July 1, 2010 Dear Parents/Guardians, It is enrollment time again! Enclosed are the necessary forms for you to complete and return with your enrollment fees. Please read and review the packet and revise any of the information listed regarding your student that has changed. Make any necessary changes directly on the form(s) and return to ACMS. Please make sure that you sign the permission consents. Health history forms are required to be completed on a yearly basis. We encourage you to enroll by mail; however, if you would like to enroll in person, you may do so on August 10th from 12:30 – 7:00 p.m. at the ACMS office. Please bring the entire completed packet with you when enrolling. We are requesting two separate checks--one for enrollment fees and the other for food service. Please make your meal account check payable to USD 385 Food Service and return in the white envelope with your enrollment forms and payment. Make enrollment checks payable to Andover Central Middle School and return to: Enrollment Andover Central Middle School 903 E. Central Andover, KS 67002 You may also make payments online for both enrollment fees and food service using a credit card by accessing the district website: www.usd385.org. There is a tab on the right side of the homepage “Online Fee Payment”, just click and follow the directions. Please make note on the fee/permission page that you sign and return that you made your payment online. You may also access additional forms you may need, such as KSHSAA physical form, medication and food service specialized forms, etc by clicking on the “Enrollment” tab at the top of the district homepage. In addition there has been an upgrade in PowerSchool that will allow you to access all your student’s records with only one log in. More information will be made available at a later date or check the web page. th
The first day of school is Thursday, August 19th for only all 6 grade students and students new to the Andover District. All students will report to school on Friday, August 20th. Your student’s schedule and locker assignment will be mailed the week of August 2nd. If you have questions and/or concerns regarding your student’s schedule, you may contact the school counselor, Lisa Stinson, at 218-4710 after August 10th. If you would like to stop by and tour the building it will be open from 8:00 a.m. through 3:00 p.m. beginning August 2nd. The staff of ACMS is looking forward to a great school year!
