dates all the way back to the turn of the 20th century. This late-Victorian, sage green dress features puffed sleeves – so representative of that era – with lovely pink lace trim on the fitted bodice and cuffs. Following the fashion show, guests will be treated to a reception – just like a real wedding. And of course, wedding cake is a must! This sweet indulgence will be served, along with homemade mints (a wedding staple of yesteryear, but a rarity these days), nuts, punch, and coffee. A time of chatty conversation with friends, this gesture of wedding hospitality presents a perfect way to round out the event. For more details about this year’s participating brides and models, turn to Facebook. The organization’s page (ESA – Theta Omega) is full of fun, newsy tidbits. And be sure to check out the Facebook event for this show as well.

She Said Yes to the Dress will be held on Saturday, November 4th at Trinity Lutheran Church in Vermillion (a mere 60 miles south of Sioux Falls, this short trip is well worth it!). The doors will open at 1:00 pm, and the show will commence promptly at 1:30. Tickets for this “can’t miss” event will be available at the door ($15 for adults and $13 for students/children). Plan to arrive early and enjoy this memorable experience…it is a delightful event for an exceptional cause!

When the

“FIERY CROSS” Came to Sioux Falls BY RON ROBINSON

The modern KKK emblem. The cross has always figured prominently in the organization’s symbolism.

I

t made “a very picturesque scene,” wrote the Argus-Leader reporter, Ralph O. Hillgren, of the lighting of the 20-foot high cross, supported by wires, in an alfalfa field on the bluffs southeast of the city. The cross, Hillgren wrote, “was studded with flares, as was also the K. K.K. emblem,” and the event was accompanied by a fireworks display, at which, apparently, the Klan excelled. Hillgren, who was later an Argus-Leader (hyphenated at that time) state editor and state legislator and still later a journalism instructor at Augustana College, had witnessed an oration by a national officer of the Klan in a gully below the hill as “an airplane with a huge lighted cross circled over the meeting.” The dramatic event took place on a Saturday evening, May 31, 1924. It was billed as the “first state gathering of the Klan,” and was attended by an estimated 3,000 persons. Hillgren scoffed at its effectiveness, however. “Spectacular and accomplished in the Klannish art of setting off fireworks, but absolutely false in the greater numbers of its arguments was the verdict which interested witnesses rendered,” Hillgren wrote. Citizens of northern states of this nation often take on an air of superiority in the midst of news about renewed domestic terrorism and violence in the South against people of color, immigrants, or almost any non-white, non-right “other.” In South Dakota there is certainly no need to feel above the fray. The state has had its share of conflict along those lines — at Wounded Knee, for example — and its share of righteous people who have fallen for the lure of such abhorrent hate organizations as the KKK. Sioux Falls in particular has been the focus of recruitment campaigns for such groups. The appeal of such organizations is especially strong for those who already have swallowed the totally false tenet that white Europeans wrested the country from the savage “redskins” and

HISTORY

therefore have every right to claim sole ownership of it. Present in the background at every display of the fiery cross is the American flag. And by espousing such ensconced maxims as free speech, free press, and “law and order,” the KKK and other hate groups disguise their real agenda: driving out, by fright, by decree, or by force, people of color, non-Christians, non-protestants, socialists, and communists. The fiery cross has already been quenched at least twice. Confederate General Nathan Bedford Deforest, then the Klan’s Grand Wizard, declared the nebulous band abolished in 1869 and ordered its white robes, masks, and pointed hoods to be destroyed. The Klan continued, nevertheless, as a kind of ghost of its earlier existence, in small groups scattered across the South, doing whatever it could to intimidate or eliminate black legislators, Republican “carpet baggers,” and other pet shibboleths. The burning cross, however, seems to be something that the novelist Thomas Dixon dreamed up, or at least gave undue emphasis, in his book and play The Clansman. In his 1915 movie based on Dixon’s work, The Birth of a Nation, D. W. Griffith made the KKK his heroes, portrayed blacks as rapists, devils, and dolts, and reignited the blazing symbol of righteous hatred. At Stone Mountain, Georgia, in the same year the movie was released, the Klan was officially reincarnated. By the early 1920s, the Klan had reared its ugly head and raised its fearful symbols in Sioux Falls. An edition of the group’s official organ, The Fiery Cross, appeared shortly before the rally Ralph Hillgren described. The Klan publication claimed that the first Sioux Falls meeting actually occurred on May 19, 1924. The organization had procured the farm upon which the meetings were held because it had been kept from having a meeting in the Sioux Falls Coliseum. The four-

