Head Trauma

How to avoid hard knocks and potential brain injuries By Samuel Inouye, MD Mat-Su Regional Urgent Care Center Most adults have a nostalgic childhood story about bike rides with no helmets, family car trips with no seat belts, and other death-defying tales from their “tougher” generation in simpler (and more dangerous) times. Those who survived the occasional bump on the head with no negative outcome probably weren’t tougher – just lucky. Head injuries can happen to children and adults, athletes and non-athletes, during strenuous physical activity or a simple slip or trip. The potential heath effects of a brain injury can be devastating, ranging from cognitive or motor impairments, to disability or death. A concussion is actually a mild TBI, and happens when the brain is jarred or shaken hard enough to hit the side of the skull. Interestingly, a concussion doesn’t always involve a direct hit to the head; it can be caused by impact to any part of the body that is hard enough to jar the brain. Concussions can be mild or severe, and can temporarily – or permanently – affect the brain’s function. Other brain injuries include a contusion (a bruise and/or swelling), a hematoma (bleeding that forms a clot), and a skull fracture. Of the TBIs reported each year, 75 percent are concussions or other mild forms of TBI. Sometimes, brain injury symptoms can be subtle, and won’t appear until days or weeks after the original injury. Long-term effects of a head injury can include permanent changes in the function of the brain or motor skills, epilepsy, and increased risk for age-related brain disorders like Alzheimer’s disease and Parkinson’s disease. That’s why it’s critical to seek immediate medical care after a head injury– even if you think the injury isn’t serious. The Centers for Disease Control ranks the leading causes of brain injuries as falls, motor vehicle crashes, and impact-related events, in which the brain is struck by another object. TBIs range from “mild,” in which the person experiences a brief change in mental status and possibly, unconsciousness, to “severe” – an extended period of uncon-

sciousness or amnesia. Children under age 5 and teens ages 15 to 19 are at the highest risk for a TBI. Certain sports-related activities carry a high degree of risk. According to the Brain Injury Association of America, Samuel Inouye, MD cycling, football, basketball, baseball/softball, and riding recreational vehicles – a pastime we love in Alaska – result in the highest number of head injuries. The organization also ranked the top five activities responsible for concussions in children from ages 5-18: cycling, football, basketball, playground activities, and soccer. Often, sports-related concussions and their symptoms go unrecognized. A 2008 study by Washington University School of Medicine found that a concussion diagnosis was widely linked with a loss of consciousness while other symptoms, such as a mild headache or confusion, were overlooked. Know the signs and symptoms of a TBI – and don’t wait to seek help if an injury does occur. Watch a head injury victim for signs that a suspected concussion or mild TBI may be something more serious, such as change in pupil size, clear or bloody fluid draining from the nose, mouth or ears, a drop in blood pressure, changes in behavior, sudden sleepiness, or convulsions. It’s important to remember, however; that head injuries don’t happen only during sports activities. The chief culprit for head injuries among the elderly are slips, trips and falls. Besides knowing the signs of a brain injury, take precautions to stay safe. - Always buckle up in an automobile. - Always wear a helmet and other protective gear during sporting activities, bicycling or riding recreational vehicles. - Install window guards to keep young children from falling out of open windows. - Make sure the area surrounding your child’s playset has a shock-absorbing material such as hardwood mulch or sand. Signs of a Brain Injury • Headaches or neck pain that do not go away • Difficulty remembering, concentrating, or making decisions • Slowness in thinking, speaking, acting or reading • Getting lost or easily confused • Lack of energy or motivation • Mood changes (feeling sad or angry for no reason)

• Changes in sleep patterns (sleeping a lot more or having a hard time sleeping) • Light-headedness, dizziness or loss of balance • Urge to vomit • Increased sensitivity to lights, sounds or distractions • Blurred vision or eyes that tire easily • Loss of sense of smell or taste • Ringing in the ears

