Improving Healthcare Transparency in Greater Columbus Primary Care Quality Reporting Overview (revised: 11/3/14) Purpose: Measuring and publicly reporting the performance of physician practices is an essential element of improving healthcare quality. Public reports enable patients to make better informed choices about their care, help health care professionals see where they can improve, and allow purchasers to know the value of the care that they are buying. Project Goals: • Increase the number of primary care practices using quality data to improve the management of chronic conditions based on all-payer clinical data exported from electronic medical records. • Increase the number of consumers using quality data to make informed choices about their care. Overview

Current Participants (at this time) Based on nationally endorsed measures for managing chronic • Central Ohio conditions: Primary Care • Compile all-payer data every six months from electronic medical (34 practice sites) records (EMRs) at participating primary care practice sites. • Mount Carmel • Share comparable quality data across with participating providers Medical Group and regional purchasers. (17 practice sites) • Apply learning from quality reports to catalyze quality improvement. • The Ohio State University Initial measures: Wexner Medical Center • Diabetes: Hemoglobin A1c Poor Control (13 practice sites) • Diabetes: Low Density Lipoprotein (LDL) Management • OPEN invitation • Diabetes: Blood Pressure Management to providers • Controlling High Blood Pressure In partnership with Consumer Reports, Robert Wood Johnson Foundation, and regional primary care leaders: • 2015 - develop consumer-friendly reports that measure regional primary care quality data focused on chronic disease management. • 2015 - design and launch a regional consumer website for sharing primary care quality data with consumers and other stakeholders. • Early 2016 - results will be published in Consumer Reports magazine via a regional supplement.

Pending federal funding of Ohio’s SIM grant: • Standardize reporting requirements across public-private payers and publicly report to demonstrate value of enhanced CPC incentives. • 2015: spread CPC across SW Ohio and selection of either Cleveland or Columbus as next expansion region • 2016: 2nd CPC expansion region begins • 2017 – 2018: spread CPC across additional regions