page, “blanket” sized paper was filled with reports about groups organizing throughout North and South Dakota, claiming large memberships In Mitchell, Huron, Highmore, Pierre, Wessington, and Iroquois, and estimated that the Black Hills membership numbered 10,000. That all could be hogwash, of course, or as the phrase goes today, “fake news.” What is factual, however, is that the organization was actively recruiting in the state and that at least a certain number of persons with ethnic or religious grudges responded by signing up. Protestant ministers were especially sought, since the later bugaboos for the Klan were less blacks than Roman Catholics. The Argus-Leader cited the first Klan foray into the state to be in July, 1921, and Ralph Hillgren reported that the minister of a Methodist church on the east side of Sioux Falls, the Rev. William Allan, was called upon to outline the current goals of the Klan at the 1924 meeting before the appearance of the main speaker, who was assumed to be Dr. Hiram W. Evans of Indianapolis, Indiana, Imperial Wizard. (He was “assumed to be” because he was never announced by name, although Hillgren spotted the car he arrived in, which had Indiana license plates.) Evans championed “Nordic Americans,” a factor that may have had some leverage among the Scandinavian Americans who populated much of the region and near Sioux Falls. In September 28, 1924, according to the Argus-Leader, a parade of some 500 robed and hooded Klansman marched down Main Avenue as a show of force. After the KKK campaign, Sioux Falls seems to have boasted a “Klavern” of its own, supposedly “number sixteen.” Evidence of its existence was revealed much later when regional historian and lawyer J. Bruce Blake dug up photographs of Klansmen praying on

their “Kampground” and marching at Woodlawn Cemetery at the burial of one of their members. By then the official Kampground had moved about the eastern fringe of Sioux Falls several times as the group sought a more-or-less permanent meeting place. The KKK faded during WW II, possibly because the country was relatively united against Nazism and Fascism, both of which much resembled the Klan cause, but following the war it flared up again. The appeal to righteous hatred of “others,” while abrasively obvious in clashes with protestors and torchlight parades, is insidious in its subtle and nuanced arguments and its alliances with patriotism and Christianity. Hate speech is protected as free speech under the First Amendment to the Constitution, a fact that should not be derided or denied by those who oppose such groups. But hate speech is often a prelude to hate acts or excuses for racial oppression in the past, things that cannot be tolerated by American democracy. When in 1994 young sympathizers of the KKK burned a cross in Aberdeen, native American columnist Tim Giago complained that it was so little reported in the state’s press. The Argus Leader has recently cited Klan activity around South Dakota, and politicians in the state are curiously quiet about the renewed clashes with hate groups in the South. One imagines that white robes and hoods must be folded carefully away, mothballed, boxed, and mouse-proofed, in back rooms and attics across Siouxland, ready to be donned again in response to the right charismatic leader or an irresistibly moving cause. Ron Robinson in is a member of the Minnehaha Century Fund, whose mission is to preserve and renovate historic sites and to educate citizens about local and regional history.

