To provide affordable and appropriate pharmacy services* and coordinate access to health care for people who are vulnerable** in our community *Pharmacy Services – medicines and necessary services to ensure optimum clinical outcomes ** Vulnerable – those living at or below 200% of the federal poverty level who are uninsured or underinsured for prescription medications.


Facts • 5 Sites • 2 Counties • 9,774 Users/Patients • 45,000 Annual Visits • 25% of Patients with a depression diagnosis receive counseling at LLCHC • >70% of Patients at LLCHC have a Comorbid Mental Health or Substance Abuse diagnosis

Why Partner? • Lower Lights and Charitable Pharmacy have commonalities – Both Non-Profit – Both Support Underserved Populations – Both have primary locations near downtown – Both want comprehensive, whole person care for their patients – Both wanted to utilize the 340B Pharmacy program to further care for the uninsured


Desired Impact Integrating Pharmacy Services with the Health Center • Increased medication access to LLCHC patients • Revenue stream for both LLCHC and CPCO to help offset the cost of uninsured patients • Improved care coordination and quality with the Pharmacists as part of the Care Team • Efficiency in Drug Selection • Additional, convenient access to services by current/new patients


Lessons Learned • 340B pharmacy program is complex – especially with a non-traditional model • Change in organizational structure/ issues take time. We phased in the integration. • We improved Provider and Patient satisfaction through the integration – even through our phased approach