If you or a loved one has any of the above symptoms or a TBI is even slightly suspected, quick action is critical: get medical help right away. Proceed to the nearest E.R., where physicians will administer a physical exam as well as test visual, verbal and motor skills. They may refer the patient to a brain injury specialist, or use other diagnostic methods to include X-rays or a CT scan to determine the nature of the injury and appropriate treatment. The classroom, to your left as you enter the building, serves as a display for local products made from re-purposed material. You can see purses, hats, yard art and much more made from all kinds of used materials including aluminum cans, plastic film, even used juice packets. The classroom also contains a lending library with books and movies. Pick up a Nature Trail Guide or Scavenger Hunt and enjoy the nature walk in back of the building. This short trail is decorated with imaginative recycled art. The Trail Guide helps you identify native vegetation and a culturally modified birch tree (find out more in the VCRS Library). The Valley Arts Alliance meets at VCRS every Thursday to find inspiration for such projects as the Wearable Arts Show, Glenn Massey Theater’s Trees of the Valley Christmas display, and the Recycled Yard Art competition at the State Fair. Recently, a class was held on making decorative picture frames from aluminum cans. through the classroom window to the processing area below, discover how landfills operate, learn how the different types of materials are made, how they are recycled, and practice sorting them. VCRS even has a curriculum for teachers that meets State Standards in several subject areas. Interested in learning how renewables work, or how to reduce energy costs in your home? VCRS was the first LEED (Leadership in Energy and Environment Design) certified building in the Valley at the Gold level. A transparent section of drywall reveals the super thick Thermo-Kool insulation that reduces heating costs. Thermo-Kool is a local Valley manufacturing company that turns recycled newspaper and cardboard into insulation. Solar panels can be seen on the south side of the building. The wind generator is currently off line due to damage from a severe wind storm, but an anemometer is still in place. VCRS is proud to report that none of its products will end up in a landfill. In spite of this crisis, VCRS has committed U.S. manufacturers who will buy their products because of their quality. However the prices for materials have dropped so low that in some cases they don’t even cover costs. As a result, VCRS began the Sustain Campaign, asking for contributions to provide a safety net, to cover operations when the markets go down like this, and unexpected events, such as equipment failures happen. With its building, equipment and new baler, VCRS is set to handle 5 times what it now collects, so it can meet the demands of the Valley’s rapid growth for years to come, and is a great investment. I wrote this series of articles because I’ve been a volunteer for VCRS for the past 10 years, and I’ve never worked with an organization that is more productive. Be bear aware Julie Cascio, University of Alaska Fairbanks Cooperative Extension Service This time of year is a great time to be outside the house. Hiking, biking, gardening, fishing, bird watching, camping, exploring Alaska are a few examples of what we may do in our state. However, Alaska is bear country. As Julie Cascio we enjoy our outdoor activities, it is important to understand what bears need and learn behavior to avoid conflicts.

Don’t Feed Bears Bears spend summer to find their food before hibernating in dens over the winter. Food we humans eat may appeal to them. Do not let bears learn that food is near. Secure food away from bears. Keep garbage cans sealed. Keep the area around your home

or camp clean. Smelly and greasy foods such as bacon or smoked fish can attract them. Keep food smells off your clothing. Don’t Surprise a Bear Bears tend to avoid or ignore people. Bears can be dangerous! So, pay attention to your surroundings. Make a special effort to be noticeable in an area where bear activity may occur. Bears, like humans, use trails and roads. Always let bears know you are there. Make noise, sing or talk loudly if doing work or hiking in an area that bears may be. Avoid thick brush whenever possible. When the terrain or vegetation makes it hard to see, make extra noise. If out hiking, go in a group; groups are easier for bears to detect. Walk with the wind at your back, if possible. Bears can see but trust their noses more than their eyes or ears.

Give Bears Space When a bear can be seen, keep your distance from the bear.

Alaska Fish and Gamer reports: • If the bear appears not to have sensed you, move away without alerting it. Keep your eyes on the bear. • If the bear does notice you, face the bear, stand your ground and talk to it calmly. Let the bear know you are human. Talk in a normal voice. Try to appear larger by standing close to others in your group or wave your arms slowly above your head. Try to back away slowly, but if the bear follows, stop and hold your ground. Prepare your deterrent, either a bear spray or firearm, if you have one, that you have been trained in, and practiced using. • If a bear cannot tell what you are, it may come closer or stand on its hind legs to get a better look or smell. A standing bear is usually curious, not threatening. • If you take the above actions and the bear continues to focus on you or approach, you should become more assertive: raise your voice, beat on pans, use noisemakers, throw rocks or sticks. Use

your deterrent if you have one. Drive a bear off rather than let it follow you. If you are with others, group together to look big and stand your ground. Go to www.adfg.alaska.gov or www.nps. gov for more information.

Enjoy rhubarb this summer Julie Cascio, Mat Su District Cooperative Extension Service, UAF Rhubarb grows well in our part of Alaska. Just this week I made pies, cookies and rhubarb lemonade from the rhubarb in my garden. Rhubarb botanically is a vegetable from the Polygonaceae family and is a close Julie Cascio cousin to sorrel and buckwheat. The stalk is the part of the rhubarb plant to eat. Both green and red stalks are edible. The deeper the red color of the stalks, the more flavorful. Harvest rhubarb when the stems are young and tender. Rhubarb contains fiber, potassium and a small amount of vitamin A, thiamin, riboflavin, niacin, and iron. It is composed of 95 percent water Rhubarb can be eaten raw; 26 calories

per raw cup. It has a tart flavor so is often cooked and sweetened with sugar. Rhubarb can be used to make jellies, jams, cakes, muffins, sauces, salsa and beverages. It can be added to pilaf or soup, or combined with other fruits in recipes. Never eat the leaves of rhubarb as they contain a toxic poison called oxalic acid. When cooking fresh rhubarb, remove any brown or scaly spots, trim ends and wash thoroughly. Do not use aluminum, iron or copper pans to cook rhubarb. The high acidity will react with the metal causing the

rhubarb to turn a brownish color as well as discolor the pan. Before storing rhubarb, remove the leaves from the stalks. The stalks can be kept in the refrigerator, unwashed and wrapped tightly in plastic for up to three weeks. Rhubarb can be dried, frozen or canned to preserve.