O

nce upon a time (in 1998), an old farmhouse was moved to a new setting on six-acres near Baltic, South Dakota. The property is located away from well-traveled roads, surrounded by mature trees and offers picturesque views of surrounding hills and farmland. And then, 12 years ago, Jimmy Schad bought the property with a notion to redo the old farmhouse “someday.” For the next several years, Schad planned and sketched and filed away ideas for his dream project. He had been in the coffeeshop business and, when an opportunity came around to sell that business and focus on his house project full-time, he took it. His “labor of love” started three years ago when he stripped the old farmhouse down to the two-by-fours of the exterior frame. “There was good structure there,” says Schad, “and it gave me a foundation to create what I wanted.” The creative process included taking out the original staircase leading from the main floor to the second story and building a new one in a different place, doubling the number of windows in the house to provide spectacular views in all directions, expanding the garage from a two-stall to a four-stall (and heating it), adding dormers on the front of the house and creating an impressive 25-foot entrance with cedar columns and a roomy front deck. But that is hardly the extent of the work Schad put into the house, primarily by himself. He framed all the new additions, did drywall, interior woodwork, built the decks and more. Schad says he did call in a little help for projects like plumbing and electrical work, as well as the detailed exterior shingle siding. With the project nearly completed, it

is a little bittersweet for Schad, his wife Natalie and their three young children. The family, who has been living in Sioux Falls during the building process, will be moving to Rapid City. “My dad is a homebuilder, and I learned a lot working for him before we came over to the eastern part of the state,” says Schad. “I have a great opportunity to go back to Rapid City now and work with him again. It’s going to be really hard to leave this house when we haven’t even lived here, but this move is the right thing for our family.” In the meantime, though, he is still putting a few finishing touches on the house as Ulmer & Barth Real Estate, part of the Keller Williams Company in Sioux Falls, get the house on the market.

Walking through the main door and looking up, it is 23 ½ feet up to the ceiling. Schad created levels and dimension throughout the house using archways and cosmetic beams wrapped in drywall. He used Knotty Alder for the interior doors, columns, trim and cabinets. The dark wood has a rustic look, but it can very easily function in any type of decorating style. The wood floor that covers much of the main level is the original oak from the farmhouse. Schad pulled up the inch-anda-quarter boards, cleaned and refurbished them, and then put the boards back down. He added visual interest by using light and dark hickory boards laid diagonally in the kitchen. The lucky homeowners-to-be will have

Between the kitchen and the dining room are a handy utility sink and doors that lead out to the back deck overlooking the yard and the fields beyond. Schad used every available space as he was crafting the home, whether for sitting areas, closets or other “nooks” that could be used for an office, for sewing and crafts or just little spots to curl up with a good book.

The kitchen flows into the living room, with the only real divider being a 3-sided fireplace that sits on a granite-topped rock platform. Another door leads out to the front deck, where you can get a closeup view of the shingled siding, finished in shades of blue and green. Schad found some copper roofing from the old farmhouse and used that to top the bumpout window from the living room. He tied that into the cedar deck by using copper spindles. Walnut stairs lead to the second floor which includes the master suite, two bedrooms, a bathroom and a couple of those creative spaces Schad worked into the floor plan. The master bedroom is a luxurious getaway spot roomy enough for any size

In the home’s lower level, there is another bedroom, bathroom, bar area, open family room with a fireplace and a walk-out to the patio and yard. For example, instead of having solid wood pillars throughout the house, Schad incorporated the same three-dimensional designs that he used in the ceilings to create depth and visual interest. Each pillar has 80 pieces to it! And the truly astounding part of the project is that Schad did not use one

ou’ve probably heard the phrase “an apple day keeps the doctor away” a thousand times. And yes, there’s some truth to it. remedies used by ancient Eastern cultures and indigenous people are being used again, which scientific corroboration. Here are a few foods that have been known to cure what ails you, and make for really tasty dinner at the same time.

Cruciferous Vegetables Cruciferous Vegetables – named for the cross-shaped flowers with four petals – include cauliflower, cabbage, bok choy, broccoli, and Brussels sprouts are a great source of vitamins C, E and K. There’s also folate, and since it’s believed to help ward off breast and colon cancer, why not add some to your diet?

Garlic & Onions It’s not only good at keeping vampires at bay, but garlic is believed to reduce the risk of breast and colon cancers. Thanks to folate and sulfur, both garlic and onions have been shown to lower cholesterol and improve cardiovascular health, all while making dishes taste great!

