BLAST FROM THE PAST: MINDFUL MANAGEMENT OF BLEEDING AFTER MENOPAUSE ENLIGHTENMENT
Menopause is the phase in a woman’s life that usually occurs around age 50, when the natural fall in hormone levels may cause several changes, one of which is marked by no menstrual cycle for at least one year. Any bleeding that occurs after menopause should be promptly evaluated and not ignored. The first and most important step is to have an evaluation to identify where the bleeding is actually coming from (cervix, vulva, vagina, urethra, uterus, or rectum); second step - to investigate why the bleeding is occurring; and third step - to develop a treatment plan that is appropriate for you.

Many of the causes for postmenopausal bleeding from the uterus are not related to cancer and can be addressed. However, it is especially important not to delay the appropriate assessment to rule out cancer. Some causes include: 1) Medications – such as hormone replacement therapy, hormonal medication used to treat women with breast cancer, or blood thinners; 2) Endometrial Atrophy – thinning of the lining of the uterus; 3) Endometrial Hyperplasia – thickening of the lining of the uterus, some types may be more likely to develop into cancer, 4) Endometritis – infection in the lining of the uterus; 5) Polyps/ Fibroids – growths along the lining of the uterus; 6) Thyroid Problems – abnormal levels of thyroid hormone can have an effect on the uterus; 7) Trauma/Injury to the Pelvis – recent injury may be a sign of damage to the uterus; and 8) Endometrial Cancer – although it occurs in about 1 out of 10
cases, it is the most serious reason and will require the most aggressive treatment.
EMPOWERMENT
Your visit to your doctor will include a detailed personal, family, surgical, and medication history; a thorough physical exam; and a discussion about what other tests may need to be done and where they may need to be done. Initial tests in the office may include lab work, a pap smear, taking a sample of the lining of the uterus (endometrial biopsy), and an ultrasound. Other testing in the hospital setting may include a dilation and curettage (a procedure to open the cervix and taking a sample of the uterine lining), and/or a hysteroscopy (using a camera to look inside the uterus). Once all the results are available, you will be counseled about your options for treatment.
ENCOURAGEMENT
It is unnerving to experience bleeding once you have transitioned to menopause. However, the easiest way to claim your peace of mind is to seek medical attention in a timely fashion, and to be evaluated for the source and cause of your bleeding. Once the cause is identified, you can start on your journey to treatment, and finally lay those past memories of bleeding to rest, once and for all.

Well, it turns out there’s a scientific explanation for the brain activity that triggers -- or avoids -- stuttering. Whether it’s initiated by genetics, a head trauma, premature birth, a birth complication or some unknown factor, it’s a disruption in the motion or muscle movement involved in speaking that causes the speaker to “get stuck.� In a stutterer’s brain, hyperactivity in regions of the right hemisphere causes other brain areas involved in the initiation and termination of motor movements to malfunction.
What does this mean for those who stutter? One day soon, there will be ways of restoring normal function to those brain areas, so motion related to speech starts and stops normally. But until then, the best options are speech therapy, cognitive behavioral therapy and using electronic devices that can improve fluency. It also may be smart to consider a future on the stage.
