CliniSync Reporting capabilities are currently limited. Referral tool does show what services an organization offers. Service eligibility could be set by setting criteria to make a referral. Product does not display the capacity of an organization. Each organization could indicate what capacity they have in their instructions window, or when they reject a referral. A patient’s referral history will be tracked once they are placed in an organization’s database. Will track all referrals, not just the ones involving your organization. Organizations have the ability to set criteria for making a referral. Sensitive information can be sent via the direct messaging portal. Most providers in the state will have a direct messaging address.

Documents can be attached to referrals and direct messages.

 

System will consolidate patient records to eliminate duplicates. Working to implement a system that would alert organizations via text message or email.

Community Health Record will soon have the ability to track a patient’s medication history.

Messaging Patient Portal Key Take-Aways from CliniSync Demonstration o CliniSync’s referral and direct messaging tools are offered at no cost to local community-based organizations and healthcare providers. o Many hospitals, providers, long-term care and rehab facilities, and behavioral health agencies in Ohio are already using CliniSync. o CliniSync’s Community Health Record is connected directly into an organization’s EMR. Next Steps During the November meeting the design team will be collaboratively identifying if one of the tools demonstrated could be successfully implemented in Greater Columbus.
