Medical Neighborhood Project 2014 Design Team Charter (revised: July 2014) Purpose: Improve referral infrastructure across the medical neighborhood in Greater Columbus. Objectives: Inventory current best practices and associated client-centric shared-infrastructure used in other communities for improving referrals across the medical neighborhood. Identify client-centric shared-infrastructure that will meet the needs of local communitybased organizations and healthcare providers. Create a collaborative process for oversight of shared-infrastructure Assist in securing commitments from community-based organizations, and local healthcare providers to participate in a pilot project. Develop measures of success for pilot phase of project.
