Medical Neighborhood Project Design Team Charter

Medical Neighborhood Project 2014 Design Team Charter (revised: July 2014) Purpose: Improve referral infrastructure across the medical neighborhood in Greater Columbus.

Objectives:  Inventory current best practices and associated client-centric shared-infrastructure used in other communities for improving referrals across the medical neighborhood.  Identify client-centric shared-infrastructure that will meet the needs of local communitybased organizations and healthcare providers.  Create a collaborative process for oversight of shared-infrastructure  Assist in securing commitments from community-based organizations, and local healthcare providers to participate in a pilot project.  Develop measures of success for pilot phase of project.