Eggs Remember just a few years ago when we thought a couple eggs for breakfast was going to kill you with cholesterol? Well, thankfully, those days are behind us, and we can enjoy our eggs and all of their iron, protein, vitamins A and B and folate.

Grass-fed Beef I bet you weren’t expecting to see beef on this list! But meat from cows that graze on grass for most of their lives has been shown to have more vitamin B and iron than corn-fed beef. I take that as approval to go out and buy a couple of steaks for dinner!

Green Beans With a low glycemic index, green beans help to regulate blood sugar and they’re a good source of fiber, so they will help keep things moving. My mother used to make big pots of green beans with smoky ham hocks that were delicious. Clearly, she had figured out the way to get kids to eat vegetables was to cook them with pork!

Dark, Leafy Greens While kale is getting a lot of attention these days, spinach, chard, mustard greens, collard greens and even dandelion greens are all chock full of minerals, vitamins, phytonutrients (which boost the immune system), calcium, sulforaphanes (disease fighter) and folate.

Anchovies & Sardines If you’re worried about mercury from fish, these little guys on the small end of the oceanic food chain contain very little. But they are good sources of omega-3s, and B vitamins. They also have a good combination of calcium and vitamin D which our bodies need for healthy bones. I can already hear you saying you’re not going to eat sardines or anchovies, but they get a bad rap. Anchovies in pasta sauces, in braises and salad dressings add a complexity and saltiness, but you’ll probably never know they were there. My father in-law loved canned sardines and he lived to be 96; maybe he was on to something.

Mushrooms

Shop with us in the Big Pink Truck at the Expo! Book "The Pod" for your next "girl's night out" at: www.thepaisleypod.com

I love mushrooms. Not just the pricey morels but trumpets, oysters, shitake, porcini, or even those white button mushrooms. I love them all. Now that there’s a place in Tea that is growing fresh king trumpet and oyster mushrooms year ‘round, you can pick up fresh local fungi at the Falls Park Farmers Market, the Co-op or Pomegranate. Mushrooms are said to be anti-inflammatory, include minerals, vitamins B and D. Mushrooms are also a good source of polysaccharides which are said to fight tumors.

Organic Chicken Sure it has lots of protein, but organic chicken raised without antibiotics has been shown to be higher in omega-3s. Yard birds are also a good source of vitamin B6 and iron. It’s getting easier to find chicken raised without antibiotics, most groceries stores sell several brands, and if the people raising the chickens went to the extra effort, the label will be plain to see. I probably don’t need to tell you about whole grains and the associated fiber or avocados with the “good fat” but there are so many good things hiding in good food. If you follow the simple rule of not eating anything with words on the label that you can’t pronounce, you’ll probably do pretty well at adding most of these to your diet. And keep your eyes out for the great discoveries that happen all the time. A study that came out in July of this year said that people who drink coffee live longer that those who don’t. Not sure why, but hey, that’s science a person can get behind. Do yourself a favor, eat something good today!

ne night in the small town of Garfield, NJ, a fire started inside the local sausage factory. In a blink, the building was engulfed in flames. The alarm went out to all the fire departments for miles around. When the first volunteer fire fighters appeared on the scene, the sausage company president rushed to the fire chief and said, “All of our secret sausage recipes are in the vault in the center of the plant. They have to be saved, so I will donate $50,000 to the fire company that brings them out and delivers them to me.” But the roaring flames held the firefighters off. Soon more fire departments had to be called in because the situation became desperate. As the firemen arrived, the president shouted out that the offer to extricate the secret recipes was now $100,000 to the fire department that could save them. Suddenly from up the road, a lone siren was heard as another fire truck came into sight. It was the fire engine of the nearby Lodi, NJ, volunteer fire department composed mainly of Italian firefighters over the age of 65. To everyone’s amazement, the little run-down fire engine, operated by these Italian firefighters, passed fire engines parked outside the plant, and drove straight into the middle of the inferno. Outside, the other firemen watched in amazement as the Italian old timers jumped off and began to fight the fire as if they were fighting to save their own lives. Within a short time, the Lodi old timers had extinguished the fire and saved the secret recipes. The grateful sausage company president joyfully announced that for such a super human accomplishment he was upping the reward

