STUDENT INFORMATION AND CHECKLIST
The Medical Assistant Program at Santa Clara Adult Education includes 375 hours of class instruction and 160 hours of externship.
Enrollment is on a first come - first serve basis. Students can enroll as soon as they have attended an orientation

• will have completed by at least one week prior to the start of class.

REQUIREMENTS
Students must sign up for a FREE orientation to learn about the program requirements. Orientation dates are listed in the course catalog.
Financial assistance may be available to assist you with the cost of the class. You can contact NOVAworks at (408) 730-7232 for an orientation and more information.
Students will upload Immunization Verification, BLS CPR, High School Diploma/High School Equivanlency, Liability Insurance and Signed forms. Students will also complete a drug screen and background check.
Students should submit proof of their highest level of education. The minimum requirement is a HS Diploma or equivalent. Proof can be submitted in the form of a Diploma or transcript. Students who have a foreign degree must have their transcripts translated and evaluated by an accredited agency, Academic & Credential Records Evaluation & Verification Service in Milpitas: www.acrevs.com We only require a general evaluation which states that the foreign degree is equal or higher to a US HS Diploma

Students must have a valid Basic Life Support (BLS) card. The card must be valid for at least one year and must be provided by the American Heart Association: American Heart Association website American Red Cross BLS will not be accepted.
The BLS certificate must be uploaded to Castlebranch Solutions.
Students must obtain professional liability insurance for Medical Assistant Students, e.g. through www.hpso.com. A one-time fee of $28 provides coverage for a full year and for up to $3 million.
The proof of insurance must be uploaded to Castlebranch Solutions.
Students must upload their +vaccination records to their Castlebranch account. Vaccination information is available at https://myvaccinerecord.cdph.ca.gov/.

Uniform and Nurse Shoes
STUDENT SUPPLIES
Students must wear ROYAL BLUE scrubs and white or black nurse shoes at all times. Shoes must be leather or faux leather and closed all around for safety reasons. Uniforms and shoes can be purchased with various online providers or at local stores.

CERTIFICATION TESTING
Students will be encouraged to complete a National Center for Competency Certification (NCCT) certification exam at the end of class. Passing the NCCT exam is not required to earn the program certificate. The program certificate is based on successfully completing the course work and completing the externship. The cost of the exam is not included in the class fee.

ADDITIONAL EXPECTATIONS
INJECTIONS
• You will be expected to administer injections as well as phlebotomy (blood draws). This will be required of all students.

PARTICIPATION
• In order to learn and gain skills, attendance is required for all classes. More than 3 absences may be cause for the student to be dropped from class. Three unexcused tardies will be counted as one absence.
• Students are expected to participate, interact with your peers and turn in assigned worksheets and homework on time.
• Cell phone use is not permitted unless used for instructional purposes, during breaks, and if an emergency arises.

As a registered student of the Santa Adult Education Medical Assistant class, I agree that I will not hold the instructor, or the facility or Santa Clara Unified School District responsible for personal injury, injury to others and/or property damage as a result of my actions while participating in this course.
I accept full responsibility for any known or unknown medical condition that may exist. I certify that I am aware of the general physical requirements of this class and that my physical condition allows me to participate in this activity. If a medical problem occurs prior to or during this class session, I agree to stop participation in this course until I receive written approval from my physician. EXTERNSHIP INFORMATION
• I understand that my externship will begin after the class is completed. It may take up to 6 weeks after the coursework is complete to start the externship.
• I understand that the externship site assignment will be based on availability, attendance, participation, grades on assignments and instructor recommendations.
• I understand that my instructor or program administrator will determine my externship site, which may be at any location throughout Santa Clara County.
• Typical work days are 8 hour daily shifts during the day, but can vary depending on the clinic.
• Evening students will need to complete the externship during normal business hours.
• I understand that I need to have transportation to any location in Santa Clara County for my externship.
• My records, including program, enrollment and immunization records may be requested by my externship site.
• I understand that attendance at the externship site is important for my learning. The school and site reserves the right to terminate the externship of any student who has more than 3 absences.
• I understand that the course certificate is not issued until the classroom and externship hours are completed.
