Sleep Behavior Disorder (RBD) is a condition where subjects retaliate in response to a disturbing dream and may injure themselves or their bed partner. Patients with Parkinson’s sometimes have RBD. Dementia and Alzheimer’s disease may be a major cause of sleep disturbance that sometimes causes agitation and confusion. Advanced Sleep Phase Syndrome (ASPS) is when the circadian rhythm or body clock

to sleep. Sleep Apnea, where pauses in breathing occur during sleep, is a sleep disorder seen with greater frequency in older adults. Restless Leg Syndrome is another disorder that could keep people up at night. Symptoms include creepy-crawly or irritable feelings in the feet and legs particularly in the evening while relaxing and watching television, for example. Rapid Eye Movement

• Keep regular sleep habits • Develop a period of relaxation or “downtime” before bedtime • Go to bed only when sleepy • Exercise regularly, but not close to bedtime • Maintain a healthy diet and avoid eating close to bedtime • Minimize napping, especially in the evening • Avoid caffeine, nicotine and alcohol before bedtime • Create an environment conducive to sleep (cool, dark and quiet) • Use your bed for sleeping and intimacy only (no TV, checking social media, emailing)

Getting a restful night sleep can become a nightmare, especially as we age. One big misconception is that as we age, the amount of sleep we need decreases. The truth is adults need seven to eight hours of sleep per night. This is harder to attain as adults become seniors. With age, sleep becomes lighter and deep sleep (known as delta sleep) may be entirely absent by the age of 80 (earlier in some patients). Sleep is often fragmented, especially in retirees, who nap during the day and sleep less at night. Certain medications can also interfere with sleep as well as forced changes in lifestyle or worrisome medical conditions. Nighttime sleep may also be interrupted by physical complaints such as arthritis, noise, heart disease, reflux, hot flashes (in women) and urges to urinate. Fragmented sleep leads to unrefreshing sleep and a sense that one has slept less. Other sleep disruptors include: Insomnia may be more frequent in the elderly, where nighttime disturbances are followed by difficulty in returning

Alpha Family Medicine Are you having a hard time finding a primary care doctor? According to a recent report by the Georgia Board for Physician Workforce, the state ranked 39th in the ratio of doctors per 100,000 population. The doctor shortage continues to limit patients’ access to care and is most critical in primary care. Some doctors have stopped taking new patients altogether. If you do find a doctor that will accept you as a new patient, you may still have to wait if you want to schedule a routine visit. If you are on Medicare or Medicaid you might not be able to find a doctor and may have to visit the emergency room. In which case, you will have to wait and may not see the same doctor again. There is a solution – the best time to find a new primary care doctor is when a new practice opens. You need to sign up early on, even if it’s for a routine physical. Most insurances pay for a physical once a year without requiring any co-pay. If you sign up early on, the doctor will be familiar with your medical history and it will be easier to get an appointment later on when you have a medical condition. You need to sign up before the doctor stops accepting new patients! Anterior Approach Hip Replacement An alternative to conventional hip replacement leaves a smaller surgical footprint The anterior approach is a technique for performing hip replacement through an incision in the front of the hip rather than through a standard lateral or posterior incision. It is considered to be a less invasive surgery than a traditional total hip replacement because it allows a surgeon to use a smaller incision and access the joint by going between important muscles rather than cutting and reattaching them. A major benefit of leaving the muscles intact is that the joint is more stable, thereby reducing the risk of dislocation. Also, the patient’s position makes it easier to use fluoroscopy, which makes it easier to position the hip implant and accurately assess leg length. There are also fewer post operative restrictions such as a pillow between the legs, high toilet seats and limited sleeping positions. Most total hip replacement patients whose surgeries are done with an anterior approach are able to freely move their hip and bear full weight soon after surgery. This can mean a shorter hospital stay (24) days with faster recovery and rehabilitation because patients are able to be mobile more quickly. The anterior approach is not performed by all surgeons, and requires special training because it is technically advanced. Even surgeons who use this technique will evaluate each patient individually to see if they are a candidate for the anterior approach. Recently, improvements in surgical technique, instrumentation and patient selection have modernized total hip replacement. Some local surgeons have been performing anterior approach hip replacement for years, while others prefer to operate using conventional methods. New Study Shows 3D Mammography Reveals Important Details (NAPSI)—Here’s important information for women. Human breasts are made up of different types of tissue: fatty, fibrous and glandular. If most of the tissue is fibrous and/or glandular, it is considered to be dense. It is not unusual for a woman to have dense breasts. For most women, density decreases with age, but in some women there is little change. Breast density can only be determined by a mammogram, yet finding cancer in a woman with dense breasts is more difficult for a doctor when using traditional technology. The good news is that there’s a more accurate exam— 3D mammography—that can make detecting breast cancer easier in women with dense breasts. A new study published by the Radiological Society of North America showed that the 3D mammography exam finds significantly more cancers in women with dense breasts than a traditional mammogram. That’s because 3D mammography technology allows a doctor to examine a woman’s breast tissue layer by layer. So instead of viewing all the complexities of the tissue in a flat image, as with traditional

mammography, fine details are more visible and no longer hidden by the tissue above or below. Determining Density Is Key It is important for women to know their breast density. That’s because women with dense breasts have an increased risk of cancer. Experts are not certain at this time what it is about dense breast tissue that increases a woman’s risk. Previous studies have shown that the 3D mammography exam finds more invasive cancers than a traditional mammogram-41 percent moreand decrease the number of women called back for unnecessary tests due to false alarms by up to 40 percent. With the 3D mammography exam, women, including those with dense breasts, can feel more confident that their annual screening exam will find the truly troubling cancers while reducing the need for additional, unnecessary tests.