to $200,000, and walked over to personally thank each of the brave elderly Italian firefighters. A TV news crew rushed in after capturing the event on film. The reporter asked the Italian fire chief, “What are you going to do with all that money?” “Wella,” said Chief Pasquale De Luccinellavanti, the 70-yearold fire chief, “De fursta tinga we gonnna do isza fixa de brakes on dat truck!!” I love stories that make me smile. Although a fictitious fable, accidental heroes are sometimes the most memorable ones. In the wine world, one of the most well known wines in the world was created one day over 40 years ago… by accident. The first zinfandel vineyards were planted in California in the mid 1800s, during the time of the gold rush. Brought by eastern European immigrants wanting to make sure they would have wine to drink, it is one of the oldest wine grape varieties in the US. Zinfandel (or zin) had the advantage of not needing a trellis system to thrive. Gnarled head pruned vines can still be found in the Sierra Foothills and a few other regions. Some of the vines date back over 100 years. Prior to the mid ‘70s, zinfandel was produced primarily as a red wine. Ironically, it was the introduction of “white zinfandel” that saved the old vineyards from being ripped out as sales of red zinfandel fell when other varieties gained popularity. White zinfandel which accounts for 10% of US wine sales was discovered quite by accident. A California winemaker, in the process of making a dry zinfandel rose’ experienced a “stuck fermentation” where the yeast died before the sugar was fully converted to alcohol. The winemaker

liked the resulting sweet wine and the rest is history. White Zinfandel gets its blush of color from the deep red skin of the Zinfandel grape. When the grapes are harvested, the skins are left on for only a short time, a few hours to a day, perhaps. The weight of the grapes, pressing against each other, extracts a pretty pink-tinged juice. In contrast, a winemaker making red Zinfandel wine will leave the juice and the red skins in contact for days or weeks, producing a deep red juice. Not all blush wines are equal and here is where White Zin shines. It is made from a quality grape. The Zinfandel grape is known for its luscious deep, berry-fruit flavors and a characteristic spiciness (think “blackberries”) - which transfers to White Zinfandel in a delectable, but less intense manner, than in the red version. If you are new to wine drinking, try starting with a White Zin, it’s sweet and easy to drink. If you already are a White Zin drinker, I urge you to expand your palate. Try a Moscato d’Asti or a Late Harvest Riesling. Expand your palate! Try a new wine today! Carpe Vino!

Cervical Disc Replacement MATTHEW K WINGATE M.D. | ORTHOPEDIC INSTITUTE

C

ervical disc replacement (also known as cervical arthroplasty) has been around for use in the United States for the past 10-15 years. The technology was first developed and used in Europe prior to being studied and cleared by the Food and Drug Administration (FDA) for use in the U.S. Prior to development of a cervical disc replacement, patients with herniated discs or degenerative disc disease in the cervical spine (neck) were treated with anterior cervical discectomy and fusion (ACDF). This is a very common procedure that has been performed for decades in America. ACDF involves performing a surgery through an incision in the front of the neck. After the surgeon exposes the spine the diseased or herniated disc is removed in its entirety. The disc is replacement with a spacer and the spine is stabilized with a plate and screws. The purpose of ACDF is to remove the diseased or herniated disc

and to fuse the bones (vertebrae) on either side of the disc. This way the pain caused by the herniated or arthritic disc will no longer be present. Historically ACDF has been considered the “gold standard” for individuals dealing with degenerative disc disease or disc herniations in the neck. Patients do exceedingly well. Satisfaction and success rates are well over 90%. Complication rates are very low. However, two of the most common risks involved with ACDF are related to the fusion procedure itself: nonunion and adjacent segment degeneration (ASD). Nonunion occurs when the vertebrae surrounding the removed disc failed to heal together. Nonunions can occasionally lead to recurrence of arm pain, neck pain for breakage of the titanium plate or screws. Adjacent segment degeneration is breakdown of the disc level above or below the area that was previously operated on. This

