Purpose: Improve chronic condition management by advancing results-based practices with clinical and social service organizations. Shared Learning What depression screening activity currently takes place and what are we learning? • Depression screening occurring with diabetes management program • Depression screening taking place in behavioral health organizations • Increased knowledge of health issues is important • Stratifying the needs of patients is imperative and can create a challenge when trying to address diabetes and depression on top of other issues What process improvement opportunities are emerging? • Consistent education for professionals around stigma and shame/blame • Connectivity to resources and developing a deeper understanding of what exists in our community Strategic Guidance What regional targets should be considered to measure progress? • Developing relationships across primary care, behavioral health, diabetes management programs • Measuring how often a communication loop is completed either back to primary care or behavioral health Next Steps • Meeting quarterly in 2017
