Parents – Below you will see an illustration of a Textbook Condition Form. Every student is supplied with one upon checking a textbook out and is required to fill one out for every book they check out. Please encourage your student to fill these out properly according to the directions on the sheet, noting only the present damage to their book. Many students do not take this form seriously. Filling these sheets out properly and returning them, along with COVERING THEIR BOOKS, is important in reducing damage to the books and limiting the obligations they may be responsible for at the end of the year.

TEXTBOOK CONDITION FORM PLEASE PRINT CLEARLY!

Print Last name, First name, Middle initial: ___________________________________________________________________ Print Teacher’s Name: ___________________________________________Hour____________________________________ Print Book Title: ________________________________________________________________________________________ Print Barcode #: ________________________________________________________________________________________ The textbook you are renting for this class is the property of ACHS and is rented to you for the duration of the class. You are required to cover this book with a paper cover and keep damage to a minimum. Print your name on the space provided on the inside cover. We expect that no more than one year of wear will occur while the text is in your possession. The protection of the textbook is your responsibility. Damage to the cover or binding, especially water damage, will result in fines or your having to pay the textbook replacement cost. The purpose of this form is to protect you from any unfair financial assessments. These fines will be in ADDITION to your book rental fee. It is very important that you carefully look over your book, NOTE ANY DAMAGE IN DETAIL (Do not just draw arrows or circle the pictures below to indicate a damaged area!) and hand your form back in to your teacher. Things to look for and make note of at the bottom of this sheet are: pen/pencil marks (even if erased), cover and spine damage (both inside and outside), missing/torn pages, and stains or water damage done to the pages. If at any time during the school year you notice that your textbook is beginning to fall apart through no fault of your own, please bring your book into the library where it will be repaired at no cost to you. By your signature below you are indicating that you have read this form and had the opportunity to assess the condition of this textbook. YOU MUST COVER YOUR BOOKS TO PREVENT DAMAGE!

IMPORTANT TEXTBOOK INFORMATION Textbooks are a very important and sometimes costly part of daily high school life. Parents, below you will find a few guidelines that we have found useful and can help avoid the extra cost students will incur due to a lost or damaged book. • ALWAYS COVER YOUR TEXTBOOKS! Covers provide protection from excess wear and tear which will cost parents less money in damage fines at the end of the year! We recommend a paper book cover (remember the old fashioned brown paper bag covers?) or an oversized stretchy cover. Normal stretchy covers fit too tightly most times and will cause damage to the textbook. • Do not use sticky covers! Sticky covers damage textbooks. • This may be an obvious tip: DO NOT WRITE ON YOUR TEXTBOOKS. • DO write your name on the inside front cover. This is the best way to prevent book mix-ups. • Avoid a fine when you return your textbooks by erasing any stray marks and smoothing out any wrinkled pages. • Liquids and textbooks don’t mix! Liquids cause mold which can make you sick. Take care of your books during rainy days and avoid storing drinks in your backpack. Expect to pay for your textbook if it has severe water damage, smells bad or is showing any signs of mold. • DO NOT STORE YOUR TEXTBOOKS IN CLASSROOMS EVEN IF YOUR TEACHER ALLOWS IT! You will still be responsible for any damages or lost books. • Don’t share your books with friends. You will be responsible for all damages or losses. • Each textbook has a unique barcode. When you return a book to the library, the book must have the same barcode label and number as on your library account and record. (There will be a fine for a missing or damaged barcode.) • Remember if your book is stolen you are still responsible for it, so keep your textbooks safe. • If you do lose your textbook, you must pay for the lost book before you will be issued a new book.