increased stress can result in breakdown or disc degeneration above or below the fused segment. This is called adjacent segment degeneration. This process may never occur after a neck fusion but may occur anywhere from 1-5 years after the initial procedure. This process is still not fully understood and is the topic of current and future research in the spine field. To combat the issue of ASD surgeons around the world developed cervical disc replacements (CDR). Similar to the ACDF, CDR involves making an incision on the front of the neck. Through this anterior incision the diseased or herniated disc is removed. This is where the CDR procedure differs from the ACDF. The next steps involve inserting a disc replacement device. The disc replacement is made from metal and a hard plastic called polyethylene. The metal and plastic parts can move smoothly on each other. No plates or screws are

needed with a disc replacement. The usual incision is approximately 1- 1 ½ inches in length near the throat (Adam’s apple) left or right of center. The surgery can be performed on an outpatient basis where the patient is allowed to go home the same day as surgery. Depending on overall health, some patients are kept overnight. Most patients do not need a neck brace, but some feel comfortable wearing a soft collar for several days as the incision heals. Return to work can be as soon as 1 week for those performing desk jobs, and 2-4 weeks for those in labor type positions. The most widely discussed benefit of CDR is the maintenance of movement in the spine. In contrast to fusion, the CDR allows natural motion of the spine. Preserving the natural motion in the neck significantly decreases development of arthritis or degenerative disc disease. In fact, research has shown that compared to ACDF, patients with CDR have less development of arthritis in their remaining spine and have fewer future neck surgeries.

Very high satisfaction rates with very low complication rates make this a promising procedure for many of those dealing with disc herniations and degenerative disc disease. Cervical disc replacement is not for everyone. Ideal patients for cervical disc replacement are younger, have one or two level disc disease, lack significant arthritis in the back of their spine and have not had a previous fusion. As with all spine surgery, patients are first treated with non-operative treatment. On most occasions patients with herniated discs and degenerative disc disease get better with conservative treatment such as arthritis medicines, physical therapy and injections. Those who do not see improvement with those treatments may be candidates for surgery. Several CDR devices are now FDA cleared for use in one or two level cervical surgery. Most commercial insurance carriers are now paying for CDR.

If you would like to learn more about CDR, and whether or not you are a good candidate, join Dr. Wingate for a FREE lunch and learn more!

Is it Breast Cancer or Not? M

any women have experienced the fear and uncertainty of feeling a lump in their breast. They know that most lumps turn out to be benign, yet they also know that some lumps turn out to be cancerous. Finding out for sure just became a little easier through a new blood test that has found to be effective in ruling out cancer when results of mammograms and ultrasounds are inconclusive. Before this test, a woman would have had to have a biopsy, which most often involves needle-guided ultrasound, mammography or MRI biopsy. The Videssa® Breast blood test looks for specific protein biomarkers and autoantibodies in the blood that might point to cancer. Studies show that it is nearly 99 percent effective in proving a patient does not have breast cancer. “This test is wonderful in that it saves numerous women from

having unnecessary biopsies,” said Tricia Merrigan, MD, Avera Medical Group breast surgeon. “The vast majority of abnormal mammograms turn out to be benign, but before Videssa, we have needed to do a biopsy in many cases to find out for sure. Now, if the Videssa test comes back negative, we can give strong assurance that it is not breast cancer. If the test is positive, that tells us we should do more tests, such as a biopsy, to confirm whether or not it’s cancer.” Kirsten Stotz of Sioux Falls is one of those patients who could go on with her life with confidence that she was cancer free. “I had felt a lump in my breast, and waited a couple of months to see if it changed with my cycles. It didn’t,” Stotz said. She had a diagnostic mammogram and ultrasound, as well as contrastenhanced spectral mammogram (CESM). These tests showed an area of abnormality that was in a different place than the lump

that Stotz felt. “I had a simple blood test and it came back in two weeks – thankfully, it was negative,” Stotz said. “So now I know I’m free from disease, and good to go without further testing.” Avera Cancer Institute in Sioux Falls took part in a national research study of Videssa. Now that the test has been proven effective, Avera was chosen to provide early access to this test and continues to offer it in day-to-day breast diagnostic care. “We’ve been involved in this research for the past five years,” said Josie Alpers, MD, Director of Mammography at Avera Breast Center. “This is very exciting work,” said Alpers, noting that Videssa and the research surrounding it has been accepted as part of the federal government’s Cancer Moonshot program to transform cancer care and research. “When a mammogram yields an abnormal result, the challenge

Oktoberfest Fun BY JESSICA WEISCHEDEL

O

ctober is a month of many fall festivities, including harvest celebrations and holiday shenanigans. The world’s largest beer festival, Oktoberfest, is held annually in Munich, Bavaria, Germany, and runs late September to the first weekend in October. More than 6 million people from around the world attend the event each year. It all began with a royal marriage on October 12, 1810, and since then, mini-festivals throughout the world have been recreated to honor the event’s German tradition.

You don’t have to travel very far to enjoy this annual celebration. Here are some notable Oktoberfests in the United States that would be worth visiting.

New Ulm Oktoberfest - New Ulm Minnesota During the first two weekends in October, you can travel to our nearby state of Minnesota and visit New Ulm for its 24th annual Oktoberfest. Local bands, food vendors, horse-drawn trolley

rides, and the popular Schell’s Beer Wagon are all showcased at the Best Western Plus, Downtown, Schell’s Brewery, and Morgan Creek Vineyards. Downtown, the 45-foot Musical Glockenspiel chimes throughout day, while 12 figurines that depict characters from the city’s history rotate on a circulating stage during performances. Public tours are offered at the John Lind House, Wanda Gag House, Minnesota Music Hall of Fame, and the Hermann Monument. Also be sure to tour the Schell’s Brewery,

German beers, Bavarian cheesecake, bratwurst and more are highlights of this incredible event. The highly anticipated Dachshund Dash is sure to provide the entire family with entertainment, as well as chicken dances, face painting, and midway rides. Entering its 39th year, Linde Oktoberfest has been around since 1978. A craft beer garden, food trucks, carnival rides, and live music make it a party to remember. The beer-stein holding contest, Bavarian Masskrustemmen, challenges participants to hold a liter of beer for as long as possible in one out-stretched hand. Stelzenplatz also offers craft beers, additional food, music, and shopping.

Instructions: Using your imagination, create different Halloween characters from toilet paper rolls. Some ideas for characters include a pumpkin, a witch, a bat, a ghost, Frankenstein, a spider, and more. Have your child come up with ideas and let them create funny faces and accessories out of construction paper of various colors. Display the characters as decorations for the spooky holiday.

Instructions: Cut a skeleton’s head out of white paper, then draw his face with a black marker. Glue the head to the top of a piece of black construction paper. Create the skeleton’s body using Q-tips, cutting some in half for the feet and any fingers you might want to add. Make sure to lay it out before gluing to estimate how much room you have on the paper. Glue the body in place, and let dry completely.

How to Nurture Learning with Play BY SANFORD HEALTH

Enjoy Tummy Time

Chat and Sing

Make Expressions

This gives her a chance to see the world from a new perspective. Making tummy time into playtime will encourage your baby’s development. While she’s spending a few minutes (at first) in this position, you can use soft toys that shake or make noise to get her attention, encouraging her to try to lift her head toward the sound. Get down on the floor and talk to her at her eye level. As her neck and arm muscles get stronger she will be able to hold herself up to look at more things.

Use different inflections in your voice and watch how your baby reacts. Soon he will start to respond to you with cooing, gurgling and simple inflections. Mimicking those sounds back to your baby will encourage more interaction and bonding. Singing will help your baby develop language and rhythm skills. It can soothe your child as he is comforted by the sound of your voice. If there is a particular lullaby your baby is used to, that can be helpful to calm him during fussy times.

Your baby loves seeing your face. Experiment to see how many different reactions you can get from making different faces in front of her. Babies will instinctively try to copy emotions that are modeled for them – so a beaming smile should have your baby grinning back at you.

Play Simple Games Games like peekaboo or simple interactions like splashing around in a shallow bath will teach your baby how to anticipate events. Stick out your tongue,

puff out your cheeks or blow raspberries on her tummy. The unpredictability of your play will cause delight and your baby will start to understand the basics of cause and effect. Bonus: That cascade of giggles is the early development of a sense of humor. Your baby doesn’t need a lot of fancy toys and gadgets to enjoy playtime. What your baby needs most is your time, your face, your voice and your loving touch – but some basic toys that can add to the fun for both of you include: • Rattles • Large rings • Mobiles with contrasting colors • Teething toys • Soft dolls • Stackable cups • Textured balls • Board or vinyl books • Unbreakable mirrors • Musical toys Remember babies explore the world by putting things in their mouth. Do not give your baby anything that can fit through an empty toilet paper cardboard tube. Enjoy playing and watching your baby as she grows. At this age, playing is about spending time together and learning about each other as your baby discovers the world around her.

THESE ARE JUST SOME OF THE WONDERFUL BOOKS FOR CHILDREN WE HAVE COME ACROSS THIS MONTH. WE HOPE TO SHARE WITH YOU SOME YOU HAVE NOT SEEN BEFORE AND ALSO INTRODUCE OTHERS BEING RELEASED IN THE NEAR FUTURE. ENJOY. I Want to Be in a Scary Story by Sean Taylor Our author would like to write a funny story, but his main character — Monster — has a different idea. He wants to be the star of a chilling, petrifying, utterly terrifying SCARY story. But scary stories . . . well, they can be very scary — especially for their characters! Particularly when they involve dark forests and creepy witches and spooky houses . . . Oh yikes and crikes, this is definitely not the scary story Monster had in mind! Maybe he wants to be in a funny story after all! Ages 2 yrs - 5 yrs Candlewickpress.com

The Princess in Black and the Mysterious Playdate by Dean Hale; Shannon Hale Princess Magnolia and Princess Sneezewort have plans . . . mysterious plans, like a princess playdate! They dress-up slam! They karaoke jam! They playhouse romp and snack-time stomp! But then a shout from outside Princess Sneezewort’s castle interrupts their fun. It’s a monster trying to eat someone’s kitty! This is a job for the Princess in Black. Yet when the Princess in Black gets there, she finds only a masked stranger and no monster in sight . . . or is there? Action and humor abound in this ode to friendship that proves that when shape-shifting monsters intrude on your plans, two heroes are better than one. Ages 5 yrs - 8 yrs Candlewickpress.com Bizzy Bear: Spooky House Benji Davies The ever-intrepid Bizzy Bear is visiting a super-spooky Halloween house. As he climbs the rickety stairs and walks the cobwebby corridors, all sorts of creepy characters appear from doors and hidey-holes. Bizzy remains undaunted — but where could he be going and what will he find there? Bizzy’s latest adventure makes for a splendidly seasonal new addition to this popular series, which has sold more than a million copies worldwide. Ages 0 mos - 3 yrs Candlewickpress.com

Halloween can be the spookiest night of the year, but keeping your pets safe doesn’t have to be tricky. Stash the Treats The candy bowl is for trick-or-treaters. Several popular Halloween treats are toxic to pets.

Watch the Decorations While a carved jack-o-lantern certainly is festive, pets can easily knock over a lit pumpkin and start a fire

Be Careful with Costumes For some pets, wearing a costume may cause undue stress

Keep Pets Calm and Easily Identifiable While opening the door for guests, be sure that your dog or cat doesn’t dart outside. And always make sure your pet has ID tags and/or microchip.
